Spleen Volume Reduction and Transfusion Independence With Momelotinib Versus Ruxolitinib and Associated Overall Survival With Momelotinib in JAK Inhibitor-Naive Patients With Myelofibrosis and Anemia: Subgroup Analyses of SIMPLIFY-1.

INTRODUCTION: Improvements in splenomegaly, anemia, and overall survival (OS) are key considerations in the management of patients with myelofibrosis. In the phase III SIMPLIFY-1 trial (NCT01969838), momelotinib was noninferior to ruxolitinib for spleen volume reduction ≥ 35% (SVR35) and nominally superior for transfusion independence (TI) at week 24 in Janus kinase (JAK) inhibitor-naive patients. However, the relative impact of these endpoints on OS in anemic patients has not been described. MATERIALS AND METHODS: The present post hoc analysis evaluated week 24 SVR35, TI, and dual responses

Experimental evaluation of Salmonella Choleraesuis pathogenicity and porcine reproductive and respiratory syndrome virus synergy in weaned pigs.

BACKGROUND: Salmonella enterica serovar Choleraesuis (S. Choleraesuis) causes septicemic salmonellosis in pigs, which requires differential diagnosis from African swine fever (ASF). Porcine reproductive and respiratory syndrome virus (PRRSV) facilitates S. Choleraesuis dissemination. Herein, we determined the pathogenicity of S. Choleraesuis and the synergistic effect of PRRSV in septicemic salmonellosis. Furthermore, we compared the pathological characteristics of septicemic salmonellosis and ASF. RESULTS: Two experimental studies were conducted using weaning pigs (8- and 6-week-old pigs). In

Partial splenic embolization plus antitumor therapy for treating patients with hepatocellular carcinoma and splenomegaly: a case series study.

BACKGROUND: Hepatocellular carcinoma (HCC) is a significant global health challenge and is frequently complicated by splenomegaly and consequent thrombocytopenia, which adversely impact survival outcomes. Although partial splenic embolization (PSE) combined with transarterial chemoembolization (TACE) has demonstrated clinical benefits, its integration with contemporary systemic therapies, including tyrosine kinase inhibitors (TKIs) and immune checkpoint inhibitors (ICIs), remains largely unexplored. This case series aims to present the platelet elevation and survival outcomes in HCC patients w

Predictors of ineffective platelet transfusion in surgical ICU: A retrospective study.

Platelet transfusion is commonly utilized in the surgical intensive care unit (ICU) to provide hemostatic support for patients with severe thrombocytopenia or bleeding. Ineffective platelet transfusion is described as either the expected platelet count increase or insufficient hemostasis following transfusion. This study sought to identify risk factors and assess their predictive value in improving patient outcomes. This was an observational, retrospective study. Clinical information was gathered from 302 platelet transfusion episodes that occurred in the surgical ICU at the First Affiliated H

Case Report: Renal hemangiosarcoma in a free-ranging red fox (Vulpesvulpes).

This report describes the first documented case of primary renal hemangiosarcoma in a male red fox (Vulpes vulpes Linnaeus, 1758) admitted to the Wildlife Rescue Center "Federico II"-Veterinary Hospital of the Local Health Authority (ASL) Napoli 1 Centro in Naples, Italy. The fox, originating from a sparsely urbanized area, presented with progressive debilitation. Diagnostic imaging (ultrasound, radiography, CT) identified a mass in the right kidney. Citology suggested a malignant neoplasm, and histopathology confirmed hemangiosarcoma with multiple metastases to abdominal organs and to the hea

T-prolymphocytic leukemia with three distinct immunophenotypic subsets.

T-prolymphocytic leukemia (T-PLL) is a rare, aggressive mature T-cell neoplasm usually affecting older adults, often presenting with marked leukocytosis and splenomegaly. It is characteristically associated with recurrent chromosome 14 rearrangements that drive overexpression of TCL1 family oncogenes. Neoplastic cells generally show a mature T-cell immunophenotype with preserved pan-T cell markers, most commonly CD4⁺ with or without CD8 expression. Although T-PLL can harbor substantial genetic complexity that may lead to subclonal evolution, overt phenotypic diversification into distinctly sep

Malignant ovarian granulosa cell tumour of corpus luteum origin in a California sea lion (Zalophuscalifornianus).

A 19-year-old female California sea lion (Zalophus californianus) in managed care was euthanized following a 3-week history of hyporexia, diarrhoea, lethargy, abdominal discomfort, dyspnoea and tremors. Ante-mortem ultrasound confirmed splenomegaly with multifocal infiltrates and splenic aspirates revealed malignant cells of undetermined origin. Necropsy confirmed a malignant trabecular to diffuse granulosa cell tumour (GCT) that had metastasized to the liver, spleen, kidneys, adipose tissue, pancreas, adrenal glands, lungs, lymph nodes, brain and bone marrow. The GCT was arising within a corp

Lactate-mediated macrophage polarization promotes splenomegaly in acute erythroleukemia.

Acute erythroleukemia (AEL) is a rare and highly aggressive subtype of acute myeloid leukemia (AML) that is often accompanied by splenomegaly in some patients. Using the Friend murine leukemia virus clone 57 (F-MuLV clone 57) mouse model, we observed lactate accumulation in the spleens of mice with late-stage disease. Proteomic profiling indicated dysregulation of the glycolysis/gluconeogenesis pathway and aberrant activity of its key enzymes. In vitro, lactate alone directly induced macrophage polarization to the pro-inflammatory M1 phenotype. This lactate-rich milieu reprograms macrophage fu

Development of a Bedside Scoring Tool to Screen for Biliary Atresia in Infants with Neonatal and Infantile Cholestasis in a Resource-limited Setting: an Observational Study.

OBJECTIVE: To develop an easy-to-use bedside scoring system to help physicians distinguish extrahepatic biliary atresia (EHBA) from other causes of neonatal and infantile (NIC) cholestasis. METHODS: Medical records of all infants between 0 and 12 months of age diagnosed as NIC between January 2016 and December 2024 were reviewed. A diagnosis of EHBA was made based on histopathology findings, per-operative cholangiogram, and/ or intraoperative findings. Using logistic regression analysis, candidate variables that predicted biliary atresia were identified, and a scoring tool was developed. RES

Assessment of flonoltinib maleate versus ruxolitinib phosphate in intermediate- to high-risk myelofibrosis (FMF-02): study protocol for a multicenter, randomized, open-label phase IIB trial.

BACKGROUND: Myelofibrosis (MF) is a chronic myeloproliferative neoplasm characterized by progressive bone marrow fibrosis, splenomegaly, debilitating constitutional symptoms, and cytopenias. Despite the benefits of ruxolitinib in controlling symptoms and spleen volume, challenges such as myelosuppression and limited impact on underlying fibrosis persist, particularly in cytopenic patients. Flonoltinib maleate (FM), a novel JAK2/FLT3/CDK6 inhibitor, shows preliminary potential in improving hematologic parameters and reducing fibrosis. OBJECTIVES: To evaluate the efficacy of low-/high-dose FM c

Combination treatments for myelofibrosis in JAK inhibitor-naive patients: evidence, rationale, and clinical direction.

INTRODUCTION: Myelofibrosis (MF) is a myeloproliferative neoplasm (MPN) characterized by splenomegaly, constitutional symptoms, cytopenias, bone marrow fibrosis. Frontline therapy relies upon Janus kinase (JAK) inhibition improving symptoms and spleen size, though limited durability and disease modification. Prompting interest in combination strategies. AREAS COVERED: This review focuses on combination therapies in patients with MF who are naive to JAK inhibitors. We summarize the available clinical evidence from phase 2 and particularly 3 studies that evaluate combinations of JAK inhibitors

Genomic characterization and pathogenicity assessment of a novel strain of duck reovirus identified in China.

The global duck industry faces substantial challenges due to the emergence of novel duck reovirus (NDRV) infections, which are pathologically characterized by splenomegaly, hemorrhagic manifestations, and necrotic lesions in affected birds. Surviving ducklings often suffer from severe growth retardation. Here, we investigated an outbreak of hepatic necrosis in commercial ducklings from Shandong Province, China. After excluding other potential pathogens, an NDRV strain was isolated and designated SD416. This strain demonstrated infectivity in both chicken and duck embryos and induced syncytial

Neuroimaging Findings and Treatment Update for Neuroinflammation, Autoinflammation, Splenomegaly, and Anemia, a Novel Autoinflammatory Disorder Caused by Bi-Allelic Variants in IRAK4.

Neuroinflammation, autoinflammation, splenomegaly, and anemia (NASA) is a newly described autoinflammatory disorder caused by bi-allelic variants in the interleukin receptor-associated kinase 4 gene, IRAK4. Since our first description of the disorder in 2023, we now report that all 5 cases from 2 unrelated kindreds have developed severe neuroinflammation that is resistant to systemic immunosuppressive treatment. In these 5 individuals, we describe the chronology of clinical features and their correlation with neuroimaging findings, which were notable for a "waxing and waning" pattern of neuroi

Role of Chitinase 3-like-1 in Myelofibrosis via Fibroblast-Produced Extracellular Matrix Enhancement.

Cluster of differentiation 14+ monocytes produced spindle-shaped fibrocytes, similar to fibroblasts. Recent studies have reported that fibrocytes are crucial for the development and progression of primary myelofibrosis (MF); however, their functional role remains unclear. We compared monocytes and fibrocytes using RNA sequencing for gene expression profiles. We focused on chitinase 3-like-1 (CHI3L1), which causes inflammation, organ fibrosis, and extracellular tissue remodeling. We found higher CHI3L1 levels in patients with myeloproliferative neoplasm with MF than in those without MF. Further

Prim-O-glucosylcimifugin ameliorates DSS-induced ulcerative colitis via suppressing NLRP3 inflammasome activation.

BACKGROUND: Ulcerative colitis (UC) is a chronic and relapsing inflammatory bowel disease (IBD), that lacks specific therapeutic drugs. Prim-O-glucosylcimifugin (POG), a furanochromone glycoside derived from Saposhnikovia divaricata, exhibits anti-inflammatory, antioxidant, and anti-inflammasome activities. It has also been shown to inhibit the activation of the NLR family pyrin domain containing 3 (NLRP3) inflammasome. Given that ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by intestinal barrier dysfunction and NLRP3 activation, this study aims to investigate

A pharmacological profile of pacritinib for the treatment of myelofibrosis.

INTRODUCTION: Myelofibrosis (MF) is a chronic myeloproliferative neoplasm characterized by splenomegaly, constitutional symptoms, progressive cytopenias, and limited overall survival. While Janus kinase (JAK) inhibitors have transformed symptom- and spleen-directed management, many patients, particularly those with baseline thrombocytopenia, historically had limited therapeutic options due to treatment-related myelosuppression and exclusion from pivotal trials. Pacritinib was developed to address this unmet need through selective JAK2, ACVR1, and IRAK1 inhibition with relative sparing of JAK1.

A paradigm shift in the treatment of patients with polycythemia vera. The initial early use of recombinant interferon-alpha.

During the past 35 years, single-arm studies documented the efficacy and safety of recombinant interferon alpha (rIFNα) for treating polycythemia vera (PV). In some patients, 2-5 years of disease-modifying treatment resulted in symptom relief, regression of splenomegaly, normalization of abnormal blood counts and marrow morphology, and sustained JAK2V617F molecular remission. The PROUD-CONTI study showed the superiority of rIFNα compared to hydroxyurea (HU), which led to the European Medicines Agency approval in 2019 of ropeginterferon alpha-2b ("ropegIFN") for ELN-defined high-risk PV patient
