When Fruit Turns Harmful: Late Diagnosis of Hereditary Fructose Intolerance in a Pediatric Patient-A Case Report and Literature Review.

INTRODUCTION: Hereditary fructose intolerance is a rare but potentially severe and fatal disorder if it is not recognized promptly. It is caused by biallelic mutations in the ALDOB gene, which encodes the Aldolase B enzyme. Deficiency leads to intracellular accumulation of fructose-1-phosphate, causing secondary inhibition of gluconeogenesis and glucogenolysis and, consequently, hypoglycemia along with hepatic and renal dysfunction. The clinical presentation is variable and often nonspecific, making diagnosis challenging. CASE REPORT: A 12-year-old boy, born small for gestational age, with a

Carcinoid Heart Disease Presenting as Cardiogenic Shock: A Case Report.

Carcinoid heart disease (CHD) is a complication of carcinoid syndrome that results from chronic exposure of the endocardium to circulating vasoactive substances, particularly serotonin. It leads to progressive right-sided valvular dysfunction and right heart failure. Presentation as cardiogenic shock is uncommon and may delay recognition of the underlying disease. We present the case of a 44-year-old woman with no prior medical history who developed progressive dyspnea, ascites, and lower extremity edema over four months and was admitted with cardiogenic shock. Transthoracic echocardiography r

Identification of an unusual variant of Fanconi-Bickel syndrome presenting as proximal tubulopathy and short stature.

Fanconi-Bickel syndrome (FBS) is caused by biallelic pathogenic variants in the SLC2A2 gene, which encodes the glucose transporter protein 2 (GLUT2), leading to a rare disorder that affects glucose homeostasis. The exact mechanisms by which FBS leads to dysglycaemia are not clearly understood. The clinical manifestations are those related to dysglycaemia, proximal tubulopathy (glycosuria, galactosuria, aminoaciduria, proteinuria, phosphaturia), hepatomegaly, galactose intolerance, rickets and short stature. We report a teenage girl with persistent glycosuria and short stature. Developmental Environmental Chronotoxicity by Polystyrene Nanoplastics Exacerbates High-Fat Diet-Induced Pediatric MAFLD Through Circadian Disruption and Mitochondrial Dysfunction.

Environmental nanoplastic exposure is increasingly recognized as a contributor to metabolic dysfunction, yet its impact on susceptibility to diet-induced metabolic dysfunction-associated fatty liver disease (MAFLD) during development remains unclear. Here, we show that polystyrene nanoplastics (PS-NPs) exacerbate high-fat diet (HFD)-induced hepatic steatosis in an age-dependent manner. In juvenile, but not adult, mice, PS-NPs augmented HFD-driven dyslipidemia, hepatomegaly, and hepatic lipid accumulation without altering energy intake. Mechanistic analyses revealed disruption of the hepatic ci
