Detection of extracardiac abnormalities by early comprehensive abdominal ultrasound screening in neonates with congenital heart disease.

UNLABELLED: Congenital heart disease (CHD) is frequently associated with extracardiac abnormalities that can significantly affect prognosis and management. Although abdominal ultrasound is a readily available and noninvasive tool, detailed observational studies on comprehensive screening for neonates with CHD remain lacking. We aimed to evaluate the prevalence of significant abdominal abnormalities detected during early comprehensive ultrasound screening in neonates with CHD. This single-center, retrospective case series included neonates admitted to a tertiary neonatal intensive care unit (NI

Uncommon presentation of hepatic Langerhans cell histiocytosis in a young adult: a diagnostic challenge.

Langerhans cell histiocytosis (LCH) is a rare clonal myeloid neoplasm with varied and often organ-specific manifestations. Hepatic involvement in adult LCH is rare and frequently results in delayed diagnosis due to its non-specific clinical and biochemical features. We describe the case of a woman in her late twenties who presented with intermittent upper abdominal discomfort, fatigue and weight loss. Despite extensive imaging, serological workup and initial liver biopsy, no definitive diagnosis could be established. Ultimately, diagnostic laparoscopy and histopathological examination with imm

PFOS and PFOA exposure induces liver injury and sex-dependent immune effects in C57BL/6 mice.

Per- and polyfluoroalkyl substances (PFASs) are persistent environmental contaminants. Perfluorooctanoic acid (PFOA) and perfluorooctanesulfonic acid (PFOS), representing carboxylate and sulfonate subclasses, are among the most frequently detected PFASs in biomonitoring studies. We characterized the hepatotoxicity and immunotoxicity of PFOS and PFOA in male and female C57BL/6-Elite mice exposed via oral gavage for 28 (1.5 mg/kg/day) or 56 days (0.166-1.5 mg/kg/day). Both compounds caused pancreatic atrophy, hepatomegaly, elevated serum biomarkers of liver injury, and decreased serum triglyceri

Dual Monogenic Cystic Disease Case Report: Autosomal Dominant Polycystic Kidney Disease and Autosomal Dominant Polycystic Liver Disease.

Autosomal dominant polycystic kidney disease (ADPKD) and autosomal dominant polycystic liver disease (ADPLD) are inherited cystic conditions with overlapping features but distinct genetic causes and clinical courses. Here, we report a case of a 50-year-old woman with a clinical diagnosis of ADPKD, hypertension, preserved kidney function, significant abdominal distention consistent with hepatomegaly, and innumerable kidney and hepatic cysts. Family history was remarkable for ADPKD clinical diagnosis in the patient's mother, maternal grandmother, and the grandmother's siblings. Genetic testing w

Statins halt polycystic liver disease by reprogramming metabolism and normalizing mitochondrial bioenergetics in cystic cholangiocytes.

BACKGROUND AND AIM: Polycystic liver disease (PLD) is a hereditary disorder characterized by the progressive development and enlargement of intrahepatic biliary cysts, which can lead to significant morbidity. Liver transplantation remains the only curative treatment. Mutations in endoplasmic reticulum (ER)-related genes contribute to ER stress in cystic cholangiocytes, promoting disease progression. Given the functional interplay between ER and mitochondria, we investigated mitochondrial dynamics and metabolism in cystic cholangiocytes from both PLD patients and Pkhd1mut rats to identify novel

Case Report: Budd-Chiari-like syndrome in a cat with polycystic kidney and liver disease.

A 9-year-old spayed female Persian Chinchilla cat presented with progressive lethargy and acute right hindlimb pain and lameness. Diagnostic imaging revealed diffuse renal and hepatic cysts, resulting in marked hepatomegaly. Computed tomography (CT) further identified a localized narrowing of the intrahepatic caudal vena cava (CVC), likely due to extrinsic compression by the enlarged liver. Laboratory tests revealed moderate anemia, leukocytosis, hypoalbuminemia, and a hypercoagulable state with markedly elevated serum amyloid A levels. Based on these findings, the cat was diagnosed with polyc

Polycystic liver disease: Evidence-based management and critical gaps in surgical decision-making.

BACKGROUND: Polycystic liver disease (PLD) comprises a spectrum of inherited disorders characterised by progressive cyst development and highly variable clinical manifestations. A significant subset of patients develops debilitating symptoms and despite advances, a unified treatment algorithm is lacking. AIM: To review current evidence for medical, interventional and surgical management of PLD and identify gaps preventing an integrated, evidence-based care pathway. SUMMARY: Somatostatin analogues (SSAs) are the principal disease-modifying therapy, producing modest but reproducible liver-volu

[An autopsy case report of combined hepatocellular-cholangiocarcinoma in a young woman without chronic liver disease].

This case concerns a woman in her 20s with abdominal pain. Abdominal computed tomography revealed a large hepatic tumor with extensive infiltration of both liver lobes. She developed acute liver failure and was immediately admitted to our hospital. Her condition rapidly deteriorated, progressing to multiple organ failure, leaving insufficient time for comprehensive evaluation. She died 4 days after admission. Autopsy showed marked hepatomegaly due to tumor, with the liver weighing 3400g. Pathological diagnosis was combined hepatocellular-cholangiocarcinoma (cHCC-CCA), a rare primary liver canc

Protein phosphatase 6 protects hepatocytes from endoplasmic reticulum and oxidative stress by suppressing Akt/mTOR signaling.

Protein phosphatase 6 (PP6) is a serine/threonine phosphatase involved in diverse cellular processes. A recent study using a diet-induced metabolic dysfunction-associated steatohepatitis (MASH) model has shown that PP6 regulates hepatic pathology via mammalian target of rapamycin complex 1 (mTORC1) signaling; however, the role of PP6 in liver homeostasis under physiological conditions has not been systematically examined. Here, we demonstrate that hepatocyte-specific Ppp6c-deficient (PP6 HKO) mice develop spontaneous liver abnormalities under chow-fed conditions, including hepatomegaly, elevat

Giant polycystic liver disease with thoracic involvement: An anaesthesia and surgical challenge in liver transplantation.

Polycystic liver disease is a rare inherited disorder that is usually associated with autosomal dominant polycystic kidney disease. Advanced forms of the disease can cause massive hepatomegaly, chest compression, and severe respiratory compromise. We present the case of a 46-year-old woman with hepatorenal polycystic disease who developed progressive dyspnoea, early satiety, and abdominal distension despite treatment with lanreotide. The CT scan revealed a liver measuring 14,200 cc, partial lung collapse, and mediastinal displacement. Given the risk of thoracic compression, an orthotopic liver

Slippery dopamine-fluoropolymer hybrid surface for improving biliary stent longevity.

Biliary obstruction leads to bile retention and triggers a cascade of pathological events. Bile accumulation induces cholestasis and inflammation, progressing to hepatocellular injury, fibrosis, and ultimately liver failure. To restore bile flow, biliary stents are a necessary option due to their immediate patency. However, their high susceptibility to foreign body reaction (FBR) associated fibrosis, biofilm formation, and biliary sludge accumulation leads to frequent occlusion. To address this limitation, we developed the Enhanced Longevity by anti-fouling Functional coating for Stent (ELFS),

Arteriopathies: Too big to be true.

Arteriopathies in early childhood present significant diagnostic challenges due to phenotypic overlap between Marfan syndrome, Loeys-Dietz syndrome, Ehlers-Danlos syndrome, and hereditary cutis laxa. We report a 2-year-old male, born of consanguineous parentage, evaluated for persistent cough and hypertension. Clinical features included coarse facies, pectus excavatum, and mild hepatomegaly. Echocardiography revealed a massively dilated ascending aorta (56 mm), aortic annular dilatation, moderate aortic insufficiency, and severe post-subclavian coarctation with marked post-stenotic dilatation.

Spontaneous remission of congenital acute megakaryoblastic leukemia in a neonate with down syndrome.

OBJECTIVES: Congenital acute megakaryoblastic leukemia (AMKL) is an extremely rare and aggressive malignancy with poor prognosis. CASE PRESENTATION: We report a female neonate with Down syndrome (DS) who presented with respiratory distress and hepatomegaly at birth. Laboratory evaluation revealed hyperleukocytosis (253.45 × 109/L) and bone marrow studies confirmed AMKL with trisomy 21. Remarkably, spontaneous hematologic remission occurred within 2 months without chemotherapy. CONCLUSIONS: This case highlights the unique clinical course of DS-associated AMKL and underscores the potential for

Metabolic and circadian bioenergetic alterations following perfluorooctanoic acid exposure and high-fat diet in female C57BL/6 mice.

Exposure to per- and polyfluoroalkyl substances (PFAS) elicits changes in various metabolic responses but few studies have evaluated whole-body metabolic changes. This study compared whole-body bioenergetics in adult female C57BL/6 mice orally dosed with 0 or 7.5 mg/kg perfluorooctanoic acid (PFOA) or fed a 60% high fat diet (HFD) for 24 h or 15 d. After 24 h, HFD mice showed increased oxygen consumption (VO2) and reduced active-phase respiratory exchange ratio (RER), indicating early lipid utilization. After 15 d, PFOA-exposed mice exhibited decreased resting RER and suppressed active VO2, su

Transaldolase deficiency - natural disease course towards adulthood.

Transaldolase deficiency is a rare metabolic disease caused by pathogenic variants in the TALDO1 gene. Transaldolase plays an important role in the ribose-5-phosphate production, maintaining the NADPH-dependent lipid biosynthesis and cellular redox homeostasis. A small number of patients, predominantly children, have been reported, with a wide range of phenotypic presentations, including liver and kidney disease, involvement of the hematopoietic and endocrine systems, as well as possible early death. We aim to provide further insight into the clinical progression of transaldolase deficiency in

ICMT deficiency ameliorates weight loss and mortality, but not tumor formation in a mouse model of liver cancer.

Liver cancer often arises in the setting of chronic liver disease and remains a major global health challenge with limited effective treatments. Isoprenylcysteine Carboxyl Methyltransferase (ICMT) is an enzyme that catalyzes the methylation of prenylated proteins containing a CAAX motif, such as RAS, a modification thought to be essential for their function. Given its role in oncogenic signaling, ICMT has emerged as a potential therapeutic target in cancer. Notably, ICMT is overexpressed in human hepatocellular carcinoma (HCC), and previous studies have shown that Icmt deletion impairs BrafV60
