Progressive Dysphagia Revealing Glioma: Diagnostic Overshadowing by Suspected Myasthenia Gravis.

UNLABELLED: Progressive dysphagia in older adults presents a diagnostic challenge with a broad differential including neuromuscular, structural and malignant causes. Dysphagia associated with aspiration may lead to significant respiratory compromise requiring intensive care management. We report the case of a 78-year-old patient with rheumatoid arthritis treated with methotrexate who presented with progressive dysphagia to solids and liquids, weight loss, and cough. She subsequently developed aspiration pneumonia complicated by repeated type 2 respiratory failure and required mechanical ventil

Masseter muscle quality in acute ischemic stroke: Associations with functional outcome, dysphagia, and cerebral small vessel disease burden.

BACKGROUND: The masseter muscle-a surrogate of systemic muscle mass and quality-has been linked to dysphagia and mortality in oncology, but evidence in acute ischemic stroke (AIS) is limited. Cerebral small vessel disease (SVD), via oxidative stress and inflammation, may contribute. We investigated the prognostic value of masseter status in AIS, its relation to dysphagia, and association with SVD. METHODS: Consecutive AIS patients admitted between January 2022 and March 2024 within one week of onset were screened. Unfavorable outcome was defined as mRS 3-6 at 3 months. On contrast-enhanced CT

Cervical Spine Oncology: Perioperative Management and Complications.

STUDY DESIGN: Narrative review. OBJECTIVE: To synthesize contemporary evidence on perioperative complications in primary and metastatic cervical spine tumor surgery and to provide an evidence-informed framework for their prevention and management. SUMMARY OF BACKGROUND DATA: Cervical spine tumor surgery converges dense neurovascular and aerodigestive anatomy with a medically fragile, frequently irradiated patient population. Although cervical metastases represent only about 8-20% of spinal metastatic disease, their mismanagement carries disproportionately severe consequences, ranging from di

Peroral plication of esophagus for end-stage achalasia: a systematic review.

BACKGROUND AND AIMS: End-stage achalasia is marked by persistent symptoms even after lower esophageal sphincter (LES)-directed therapies and may require esophagectomy, which is associated with significant adverse events. Peroral plication of the esophagus (POPE) is a novel minimally invasive, intermediate alternative. This review aimed to assess the safety and efficacy of POPE in treating end-stage achalasia. METHODS: A systematic search was performed in PubMed, Embase, Web of Science, and the Cochrane Library from January 2016 to May 2025 for studies on POPE in end-stage achalasia. Primary o

Two pediatric type II achalasia cases complicated by candidal and eosinophilic esophagitis: Successful peroral endoscopic myotomy treatment.

Achalasia is a rare esophageal motility disorder in children, often leading to delayed diagnosis. We report the first two pediatric cases treated with peroral endoscopic myotomy (POEM) at our center, both classified as type II achalasia and complicated by mucosal disease. A 15-year-old girl presented with progressive dysphagia and weight loss, diagnosed with type II achalasia and Candida esophagitis. A 13-year-old boy exhibited similar symptoms with a 27-kg weight loss; histology revealed concurrent eosinophilic esophagitis. Both underwent successful POEM. These cases illustrate the clinical h

Memantine-Induced Sialorrhea: A Rare Adverse Reaction and Review of Pathophysiological Mechanisms.

Memantine is a noncompetitive N-methyl-D-aspartate (NMDA) receptor antagonist widely utilized in the management of moderate-to-severe neurocognitive disorders, specifically Alzheimer's disease. While its common adverse effects, such as dizziness, headache, and confusion, are well documented, parasympathetic or motor-related side effects such as drooling are not listed in the standard prescribing information and remain rarely reported in clinical practice. We present a case of a 68-year-old man with a primary diagnosis of moderate-to-severe major neurocognitive disorder due to Alzheimer's disea

Suspected Bulbar-Predominant Myasthenia Gravis Exacerbation in an Older Adult With Respiratory Deterioration and Silent Aspiration: A Case Report.

Myasthenia gravis (MG) is an autoimmune disorder of the neuromuscular junction characterized by fluctuating skeletal muscle weakness. Bulbar-predominant exacerbations may be difficult to recognize in older adults with multiple comorbidities, particularly when respiratory deterioration is initially attributed to cardiopulmonary disease. We report the case of an 83-year-old woman with generalized acetylcholine receptor antibody-positive MG, dementia with behavioral disturbance, moderate chronic obstructive pulmonary disease (COPD), chronic kidney disease, and previous deep vein thrombosis who wa

Delayed diagnosis of cerebrotendinous xanthomatosis with progressive neurological involvement: a case report.

BACKGROUND: Cerebrotendinous xanthomatosis (CTX) is a rare autosomal recessive metabolic disorder caused by pathogenic variants in the CYP27A1 gene. Although effective treatment with chenodeoxycholic acid (CDCA) is available, diagnosis is often delayed due to its heterogeneous clinical manifestations and involvement of multiple organ systems. CASE: We report a 34-year-old man with genetically confirmed CTX who presented with progressive gait instability, dysarthria, dysphagia, and cognitive impairment. His medical history revealed childhood-onset bilateral cataracts, chronic diarrhea, and poo

Kommerell diverticulum in right aortic arch with aberrant left subclavian artery: Routine resection or selective management?

Right aortic arch with aberrant left subclavian artery (RAA-ALSA) is one of the most common complete vascular rings encountered in pediatric cardiovascular practice. The associated Kommerell diverticulum (KD), a remnant of the primitive fourth dorsal aortic arch from which the aberrant subclavian artery arises, forms part of the posterior ring and may contribute importantly to persistent airway and esophageal compression. Historically, repair consisted of division of the ligamentum arteriosum. Recognition that posterior compression can persist after ligamentum division has led many surgeons to

MRI Characterization of Structural Brain Abnormalities in NGLY1 Deficiency.

BACKGROUND AND OBJECTIVES: NGLY1 (N-Glycanase 1) deficiency is an ultrarare autosomal recessive disorder affecting ~165 patients worldwide, characterized by developmental delay, hyperkinetic movement disorders and shortened life expectancy. Despite its severe neurological manifestations, comprehensive neuroimaging characterization has been limited to case reports and small descriptive studies. Our objective was to investigate alterations in brain morphology in patients with NGLY1 deficiency and determine whether these metrics associate with clinical phenotypes. METHODS: This case series analy

Surgical repair of traumatic right subclavian artery pseudoaneurysm after war shrapnel injury: A case report.

A rare right subclavian artery pseudoaneurysm developed after a war-related shrapnel injury in a 46-year-old female with a prior thyroidectomy. There was no initial bleeding or neurologic deficit, and initial noncontrast computed tomography findings were negative. One month later, the patient presented with chest pain, hoarseness, and dysphagia. A chest X-ray showed tracheal compression, Doppler ultrasound revealed leakage from the posterior wall of the right subclavian artery, and computed tomography angiography confirmed a 6 × 7 cm pseudoaneurysm causing 70% tracheal narrowing. Differences in Tongue Muscle Activity During Chewing of Rice With Various Textures.

BACKGROUND: The tongue performs finely coordinated movements during mastication in response to bolus properties; however, few studies have quantitatively evaluated tongue muscle activity during this process. OBJECTIVE: This study aimed to quantify tongue muscle activity during chewing of rice with different physical properties. METHODS: Ten healthy young adults chewed three types of rice-based foods with different textures (regular, soft and extra soft) unilaterally on either the left or right side in random order. Electromyographic (EMG) activity of the tongue was recorded using suction sur

From Feeding Difficulties to Nutritional Risk in Children with Down Syndrome.

Children with Down syndrome (DS) may experience feeding difficulties arising from interactions among hypotonia, cardiorespiratory and gastrointestinal conditions, swallowing dysfunction, oral-motor and sensory differences, delayed feeding skill development, and psychosocial or environmental factors. These difficulties may compromise feeding safety and efficiency, restrict dietary diversity, and contribute to inadequate fluid or nutrient intake, faltering growth, micronutrient deficiencies, or excess adiposity. This narrative review synthesized the literature selected for its relevance to feedi
