Methionine challenge test: methyl mercaptan (CH₃SH) response in periodontitis.

Halitosis, frequently associated with volatile sulfur compounds (VSCs) produced by oral microbiota, affects a large proportion of adults. Among VSCs, methyl mercaptan (CH₃SH) is a critical biomarker for periodontitis-related halitosis due to its strong correlation with periodontal pocket depth and attachment loss. This study investigated the utility of a methionine challenge protocol to selectively stimulate CH₃SH production and enhance the standardization of oral air-based screening for periodontal disease. Thirty adults were enrolled and divided equally into control and periodontitis groups.

Exploring the potential of extracellular matrix modulation to modify oral biofilm architecture.

Biofilms are well-organized, surface-attached colonies of microorganisms that can thrive in host cavities. A balanced, diverse mix of protective microbes in these biofilms helps preserve host health. Dysbiosis in biofilms is responsible for consequential diseases in associated tissues. Dietary factors and other xenobiotics can cause ecological dysbiosis in the oral cavity that underpins dental caries, periodontal diseases, halitosis, and periapical infections. Although the gold standard for oral biofilm elimination remains mechanical removal, it is achieved with manual curettage or an ultrason

Stannous Fluoride Toothpaste: Improving gingival health without a new habit adoption.

Purpose Dental hygienists are uniquely positioned to improve oral and overall health outcomes with evidence-based treatment plans. One recommendation that has yet to see its full potential is the use of stannous fluoride (SnF2) toothpaste. Stannous fluoride is more than just an anticaries agent. Its formulation within a toothpaste offers superior performance in reducing plaque, gingivitis, sensitivity, halitosis, and tooth erosion, while maintaining the sulcular barrier integrity. However, most patients are unaware of these easily available benefits for oral health. This short report highlight

The oral cavity as a bioreactor in halitosis.

Halitosis is a prevalent yet often underestimated clinical condition with multifactorial etiology, predominantly arising from intra-oral sources such as periodontal disease, tongue coating and xerostomia. Central to its pathogenesis is the microbial degradation of sulfur-containing substrates, which lead to the production of volatile sulfur compounds and other volatile organic compounds responsible for the characteristic odor. These metabolites are produced by proteolytic anaerobes inhabiting the tongue dorsum, periodontal pockets as well as the dental plaque and predominantly exist as structu

A brief xerostomia inventory for young adults.

OBJECTIVE: Xerostomia (dry mouth) is a common condition that negatively impacts life quality for adults of all ages. Yet, xerostomia has primarily been studied among older adults and populations at high risk of dry mouth, with no widely-available self-reported instrument to assess xerostomia accurately among younger populations. This study aimed to develop a brief xerostomia assessment tool specifically designed for young adults. METHODS: Students ages 18-24 at two California (USA) public universities (N = 278) completed a cross-sectional survey that included a 14-item xerostomia inventory, i

A Chewing Gum Containing Heyndrickxia coagulans SNZ1969® and Volatile Sulphur Compounds: An Exploratory Secondary Analysis of a Double-Blind Randomized Controlled Trial.

Background: Chewing gum may reduce oral malodor through salivary stimulation and may also serve as a carrier for oral probiotics. Evidence on Heyndrickxia coagulans in this setting is lacking. Methods: This exploratory secondary analysis used data from a randomized, blinded, placebo-controlled study in which 52 healthy adults were assigned to a sugar-free gum with H. coagulans SNZ1969® or to a matched placebo gum for four weeks. Hydrogen sulphide (H2S) and methyl mercaptan (CH3SH) were measured by OralChroma™ at baseline (T0), during intervention (T1-T2), and one week after gum discontinuation

Halitosis: A Prospective Cohort Study Comparing Periodontal Debridement with Tongue Cleaning, Denture Biofilm Management, and Restorative Maintenance.

Background: Halitosis in elderly dental patients is rarely attributable to a single etiology; tongue coating, periodontal inflammation, denture biofilm, and reduced salivary flow may coexist and modify treatment response. We compared three procedure pathways and examined whether tongue-coating reduction mediates procedure-related improvement. Methods: A prospective cohort of 94 patients aged 65 years or older was assigned by dominant indication to periodontal debridement with tongue cleaning (PDT, n = 33), denture biofilm management (DBM, n = 31), or restorative/prosthetic maintenance (RPM, n

Gastrointestinal manifestations and enzyme replacement therapy in late-onset Pompe disease: insights from a cross-sectional analysis.

BACKGROUND: Late-onset Pompe disease (LOPD) is an autosomal-recessive disorder caused by acid α-glucosidase (GAA) deficiency, typically presenting after the first year of life and resulting in systemic glycogen accumulation. Beyond motor and respiratory manifestations, gastrointestinal (GI) symptoms have been increasingly reported. Although enzyme replacement therapy (ERT) improves skeletal and respiratory function, its impact on GI symptoms remains unclear. METHODS: There were 124 LOPD patients recruited to complete the Gastrointestinal Symptom Rating Scale (GSRS), together with questionnaire

The Effect of a Novel Natural Oil-Pulling Oral Rinse on Oral Microflora, Dental Plaque, and Gingivitis: A Double-Blinded Randomized Controlled Clinical Trial.

AIM: Oral health conditions such as plaque accumulation, gingivitis, and halitosis are primarily caused by pathogenic oral microflora and remain a global concern due to their links with systemic diseases. While essential oil-based mouthwashes are effective, concerns over side effects such as dryness and taste alterations have spurred interest in natural alternatives such as oil-pulling rinses. This randomized, double-blind, placebo-controlled clinical trial evaluated the efficacy and safety of a novel oil-pulling oral rinse compared to an alcohol-based anti-gingivitis mouthwash and placebo in

The Burden of Chronic Rhinosinusitis Without Nasal Polyps: A Systematic Review of Epidemiological, Clinical, Humanistic, and Economic Evidence.

Chronic rhinosinusitis (CRS) is a heterogeneous and often debilitating disease characterized by persistent sinonasal symptoms and inflammation and is phenotypically classified as CRS with nasal polyps (CRSwNP) or without nasal polyps (CRSsNP). Although CRSsNP is the more prevalent phenotype (approximately 80%), it remains disproportionately understudied, leading to critical gaps in understanding and management. This systematic literature review synthesized current global evidence on the epidemiological, clinical, humanistic, and economic burden of CRSsNP, highlighting areas of unmet need and r

[Research Progress on Microbiome-Driven Mechanisms and Intervention Strategies for Oral Malodor].

Oral malodor, a common oral symptom, is primarily caused by the metabolic activities of oral microorganisms. The characteristic odors mainly originate from volatile sulfur compounds produced and released by oral bacteria (such as Fusobacterium nucleatum and Porphyromonas gingivalis) through specific enzymatic systems that break down sulfur-containing amino acids in saliva, gingival crevicular fluid, and food debris. Research indicates that various factors, including periodontitis, fungal infections, smoking, and obesity, can worsen oral malodor by altering the oral microenvironment. For preven

Halitosis: An 8-Type Clinical Classification With Integrated Diagnostic and Therapeutic Algorithms.

Misclassification of halitosis frequently results in ineffective treatment. A clinical classification system is presented that organizes halitosis into eight types based on etiology and clinical presentation. Intraoral causes include tongue biofilm, periodontal disease, peri-implant disease, and xerostomia-associated halitosis, while extraoral causes comprise airway related conditions and metabolic/systemic disorders with pulmonary elimination of bloodborne compounds. Perceptual conditions are categorized as pseudohalitosis and halitophobia. A diagnostic workflow is proposed to guide clinician

Superiorly based nasolabial flap for closure of oronasal fistula: a case report.

Oronasal fistula (ONF) is an epithelialized tract between the oral and nasal cavities, for which various flaps are used in surgical closure. ONF may lead to regurgitation of food into the nasal cavity, nasal obstruction, halitosis, and other symptoms that contribute to patient discomfort and impaired quality of life. Surgical closure is challenging, occasionally requiring multiple procedures and carrying a risk of recurrence. The nasolabial flap (NLF) is a well-established local flap for soft tissue reconstruction in the orofacial region; superiorly based flaps are typically used for defects o

Dietary Sulfur Compounds and Halitosis: Bridging Food Science, Microbial Metabolism, and Oral Health: A Comprehensive Review.

Intraoral halitosis is predominantly caused by anaerobic microbes within the tongue biofilm that break down sulfur-containing amino acids, especially cysteine and methionine, to volatile sulfur compounds (VSCs). In addition to microbial activity, there is growing evidence to suggest that dietary factors are able to influence VSC formation by affecting substrate availability, redox equilibrium, and oral ecological stability and that some of these effects are due to extraoral metabolic activity. This review integrates food chemistry, microbial ecology, and oral health to explain how dietary expo

A Rare Coexistence of Gastric Fundal False Diverticulum and Sliding Hiatal Hernia: Technical Considerations and Literature Review.

Gastric fundal false diverticula are exceedingly rare, and to our knowledge, this is the first reported case occurring in association with a sliding hiatal hernia. We describe a 49-year-old woman who presented with persistent dysphagia, regurgitation, heartburn, and halitosis refractory to medical therapy. Imaging revealed a 5.0 × 3.6 × 4.7 cm false diverticulum at the posteromedial fundus and a sliding hiatal hernia. Esophageal manometry demonstrated normal motility with ineffective bolus clearance. Laparoscopic management was employed, combining intraoperative endoscopy for diverticulum loca
