Straight to flexible sigmoidoscopy: rationalization of 2-week wait referrals in suspected colorectal cancer.

AIM: The 2-week wait pathway was designed to decrease the time from presentation to primary care of patients with 'red flag' symptoms of suspected cancer for review by a specialist for the diagnosis or exclusion of cancer. In our tertiary referral centre we have found that 968 colonoscopies per year are required to satisfy the demand for the 2-week wait, leading to limited colonoscopy availability for other services. We sought to determine the yield of colorectal cancer found at colonoscopy referred via the 2-week wait and referenced to the original red flag symptoms. This was in order to sele

Chronic Intermittent Abdominal Bloating and Change in Bowel Habit: An Eight Year Diagnostic Problem Associated with Intra-Abdominal Adhesions.

Abdominal bloating is a common clinical presentation and can impact significantly on the quality of life. It can be functional or signify more serious pathology. We present a case presentation of a 38-year-old man who presented with recurrent episodes of abdominal bloating and severe debilitation for many years. He had suffered weight loss and was unable to tolerate solids. Ten years ago, he underwent an emergency laparotomy for sigmoid volvulus. In the presence of anaemia, normal imaging, and normal endoscopy, a laparoscopy was performed. At surgery, several adhesional bands were identified a

Irritable bowel syndrome: Is it "irritable brain" or "irritable bowel"?

Irritable bowel syndrome (IBS) has been recognized as one of the most common and best studied disorders among the group of functional gastrointestinal disorders. It is a functional bowel disorder in which abdominal pain or discomfort is associated with defecation or a change in bowel habit. In the Western world, IBS appears to affect up to 20% of the population at any given time but in Asian countries, the median value of IBS prevalence defined by various criteria ranges between 6.5% and 10.1%, and community prevalence of 4% is found in North India. Those attending gastroenterology clinics rep

A case of multicentric carcinoid in a patient with psoriatic spondyloarthropathy.

We describe the first case of a patient presenting with multicentric carcinoid occurring in the lung and subsequently in the rectum, with chronic psoriatic arthritis. Although reports have been published regarding carcinoid syndrome occurring alongside rheumatoid arthritis, no reports have been made on such a case. Initial presentation of carcinoid syndrome in this patient was insidious and atypical with few symptoms, including shortness of breath and long standing abdominal bloating. Several years later a sudden change in bowel habit prompted a colonoscopy with biopsy that revealed a carcinoi

Two-week wait symptoms are prevalent in screened patients with a positive faecal occult blood test but do not predict cancer.

AIM: Lower gastrointestinal (LGI) symptoms are prevalent in patients screened for bowel cancer yet do not predict a finding of cancer. This study evaluates symptoms in patients with these characteristics against the 2-week wait (2ww) criteria to determine whether they predicted cancer in these patients. METHOD: A prospective cohort study was performed. Patients with a positive faecal occult blood (FOB) test attending our unit over a 7-month period were included. Data on symptom prevalence, frequency and duration were collected and assessed against the 2ww criteria. Associations between sympto

Diagnostic value of rectal bleeding in predicting colorectal cancer: a systematic review.

This study aimed at summarizing published study findings on the diagnostic value of rectal bleeding (RB) and informing clinical practice, preventive interventions and future research areas. We searched Medline and Embase for studies published by September 13, 2013 examining the risk of colorectal cancer in patients with RB using highly inclusive algorithms. Data for sensitivity, specificity, positive likelihood ratio, negative likelihood ratio and positive predictive value (PPV) of RB were extracted by two researchers and analyzed applying Meta-Disc (version 1.4) and Stata (version 11.0). Constipation: take it seriously.

A case report of a young, healthy female who presented as an emergency with large bowel obstruction secondary to a rectosigmoid tumour. Acute presentation was preceded by a progressive history of change in bowel habit managed medically in the community with laxative treatment. The family history of note included breast cancer of an identical triplet aged 41 years. An emergency Hartman's procedure was successfully performed the same day as presentation due to the high risk of perforation. The patient was discharged home within a week. Histology confirmed adenocarcinoma, Dukes' Stage C and oncol

Primary gallbladder lymphoma presenting as a polyp.

We present an unusual case of a 75-year-old woman, with no significant medical history, presenting with ongoing weight loss and change in bowel habit. Her physical examination and initial blood tests were all normal, and, therefore, radiographic imaging was undertaken. Ultrasound and CT of the abdomen confirmed a gallbladder polyp and a laparoscopic cholecystectomy was subsequently performed. Histological analysis confirmed primary gallbladder lymphoma. This case report is the first to present gallbladder lymphoma presenting as a polyp. The authors discuss the incidence, presentation and manag

Development of a risk score to stratify symptomatic adults referred for colonoscopy.

BACKGROUND AND AIM: With an increasing burden on overstretched colonoscopy services, a simple risk score for significant pathology in symptomatic patients may aid in the prioritization of patients. METHODS: A derivative study of a risk score model for colonic neoplasia (colorectal carcinoma [CRC] and advanced adenoma) and CRC alone was conducted in symptomatic adults referred for an index colonoscopy. The accuracy of the final model was assessed by the area under the curve (AUC) of the receiver operating characteristic curve and the Hosmer-Lemeshow goodness-of-fit statistic. RESULTS: A total

Rectal pseudodiverticulum.

We present the case of a 77-year-old woman who initially presented 13 years ago to a colorectal clinic with a change in bowel habit and bleeding per rectum over 7 months. These symptoms were attributed to a tubulovillous rectal adenoma which was excised transanally, but recurred five times in 12 years. The most recent endoscopy showed recurrence of the rectal adenoma and a new rectal diverticulum. Diverticula in the rectum are rare and only a few reports of this condition exist in the literature. Repeated surgical resections and endoscopic polypectomies may have caused a weakness in the rectal

Enterobiasis-related inflammatory caecal polyp masquerading as a malignancy.

A 55 -year-old Asian man was seen in the emergency department with bleeding per rectum. He was a teetotaller and had no previous abdominal surgery. He did, however, report a change in bowel habit towards constipation. He underwent colonoscopy which revealed a lesion, highly suspicious of malignancy, in the caecum. On review by two consultants, a decision to completely resect this lesion was made. Histological analysis of the polypoidal growth showed it to be a consequence of chronic infection with the helminth Enterobius vermicularis. Importantly, there was no evidence of dysplastic or maligna

Investigating the frail elderly patient with lower bowel symptoms: what do we do now and can we improve?

AIMS: To assess the utility of flexible sigmoidoscopy (FS) and minimal preparation CT (MPCT) in investigating lower gastrointestinal (LGI) symptoms in elderly patients who are too frail to undergo colonoscopy or spiral CT. METHODS: All FS examinations performed in patients aged over 70 between 1 January and 31 December 2008 were analysed. Predictors of usefulness were determined using multivariable analysis. In patients who also underwent MPCT, we analyzed the correlation between FS and MPCT. RESULTS: 426 FS were performed. Bowel preparation was inadequate in 24% of procedures. Indications i

Primary lymphoma of the colon.

BACKGROUND: primary colorectal lymphoma is a very rare disease, representing less than 0.5 % of all primary colorectal neoplasms. The gastrointestinal tract is the most frequently involved site of all extranodal lymphomas, the most common type of that is non-Hodgkin s lymphoma. Early diagnosis is often difficult because of unspecific symptoms. Therapeutic approaches have classically included radical resection, chemotherapy and radiotherapy. MATERIALS AND METHODS: we present our experience in the management of primary colorectal lymphomas over a 17-year period (1994-20011). RESULTS: in this p

[A man with colorectal signet ring cell carcinoma: which therapy is required?].

BACKGROUND: Primary colorectal signet ring cell carcinoma (SRCC) is rare and often affects younger patients. It demonstrates a different biological behaviour and has a less favourable prognosis than the more conventional colorectal adenocarcinoma. CASE DESCRIPTION: Patient A, a 65-year-old man, presented with a change in bowel habit. Colonoscopy showed a stenosis with signs of a nonspecific inflammation. However, laparoscopy revealed a primary colorectal SRCC with diffuse peritoneal metastases. Standard chemotherapy was unsuccessful. Patient A died 13 months after diagnosis. CONCLUSION: Base

Treating symptomatic adhesions to the sigmoid colon: colectomy improves quality of life.

INTRODUCTION: When severe chronic pelvic pain, constipation, and bloating are due to dense pelvic adhesions fixing the sigmoid loops, patients may be offered the option of sigmoid colectomy. This study examines the effectiveness of colectomy in the treatment of such patients. METHODS: Patients were identified from a surgical database, demographic data were abstracted, and charts were reviewed. Patients were interviewed postoperatively. Primary end points were morbidity, length of stay, change in bowel habit, and patient satisfaction. RESULTS: There were 46 patients (44 women) with a mean age

Validation of the Rome III criteria for the diagnosis of irritable bowel syndrome in secondary care.

BACKGROUND & AIMS: There are few validation studies of existing diagnostic criteria for irritable bowel syndrome (IBS). We conducted a validation study of the Rome and Manning criteria in secondary care. METHODS: We collected complete symptom, colonoscopy, and histology data from 1848 consecutive adult patients with gastrointestinal symptoms at 2 hospitals in Hamilton, Ontario; the subjects then underwent colonoscopy. Assessors were blinded to symptom status. Individuals with normal colonoscopy and histopathology results, and no evidence of celiac disease, were classified as having no organic
