Giant hepatic angiomylipoma presenting with painful hepatomegaly.

Hepatic angiomyolipoma (HAML) is a rare mesenchymal neoplasm belonging to the perivascular epithelioid cell tumor (PEComa) family. Although usually asymptomatic and incidentally detected, large lesions may cause symptoms and pose diagnostic challenges. We report the case of a 69-year-old man presenting with painful hepatomegaly. Computed tomography revealed a 12-cm, predominantly fat-containing hepatic mass. Percutaneous biopsy demonstrated characteristic morphological features and an immunohistochemical profile showing HMB-45 and Melan-A positivity, with negative S100 and smooth muscle actin

Loss of hepatic circadian organization in fatty liver syndrome identifies CLOCK as a regulator of lipid homeostasis in laying hens.

Fatty liver syndrome (FLS) is a major metabolic disorder in laying hens, but how hepatic lipid dysregulation is linked to circadian disruption remains unclear. Here, we investigated the role of circadian regulation in FLS pathogenesis and focused on the core clock gene Clock Circadian Regulator (CLOCK). In a high-fat diet-induced FLS model in Lohmann pink layers, affected birds showed hepatomegaly, hepatic lipid accumulation, elevated serum total cholesterol, triglycerides, alanine aminotransferase, and aspartate aminotransferase, increased hepatic pro-inflammatory gene expression, and reduced

Serologically supported pediatric brucellosis presenting with marked conjugated hyperbilirubinemia and marked systemic inflammation: A case report.

Brucellosis is a multisystem zoonosis that can mimic complicated bacterial infection in children. A 10-year-old girl presented with prolonged high-grade fever, jaundice, abdominal pain, anorexia and weight loss. The initial syndrome was managed empirically as an undifferentiated complicated bacterial illness because enteric fever and a hepatobiliary or intra-abdominal source were considered. CRP was 333.2 mg/L and total/direct bilirubin 7.24/5.21 mg/dL despite only mild aminotransferase elevation; ultrasonography showed hepatomegaly and nonspecific gallbladder-wall edema without biliary dilata

Enhanced hepatoprotective effect of Phoenix dactylifera L. and carvedilol in dexamethasone-induced toxicity in rats by reducing oxidative stress and apoptosis: possible role of Hotair-1/mir-148a pathway.

BACKGROUND: Corticosteroid toxicity can predispose individuals to metabolic dysfunction-associated steatotic liver disease (MASLD), which may progress to metabolic dysfunction-associated steatohepatitis (MASH). Date palm fruit (Phoenix dactylifera L.) is widely recognized as a nutraceutical due to its rich content of bioactive phytochemical compounds. OBJECTIVES: This study aimed to investigate the enhanced hepatoprotective effects of date palm fruit (pulp and ethanolic extract) in comparison with carvedilol, as well as their combined effects, in a model of dexamethasone-induced hepatotoxicit

Immune responses in the olfactory organ and brain of Atlantic cod (Gadus morhua) following nasal vaccination against Francisella noatunensis.

Bacterial infections caused by the intracellular bacterium Francisella noatunensis pose a major health challenge in Atlantic cod (Gadus morhua), largely due to the lack of effective prophylactic and therapeutic measures. Here we evaluated the immune and physiological responses of cod to two candidate live attenuated vaccines (i.e. clpB and iglC) against F. noatunensis delivered intranasally. These experimental vaccines were constructed by a single knockout mutation of a virulence gene in F. noatunensis. Cod was vaccinated intranasally and reared for 45 days until infection with the wild-type s

[AL amyloidosis associated with multiple myeloma revealed by cholestatic hepatomegaly: A case report].

INTRODUCTION: AL amyloidosis is a rare disorder characterized by the extracellular deposition of immunoglobulin light chain fibrils produced by a clonal plasma cell population. Hepatic involvement as the initial manifestation is rare, and forms associated with jaundice and cholestasis carry a poor prognosis. CASE REPORT: We report the case of a 53-year-old man from Togo presenting with massive hepatomegaly, cholestatic jaundice, and weight loss. After a negative etiological workup, diffuse hepatic infiltration associated with a plasma cell dyscrasia raised suspicion of AL amyloidosis. Periphe

Beyond Postprandial Symptoms: Fasting Hypoglycemia in Post-Bariatric Hypoglycemia.

BACKGROUND: Post-bariatric hypoglycemia (PBH) is characterized by postprandial neuroglycopenia resulting from an exaggerated hyperinsulinemic response to nutrient intake in patients who have undergone Roux-en-Y gastric bypass, sleeve gastrectomy, or other upper gastrointestinal surgical procedures for weight loss. Fasting hypoglycemia is atypical. OBJECTIVE: We present a cohort of patients who developed symptomatic fasting hypoglycemia in the setting of established PBH and discuss the possible pathophysiological mechanisms. We also examine the role of β-cell hyperfunction in PBH. METHODS: A

Clinical, biochemical, and genetic perspectives in hepatic glycogen storage diseases: HEPA-GSD study.

Glycogen storage diseases (GSDs) are a group of inherited metabolic disorders resulting from genetic deficiencies in enzymes involved in glycogen synthesis or breakdown, transport proteins, or proteins that regulate glycogen metabolism. Liver GSDs are further subdivided into subtypes based on the specific enzyme deficiency. This multicenter, cross-sectional, descriptive retrospective study, conducted across 24 centers, aimed to document the clinical and genotypic findings in patients with hepatic GSD. The study included 282 patients (152 males, 53.9%) from 278 unrelated families. At the time o

Histone lysine demethylase inhibition is a disease-modifying therapy for hypertrophic cardiomyopathy.

RATIONALE: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiac disorder characterized by cardiac hypertrophy, fibrosis, arrhythmias, and sudden cardiac death (SCD). Although current therapies primarily target sarcomere dysfunction, the contribution of epigenetic dysregulation to HCM pathogenesis and its therapeutic potential remain poorly understood. OBJECTIVE: To determine whether pharmacological inhibition of histone lysine demethylases (KDMs) with JIB-04 can prevent or reverse HCM progression and to identify the underlying epigenetic mechanisms. METHODS AND RESULTS: We evaluat

Hepatic involvement in children with diabetes: Pathophysiology, clinical manifestations, and current management.

Diabetes is a chronic disease characterized by hyperglycemia resulting from insulin deficiency or resistance to insulin action. Its onset may result from polygenic or monogenic mechanisms. The liver plays a central role in glucose metabolism and can be affected by various conditions. In poorly controlled type 1 diabetes, glycogenic liver disease presents with hepatomegaly and elevated liver enzymes, which is reversible with good metabolic control. Autoimmune hepatitis may coexist with type 1 diabetes due to shared immunological mechanisms or may develop during treatment with corticosteroids. I

Hepatic Sialylation Defects Increase Susceptibility to Obesity and Steatosis in Mice.

Sialic acid is a well-characterized monosaccharide known to regulate early development and lifelong health, and its aberrant metabolism has been closely linked to multiple pathologies including cancer, cardiovascular disease, and neurological disorders. However, its role in early-life metabolic programming and adult metabolic homeostasis remains poorly understood. Here, we investigated Cmp-Neu5Ac synthase (CMAS), a key enzyme in the sialylation pathway, using two complementary mouse models to address this gap. In a mammary-specific Cmas knockout model, reduced milk sialyllactose levels signifi

The emerging contaminant HFPO-TA impairs hepatic glucose metabolism and induces pparγ-associated lipid metabolic disorders in zebrafish.

Hexafluoropropylene oxide trimer acid (HFPO-TA) is a new alternative to PFOA; it is already widely used and has caused global pollution. Previous studies have suggested that HFPO-TA may influence hepatic glucose and lipid metabolism via PPAR signaling pathway, however, the underlying molecular mechanism remains unclear. Here, a model of pparγ-knockout (pparγ-KO) zebrafish was used to investigate the mechanisms involving the disorders of glucose and lipid metabolism in the liver induced by HFPO-TA. In this study, a range of techniques were employed, including pparγ-KO (pparγ-/-) zebrafish, tran

Anterior Osteophytes at L3-L4 as a Primary Predictor of Adjacent Segment Degeneration After Lumbar Instrumented Fusion.

INTRODUCTION: Degenerative lumbar spine disorder (DLSD) is a significant public health concern, presenting with low back pain, radicular leg pain, and reduced quality of life. Although conservative management is the first-line treatment, some patients require surgery due to persistent symptoms or progressive neurological deficits. Lumbar instrumented fusion is a commonly performed procedure for DLSD. Adjacent segment degeneration (ASD) is a frequent complication after spinal fusion, occurring at either the cranial or caudal side of the fusion mass. While the role of spinopelvic parameters in A

Medical Management of Polycystic Liver Disease: A Position Statement From the European Reference Network on Hepatological Diseases.

Polycystic liver disease (PLD) is a rare genetic disorder characterised by progressive liver enlargement due to multiple cysts. The main symptoms are liver volume-related. Although randomised controlled trials have shown that somatostatin analogues (SSAs) reduce liver volume as well as symptoms, specific guidance on when and how to use SSAs in clinical practice is still lacking. A panel of 15 hepatologists and nephrologists developed practical guidance on SSA use, based on a systematic literature search, expert surveys, and clinical experience. This consensus was reached during a two-day works

Antiobesogenic, Hypocholesterolemic and Antioxidant Effects of the Fungal (1→3)(1→6)-β-D-Glucan (Botryosphaeran) in Obese and Non-Obese Female Wistar Rats.

Botryosphaeran is an exocellular (1 → 3)(1 → 6)-β-d-glucan that promotes significant metabolic effects in male rats, as antiobesogenic and hypoglycemic effects. This study aimed to investigate its metabolic effects in female Wistar rats subjected to a high-fat high-sucrose diet. Obesity induction resulted in increased body weight, accumulation of adipose tissue, glucose intolerance, insulin resistance, hepatomegaly and high levels of TBARS (oxidative stress marker) in the liver, compared with the controls; all differences were statistically significant (p ˂0.05). Treatment with botryosphaeran
