Temporal characterization of the gut microbiome and metabolome in preterm infants.

Preterm infants experience abnormal microbial colonization, which coupled with their vulnerable physiology can increase the risk of disease. Understanding the factors influencing the complex relationship between the temporal development of the gut microbiome and functional metabolites derived from microbe-microbe and microbe-host interaction is therefore critical. In this study, 266 longitudinal stool samples from 66 very preterm infants underwent 16S rRNA gene sequencing to analyse gut microbial structure. To further explore the functional status of these gut members, a subset of these sample

Effects of a Phytogenic Mycotoxin Detoxifier on Oxidative Status, Health, and Performance in Dairy Sheep.

Mycotoxins are common feed contaminants that can affect the health, immune function, and productivity of ruminants by causing oxidative stress and organ dysfunction. In this field study, the effects of a phytogenic multicomponent mycotoxin detoxifier on oxidative status, liver function, udder health, and productive parameters were investigated in dairy ewes. One hundred clinically healthy ewes were randomly assigned to either a control group or a treatment group, with the latter receiving 1.5 kg/ton of the detoxifier over a 90-day period during lactation. The detoxifying agent contained adsorp

Current advances in the management of hepatic encephalopathy: an updated and critical review.

Hepatic encephalopathy (HE) is a serious and potentially reversible neuropsychiatric syndrome resulting from severe liver insufficiency. HE is primarily considered a gliopathy in which astrocyte dysfunction is the main neuropathological hallmark, while recently microglia and neuronal alterations have been reported. It is believed that reversible factors trigger more than 80% of the cases. Several causative factors, including ammonia, inflammation, neuroinflammation, and oxidative stress, contribute to its pathogenesis, ultimately leading to abnormalities in neurotransmission and altered neuron

Potential of Limosilactobacillus reuteri in modulating neuroplasticity and ameliorating behavioral deficits in a maternal separation rat model of autism.

Limosilactobacillus reuteri has attracted attention for its potential to modulate the gut-brain axis. This study investigates whether L. reuteri administration mitigates autism-related abnormalities in a maternal separation rat model of autism. Male Wistar rats were divided into maternal-separated L. reuteri-treated (MS+LR), maternal-separated placebo-treated (MS), and non-separated control groups. Behavioral assessments evaluated autism-related behaviors, including social interaction, repetitive behavior, anxiety, and hyperactivity. Immunohistochemical analysis of the hypothalamus assessed ox

Fecal elastase measurement- beware; A cautionary tale.

BACKGROUND: Fecal elastase testing of stool is increasingly utilized for the diagnosis of exocrine pancreatic insufficiency (EPI). However, testing is being used in other settings such as evaluation of diarrhea where interpretation may be inaccurate resulting in unwarranted additional testing and prescription of pancreatic enzymes. METHODS: Over a 24-month period, we prospectively evaluated all patients referred to a pancreas clinic for evaluation of EPI using the fecal elastase (FE-1) measurement. A thorough history was obtained to determine the nature and duration of the complaints, risk fa

From Cytokines to Biomarkers: Mapping the Immunopathology of Inflammatory Bowel Disease.

Inflammatory bowel disease (IBD) is a chronic immune-mediated condition of the gastrointestinal tract, characterized by dysregulated inflammatory responses throughout the gastrointestinal tract. It includes two major phenotypes, Crohn's disease (CD) and ulcerative colitis (UC), which present with varying gastrointestinal and systemic symptoms. The pathophysiology of IBD is multifactorial including genetic predisposition, mucosal and epithelial dysfunction, environmental injury, and both innate and adaptive immune response abnormalities. Several predisposing genetic factors have been associated

Assessment of Water Genotoxicity in Four Coastal Lagoons in Southern Brazil Using Fish Bioassays.

The water in the coastal lagoons of the Tramandaí River Basin (TRB) in southern Brazil is affected by domestic, industrial, and agricultural effluents. The towns within the TRB experience significant population surges during peak vacation periods, which may further impact water quality. The micronucleus (MN) test is a useful tool for detecting genotoxic effects induced by pollutant mixtures and can serve as an indicator of water quality. This study aimed to evaluate the temporal variation in genotoxic potential and fecal contamination of water samples from four coastal lagoons within the TRB.

Imperforate Anus With Perineal Fistula in an Infant Presenting With Constipation and Abscesses: A Case Report.

Chronic constipation and failure to thrive in infants can sometimes mask underlying congenital anorectal malformations. Here, we present a female infant who initially presented with refractory constipation, slow weight gain, and intermittent vomiting, later found to have an imperforate anus with a perineal fistula. Her course was complicated by recurrent fecal impactions, perianal and gluteal abscesses, and repeated hospitalizations. Multiple interventions were required, including manual disimpactions, rectal dilations, full-thickness biopsies, a colostomy with mucus fistula creation, a poster

A Qualitative Study of Staff and Provider Experiences with Follow-up Colonoscopy in a Community Health Center and Gastroenterology Specialty.

Colorectal cancer screening is an effective strategy to prevent disease, reduce the risk of advanced-stage diagnosis, and improve survival. Timely follow-up of abnormal screening results, particularly abnormal fecal immunochemical tests, is essential to realizing these benefits. This qualitative study examined routine processes related to colonoscopy referral and completion for patients of a federally qualified health center (FQHC) referred to community gastroenterology (GI) practices. Using a snowball sampling approach, five FQHC individuals and eight community GI practice individuals were in

A study on the utility of inflammatory biomarkers in primary care irritable bowel syndrome: a sub analysis of the DOMINO randomized trial.

BACKGROUND: The role of low-grade-inflammation in Irritable Bowel Syndrome (IBS) a common disorder of gut-brain interaction is unclear. Association with symptom severity, as well as effects of first line treatments are understudied. In a large cohort of primary care IBS patients (DOMINO trial), a FODMAP-lowering diet application was superior in improving symptoms compared to standard medical therapy (otilonium bromide, OB). We investigated the level of pro-inflammatory markers in this IBS cohort and its changes by the DOMINO application and OB. METHODS: Inflammatory markers fecal calprotectin

Prevalence, determinants, and barriers to follow-up colonoscopy after a positive fecal immunochemical test in Qaemshahr, Iran: A population-based cross-sectional study from 2022 to 2023.

OBJECTIVE: Fecal Immunochemical Test (FIT) screening reduces mortality by detecting early-stage colorectal cancer (CRC), but its success depends on follow-up colonoscopy after a positive result. Follow-up rates vary widely, influenced by several barriers, which need to be addressed. Thus, the present study aimed to assess the prevalence, determinants, and barriers to a follow-up colonoscopy after a positive FIT result. METHODS: This cross-sectional study evaluated FIT-based CRC screening from April 2022 to March 2023 in Qaemshahr, Iran. Positive FIT results triggered follow-up colonoscopies.

Bile Derivative T3K Ameliorates Colitis by Regulating the Intestinal Microbiota-Bile Acid Axis.

Background/Objectives: The pathogenesis of ulcerative colitis (UC) is complex, and there is an urgent need for effective therapeutic agents with low side effects. Recent studies highlight the critical roles of abnormal bile acid (BA) metabolism and gut microbiota dysbiosis in UC progression. However, there is a significant knowledge gap about the relation between BA and gut microbiota. The BA derivative T3K exerts good anti-UC effect, and its mechanism is still unknown. In this study, we investigate how its anti-UC mechanism is involved in the modulation of the gut microbiota-BA axis and BA me

Correlation of meconium-stained amniotic fluid with subclinical chorioamnionitis and associated cardiotocographic changes.

Our study aims to correlate meconium-stained amniotic fluid with subclinical chorio-amnionitis and associated cardio-tocographic (CTG) changes in women of >34 week gestation. We found CTG abnormalities in 47.2% cases and 24.1% controls, showing a significant correlation between meconium stained amniotic fluid and CTG abnormalities. These were not significantly associated with subclinical chorio-amnionitis, though the neonatal outcome of these babies was worse. Consequently, it is essential to balance the use of CTG with clinical judgement and other diagnostic tools to avoid unnecessary Caesare

Role of the intestinal flora-immunity axis in the pathogenesis of rheumatoid arthritis-mechanisms regulating short-chain fatty acids and Th17/Treg homeostasis.

BACKGROUND: The pathogenesis of rheumatoid arthritis (RA), a chronic systemic autoimmune disease, is closely linked to the interactions between intestinal flora and metabolites. Recent research has shown that the "gut-joint axis" is an important regulator of immune homeostasis, gut microbiota dysbiosis not only causes pro-inflammatory bacteria to proliferate abnormally, but it also decreases the biosynthesis of short-chain fatty acids (SCFAs). This dual imbalance ultimately exacerbates synovial inflammation and encourages bone destruction by upsetting the balance of Th17/Treg cells, that is, t
