Viable appendico-ileal knotting resulting in acute small bowel obstruction: a case report from Ethiopia.

Appendico-ileal knotting is a rare cause of small bowel obstruction, usually complicated by strangulation and resection of bowel. A 32-year-old woman presented with 24-hour abdominal pain, bilious vomiting, distension, and obstipation. Visible peristalsis was noted on physical examination; a radiograph showed multiple air-fluid levels and a dilated small bowel. Emergency laparotomy revealed an inflamed appendix, which formed a constricting ring around a loop of 15 cm of distal ileum, which was viable. The knot was released with retrograde appendectomy without bowel resection. A follow-up 1 yea

Uncommon cause of large bowel obstruction in a pediatric patient: A case of phytobezoar induced by peas.

Phytobezoars, which are indigestible plant materials located in the gastrointestinal tract, are an uncommon cause of intestinal blockage, especially in pediatric patients. This report details a case involving a 2-year-old girl who experienced a large bowel obstruction caused by phytobezoars resulting from excessive consumption of peas who exhibited symptoms of abdominal swelling, an inability to pass stool and gas, and vomiting, abdominal distension with visible peristalsis with a plain abdominal X-ray finding of multiple air-fluid levels and enlarged bowel loops. Surgical intervention was ini

Obturator hernia: a persisting clinical diagnostic challenge - a case report.

INTRODUCTION: The presentation of a strangulated obturator hernia is rare, with it accounting for less than 0.04% of all hernias. Delay in presentation and diagnosis results in complications like bowel ischemia, necrosis, perforation, and peritonitis, thereby increasing morbidity and mortality. CASE PRESENTATION: The authors report the case of an 85-year-old multiparous woman who presented with a 3-day history of abdominal pain and vomiting. Upon examination, she exhibited hypotension, altered sensorium, and a distended abdomen with visible peristalsis. An abdominal pelvic computed tomography

Adult-Onset Annular Pancreas: When to Intervene?

Annular pancreas in adults is a rare congenital abnormality, often detected after the onset of complications or incidentally during autopsy. Diagnosis in adults is challenging due to the similarity of symptoms with other conditions. We report a case of a 55-year-old female who presented with a six-month history of intermittent colicky pain in the epigastric region, radiating to the back, accompanied by periodic nonbilious vomiting. Abdominal examination revealed mild tympany and succussion splash in the epigastric region, with no organomegaly, lumps, or visible peristalsis. The patient was tre

Epithelioid Mesothelioma of Peritoneum Masquerading as Peritoneal Carcinomatosis.

INTRODUCTION: Mesothelioma is an insidious neoplasm that develops from mesothelial cells. About 80% of mesotheliomas originate in the pleural cavity. Other sites where it has been reported are the peritoneal cavity, tunica vaginalis, and the pericardium. CASE PRESENTATION: A 45-year-old female complained of abdominal distention and pain for three months. There was a significant weight loss of approximately 15 kg in the past three months, and there was no family history of any malignancy, tuberculosis, substance abuse, or asbestosis exposure. Physical examination revealed signs of muscle wasti

SPONTANEOUS TRANSVAGINAL INTESTINAL EVISCERATION IN AN ELDERLY WOMAN.

INTRODUCTION: Transvaginal intestinal evisceration is a rare surgical emergency that is associated with morbidity and mortality. Only a few cases of transvaginal evisceration have so far been described. The predisposing risk factors associated with this clinical condition are multifactorial. CASE PRESENTATION: We report a case of an 85-year-old female that presented with spontaneous small bowel evisceration through the vagina. The loops of the small bowel appeared edematous and thickened but there was demonstrable visible peristalsis. She had no previous laparotomy or vaginal surgery. An emer

Left sided gallbladder and liver without situs inversus and recurrent sigmoid volvulus after alleged previous total sigmoid resection; a case report and review of literatures.

INTRODUCTION AND IMPORTANCE: Acute abdomen is an emergency condition which necessitates urgent management. Obstruction accounts to the majority of causes of acute abdomen. As acute abdomen can lead to a dramatic and fatal complication, rapid diagnosis and management are utterly important. Sigmoid volvulus is the commonest cause of large bowel obstruction in Africa and particularly in Ethiopia. Left side gallbladder and liver without situs inversus is a very rare clinical finding with few reports in the history of literatures. CASE PRESENTATION: A 53 years old male patient presented to our eme

Ileocolic Intussusception in Infancy is not Always Idiopathic - Adenomyoma as the Leading Point.

BACKGROUND Intussusception is a frequent abdominal emergency in infancy, requiring immediate diagnosis and therapeutic intervention. In approximately 90% of cases, intussusception seems to be idiopathic. There has been a reported association of intussusception with lymphoid hyperplasia of Peyer patches in the terminal ileum, possibly acting as the triggering factor. Clinical presentation varies substantially, while the etiology seems idiopathic in most reported cases. CASE REPORT This case describes a previously healthy 2-month-old girl who presented with an episode of non-bilious vomiting and

Primary Gastrointestinal Amyloidosis: An Unusual Cause of Acute Intestinal Pseudo-Obstruction.

Amyloidosis of the gastrointestinal tract is an uncommon disorder characterized by the extracellular deposition of an abnormal fibrillar protein. It is rarely proven by biopsy. Amyloid deposition interferes with organ structure and its function. We report a case of a 64-year-old male who presented with severe colicky pain, unable to pass feces, and progressive abdominal distension for 2 days. Physical examination revealed marked abdominal distension, visible peristalsis, high-pitched hyperactive bowel sounds, and generalized tenderness. Plain abdominal radiograph showed markedly diffuse dispro

Transient versus surgically managed small bowel intussusception in children: Role of ultrasound.

BACKGROUND: To evaluate and compare the ultrasound (US) features of transient small bowel intussusception (SBI) with those which required surgical management. MATERIALS AND METHODS: US features of 26 children with 32 intussusceptions from January 2014 to August 2014 were recorded and compared with follow-up imaging or surgical findings. RESULTS: Transient SBI when compared to surgically managed intussusception has shorter length of intussusception (mean 2.25 cm, range 1.8-4.5 cm vs. mean 5.6 cm, range, 2.3-7.8 cm), smaller transverse diameter (mean, 1.2 cm, range 0.8-2.3 cm vs. mean, 3.3 cm,

Function of mechanically lengthened jejunum after restoration into continuity.

PURPOSE: Distraction enterogenesis is a potential treatment for patients with short bowel syndrome. We previously demonstrated successful lengthening of jejunum using a degradable spring device in rats. Absorptive function of the lengthened jejunum after restoration into intestinal continuity needs to be determined. METHODS: Encapsulated polycaprolactone springs were placed into isolated jejunal segments in rats for four weeks. Lengthened segments of jejunum were subsequently restored into intestinal continuity. Absorption studies were performed by placing a mixture of a non-absorbable substr

Gastric outlet obstruction with an elevated serum pancreatic lipase secondary to an infraumbilical hernia.

We report the case of a 68-year-old woman who presented with symptoms and signs of gastric outlet obstruction with a history of a ventral hernia. Clinical examination revealed a large ventral hernia with visible peristalsis of the herniated viscera. Initial serum biochemistry revealed a markedly elevated lipase level and deranged renal function. Computed tomography demonstrated an infraumbilical hernia with herniation of the stomach through the ventral defect and distortion of the pancreatic anatomy. The hernia was reduced operatively and repaired, leading to an uneventful recovery.
