Beyond Postprandial Symptoms: Fasting Hypoglycemia in Post-Bariatric Hypoglycemia.

BACKGROUND: Post-bariatric hypoglycemia (PBH) is characterized by postprandial neuroglycopenia resulting from an exaggerated hyperinsulinemic response to nutrient intake in patients who have undergone Roux-en-Y gastric bypass, sleeve gastrectomy, or other upper gastrointestinal surgical procedures for weight loss. Fasting hypoglycemia is atypical. OBJECTIVE: We present a cohort of patients who developed symptomatic fasting hypoglycemia in the setting of established PBH and discuss the possible pathophysiological mechanisms. We also examine the role of β-cell hyperfunction in PBH. METHODS: A

Clinical, biochemical, and genetic perspectives in hepatic glycogen storage diseases: HEPA-GSD study.

Glycogen storage diseases (GSDs) are a group of inherited metabolic disorders resulting from genetic deficiencies in enzymes involved in glycogen synthesis or breakdown, transport proteins, or proteins that regulate glycogen metabolism. Liver GSDs are further subdivided into subtypes based on the specific enzyme deficiency. This multicenter, cross-sectional, descriptive retrospective study, conducted across 24 centers, aimed to document the clinical and genotypic findings in patients with hepatic GSD. The study included 282 patients (152 males, 53.9%) from 278 unrelated families. At the time o

Incidence and Risk Factors for Development of Portal Hypertension and Sinusoidal Liver Injury Associated with T-DM1 in HER2-Positive Breast Cancer.

PURPOSE: While an association between trastuzumab emtansine (T-DM1) and development of portal hypertension has been suggested, clinical studies on the incidence and risk factors remain limited. MATERIALS AND METHODS: HER2-positive breast cancer patients treated with T-DM1 as adjuvant or palliative therapy were retrospectively enrolled. The composite endpoint of portal hypertension and sinusoidal liver injury was defined as development of splenomegaly and at least one among gastroesophageal varices, spontaneous portosystemic shunt, or portal hypertensive ascites; or biopsy-confirmed sinusoidal

Histone lysine demethylase inhibition is a disease-modifying therapy for hypertrophic cardiomyopathy.

RATIONALE: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiac disorder characterized by cardiac hypertrophy, fibrosis, arrhythmias, and sudden cardiac death (SCD). Although current therapies primarily target sarcomere dysfunction, the contribution of epigenetic dysregulation to HCM pathogenesis and its therapeutic potential remain poorly understood. OBJECTIVE: To determine whether pharmacological inhibition of histone lysine demethylases (KDMs) with JIB-04 can prevent or reverse HCM progression and to identify the underlying epigenetic mechanisms. METHODS AND RESULTS: We evaluat

Hepatic B-Cell Lymphoma in a Common Mynah (Acridotheres tristis): A Case Report.

BACKGROUND: Hepatic lymphoma is rarely characterized in mynah birds, and severe hepatomegaly in this group is often interpreted in relation to iron storage disease or other metabolic hepatopathies. CASE DESCRIPTION: This report describes hepatic B-cell lymphoma in a 14-year-old common mynah (Acridotheres tristis) presented with lethargy, altered vocalization, poor body condition, ascites, dyspnea, chronic ataxia, and left-leg lameness. Radiography revealed a large soft-tissue opacity in the middle coelomic cavity, with caudodorsal displacement of the intestinal loops and compression of the th

Mast Cells Regulate Epithelial Barrier Integrity and Myeloid Immune Responses in Experimental Colitis.

Mast cells (MCs) are increasingly recognized as modulators of intestinal homeostasis and inflammation; however, their precise contribution during colitis remains incompletely defined. Here, we investigated the role of MCs in dextran sulfate sodium (DSS)-induced colitis using a Kit-independent MC-deficient model (Cpa3-Cre; Mcl1fl/fl), in which selective deletion of the anti-apoptotic gene Mcl1 ablates MCs. Despite efficient and tissue-wide MC depletion, MC-deficient mice exhibited macroscopic and histological disease severity comparable to control mice. However, MC deficiency significantly alte

Spontaneous splenic rupture in a patient with congenital factor XIII deficiency: A case report and literature review.

Spontaneous (atraumatic) splenic rupture is a rare but potentially life-threatening clinical condition. Most reported cases of spontaneous splenic rupture are associated with splenomegaly due to infectious, neoplastic, or inflammatory diseases. Spontaneous rupture in the absence of these conditions is extremely rare. There is often an underlying trauma history. Factor XIII deficiency, a rare autosomal recessive bleeding disorder, is associated with abnormal fibrin stabilization despite normal routine coagulation parameters. Therefore, it can also lead to defects in wound healing. For the same

Fatal haemophagocytic lymphohistiocytosis and atypical haemolytic uraemic syndrome following coronavirus disease 2019 infection.

A previously healthy woman in her early 30s developed rash, fever and arthralgia 3 weeks after SARS-CoV-2 infection, rapidly progressing to multiorgan failure. Laboratory studies revealed severe thrombocytopenia (23×109/L), microangiopathic haemolytic anaemia, acute kidney injury and ferritin >100 000 ng/mL. Kidney biopsy demonstrated thrombotic microangiopathy with negative antinuclear antibody, supporting atypical haemolytic uraemic syndrome (aHUS). Concurrently, low haptoglobin, elevated lactate dehydrogenase (2500 U/L), interleukin-2 receptor (18 644), triglycerides (425 mg/dL) and splenom

Massive Splenomegaly Secondary to Lymphangioma With Gluteal Component in an Adolescent: A Case Report.

Splenic lymphangioma (SL) is a rare benign lymphatic malformation resulting from abnormal development of lymphatic vessels. It primarily affects children and may coexist with lymphatic malformations at other anatomical sites. Most patients are asymptomatic, and the diagnosis is often incidental. When symptoms occur, they are usually related to the size of the lesion and the compression of adjacent organs. We present the case of a 14-year-old male with a history of congenital gluteal and perineal lymphatic malformations, initially misdiagnosed prenatally as myelomeningocele. During follow-up, t

Atraumatic Spontaneous Splenic Rupture Complicating Decompensated Chronic Liver Disease With Portal Hypertension: A Case Report.

Atraumatic spontaneous splenic rupture (ASR) is a rare, potentially fatal condition accounting for fewer than 0.5% of all splenic ruptures, with reported mortality of up to 20% when diagnosis is delayed. We report a 63-year-old male with decompensated chronic liver disease and portal hypertension who presented with acute epigastric pain, progressive abdominal distension, and absolute constipation without antecedent trauma. Abdominal ultrasonography identified splenomegaly with a 5 × 6 cm anechoic intraparenchymal collection with subcapsular and perisplenic extension and moderate hemoperitoneum

Ras-associated autoimmune leukoproliferative disorder associated with immune thrombotic thrombocytopenic purpura.

BACKGROUND: Ras-associated autoimmune leukoproliferative disorder (RALD) is a rare, nonmalignant condition characterized by monocytosis, lymphoproliferation, and autoimmunity, driven by KRAS or NRAS variants. Immune thrombotic thrombocytopenic purpura (iTTP) results from autoantibodies against ADAMTS13 (a disintegrin and metalloproteinase with a thrombospondin type 1 motif, member 13). KEY CLINICAL QUESTION: Can RALD co-occur with iTTP, and how can it be diagnosed? CLINICAL APPROACH: A patient presenting with neutropenia, monocytosis, and thrombocytopenia was initially diagnosed with chronic

Sustained Regression of Parotid Mucosa-Associated Lymphoid Tissue (MALT) Lymphoma After Nutritional and Nutraceutical Intervention Without Chemoimmunotherapy: A Case Report.

Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT) in the setting of Sjögren's syndrome (SS) is a well-recognized complication of chronic autoimmune salivary gland inflammation. Standard management options include radiotherapy, immunotherapy, and combination chemoimmunotherapy, with active surveillance reserved for stable, low-burden disease. The potential role of structured nutritional and nutraceutical intervention in this setting has not been previously described. We report the case of a woman who presented at age 52 with SS-associated bilateral parotid MALT lymph

Splenomegaly With Thrombocytopenia: A Case of the Rare and Obvious.

BACKGROUND: A 12-year-old boy presented with incidental splenomegaly, mild anemia, and thrombocytopenia, prompting evaluation, with Gaucher disease considered in the differential diagnosis. OBSERVATION: Enzyme testing revealed low β-glucosidase activity twice; however, Sanger sequencing was negative for mutations in the glucocerebrosidase 1 (GBA1) gene. A peripheral smear review was the turning point; it showed stomatocytes and giant platelets, suggestive of sitosterolemia. Next-generation sequencing confirmed a homozygous, likely pathogenic variant in ATP-binding cassette subfamily G member

Case Report: Breaking the diagnostic dilemma of "lymphoma-like" presentation: a case of adult-onset Still's disease characterized by generalized lymphadenopathy and extremely elevated ferritin.

Adult-onset Still's disease (AOSD) is a rare autoinflammatory disease, the clinical manifestations of which often overlap with other diseases such as lymphoma. This overlap makes differential diagnosis difficult, especially in patients with generalized lymphadenopathy, splenomegaly, and systemic metabolic hyperactivity. In this article, we discuss a 53-year-old East Asian (Chinese) female patient who had a fever, transient rash, generalized lymphadenopathy, and splenomegaly. An 18F-FDG PET/CT scan showed diffuse hypermetabolism in multiple lymph nodes, her spleen, and her bone marrow, and her

Hepatic involvement in children with diabetes: Pathophysiology, clinical manifestations, and current management.

Diabetes is a chronic disease characterized by hyperglycemia resulting from insulin deficiency or resistance to insulin action. Its onset may result from polygenic or monogenic mechanisms. The liver plays a central role in glucose metabolism and can be affected by various conditions. In poorly controlled type 1 diabetes, glycogenic liver disease presents with hepatomegaly and elevated liver enzymes, which is reversible with good metabolic control. Autoimmune hepatitis may coexist with type 1 diabetes due to shared immunological mechanisms or may develop during treatment with corticosteroids. I

Multisystem imaging findings as diagnostic red herrings in Wilson's disease.

A previously well man in his 20s presented with fever, cough, new-onset jaundice and a right-sided pleural effusion. Initial investigations demonstrated marked hepatic synthetic dysfunction. Subsequent imaging demonstrated cirrhotic liver morphology with splenomegaly and ascites. Additional findings of gallbladder wall oedema and focal colonic wall thickening broadened the differentials but following specialist radiology review, these abnormalities were recognised as manifestations of portal hypertension rather than separate disease processes.Further investigations confirmed Wilson's disease i
