Symptom presentations and other characteristics of colorectal cancer patients and the diagnostic performance of the Auckland Regional Grading Criteria for Suspected Colorectal Cancer in the South Auckland population.

AIM: This study reviews the presenting symptoms of colorectal cancer in the ethnically diverse Middlemore Hospital referral population of South Auckland, New Zealand. The performance of the newly introduced Auckland Regional Grading Criteria as prediction tool for selecting colorectal cancer cases referred from primary care was evaluated in this group. METHOD: Retrospective review of all colorectal cancer (CRC) cases diagnosed between January 2006 and January 2011. Information extracted from case note review was used to grade patients using the Auckland Regional Grading Criteria. RESULTS: A

Segmental colitis associated with diverticulosis: a clinical entity to know and to look for.

Patients with diverticulosis may develop inflammatory manifestations with segmentary involvement  of the colon (segmental colitis associated with diverticulosis, SCAD), an often underestimated clinical condition. We observed the case of a 59 year-old man who presented with abdominal pain, change in bowel habit and hematochezia. Endoscopic and histological evaluation showed features suggestive for SCAD. Medical therapy has been confirmed successful in this patient.

Linaclotide for constipation-predominant IBS.

Irritable bowel syndrome is a chronic relapsing disorder characterised by abdominal pain or discomfort associated with defaecation, abdominal bloating and a change in bowel habit. IBS may be classified by the change in bowel function as 'diarrhoea predominant' (IBS-D), 'constipation predominant' (IBS-C) or mixed, or may be unclassified. Although it is not thought to be associated with the development of serious disease, IBS is a debilitating condition often resulting in reduced quality of life and significant use of healthcare resources. Current drug treatment strategies target the patient's p

Is whole colonic imaging necessary for symptoms of change in bowel habit and/or rectal bleeding?

AIM: Following the introduction of a 2-week-wait (2ww) cancer pathway, many units are triaging patients with change in bowel habit (CIBH) and/or rectal bleeding (RB) straight to colonoscopy. Evidence suggests that right-sided colonic cancer does not present with these symptoms, hence imaging the left colon only is satisfactory. If this were substantiated, patients could be offered a flexible sigmoidoscopy (FS) alone. This study aimed to review presenting symptoms of patients diagnosed with a right-sided colonic malignancy and assess whether their tumours would be missed based on this practice.

Cannabinoid hyperemesis.

A 21-year-old woman presented with a 4-week history of sudden onset vomiting, nausea and anorexia. Questioning revealed that she had a 7-year history of heavy cannabis use (smoking). She did not describe abdominal pain, change in bowel habit, antibiotic use, foreign travel or contact with gastroenteritis. Biochemistry results demonstrated mild metabolic derangement with a low potassium and a low bicarbonate, and urine toxicology was positive for cannabinoids. Other investigations, including a full blood count, renal function tests, liver function tests, a coagulation sample, an ECG, urinary β-

Current state of medical-advice-seeking behaviour for symptoms of colorectal cancer: determinants of failure and delay in medical consultation.

AIM: There are few data on the factors associated with healthcare-seeking behaviour for symptoms of colorectal cancer. This study describes the determinants of failure and delay in seeking medical advice for rectal bleeding and change in bowel habit. METHOD: In total, 1592 persons (56-88 years) were randomly selected from the Hunter Community Study and mailed a questionnaire. RESULTS: In all, 18% (60/332) of respondents experiencing rectal bleeding and 20% (39/195) reporting change in bowel habit had never consulted a doctor. The rate of delay (>1 month) for each symptom was 18% and 37%. The

Revisiting concepts of visceral nociception in irritable bowel syndrome.

BACKGROUND AND OBJECTIVE: Irritable bowel syndrome (IBS) is a common disorder characterized by abdominal pain related to defecation with a change in bowel habit. Patients with IBS often exhibit increased visceral sensitivity, which can be tested clinically by rectal balloon distension procedures. This paper aims to give an overview of mechanisms involved in visceral hypersensitivity in IBS by reviewing recent literature. DATABASES AND DATA TREATMENT: A literature search in the electronic databases Pubmed and MEDLINE was executed using the search terms 'visceral pain' or 'visceral nociception'

Identifying patients with suspected colorectal cancer in primary care: derivation and validation of an algorithm.

BACKGROUND: Earlier diagnosis of colorectal cancer could help improve survival so better tools are needed to help this. AIM: To derive and validate an algorithm to quantify the absolute risk of colorectal cancer in patients in primary care with and without symptoms. DESIGN AND SETTING: Cohort study using data from 375 UK QResearch® general practices for development and 189 for validation. METHOD: Included patients were aged 30-84 years, free at baseline from a diagnosis of colorectal cancer and without rectal bleeding, abdominal pain, appetite loss, or weight loss in the previous 12 months.

Metastatic paediatric colorectal carcinoma.

A 16-year-old girl presented to our unit with crampy abdominal pain, change in bowel habit, a subjective impression of weight loss and a single episode of haematochezia. She was found to have a rectosigmoid adenocarcinoma and proceeded to laparoscopic anterior resection, whereupon peritoneal metastases were discovered. She received chemotherapy and is alive and well ten month later with no radiological evidence of disease. Colorectal carcinoma is rare in the paediatric population but is increasing in incidence. Early diagnosis is critical to enable optimal outcomes.

Factors associated with consultation behaviour for primary symptoms potentially indicating colorectal cancer: a cross-sectional study on response to symptoms.

BACKGROUND: Little data exists on the factors associated with health care seeking behaviour for primary symptoms of colorectal cancer (CRC). This study aimed to identify individual, provider and psychosocial factors associated with (i) ever seeking medical advice and (ii) seeking early medical advice for primary symptoms of colorectal cancer (CRC). METHODS: 1592 persons aged 56-88 years randomly selected from the Hunter Community Study (HCS) were sent a questionnaire. RESULTS: Males and those who had received screening advice from a doctor were at significantly higher odds of ever seeking me

Clinical, endoscopic and pathogical characteristics of early-onset colorectal cancer in Vietnamese.

BACKGROUND: The Asia Pacific consensus for colorectal cancer (CRC) recommends that screening programs should begin by the age of 50. However, there have been reports about increasing incidence of CRC at a younger age (i.e. early-onset CRC). Little is known about the features of early-onset CRC in the Vietnamese population. AIM: To describe the clinical, endoscopic and pathological characteristics of early-onset CRC in Vietnamese. METHOD: A prospective, cross-sectional study was conducted at the University Medical Center from March 2009 to March 2011. All patients with definite pathological d

Anterior resection syndrome.

Up to 80% of patients with rectal cancer undergo sphincter-preserving surgery. It is widely accepted that up to 90% of such patients will subsequently have a change in bowel habit, ranging from increased bowel frequency to faecal incontinence or evacuatory dysfunction. This wide spectrum of symptoms after resection and reconstruction of the rectum has been termed anterior resection syndrome. Currently, no precise definition or causal mechanisms have been established. This disordered bowel function has a substantial negative effect on quality of life. Previous reviews have mainly focused on dif

Early diagnosis improves survival in colorectal cancer.

Colorectal cancer is the second most common cause of death from cancer in the UK. Common alarm symptoms include rectal bleeding, change in bowel habit and iron deficiency anaemia. Abdominal mass, weight loss, nausea and vomiting, anorexia and abdominal swelling are less common presenting symptoms. Patients meeting the NICE criteria for urgent referral should be referred via the two week wait pathway to the local colorectal department for prompt assessment to exclude colorectal cancer. Colorectal cancer has a male predominance and is strongly associated with age; 80% of new cases occur in patie

Pyogenic granuloma of the large intestine: case report and review of reported cases in the adult.

Pyogenic granuloma (PG) is a polypoid lobular capillary hemangioma rarely described in the large bowel. We describe the case of a 72-year-old man with recent weight loss, anemia, and change in bowel habit. A 3cm×2cm polypoid lesion in the right flexure of the colon was observed and treated with extended right hemicolectomy. A review of the English-language literature on PG of the large bowel in adults yielded eight previously reported cases. The age of the patients ranged from 26 to 80 years with a mean of 54.8 years. The lesions were all solitary, except one case of multiple lesions in the co

Spectrum of colonoscopy findings in Ile-Ife Nigeria.

AIMS AND OBJECTIVES: Lower gastrointestinal (LGI) diseases are the leading causes of morbidity and mortality worldwide. Colonoscopy holds an important place in screening, diagnosing and treatment of these conditions. In Nigeria, as in many other developing countries, the facility for performing colonoscopy is rarely available. This prospective report seeks to evaluate the demographic data of patients presenting for colonoscopy, the pattern and validity of referral diagnosis versus colonoscopy findings in Ile-Ife, Nigeria. SUBJECTS AND METHODS: All patients who had colonoscopy procedure done i

The diagnostic value of symptoms for colorectal cancer in primary care: a systematic review.

BACKGROUND: Over 37,000 new colorectal cancers are diagnosed in the UK each year. Most present symptomatically to primary care. AIM: To conduct a systematic review of the diagnostic value of symptoms associated with colorectal cancer. DESIGN: Systematic review. METHOD: MEDLINE, Embase, Cochrane Library, and CINAHL were searched to February 2010, for diagnostic studies of symptomatic adult patients in primary care. Studies of asymptomatic patients, screening, referred populations, or patients with colorectal cancer recurrences, or with fewer than 100 participants were excluded. The target co

Most bowel cancer symptoms do not indicate colorectal cancer and polyps: a systematic review.

BACKGROUND: Bowel symptoms are often considered an indication to perform colonoscopy to identify or rule out colorectal cancer or precancerous polyps. Investigation of bowel symptoms for this purpose is recommended by numerous clinical guidelines. However, the evidence for this practice is unclear. The objective of this study is to systematically review the evidence about the association between bowel symptoms and colorectal cancer or polyps. METHODS: We searched the literature extensively up to December 2008, using MEDLINE and EMBASE and following references. For inclusion in the review, pap

Colorectal cancer screening amongst first degree relatives of colon cancer cases in Jordan.

OBJECTIVES: Early detection and screening for colorectal cancer is important among first degree relatives (FDR) of colon cancer cases. METHODS: Our target population comprised all such FDR, above 40 years of age, registered during the years 2003-2007, Jordan. Detailed information about cancer cases was collected from Jordan Cancer Registry. The screening study was conducted through two stages, where all FDR were examined at their homes for any suggestive related symptoms of colorectal cancer, then those who were suspected to have cancer were referred to hospital for confirmatory colonoscopy.
