Preventing faecal incontinence in patients with functional gastrointestinal disorders.

Nurses assist patients with activities of daily living, including the provision of excretion care. Although excretion may be accomplished by some patients independently, others exhibit disorders in this process. Despite the high prevalence of faecal incontinence, the proper nursing care protocol in this context remains unclear. Therefore, this narrative review was conducted to investigate what types of nursing care are needed to prevent or improve faecal incontinence through a review of existing knowledge, challenges and future prospects. Outcomes of stoma for faecal incontinence.

AIM: Faecal incontinence is a complex condition with multifactorial aetiology and significant medical, social, psychological and economic implications. Stoma formation is typically considered a last resort, and evidence on outcomes remains limited. This study aimed to evaluate the characteristics, management and outcomes of patients undergoing stoma formation for faecal incontinence. METHOD: A retrospective study was conducted in a tertiary unit, including patients with stoma formation for faecal incontinence between February 2011 and August 2023. Twenty-one patients were identified, of whom

Nursing care for patients with low anterior resection syndrome using transanal irrigation.

Low anterior resection syndrome (LARS) is a common and often under-recognised consequence of sphincter-preserving rectal cancer surgery, encompassing a range of bowel symptoms including urgency, faecal incontinence, increased stool frequency, clustering and incomplete emptying. These symptoms can profoundly affect quality of life, dignity, emotional wellbeing and social participation, and may persist for years as part of long-term cancer survivorship. Transanal irrigation (TAI) is one evidence-based management option for patients with problematic LARS. Recent systematic reviews and randomised

Posterior tibial neurostimulation for faecal incontinence in community-dwelling older adults: long-term outcomes and predictors of response.

BACKGROUND: Faecal incontinence (FI) increases up to 50% with age in institutionalized older individuals, and treatment options have been scarcely studied for older people. Posterior tibial nerve stimulation (PTNS) is a minimally invasive second-line treatment available. This research aims to assess long-term clinical outcomes of PTNS using the Wexner score in community-dwelling patients aged > 65 years, and its impact on quality of life (QoL). METHODS: A prospective cohort study with 61 patients (median age 71 years; 79% women) was conducted. PTNS was administered in three phases over 12 mon

A Pragmatic Real-World Observational Study of Patient Reported Outcome Measures (PROMs) Following Treatment of Colorectal Cancer-What Matters Most?

PURPOSE: Patient reported outcome measures (PROMs) following colorectal cancer (CRC) treatment have been gaining prominence in addition to oncological outcomes. We have integrated PROMs collection through our nurse-led follow-up (NLFU) clinic of patients undergoing surgical/endoscopic treatment of CRC and report our real-world results. METHODS: PROMs were collected using the EORTC 29 Questionnaire using a pre-appointment link via NLFU clinic over a period of 18 months since April 2023. The reported outcomes were analysed and correlated with patient demographics, tumour stage, type of procedur

Digital self-management programme for pain, fatigue and faecal incontinence in inflammatory bowel disease: cost-effectiveness analysis of the IBD-BOOST randomised controlled trial.

OBJECTIVE: People living with inflammatory bowel disease (IBD) frequently experience abdominal pain, fatigue and faecal incontinence that persist despite optimal medical treatment. This study aimed to assess the cost-effectiveness of IBD-BOOST, a digital, interactive, facilitator-supported self-management intervention targeting these symptoms. METHODS: A cost-effectiveness analysis was conducted alongside the IBD-BOOST trial, which randomised people with IBD experiencing fatigue, pain and/or faecal incontinence to the IBD-BOOST intervention (N=391) or care as usual (N=389) over 12 months. Outcomes of a novel multidisciplinary approach to delivering sacral neuromodulation services away from the operating theatre.

AIM: At this tertiary referral hospital, sacral neuromodulation (SNM) evaluation procedures for faecal incontinence (FI) were performed by surgeons in theatre, using general or local anaesthetic. Growing referrals and limited theatre access prompted service reform. A clinical scientist multidisciplinary team member completed bespoke training to perform procedures in outpatients using local anaesthetic. This observational study compares outcomes of clinical scientist-led (CSL) and surgeon-led (SL) evaluation procedures, including clinical success and complications. METHOD: Demographics and cli

Preventing deconditioning and incontinence in older people in acute care settings.

Hospital-associated deconditioning is a recognised and well-known condition. It refers to a loss of physiological fitness and functional ability in a patient resulting from prolonged immobility, inactivity or bed rest during a period of hospitalisation. Many patients who experience deconditioning also have new or existing continence issues, although the link between deconditioning and incontinence is not well understood. This article explores deconditioning and incontinence among older people in acute care settings. It outlines the multifactorial links between the two conditions and reviews cu

Pushing using real-time sonographic ultrasound education (PURSUE): A study protocol for a randomized controlled trial.

INTRODUCTION: Maternal expulsive force is a key determinant of fetal head descent during the active second stage of labour. Pushing is most effective when abdominal muscle contraction is synchronised with relaxation of the levator ani muscle, thereby widening the pelvic hiatus. Trans-perineal ultrasound studies show that 15-20% of nulliparous women paradoxically contract the levator ani (co-activation) during pushing, narrowing the hiatus and slowing head descent. Consequently, about 20% of primigravidas experience a prolonged second stage, with higher rates of operative birth and perineal tra

Faecal incontinence in systemic sclerosis: A narrative review.

Although associated with impaired health-related quality of life, faecal incontinence (FI) is underscreened in cases of systemic sclerosis (SSc). Its pathophysiology is complex and multifactorial, encompassing vasculopathy, fibrosis and probably (autoimmune) neuropathy. FI prevalence in SSc seems to be 5-6-fold higher than in the age- and sex-matched general population and is severe in nearly 15% of cases. FI is associated with diarrhoea, small intestinal bacterial overgrowth, constipation, urinary incontinence, anticentromere positivity and some features of vasculopathy. FI should be regularl

Genital bar electrode for dorsal genital nerve stimulation: feasibility and safety of a novel approach for managing bowel dysfunction and overactive bladder.

BACKGROUND: Electrical stimulation of the dorsal genital nerve (DGN) using a patch electrode is effective in treating bowel dysfunction (BD) with faecal urgency and/or incontinence and overactive bladder (OAB). Keeping the patch electrode in place during stimulation may be challenging. This study evaluated the feasibility and safety of inserting a novel genital bar electrode for DGN stimulation. METHOD: A vertical clitoral hood piercing with a specially designed genital bar electrode was inserted under aseptic conditions with topical anaesthesia in women who had undergone DGN stimulation via

Safety of carbon-ion radiotherapy for prostate cancer after rectal surgery: a single-institution retrospective study.

Carbon-ion radiotherapy (C-ion RT) is effective in treating prostate cancer. However, the safety of this therapy in patients with a history of rectal surgery remains unclear. This study aimed to confirm the feasibility and efficacy of C-ion RT in patients with prostate cancer after rectal surgery. This study comprised 33 consecutive patients with prostate cancer who underwent C-ion RT at our institute from October 1995 to May 2024 with a history of rectal surgery for their colorectal cancers. The prescribed doses were 63.0 Gy [relative biological effectiveness (RBE)] in 20 fractions (fr), 57.6

The forgotten burden: nutrition impact symptoms in colorectal cancer survivorship.

PURPOSE OF REVIEW: As survival following colorectal cancer (CRC) continues to improve, an increasing number of survivors experience persistent nutrition impact symptoms (NIS) that adversely affect nutritional status, physical function, and quality of life. This review is timely in synthesising recent evidence on the prevalence, mechanisms, and clinical consequences of NIS, an area that remains under-recognised within survivorship care. RECENT FINDINGS: Recent studies consistently demonstrate that bowel dysfunction, including diarrhoea, urgency, faecal incontinence, and constipation, affects a

Long-Term Outcomes of PTQ Anal Sphincter Implants for the Management of Faecal Incontinence: A Single Centre Experience.

BACKGROUND: Faecal incontinence can be functionally debilitating, with significant impact on patients' quality of life. Injectable perianal bulking agents such as PTQ implants are shown to improve faecal incontinence in the short- to mid-term; however, long-term outcomes remain poorly understood. OBJECTIVE: To assess long-term outcomes of PTQ implants for management of faecal incontinence. METHODS: A retrospective cohort study of 101 patients receiving PTQ implants at St Vincent's Public and Private Hospitals, Fitzroy, between 2007 and 2023. Mean follow up time was 8 years (range 2-17). Prim

Change in faecal incontinence pattern after gastric bypass surgery: related to change in anal sphincter thickness?

PURPOSE: Faecal incontinence is common in persons with severe obesity. Little is known about how the thicknesses of the internal anal sphincter (IAS) and the external anal sphincter (EAS) change in relation to weight loss following metabolic bariatric surgery (MBS). This study aims to investigate any change in IAS and EAS thickness 6 months after Roux-en-Y gastric bypass surgery (RYGB) and to determine whether any such change correlates with a change in faecal incontinence pattern. METHODS: Thirty-one patients underwent three-dimensional endoanal ultrasound to measure anal sphincter thickness

Predictors of Success With Sacral Nerve Stimulation for Faecal Incontinence-Results From a Single Centre Over 15 Years.

BACKGROUND: Sacral nerve stimulation (SNS) is an accepted first-line surgical treatment for faecal incontinence (FI) with variable success. This study aims to analyse the parameters that may predict initial and sustained success with SNS in patients with FI. METHODS: A retrospective audit was conducted on 56 patients with FI managed with SNS implantation by a single Australian colorectal surgeon. A successful outcome was defined as a 50% or greater reduction in the number of FI episodes per week at the end of the two-week trial period (temporary stimulation) or at their last follow-up visit (
