Outcomes of Screening Colonoscopy in Older Adults: A Large Propensity-Matched Real-World Cohort Study.

BACKGROUND: The benefits and harms of screening colonoscopy after age 75 remain uncertain. Guidelines endorse individualized decisions given competing risks, but comparative data are limited. METHODS: In the TriNetX US Collaborative Network, adults aged 75-90 years with a colorectal cancer screening encounter were matched 1:1 by colonoscopy within 3 months. Outcomes were assessed at 30 days, 1 year, and 5 years. Sensitivity analyses excluded pre-index anemia and, separately, fecal abnormalities and alarm symptoms; subsequent colonoscopy was counted through 5 years. RESULTS: Matched cohorts c

Orlistat-Associated Gastrointestinal, Hepatobiliary, Pancreatic, and Anorectal Safety Signals: A FAERS Disproportionality and Regulatory Label Concordance Study.

Background/Objectives: Orlistat is a gastrointestinal lipase inhibitor used for weight management. Although its established safety profile is largely characterised by fat malabsorption-related gastrointestinal events, post-marketing reports suggest a broader spectrum of adverse events. Given the increasing intersection between obesity pharmacotherapy and the management of non-alcoholic fatty liver disease/metabolic dysfunction-associated steatotic liver disease (NAFLD/MASLD), this study evaluated gastrointestinal, hepatobiliary, pancreatic, and anorectal adverse-event reporting signals associa

High-fidelity simulation as a translational and educational platform for crisis ventilation: quantifying and mitigating tidal volume maldistribution during shared mechanical ventilation.

BACKGROUND: Shared mechanical ventilation has been proposed during ventilator shortages, but unequal tidal volume delivery remains a major limitation when lung compliance differs. Professional societies have cautioned against this practice, yet the intervention has never been validated clinically, and ethical constraints preclude direct patient testing. Translational simulation-simulation applied beyond education to probe clinical systems, test crisis-driven innovations, and generate actionable safety data-offers a methodological pathway for evaluating interventions that cannot be studied in v

Metastatic breast cancer diagnosed on upper gastrointestinal endoscopy.

A woman in her late 60s was referred with a positive faecal immunochemical test and change in bowel habit; a suspicion of gastrointestinal (GI) cancer led to straight-to-test vetting for colonoscopy, which was unsuccessful due to a nontraversable, fixed sigmoid colon, secondary to prior hysterectomy and adhesions.Radiological evaluation with CT scanning revealed an abnormally thickened stomach, with subsequent upper GI endoscopy and biopsy findings confirming the presence of invasive poorly differentiated carcinoma consistent with metastatic invasive lobular carcinoma originating from the brea

Does the Value of the Faecal Immunochemical Test (FIT) Result Matter, and Can This Be Used as a Triage Criterion for Symptomatic FIT-Positive Patients?

Background The Faecal Immunochemical Test (FIT) is a cornerstone in both colorectal cancer (CRC) screening programs and referral pathways for symptomatic patients. The test identifies haemoglobin in stool and, when present, is used as a referral indicator on the suspected CRC pathway. Since its introduction, the number of referrals with a positive FIT has risen, stretching diagnostic capabilities to the limit. This study examines whether the value of the result is significant and, if so, can be used as part of the triage process. Methods This retrospective cohort study analysed patients who un

Attribution of colorectal cancer symptoms to medications for pre-existing chronic conditions: a secondary analysis of a vignette study in England.

OBJECTIVE: To investigate the likelihood of attributing colorectal cancer (CRC) symptoms to medications for chronic conditions. METHODS: The online vignette survey included 1287 participants aged ≥50 years, with quota sampling to recruit sufficient participants with type 2 diabetes. Participants self-reported chronic conditions and answered questions on symptom attribution and help-seeking, after reading vignettes describing new-onset rectal bleeding or change in bowel habit. Using multivariable logistic regression, we analyzed the association between specific conditions and attributing new-o

Impact of Self-Reported Long-Term Mental Health Morbidity on Help-Seeking and Diagnostic Testing for Bowel-Related Cancer Symptoms: A Vignette Study.

OBJECTIVE: To investigate if pre-existing mental health morbidity (MHM) might influence help-seeking and willingness to undergo diagnostic investigations for potential colorectal cancer (CRC) symptoms. METHODS: An online vignette survey was completed by 1307 adults aged > 50 years recruited through Prolific, a UK panel provider. Participants self-reported any chronic physical or MHM. After having been presented with vignettes describing new onset symptoms (rectal bleeding or change in bowel habit), participants answered questions on symptom attribution and attitudes to investigations. Validation of a diagnostic prediction tool for colorectal cancer: a case-control replication study.

BACKGROUND: Early detection of colorectal cancer (CRC) is crucial for survival. Primary care, the first point of contact in most cases, needs supportive risk assessment tools. We aimed to replicate the Swedish Colorectal Cancer Risk Assessment Tool (SCCRAT) for non-metastatic CRC in primary care and examine if risk factor patterns depend on sex and age. METHODS: 2,920 adults diagnosed with non-metastatic CRC during the years 2015-2019 after having visited a general practitioner the year before the diagnosis were selected from the Swedish Cancer Register and matched with 11,628 controls, using

Colonoscopic findings in patients with pelvic floor dysfunction.

BACKGROUNDY: Colonoscopy is often performed in the initial workup of pelvic floor dysfunction, even in the absence of red flag symptoms. Current guidelines suggest colonoscopy is only required in the presence of rectal bleeding, diarrhoea or change in bowel habit. The aim of this study was to evaluate the prevalence of significant pathology found at colonoscopy in patients with pelvic floor dysfunction. METHODS: Retrospective chart review was performed on all patients presenting to a functional colorectal outpatient clinic between May 2018 and August 2019. Information was collected on present

A machine learning tool for identifying non-metastatic colorectal cancer in primary care.

BACKGROUND: Primary health care (PHC) is often the first point of contact when diagnosing colorectal cancer (CRC). Human limitations in processing large amounts of information warrant the use of machine learning as a diagnostic prediction tool for CRC. AIM: To develop a predictive model for identifying non-metastatic CRC (NMCRC) among PHC patients using diagnostic data analysed with machine learning. DESIGN AND SETTING: A case-control study containing data on PHC visits for 542 patients >18 years old diagnosed with NMCRC in the Västra Götaland Region, Sweden, during 2011, and 2,139 matched c

Stage-specific risk of colon and rectal cancer in patients presenting with rectal bleeding or change in bowel habit in primary care: A population-based cohort study.

INTRODUCTION: Rectal bleeding and change in bowel habit are red-flag symptoms for colon and rectal cancer but how they relate to advanced stage disease is not adequately understood. METHODS: We analysed primary care electronic health records data on patients aged 30-99 years. Using logistic regression, we first examined the risk of colon and rectal cancer within 12 months in patients presenting with change in bowel habit and rectal bleeding, and then the risk of advanced stage at diagnosis within cancer cases. We combined the results to estimate risk of advanced stage colon and rectal cancers

Inflammatory bowel disease (IBD) in rural and urban India: results from community colonoscopic evaluation of more than 30,000 symptomatic patients.

BACKGROUND: Traditionally, infectious diarrhoea has been the major cause of lower GI symptoms across the developing world. Increasing urbanization has been implicated for the rising IBD cases despite very limited data in the rural setting. We aimed to assess the relative proportion of IBD and other intestinal diseases among symptomatic patients from rural and urban India. METHODS: Patients with lower GI symptoms attending urban out-patient clinics and/or specially conducted mobile rural health camps were evaluated using basic laboratory parameters, abdominal ultrasound and colonoscopy. Data i

Who are the patients being offered the faecal immunochemical test in routine English general practice, and for what symptoms? A prospective descriptive study.

OBJECTIVES: The faecal immunochemical test (FIT) was introduced to triage patients with lower-risk symptoms of colorectal cancer (CRC) in English primary care in 2018. While there is growing evidence on its utility to triage patients in this setting, evidence is still limited on how official FIT guidance is being used, for which patients and for what symptoms. We aimed to investigate the use of FIT in primary care practice for lower-risk patients who did not immediately meet criteria for urgent referral. DESIGN: A prospective, descriptive study of symptomatic patients offered a FIT in primary
