A 12-year-old boy presented to our center with elevated aminotransferases, jaundice, right upper quadrant tenderness, and hepatomegaly. Evaluation revealed a highly elevated blood hepatitis C virus (HCV) ribonucleic acid (RNA) level, and liver biopsy showed canalicular and intrahepatic cholestasis, feathery degeneration of hepatocytes, and periportal/perisinusoidal fibrosis. Though there were atypical biopsy features, including absence of bile ductular reaction, we diagnosed suspected fibrosing cholestatic hepatitis (FCH). FCH secondary to HCV infection has been reported exclusively in immunos

Partial hepatectomy (PHx) and liver transplantation are the most effective interventions for end-stage liver diseases, yet druggable targets that promote donor or remnant liver regeneration are critically lacking. Pregnane X receptor (PXR), a key regulator of liver regeneration, is a promising therapeutic target. Previously, we identified the natural product Schisandrol B (SolB) as a PXR agonist to promote liver regeneration. To further improve its PXR activity and selectivity, the present study performed structural simplification of SolB and identified BZZ-034, a ring-opened analogue with an

OBJECTIVE: B-mode ultrasonography is widely used to assess hepatic steatosis in MASLD. The associations between ultrasound-assessed steatosis, liver size, and metabolic abnormalities have previously been studied without considering whether the patients had received treatment. Medications may modify liver enzymes, glycemic indices, lipid profiles, and body weight and may strongly affect these relationships. Therefore, we aimed to evaluate hepatic echogenicity grade and liver size in relation to body mass index (BMI) and metabolic markers in newly diagnosed, treatment-naïve MASLD patients.

Vitamin B12 plays a crucial role in DNA synthesis, and its deficiency can occasionally present with atypical hematological manifestations mimicking myelodysplastic syndrome (MDS). There is limited literature describing reversible chromosomal abnormalities associated with vitamin B12 deficiency. We report an 11-year-old girl who presented with generalized body aches for 1 month and fever, easy fatigability, and respiratory distress for 2 days. Examination revealed pallor and hepatomegaly, with no splenomegaly. Investigations showed severe anemia (hemoglobin 3.1 g/dL), thrombocytopenia, a low co

Rosai-Dorfman-Destombes disease (RDD) is a rare non-Langerhans cell histiocytosis with a broad clinical spectrum, ranging from localized lymphadenopathy to severe multisystem involvement. Due to its rarity, clinical data on pediatric RDD remain limited, particularly in North Africa. We report two contrasting pediatric cases of RDD managed at a tertiary center in Morocco to illustrate this clinical heterogeneity. The first was that of a five-year-old girl who presented with chronic, isolated, painless cervical lymphadenopathy. Laboratory findings were normal. Biopsy confirmed RDD with classic e

Hepatic cavernous hemangioma is the most common benign liver tumor, but giant or extensively thrombosed variants may lose their characteristic imaging features and mimic other cystic or vascular lesions. A 32-year-old man presented with recurrent fever for more than two months, massive hepatomegaly, leukocytosis (white blood cell count 14.46 × 109/L), C-reactive protein 165.30 mg/L, and greatly elevated D-dimer (48.44 mg/L) and fibrin degradation products (323.52 mg/L), with normal platelet count, fibrinogen, prothrombin time, and activated partial thromboplastin time. Lysosomal acid lipase deficiency (LAL-D, OMIM #309900), caused by mutations of the LIPA gene, is involved in two continuum disease phenotypes: Wolman disease in infants and cholesteryl ester storage disease in adults. Their major phenotypes include steatosis, hepatomegaly, splenomegaly, and elevated liver enzymes. Enzyme replacement therapy has been used for the treatment of LAL-D. In this study, we examined the therapeutic effect of liposomal clodronate in an LAL-D mouse model. When liposomal clodronate was administered intraperitoneally by a single administration at 200 mg/kg to adult mice,

Syphilis, caused by the spirochete Treponema pallidum, is a sexually transmitted infection whose incidence has risen worldwide over the past decade and which may involve virtually any organ, including the liver. Syphilitic hepatitis is an uncommon and frequently overlooked manifestation that characteristically produces a cholestatic pattern of liver injury. We report the case of a 61-year-old man with no relevant medical history and self-reported sexual risk factors, admitted with a 15-day history of bilateral leg edema, fatigue, dry cough, odynophagia, weight loss, and conjunctivitis of the r

Werner syndrome (WS) is a rare autosomal recessive progeroid disorder caused by mutations in the WRN gene and is characterized by distinctive physical features and metabolic complications, including insulin resistance, diabetes, hypertriglyceridemia, and hepatic steatosis. Although lipodystrophy is a recognized component of WS, clinical reports of metreleptin use in this setting remain very limited. We report the case of a 38-year-old woman with WS complicated by partial lipodystrophy, diabetes with suboptimal glycemic levels, hypertriglyceridemia and severe hepatic steatosis. She presented wi

Hepatic angiomyolipoma (HAML) is a rare mesenchymal neoplasm belonging to the perivascular epithelioid cell tumor (PEComa) family. Although usually asymptomatic and incidentally detected, large lesions may cause symptoms and pose diagnostic challenges. We report the case of a 69-year-old man presenting with painful hepatomegaly. Computed tomography revealed a 12-cm, predominantly fat-containing hepatic mass. Percutaneous biopsy demonstrated characteristic morphological features and an immunohistochemical profile showing HMB-45 and Melan-A positivity, with negative S100 and smooth muscle actin

Fatty liver syndrome (FLS) is a major metabolic disorder in laying hens, but how hepatic lipid dysregulation is linked to circadian disruption remains unclear. Here, we investigated the role of circadian regulation in FLS pathogenesis and focused on the core clock gene Clock Circadian Regulator (CLOCK). In a high-fat diet-induced FLS model in Lohmann pink layers, affected birds showed hepatomegaly, hepatic lipid accumulation, elevated serum total cholesterol, triglycerides, alanine aminotransferase, and aspartate aminotransferase, increased hepatic pro-inflammatory gene expression, and reduced

Brucellosis is a multisystem zoonosis that can mimic complicated bacterial infection in children. A 10-year-old girl presented with prolonged high-grade fever, jaundice, abdominal pain, anorexia and weight loss. The initial syndrome was managed empirically as an undifferentiated complicated bacterial illness because enteric fever and a hepatobiliary or intra-abdominal source were considered. CRP was 333.2 mg/L and total/direct bilirubin 7.24/5.21 mg/dL despite only mild aminotransferase elevation; ultrasonography showed hepatomegaly and nonspecific gallbladder-wall edema without biliary dilata

BACKGROUND: Corticosteroid toxicity can predispose individuals to metabolic dysfunction-associated steatotic liver disease (MASLD), which may progress to metabolic dysfunction-associated steatohepatitis (MASH). Date palm fruit (Phoenix dactylifera L.) is widely recognized as a nutraceutical due to its rich content of bioactive phytochemical compounds.
OBJECTIVES: This study aimed to investigate the enhanced hepatoprotective effects of date palm fruit (pulp and ethanolic extract) in comparison with carvedilol, as well as their combined effects, in a model of dexamethasone-induced hepatotoxicit

Bacterial infections caused by the intracellular bacterium Francisella noatunensis pose a major health challenge in Atlantic cod (Gadus morhua), largely due to the lack of effective prophylactic and therapeutic measures. Here we evaluated the immune and physiological responses of cod to two candidate live attenuated vaccines (i.e. clpB and iglC) against F. noatunensis delivered intranasally. These experimental vaccines were constructed by a single knockout mutation of a virulence gene in F. noatunensis. Cod was vaccinated intranasally and reared for 45 days until infection with the wild-type s

INTRODUCTION: AL amyloidosis is a rare disorder characterized by the extracellular deposition of immunoglobulin light chain fibrils produced by a clonal plasma cell population. Hepatic involvement as the initial manifestation is rare, and forms associated with jaundice and cholestasis carry a poor prognosis.
CASE REPORT: We report the case of a 53-year-old man from Togo presenting with massive hepatomegaly, cholestatic jaundice, and weight loss. After a negative etiological workup, diffuse hepatic infiltration associated with a plasma cell dyscrasia raised suspicion of AL amyloidosis. Periphe

BACKGROUND: Post-bariatric hypoglycemia (PBH) is characterized by postprandial neuroglycopenia resulting from an exaggerated hyperinsulinemic response to nutrient intake in patients who have undergone Roux-en-Y gastric bypass, sleeve gastrectomy, or other upper gastrointestinal surgical procedures for weight loss. Fasting hypoglycemia is atypical.
OBJECTIVE: We present a cohort of patients who developed symptomatic fasting hypoglycemia in the setting of established PBH and discuss the possible pathophysiological mechanisms. We also examine the role of β-cell hyperfunction in PBH.

Glycogen storage diseases (GSDs) are a group of inherited metabolic disorders resulting from genetic deficiencies in enzymes involved in glycogen synthesis or breakdown, transport proteins, or proteins that regulate glycogen metabolism. Liver GSDs are further subdivided into subtypes based on the specific enzyme deficiency. This multicenter, cross-sectional, descriptive retrospective study, conducted across 24 centers, aimed to document the clinical and genotypic findings in patients with hepatic GSD. The study included 282 patients (152 males, 53.9%) from 278 unrelated families. At the time o

RATIONALE: Hypertrophic cardiomyopathy (HCM) is a common inherited cardiac disorder characterized by cardiac hypertrophy, fibrosis, arrhythmias, and sudden cardiac death (SCD). Although current therapies primarily target sarcomere dysfunction, the contribution of epigenetic dysregulation to HCM pathogenesis and its therapeutic potential remain poorly understood.
OBJECTIVE: To determine whether pharmacological inhibition of histone lysine demethylases (KDMs) with JIB-04 can prevent or reverse HCM progression and to identify the underlying epigenetic mechanisms.
METHODS AND RESULTS: We evaluat

Diabetes is a chronic disease characterized by hyperglycemia resulting from insulin deficiency or resistance to insulin action. Its onset may result from polygenic or monogenic mechanisms. The liver plays a central role in glucose metabolism and can be affected by various conditions. In poorly controlled type 1 diabetes, glycogenic liver disease presents with hepatomegaly and elevated liver enzymes, which is reversible with good metabolic control. Autoimmune hepatitis may coexist with type 1 diabetes due to shared immunological mechanisms or may develop during treatment with corticosteroids. I
