Mechanisms of resistance to Tmprss6 antisense oligonucleotides in a mouse model of β-thalassemia.

Non-transfusion dependent β-thalassemia (NTDT) is characterized by ineffective erythropoiesis, suppression of hepcidin, and iron overload, which is a major cause of morbidity and mortality. Hepatic protease Tmprss6 regulates hepcidin by modulating BMP/Smad signaling. Suppression of Tmprss6 in the mouse model of NTDT (Th3 mice), increased hepcidin and improved disease outcomes, but similar benefits have not been reported in human trials. We hypothesized that the resistance to the effects of anti-TMPRSS6 treatment in patients with NTDT may be caused by high erythroferrone (ERFE) concentrations a

PITPβ Drives JAK2 V617F-Mediated Myeloproliferative Neoplasms by Promoting PtdIns(3,4)P ₂ -Dependent AKT Hyperactivation.

JAK2 is a key regulator of cytokine-mediated proliferative signaling in hematopoietic stem and progenitor cells. Activating mutations, most commonly JAK2 V617F, trigger aberrant cytokine signaling driving the pathogenesis of myeloproliferative neoplasms (MPNs). Phosphatidylinositol transfer proteins (PITPs) facilitate phosphoinositide synthesis by delivering phosphatidylinositol to lipid kinases, though their roles in oncogenic signaling have remained poorly defined. Here we show that PITPβ is critical for the development of JAK2V617F-driven MPN in mice. Deleting Pitp β across the hematopoieti

Reciprocal regulation of TNF receptor 1-mediated signaling and inflammatory damages by MARCH2 and USP22.

The tumor necrosis factor (TNF) receptor 1 (TNF-R1) plays critical roles in inflammatory response and autoimmune diseases. The underlying mechanisms on posttranslational regulation of TNF-R1 and its functional significance remain enigmatic. In this study, we identified the deubiquitinase USP22 as a positive regulator of TNF-R1. USP22 is minimally associated with TNF-R1, which is markedly increased following TNF stimulation. USP22 deconjugates K27-linked polyubiquitination of TNF-R1 at K340, which reverses its proteasomal degradation. USP22 deficiency reduces TNF-triggered signaling and transcr

Emergence of multidrug-resistant and virulent Escherichia coli with APEC‑associated traits in broiler chickens from Ismailia, Egypt.

Colibacillosis in poultry is a significant disease that leads to substantial economic losses and poses a potential zoonotic threat through the emergence of antimicrobial‑resistant strains. This study aimed to investigate the prevalence, antimicrobial resistance patterns, phylogenetic distribution, and virulence and resistance gene profiles of Escherichia coli isolates from broiler chickens on two private farms in Ismailia Governorate, Egypt. During April–July 2024, cloacal swabs were collected from diseased broiler chickens showing diarrhea and respiratory symptoms. Post-mortem examination rev

NK-cell-derived exosomes exert antitumor potency via miR-140/XYLT1/HSPG2 axis.

Non-small cell lung cancer (NSCLC) remains a leading cause of global mortality, necessitating novel therapies. This study investigated the therapeutic role of natural killer cell-derived exosomes (NK-Exo), whose antitumor mechanisms are incompletely understood. Exosomes were isolated from interleukin (IL)-2-independent NK-92MI cells via differential ultracentrifugation and characterized by nanoparticle tracking, electron microscopy, and western blotting. They exhibited cup-shaped morphology (50–150 nm), expressed CD81/TSG101, and demonstrated selective cytotoxicity against tumor cells (A549, A

Successful Haplo-Hematopoietic Stem Cell Transplantation for Juvenile Myelomonocytic Leukemia in a Child With Underlying Thrombocytopenia-Absent Radius Syndrome: A Unique Case.

BACKGROUND: Thrombocytopenia-absent radius (TAR) syndrome is a rare congenital disorder characterized by bilateral radial aplasia with preserved thumbs and early-onset thrombocytopenia. While hematologic and skeletal abnormalities define the condition, its association with hematologic malignancies is extremely rare, with only a few reported cases of leukemia. Juvenile myelomonocytic leukemia (JMML) is an uncommon pediatric myelodysplastic/myeloproliferative neoplasm frequently linked to RAS pathway mutations. To our knowledge, JMML has not previously been reported in association with TAR syndr

Immune-imaging biomarkers: Prognostic value of splenic volume and circulating lymphocytes in upper tract urothelial carcinoma.

Splenomegaly has been linked to a poor prognosis in certain cancer types, possibly due to a weakened immune system. The present study evaluated the association between splenic volume (SV) and oncological outcomes in patients with upper tract urothelial carcinoma (UTUC) after radical nephroureterectomy (RNU). A total of 465 patients with non-metastatic UTUC who underwent RNU between 2009 and 2020, with available preoperative abdominal computed tomography scans and hemogram data collected within 1 month before the operation, were retrospectively analyzed. SV (cm3) was calculated using the cross-

Case Report: The diagnostic and therapeutic crossroads: when myelofibrosis transforms into mixed phenotype acute leukemia.

BACKGROUND: Myelofibrosis (MF) is a myeloproliferative neoplasm characterized by the proliferation of fibrous tissue in the bone marrow. Transformation to acute leukemia, typically acute myeloid leukemia, represents a terminal event in its natural history; however, transformation to mixed phenotype acute leukemia (MPAL) is exceedingly rare. The diagnosis and treatment of MPAL are particularly challenging, especially in elderly patients with underlying MF. The clinical characteristics, treatment response, and prognosis of such cases remain unclear and are rarely reported. CASE PRESENTATION: Th

Case Report: Chronic myeloid leukemia in a 13-year-old-a rare pediatric case of extreme hyperleukocytosis in chronic phase.

Pediatric Chronic Myeloid Leukemia (CML), a rare hematologic malignancy, accounts for only 2-3% of cases of leukemia in pediatric patients. The case presented here is of a previously healthy 13-year-old male, who came to the emergency department with increasing fatigue, abdominal distention, and discomfort. On examination, he was pale with significant splenomegaly. His investigation findings included marked leukocytosis (WBC > 600 × 109/L), severe anemia (Hb 5 g/dL), along with thrombocytosis (Platelets > 1,530 × 109/L). The diagnosis was made by peripheral smear and bone marrow biopsy, showin

A Clinical Prediction Model for Inadequate Hemoglobin Response After Red Blood Cell Transfusion.

BACKGROUND: Transfusion of one unit of packed red blood cells is traditionally expected to increase hemoglobin concentration by approximately 1 g/dL in adults. However, substantial variability in hemoglobin response is frequently observed in clinical practice. Smaller-than-expected increments may raise concerns regarding ongoing bleeding, hemodilution, or altered erythrocyte distribution. Understanding predictors of inadequate hemoglobin response may help clinicians interpret post-transfusion laboratory values and avoid unnecessary repeat transfusion. METHODS: We conducted a prospective obser

Sequential Analysis of Murine Myelofibrosis Models Using a Novel Deep Learning-Based Fibrosis Quantitative Method.

INTRODUCTION: Primary myelofibrosis (MF) is characterized by MF, splenomegaly, and extramedullary hematopoiesis. MF initially presents with reticular fibers (RFs) and progresses to increased collagen fiber deposition in the advanced stages. Although recent clinical trials have adopted MF improvement as an evaluation criterion, current diagnostic methods rely primarily on qualitative assessments based on the presence or absence of reticular and collagen fibers. Therefore, detecting subtle changes during the early stages of MF can be challenging. METHODS: We developed a novel deep learning-base

Hemoglobin equilibration kinetics after single-unit red blood cell transfusion in stable hospitalized adults: A prospective cohort study.

BACKGROUND: A single unit of red blood cells (RBCs) is commonly expected to increase hemoglobin by approximately 1 g/dL, yet short-term equilibration kinetics and modifying factors in stable hospitalized adults remain incompletely characterized. METHODS: In this prospective cohort study, 518 hemodynamically stable adults receiving a single unit of RBC transfusion were enrolled over 12 months. Patients with active bleeding, hemodynamic instability, large-volume intravenous fluids, or multiple transfusions were excluded. Complete blood count was measured pre-transfusion and at 12, 24, and 48 h

Clinical application of a modified platelet desialylation test for mechanistic characterization of platelet transfusion refractoriness.

BACKGROUND: Platelet transfusion refractoriness (PTR) is a major challenge in transfusion medicine and may result from both immune and non-immune mechanisms. Although alloantibodies are well-established contributors, Fc-independent pathways such as platelet desialylation have emerged as alternative mechanisms of clearance. STUDY DESIGN AND METHODS: In this prospective diagnostic study, 81 patients with suspected PTR were evaluated using an integrated approach combining the platelet immunofluorescence test (PIFT) to detect antibody-mediated refractoriness and a modified platelet desialylation
