Isolated central nervous system involvement in blastic plasmacytoid dendritic cell neoplasm: a rare case of relapse during systemic remission with diagnostic and therapeutic implications.

Blastic plasmacytoid dendritic cell neoplasm (BPDCN) is a rare, highly aggressive hematologic malignancy characterized by frequent involvement of the skin and bone marrow. Central nervous system (CNS) infiltration is uncommon and may be clinically occult, thereby posing diagnostic challenges. We report the case of a 74 year-old man who initially presented with progressively enlarging cutaneous masses on the trunk. Histopathological examination of skin biopsy demonstrated BPDCN with a characteristic immunophenotype, and subsequent bone marrow studies confirmed extensive marrow involvement. The

Micheliolide ameliorates colon cancer cachexia by modulating gut microbiota-immune signaling via Phocaeicola vulgatus enrichment.

BACKGROUND: Cancer cachexia profoundly impacts patient survival and quality of life. Current treatments fail to halt this trajectory, highlighting an urgent clinical need for host-directed therapies capable of uncoupling skeletal muscle wasting from tumor progression. This study investigated the therapeutic potential of micheliolide (MCL) across distinct tumor contexts. METHODS: We employed immunocompetent murine models of colon cancer (CT26) and lung cancer (LLC) cachexia, pseudo-germ-free (pseudo-GF) mice, murine C2C12 myotubes, and primary human skeletal muscle cells. We evaluated MCL's im

Targeted treatment of anemia in myelofibrosis and the impact of JAK inhibitors.

INTRODUCTION: Anemia is a hallmark feature of Myelofibrosis (MF), frequently present at diagnosis and almost universal with disease evolution. It is a major contributor to symptom burden, transfusion dependence, healthcare utilization, and adverse prognosis, and is incorporated into contemporary risk stratification models. AREAS COVERED: This review highlights current definitions of MF-associated anemia, including trial and prognostic thresholds, and outlines its incidence, clinical impact, and multifactorial pathophysiology. Therapeutic approaches reviewed include supportive options, differe

Splenic, Nonhairy, B-Cell Lymphoma Presenting as Fever of Unknown Origin.

Fever of unknown origin (FUO) is a common diagnostic dilemma faced by internists on a fairly regular basis. A wide variety of infectious, autoimmune, and neoplastic conditions can present with FUO. Splenic B-cell lymphoma/leukemia with prominent nucleolus (SBLPN), formerly known as hairy cell leukemia-variant, is a hematologic malignancy that often presents with massive splenomegaly and pancytopenia. Initial presentation of SBLPN as FUO is exceedingly rare. Here, we present the case of an elderly gentleman in his 80s who presented with fever, night sweats, weight loss, and recurrent falls. He

The Path Forward in MF: Small Molecules in the Limelight.

Myelofibrosis (MF) is a chronic myeloproliferative neoplasm characterized by progressive bone marrow fibrosis, extramedullary hematopoiesis (particularly symptomatic splenomegaly), constitutional symptoms, progressive cytopenias, and, in a subset of patients, leukemic transformation. The advent of the JAK1/2 inhibitor ruxolitinib has revolutionized the management of MF, substantially improving splenomegaly, symptom burden, and, in some settings, overall survival. However, a substantial percentage of patients fail to achieve sustained benefit, are intolerant, or become refractory; real-world an

A Rare Case of Acute Hemolysis After Rasburicase Infusion.

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is a genetic X-linked recessive disorder that results from mutations in the G6PD gene, which provides instructions for manufacturing the G6PD enzyme. G6PD deficiency primarily impacts red blood cells. This deficiency can result in hemolytic anemia (HA), in which rapid destruction of red blood cells occurs due to the vulnerability of erythrocytes to reactive oxygen species. It is most commonly found in males and affects 1 in 10 Black males in the United States. Persons with G6PD deficiency may exhibit symptoms such as pallor, jaundice, dark ur

Integration of peripheral blood lymphocyte count kinetics and bone marrow hematogones for stratifying B-cell recovery and clinical outcomes after cord blood transplantation: a retrospective study.

Immune reconstitution after allogeneic hematopoietic stem cell transplantation influences clinical outcomes. However, the optimal clinical laboratory evaluation parameters remain unclarified. This retrospective study analyzed first-time cord blood transplant (CBT; without anti-thymocyte globulin) recipients (n = 837; cohort) and unrelated bone marrow (BM) transplant recipients (n = 122; controls). A novel lymphocyte reconstitution (LR) classification, integrating peripheral blood lymphocyte count kinetics on post-CBT days 20, 30, and 50, and hematogones in the BM test confirming neutrophil eng

A new Plasmodium Marchiafava & Celli, 1885 (Apicomplexa: Haemosporida) species in Cory's Shearwater (Calonectris borealis) [Cory]) (Aves: Procellariiformes) stranded in a coastal area in Brazil.

Cory's Shearwater (Calonectris borealis), a migratory seabird from the Procellariidae family, is widely distributed across the Atlantic Ocean. It breeds on islands of Azores, Madeira, Canary, and Berlengas, and winters in coastal areas of South America and Africa. This species' wide habitat range contributes to pathogen exchange across global regions. Plasmodium, a genus of apicomplexan parasites causing avian malaria, affects many wild bird species worldwide. Although rare in seabirds, Plasmodium infections can significantly impact these hosts. We identified and characterized a new Plasmodium

Dual-targeting phytochemicals Ergosterol and Quercetagetin implicate steroid metabolism-associated pathways in lung and liver cancer models.

INTRODUCTION: Hormone-associated cancers use steroid metabolism to grow and avoid the immune system, while traditional treatments have to deal with drug resistance and systemic toxicity. METHODS: This study is mainly divided into two parts. First, screening natural active ingredients for anti-lung cancer and anti-liver cancer and their network pharmacology research. Second, target identification and clinical drug design of potential medicinal components focusing on hormone metabolic pathways. RESULTS AND DISCUSSION: We found that ergosterol and quercetagetin are dual-targeting phytochemicals

Sophoraflavanone G alleviates ulcerative colitis in mice by reshaping the gut microbiome and restoring the Th17/Treg balance through the PI3K/AKT/NF-κB signaling pathway.

Ulcerative colitis (UC) is characterized by diarrhea, bloody stool and a feeling of incomplete defecation. Sophoraflavanone G (SFG) is one of the main chemical components of Sophora flavescens Ait and possesses various pharmacological activities. This study aims to investigate the therapeutic efficacy of naturally derived SFG in alleviating dextran sulfate sodium (DSS)-induced UC and the underlying mechanisms. A mouse model of DSS-induced UC was created and treated with SFG, antibiotics (ABX) cocktail, or the AKT agonist SC79. Clinical symptoms and signs of UC were assessed. Expression levels

An imported case of human granulocytic anaplasmosis presenting with altered mental status: A case report.

Human granulocytic anaplasmosis (HGA) is a tick-borne disease endemic to the northeastern United States but remains extremely rare in Japan. We report an imported case of HGA in a 77-year-old man who had recently traveled from the United States. He presented with a 5-day history of fever, lethargy, and loss of appetite, followed by altered consciousness. Laboratory findings revealed thrombocytopenia, elevated transaminases, and increased creatine kinase levels. Computed tomography was unremarkable except for mild splenomegaly. Considering his recent stay in Bemidji, Minnesota-a region with a h

Risk Factors and Predictive Model of Poor Graft Function After Allogeneic Hematopoietic Stem Cell Transplantation.

Poor graft function (PGF) is a serious and life-threatening complication following allogeneic hematopoietic stem cell transplantation (allo-HSCT). Current therapeutic approaches show suboptimal clinical outcomes, with substantial variability in treatment efficacy across studies, which highlights the critical need for timely identification of PGF and appropriate preventive strategies to improve prognosis. To identify PGF risk factors and establish a nomogram-based predictive model of PGF, this real-world study analyzed 795 hematological malignancies patients underwent allo-HSCT. We identified l

Patient selection, practice patterns, and perioperative outcomes for robotic splenectomy: a contemporary ACS-NSQIP analysis.

BACKGROUND: Robot-assisted surgery has increasingly been adopted across multiple surgical specialties, demonstrating benefits such as enhanced precision and visualization, and potentially improved outcomes. However, evidence for its use in less common operations, including splenectomy, remains limited. To better understand its use and outcomes, we examined practice patterns and perioperative results of robotic splenectomy in a large, contemporary national cohort. METHODS: The American College of Surgeons - National Surgical Quality Improvement Program database was queried for adult patients w

Incidental Diffuse Splenic Polyangiomatosis With Long-Term Radiologic Stability: A Case Report.

Splenic polyangiomatosis is an exceptionally rare benign vascular proliferation characterized by multiple vascular channels that may replace large portions of the splenic parenchyma. Most reported cases are discovered incidentally, as the condition is often asymptomatic and identified during imaging performed for unrelated reasons. We report the case of a 46-year-old asymptomatic woman in whom splenomegaly was detected during a routine clinical evaluation. Cross-sectional imaging with CT and MRI revealed marked splenomegaly with innumerable solid and cystic nodular lesions replacing more than

Influenza as a Less Commonly Recognized Cause of Hemophagocytic Lymphohistiocytosis: A Systematic Review of Case Reports and Case Series.

Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyper-inflammatory condition that can be triggered by viral infections. However, influenza is not commonly recognized as a cause of HLH, and there is no comprehensive synthesis of influenza-associated HLH in the literature to guide clinicians. We conducted a systematic search of Pubmed and Embase to identify case reports and case series on influenza-associated HLH, and included 29 articles involving 47 patients. Their age ranged from 2 months to 72 years. 67% were males. Influenza A accounted for 91.3% of the cases, predominantly H
