BACKGROUND: Intravenous immunoglobulin (IVIg) resistance affects 15-25% of children with Kawasaki disease (KD) and increases the risk of coronary artery abnormalities (CAA). The Kobayashi score has shown good predictive value in Japanese cohorts but limited accuracy in non-Asian populations. The Kawanet model was proposed as an alternative with improved applicability in non-Asian settings.
METHODS: A retrospective-prospective, observational, multicentre cohort study including children fulfilling the American Heart Association criteria for KD was conducted. The availability of all clinical and

Nontargeted metagenomic surveillance of the poultry enteric virome reveals underrecognized threats to poultry health and productivity in intensive production systems. In South Asia, avian rotavirus A (AvRV-A) and avian orthoreovirus (ARV) are frequently detected in broilers by conventional diagnostics, whereas chicken megrivirus genotype C (ChMeV-C) is often identified through metagenomic surveillance. Often present in both clinical disease and coinfections, these viruses may impair gut function, immune responses, and growth performance, yet their genomic diversity and evolutionary dynamics in

BACKGROUND: Multisystem inflammatory syndrome in children (MIS-C) remains one of the most complex post-infectious conditions associated with SARS-CoV-2. In Ukraine, where the healthcare system operates under uneven resource availability due to the ongoing conflict, diagnostic and therapeutic decisions often require simplified, adaptable tools for early risk assessment. This study aims to develop MIS-C severity scoring systems based on clinical and laboratory predictors and differentiate them according to healthcare facilities resource availability.
METHODS: This retrospective cohort study inc

Hepatic hemangiomas are uncommon vascular tumors in the neonatal period and are rarely reported in extremely preterm infants. Cystic variants are exceptionally rare and may exhibit atypical imaging features, making early diagnosis challenging. Spontaneous rupture with massive hemorrhage represents a life-threatening complication. We report the case of a male neonate born at 27 weeks of gestation with a birth weight of 900 g who was initially admitted to the neonatal intensive care unit for respiratory distress syndrome. During hospitalization, the infant developed recurrent severe anemia assoc

Duck Hepatitis A Virus Type 1 (DHAV-1) is a major pathogen in ducklings, characterized by severe hepatomegaly and punctate hepatic hemorrhage. In this study, we investigated host gene expression dynamics in specific-pathogen-free (SPF) ducklings infected with the DHAV-1 isolate HA5 using high-throughput RNA sequencing (RNA-seq). We performed comprehensive transcriptomic analyses, integrating Clusters of Orthologous Groups (COG) classification, Gene Ontology (GO) annotation, and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment. By combining these data with viral replication kin

We previously demonstrated that prenatal choline supplementation (CS) mitigates metabolic abnormalities in mouse offspring exposed to maternal obesity. In this preliminary study we examined whether prenatal and postnatal CS can protect against the development of metabolic dysfunction-associated steatotic liver disease (MASLD) in offspring exposed to an obesogenic environment. C57BL/6 J mice were exposed to a maternal high-fat (HF) diet in utero and a postnatal Western Diet (WD) to induce MASLD. Choline was supplemented before and during gestation and lactation, after weaning, or throughout the

Pulmonary fibrosis (PF) is a chronic, progressive, and often fatal interstitial lung disease characterized by interstitial remodeling, fibroblastic foci formation, and excessive extracellular matrix (ECM) deposition. Current therapeutic options remain limited and are associated with unfavorable prognoses. Growing evidence indicates that dysregulated coagulation and fibrinolytic pathways are intimately involved in its pathology, suggesting that modulating this axis may offer novel therapeutic opportunities. In this study, we characterize Angio-3, a plasminogen kringle 3-derived decapeptide, as

BACKGROUND: Pediatric fulminant myocarditis (FM) is characterized by a critical and rapidly progressive course. Current diagnostic approaches primarily rely on cardiac function and myocardial injury markers, which fail to comprehensively assess the associated systemic inflammation and immune dysregulation. The hemoglobin, albumin, lymphocyte, and platelet (HALP) score, a composite index integrating hemoglobin (Hb), albumin (Alb), lymphocyte count (Lym), and platelet count (Plt), offers a new perspective for evaluating the severity of pediatric fulminant myocarditis. OBJECTIVE: To evaluate the

Transient infantile liver failure due to tRNA 5-methylaminomethyl-2-thiouridylate methyltransferase (TRMU) gene mutation is a rare mitochondrial disease (MD) that typically presents within the first few months of life. We present the case of a 50-day-old female infant who was admitted with jaundice, hepatomegaly, lactic acidosis, and signs of liver dysfunction. Extensive metabolic and infectious investigations revealed a homozygous TRMU gene mutation and a high cytomegalovirus (CMV) viral load. The patient was treated with intravenous ganciclovir, supportive liver management, and metabolic cor

Hepatomegaly with hepatic cysts constitutes a common extrarenal manifestation of autosomal dominant polycystic kidney disease (ADPKD), often resulting in mass effects and occasionally necessitating cyst fenestration to alleviate abdominal distension. Nonetheless, persistent cyst fluid leakage as a postoperative complication frequently leads to refractory ascites, for which treatment options are limited. In this report, we present a 62-year-old female patient with ADPKD undergoing maintenance hemodialysis. She received bilateral nephrectomy and hepatic cyst fenestration to mitigate abdominal di

Yes-associated protein (YAP) is a critical factor of the Hippo pathway, which plays a key role in regulating organ size. Pregnane X receptor (PXR), constitutive androstane receptor (CAR), and peroxisome proliferator-activated receptor α (PPARα) are key members of the nuclear receptor superfamily, known for mediating diverse physiological and biological processes. Previous studies have demonstrated that activation of PXR, CAR, or PPARα promotes hepatomegaly and liver regeneration via YAP signaling. YAP has been reported to undergo phase separation under hyperosmotic stress, leading to enhanced

BACKGROUND AND OBJECTIVES: Mutations in the GPD1 gene, which encodes glycerol-3-phosphate dehydrogenase 1 (GPD1), are a rare cause of monogenic hypertriglyceridemia (HTG) during childhood. This study is aimed at providing a detailed analysis of the clinical, laboratory, and molecular characteristics of all patients affected by biallelic GPD1 gene variants, including three cases from our center.
METHODS: A literature search was conducted across the English MEDLINE, PubMed, and Embase databases from 1966 to 2023 for the studies that reported on GPD1 deficiency with confirmed GPD1 gene variants

Cystinosis is a genetic lysosomal storage disease with an autosomal recessive inheritance pattern. The mutation causing this disease occurs in the CTNS gene, which is located on chromosome 17 and codes for the lysosomal cystine transporter cystinosin. Cystinosis is characterized by intracellular accumulation of cystine in various organs and tissues, which may lead to impairment of organ function. Kidney injury is characterized by a progressive decline in glomerular filtration rate, leading to end-stage kidney disease if not treated. Other manifestations of cystinosis include ocular abnormaliti

BACKGROUND: Thromboembolic complications remain a significant cause of morbidity following the Fontan procedure, driven by the unique haemodynamics of the Fontan circulation.
CASE SUMMARY: We present a case of a 14-year-old patient with single-ventricle physiology who underwent an extracardiac Fontan procedure. Post-operatively, he developed low cardiac output requiring extracorporeal membrane oxygenation (ECMO) support. Despite successful weaning from ECMO, hepatomegaly, pleural effusion, and ascites persisted. Further investigation revealed a significant thrombus within the Fontan conduit.

Background/Objectives: Gastrointestinal bleeding after metabolic bariatric surgery is a relatively rare adverse event, but it has significant morbidity potential. The clinical course can be rapidly unfavorable, requiring early recognition and prompt intervention. The management of this complication often involves different approaches, surgical or endoscopic, the use of which is influenced by the particularities of the postoperative anatomy and the timing of the bleeding. This study aimed to evaluate the incidence, clinical characteristics, and factors associated with postoperative bleeding fol
