Results of the international audit for faecal incontinence on behalf of the European Society of Coloproctology (ESCP) collaborating group.

AIM: The aim of this study was to evaluate the prevalence of faecal incontinence (FI) among patients attending colorectal clinics in a global setting, treatment choices and accessibility to diagnostic and treatment tools. METHODS: An international, prospective, multi-centre snapshot audit and survey of patients undergoing consultation regarding FI was undertaken from January 9 to February 28, 2023. Participating units included patients in the audit who presented until March 28, 2023. Main outcomes measured included the prevalence of FI in clinical practice, diagnostic approaches, treatment pa

Perianal Crohn's disease: the experience of taking a multiprofessional approach in a tertiary centre setting.

Perianal fistulising Crohn's disease (PFCD) is a debilitating phenotype of Crohn's disease, with a lifetime incidence of 20-30% in people living with the dieases. Symptoms include perianal pain, perianal discharge and faecal incontinence, with repercussions for a patient's physical and mental health, psychosocial wellbeing and productivity. PFCD is challenging to treat, with existing therapeutic options achieving modest fistula remission rates only. While research initiatives are under way to characterise PFCD pathogenesis and optimal treatment approaches, the focus should be on early diagnosi

Volume-outcome relationship in corrective surgery for Hirschsprung's disease: a systematic literature review of direct evidence and an overview of indirect evidence.

Hirschsprung's disease (HD) is a rare congenital condition requiring complex corrective surgery. We reviewed available direct evidence on the volume-outcome relationship for HD corrective surgery and assessed the applicability of indirect evidence based on GRADE (Grading of Recommendations Assessment, Development, and Evaluation) recommendations for rare diseases. Three retrospective cohort studies analysing the volume-outcome relationship for HD surgery met the inclusion criteria. In all studies, the high-volume threshold was below 12 HD corrective surgeries per year. No significant volume-ou

The Ageing of the Human Lower Bowel.

Older people suffer a greater number of disorders of the gastrointestinal tract, including chronic constipation and faecal incontinence. In this review, we examine the age-related degenerative changes that have been identified in the lower bowel of humans. Firstly, older individuals may experience less abdominal pain and a lower incidence of gut-brain disorders that are defined partly by abdominal pain (e.g., irritable bowel syndrome); the causes are unclear. Secondly, an age-dependent reduction in mucosal barrier functions may follow a decline in intestinal stem cell activity, a reduced densi

Fistulotomy versus fistulectomy for simple fistula-in-ano: a systematic review and meta-analysis of randomized controlled trials.

BACKGROUND: Perianal fistulas are a common anorectal pathology. The sphincter-cutting techniques of fistulectomy and fistulotomy are associated with high cure rates for low or simple fistula-in-ano, with negligible risk of incontinence. However, the superiority of either technique has not previously been conclusively demonstrated. The aim of this systematic review is to compare the outcomes of the two surgical techniques for the management of simple fistula-in-ano. METHODS: A search of MEDLINE, EMBASE and Cochrane Databases for randomized controlled trials (RCT) comparing fistulotomy to fistu

Sphincter-preserving total fistulectomy with resection of the anoderm and skin and intersphincteric space closure for anal fistula.

Non-cure, recurrence and faecal incontinence have often been a major challenge in patients after surgical treatment for anal fistula. To overcome these problems, we have developed a sphincter-preserving total fistulectomy procedure involving resection of the anoderm and skin with intersphincteric space closure (TFRAS). The purpose of the present study was to evaluate this TFRAS technique. TFRAS was performed in 618 patients. The method involved excision of the anoderm and skin, including the internal opening, total removal of the fistula tract while preserving the sphincter, closure of the res

Management of chronic anal fissures: should fissurectomy be performed with botulinum toxin injection?

BACKGROUND: Despite the superior success rate of lateral internal sphincterotomy for managing chronic anal fissure (CAF), there is a trend towards sphincter-preserving treatment due to the increased risk of incontinence. Botulinum toxin (BT) and fissurectomy are two sphincter-preserving options for CAF. We aim to assess if combining fissurectomy with botox treatment is superior to botulinum toxin alone in the management of CAF. METHODS: This retrospective cohort study was conducted across two Sydney hospitals over 7 years. All patients with a CAF managed with either BT and fissurectomy (group

Prognostic factors for incontinence-associated dermatitis (IAD): Results of an international expert survey.

BACKGROUND: Incontinence-associated dermatitis (IAD) is a prevalent and distressing form of irritant contact dermatitis caused by prolonged exposure to urine and/or faeces. Not all incontinent individuals develop IAD, suggesting that additional prognostic factors contribute to its onset. The quality of empirical evidence supporting risk factors for IAD development is moderate to very low. Therefore, it is necessary to systematically compile and analyse expert knowledge on this topic. AIM: This study aimed to identify and prioritise key prognostic factors for IAD development through an interna

Teaching Caregivers to Implement the Picture Exchange Communication System: An Evaluation of Video Modeling.

BACKGROUND: Around 60% to 80% of patients with periampullary carcinoma are unresectable either due to distant metastasis or local vascular invasion. With the advancement of endoscopic interventional procedures, the role of surgical bypass has diminished. However, surgical bypass is still appropriate in patients with unresectable disease discovered at the time of surgery. This study was conducted to assess the results of palliative surgical bypass for patients with unresectable periampullary carcinoma at our hospital, a tertiary referral center of Northern India. METHOD: The study group compri

A qualitative exploration of perceived risks, motivations, and risk management strategies among brothel-based female sex workers in Ibadan, Nigeria.

BACKGROUND: Female sex workers (FSWs) experience a higher burden of Human Immunodeficiency Virus (HIV), and other sexually transmitted infections (STIs) compared to the general population due to the nature of their work. In Nigeria, challenges such as poverty, stigma, and poor access to healthcare continue to heighten their vulnerability, even though prevention programmes are in place. However, limited qualitative evidence exists on how FSWs perceive sexual risks, the motivations driving unsafe practices, and the strategies used to mitigate them. This study draws from the constructs of the Hea

Metabolic Implications of PCOS.

BACKGROUND: In children, necrotizing pneumonia (NP) is an uncommon, severe complication of pneumonia. It is characterized by destruction of the underlying lung parenchyma resulting in multiple small, thin-walled cavities and is often accompanied by empyema and bronchopleural fistulae. REVIEW: NP in children was first reported in children in 1994, and since then there has been a gradual increase in cases, which is partially explained by greater physician awareness and use of contrast computed tomography (CT) scans, and by temporal changes in circulating respiratory pathogens and antibiotic pre
