A new association between Kleefstra syndrome and Panayiotopoulos epilepsy.

BACKGROUND: Kleefstra syndrome is a rare genetic disorder attributed to loss of function of EHMT1, either due to a point mutation or a microdeletion in the chromosome region 9q34.3. This gene encodes an enzyme that modifies histone function and is essential for normal development. Individuals with Kleefstra syndrome typically present intellectual disability (from moderate to severe), language delay, autism spectrum disorders, generalized hypotonia, and distinctive facial dysmorphic features. Additional manifestations in children may include cardiac defects, renal and urological malformations,

Health status and disease prevalences in French bulldogs in Germany: insights from a survey-based study.

BACKGROUND: The French bulldog has recently become one of the most popular dog breeds in the UK, Germany, and the USA. Known for its brachycephalic facial structure, characterized by a short muzzle and flat nose, the breed is predisposed to brachycephalic obstructive airway syndrome (BOAS) and other health concerns, including gastrointestinal, dermatological, and orthopedic conditions. These conditions are distressing for affected dogs and present significant challenges for their owners and healthcare providers. A survey targeting French bulldog owners in Germany was conducted to investigate t

Microsporidia keratoconjunctivitis identified as an emerging zoonotic threat from pet parrots: Clinical and metagenomic next-generation sequencing evidence.

Microsporidia are opportunistic, obligate intracellular fungi capable of causing keratoconjunctivitis. Because the clinical manifestations of microsporidia keratoconjunctivitis are indistinguishable from those of other etiologies, and the organism is difficult to culture, its diagnosis is challenging. The transmission routes of microsporidia keratoconjunctivitis remain poorly defined, and zoonotic sources have long been suspected but rarely confirmed. Between September 2024 and October 2025, a total of 15 confirmed cases of microsporidia keratoconjunctivitis were identified at Peking Universit

From crisis to recovery: A case report on nursing strategies for hepatitis E post-cardiac arrest.

RATIONALE: Extracorporeal membrane oxygenation (ECMO) is a life-support technology for refractory cardiac arrest, but the massive blood transfusions required during treatment significantly increase the risk of transfusion-related infections. Hepatitis E virus (HEV) - traditionally linked to fecal-oral transmission - is increasingly recognized as a transfusion-transmitted pathogen, especially in emergency settings where urgent blood product infusion is common and routine HEV screening in blood banks is often lacking. However, nursing strategies for managing acute HEV infection after ECMO remain

Unmasking Hepatitis A: A Case Study of Atypical Presentation in a Returning Traveler From Egypt.

Hepatitis, characterized by inflammation of the liver, arises from various infectious and noninfectious causes, with viral hepatitis being caused by a diverse group of viruses including hepatitis A, B, C, D, and E. Infection with the Hepatitis A virus (HAV) can result in liver inflammation and damage, primarily spread through fecal-oral contamination. Clinical symptoms often overlap with other infections, complicating diagnosis in returning travelers from endemic regions. This case study focuses on a 46-year-old Sudanese housewife who presented with symptoms of fever, chills, headache, and mus

A Rare Case of Triple Infection with Leptospira, Hepatitis A Virus, and Hepatitis E Virus.

Background and Clinical Significance: Simultaneous, multiple infections coinfections caused by zoonotic or fecal-orally transmitted diseases are common in tropical and subtropical regions. Published data report that leptospirosis may coexist with other infections, complicating the clinical presentation and trajectory due to overlapping symptoms and leading to more severe clinical progress. Case Presentation: We describe a clinical case of a 34-year-old female diagnosed with a triple infection caused by Leptospira, Hepatitis A Virus, and Hepatitis E Virus. To our knowledge, this is the first ca

Colonic Metastasis of Previously Undetected Invasive Lobular Carcinoma in a 68-Year-Old Woman Undergoing Regular Breast Screening.

Invasive lobular carcinoma (ILC) is a breast cancer subtype known for its subtle presentation and potential for unusual metastatic spread. We present the case of a 68-year-old woman with vague abdominal pain and a positive fecal occult blood test. Colonoscopy revealed a vegetative neoplasm in the cecum with ileocecal valve stenosis. Initial histopathology, performed during endoscopy, identified an epithelial infiltrative histotype but did not allow further characterization due to limited differentiation. The tumor was staged with a contrast-enhanced thoracoabdominal CT scan, and multidisciplin

Characterization and Evaluation of Lactobacillus plantarum LC5.2 Isolated from Thai Native Pigs for its Probiotic Potential in Gut Microbiota Modulation and Immune Enhancement.

Probiotic supplementation, particularly with Lactobacillus species, enhances growth performance, maintains gastrointestinal microbial balance, and prevents infections in livestock. This study isolated Lactobacillus strains from the feces of healthy native pigs in southern Thailand and assessed their probiotic properties and safety through both in vitro and in vivo evaluations. Nine Lactobacillus strains showed probiotic potential, with Lactobacillus plantarum LC5.2 demonstrating the best characteristics. This strain tolerated both acid and bile (100% tolerance) and exhibited strong adhesion pr

An unpredictable gastrointestinal bleed.

A 65-year-old male complained of persistent melena for 6 days, and displayed anemia symptoms without hematemesis, vomiting, and abdominal distention. He was diagnosed as ruptured aneurysm of aortic sinus Valsalva, and had received coronary artery occlusion 1 month ago. After the operation, he was continually prescribed clopidogrel 75 mg once daily. The laboratory examination showed blood hemoglobin concentration was 60 g/L without other conspicuous abnormality. Unfortunately, neither esophagogastroduodenoscopy (EGD) nor colonoscopy found no obvious bleeding lesions. And abdominal computed tomo

Dual-directional regulation of spinal cord injury and the gut microbiota.

There is increasing evidence that the gut microbiota affects the incidence and progression of central nervous system diseases via the brain-gut axis. The spinal cord is a vital important part of the central nervous system; however, the underlying association between spinal cord injury and gut interactions remains unknown. Recent studies suggest that patients with spinal cord injury frequently experience intestinal dysfunction and gut dysbiosis. Alterations in the gut microbiota can cause disruption in the intestinal barrier and trigger neurogenic inflammatory responses which may impede recover

Detecting and quantifying Veillonella by real-time quantitative PCR and droplet digital PCR.

Veillonella spp. are Gram-negative opportunistic pathogens present in the respiratory, digestive, and reproductive tracts of mammals. An abnormal increase in Veillonella relative abundance in the body is closely associated with periodontitis, inflammatory bowel disease, urinary tract infections, and many other diseases. We designed a pair of primers and a probe based on the 16S rRNA gene sequences of Veillonella and conducted real-time quantitative PCR (qPCR) and droplet digital PCR (ddPCR) to quantify the abundance of Veillonella in fecal samples. These two methods were tested for specificity

Methods for scaling up an outreach intervention to increase colorectal cancer screening rates in rural areas.

BACKGROUND: Mailed fecal immunochemical test (FIT) outreach and patient navigation are evidence-based practices shown to improve rates of colorectal cancer (CRC) and follow-up in various settings, yet these programs have not been broadly adopted by health systems and organizations that serve diverse populations. Reasons for low adoption rates are multifactorial, and little research explores approaches for scaling up a complex, multi-level CRC screening outreach intervention to advance equity in rural settings. METHODS: SMARTER CRC, a National Cancer Institute Cancer Moonshot project, is a clu

Revisiting the Role of Valeric Acid in Manipulating Ulcerative Colitis.

BACKGROUND: Ulcerative colitis (UC) is characterized by a complicated interaction between mucosal inflammation, epithelial dysfunction, abnormal activation of innate immune responses, and gut microbiota dysbiosis. Though valeric acid (VA), one type of short-chain fatty acids (SCFAs), has been identified in other inflammatory disorders and cancer development, the pathological role of VA and underlying mechanism of VA in UC remain under further investigation. METHODS: Studies of human clinical specimens and experimental colitis models were conducted to confirm the pathological manifestations of

Specific gut microbiome and metabolome changes in patients with continuous ambulatory peritoneal dialysis and comparison between patients with different dialysis vintages.

BACKGROUND: In recent years, the role of gut microbiota and derived metabolites in renal disease has attracted more attention. It has been established that the gut microbiota is a potential target for medical interventions in renal disease including chronic kidney disease (CKD), acute kidney injury (AKI) and renal calculus. Emerging evidence has related dialysis treatment to the microbial composition and function of the intestines, and there are many reports related to HD, but few studies have been related to PD. Previous studies have found that PD patients have intestinal flora disturbances,

Gut microbiota and clinical response to immune checkpoint inhibitor therapy in patients with advanced cancer.

BACKGROUND: There is currently no well-accepted consensus on the association between gut microbiota and the response to treatment of immune checkpoint inhibitors (ICIs) in patients with advanced cancer. METHODS: Fecal samples were collected before ICI treatment. Gut microbiota was analyzed using 16 S ribosomal RNA sequencing. We investigated the relationship between the α-diversity of fecal microbiota and patients' clinical outcomes. Microbiota profiles from patients and healthy controls were determined. Pre-treatment serum was examined by cytokine array. RESULTS: We analyzed 74 patients, in

Modulation of gut microbiota by crude mulberry polysaccharide attenuates knee osteoarthritis progression in rats.

Mulberry (Morus alba L.), a kind of common fruits widely cultivated worldwide, has been proven various biological activities. However, its potential role in the progression of knee osteoarthritis (KOA) remains unclear. This study aims to investigate the potential protective effects of crude polysaccharide extracted from mulberry fruit, referred to as a complex blend of polysaccharides and other unidentified extracted impurities, on KOA progression. The KOA rats were established by injection of 1 mg sodium monoiodoacetate into knee, and administrated with crude mulberry polysaccharide (Mup) by

Maximizing scarce colonoscopy resources: the crucial role of stool-based tests.

During the COVID-19 pandemic, health systems, including federally qualified health centers, experienced disruptions in colorectal cancer (CRC) screening. National organizations called for greater use of at-home stool-based testing followed by colonoscopy for those with abnormal test results to limit (in-person) colonoscopy exams to people with acute symptoms or who were high risk. This stool-test-first strategy may also be useful for adults with low-risk adenomas who are due for surveillance colonoscopy. We argue that colonoscopy is overused as a first-line screening method in low- and average

Repeated (S)-ketamine administration ameliorates the spatial working memory impairment in mice with chronic pain: role of the gut microbiota-brain axis.

Chronic pain is commonly linked with diminished working memory. This study explores the impact of the anesthetic (S)-ketamine on spatial working memory in a chronic constriction injury (CCI) mouse model, focusing on gut microbiome. We found that multiple doses of (S)-ketamine, unlike a single dose, counteracted the reduced spontaneous alteration percentage (%SA) in the Y-maze spatial working memory test, without affecting mechanical or thermal pain sensitivity. Additionally, repeated (S)-ketamine treatments improved the abnormal composition of the gut microbiome (β-diversity), as indicated by

The gut microbiota regulates the depressive-type behaviors and inflammatory processes after severe burn injuries in mice.

An emerging number of studies have recently revealed the correlation between burn injuries and psychological disorders. Gut microbiota and inflammatory factors may play a vital role in this process. Nevertheless, there are few studies conducted to disclose the potential mechanism of the gut microbiota between depression and burn injuries. In this study, we constructed a burn model of C57BL/6 mice, which showed that the symptom of depression became more and more severe with the burn of mice lasted longer. Meanwhile, there are significant differences of composition of gut microbiota among mice b

Interpreting pathology test result values with comparators (< , >) in Electronic Health Records research: an OpenSAFELY short data report.

BACKGROUND: Numeric results of pathology tests are sometimes returned as a range rather than a precise value, e.g. "<10". In health data research, test result values above or below clinical threshold values are often used to categorise patients into groups; however comparators (<, > etc) are typically stored separately to the numeric values and often ignored, but may influence interpretation. METHODS: With the approval of NHS England we used routine clinical data from 24 million patients in OpenSAFELY to identify pathology tests with comparators commonly attached to result values. For each te
