Refined Vertical Neck Lift With Crisscrossed Platysma Traction Sutures: A Simple Method for Improving Mandibular Neck Definition in Face Lifts.

INTRODUCTION: Jawline and upper neck definition are crucial for aesthetically pleasing outcomes in face lifts. Still, neck rejuvenation is challenging with current techniques which can be either invasive or have minimal and short-term effects. Building upon known methods, we report on a series of patients utilizing a modified suturing technique to perform neck lifting and jaw line contouring based on submental crisscrossed platysma traction sutures. This technique is safe, reproducible and can be combined with any facelift procedure. METHODS: This series included 154 patients who underwent fa

Factors associated with severity and outcomes of diabetic foot ulcers: A single center hospital-based cross-sectional observational study in Eastern India.

BACKGROUND: Diabetes mellitus (DM) affects millions globally and is associated with high morbidity, risk of infection, and potential for severe outcomes. In India, where the prevalence of diabetic foot complications is notably high, data on factors influencing the severity and outcomes of diabetic foot ulcers (DFUs) in specific regions, particularly Eastern India, remain sparse. METHODS: This hospital-based cross-sectional study included Type 2 DM patients aged over 18 years, excluding those unwilling to participate and those with ulcers classified as Wagner grade less than 2. The study invol

Pearls & Oy-sters: Trigeminal Trophic Syndrome From Lateral Medullary Stroke With Successful Multimodal Treatment.

We describe an adult patient who developed widespread skin ulceration in the left trigeminal nerve distribution due to a Wallenberg syndrome from an acute ischemic stroke in the left lateral medulla. Trigeminal trophic syndrome is a rare consequence of a trigeminal nerve pathway injury characterized by the triad of persistent ulceration, anesthesia, and paresthesia of the same trigeminal dermatomes. It is an unusual complication of stroke that most likely developed in this patient from a combination of neurotrophic skin changes worsened by self-inflicted injury from scratching in a hypoestheti

Reducing complications using minimally invasive retrosigmoid approach for microvascular decompression in patients with trigeminal neuralgia.

BACKGROUND: The most effective treatment for a typical trigeminal neuralgia is microvascular decompression of the trigeminal nerve. An approximately 3-5 cm in diameter retrosigmoid craniotomy with an approximately 12-14 cm long skin incision is considered the standard approach. Modern neurosurgery strives for minimally invasive surgical approaches, which in turn reduces both intraoperative and postoperative complications. The use of minimally invasive approaches in skull base neurosurgery and, in particular, in microvascular decompression of the trigeminal nerve can reduce the risk of complica

Multifocal discospondylitis and osteomyelitis due to Salmonella species in a cat.

CASE SUMMARY: A 1-year-old male castrated Savannah cat presented with chronic recurrent lethargy, stiffness, right pelvic limb lameness and spinal hyperaesthesia. Eight months before referral, the cat was treated with prednisolone, remdesivir and a 1-week course of amoxicillin/clavulanic acid and marbofloxacin for suspected feline infectious peritonitis. Multiple recurrences were reported after initial presentation, and were treated with 1-week courses of amoxicillin/clavulanic acid. Neurological examination did not reveal further findings. Haematology showed neutrophilia/monocytosis. Spinal,

Peripheral Neuromodulation for Treatment of Upper Extremity Complex Regional Pain Syndrome Following Peripherally Inserted Central Catheter Placement: A Case Report.

BACKGROUND: Complex regional pain syndrome (CRPS) is a chronic pain condition that typically affects distal extremities after an injury or noxious event, with symptoms disproportionate to the primary insult. We present the first successful use of a temporary peripheral nerve stimulator (PNS) followed by a permanent PNS system to treat CRPS Type 2 after failed responses to pharmacotherapies, trigger point injections, cervical epidural injections, and sympathetic nerve blocks. CASE REPORT: An 81-year-old patient developed CRPS Type 2 following a left antecubital peripherally inserted central ca

Epistaxis and Intradural-Extramedullary Haemorrhage in a Dog With Steroid Responsive Meningitis-Arteritis.

A 2-year-old female entire Golden Retriever with a history of being subdued was seen. Her physical and neurological examinations were initially unremarkable, but she acutely progressed to non-ambulatory paraparesis, with absent cervical or thoracolumbar hyperaesthesia. Magnetic resonance imaging of the vertebral column was performed, showing a well-defined, intradural-extramedullary mass at the level of the caudal aspect of L2 causing right-sided ventrolateral marked cord compression. The lesion was T2W hyperintense and mildly hyperintense on T1W images. The signal intensity pattern of this le

Association of Dyslipidemia With Dermatological Manifestations in Patients With Type 2 Diabetes Mellitus.

Objectives Type 2 diabetes mellitus (T2DM) is frequently related with various cutaneous manifestations, which may be influenced by comorbid conditions such as dyslipidemia. Hence, this study compared the demographic to match clinical and dermatological profiles of T2DM patients with and without dyslipidemia. Methodology This cross-sectional study was conducted at a secondary care hospital using a non-probability convenient sampling technique over six months. A total of 280 T2DM patients aged 40-65 years were included in this study. Patients were distributed into two equal groups: Group A (with

Acute limb ischemia secondary to left brachial artery occlusion in a 45-year-old woman: A case report.

RATIONALE: Acute limb ischemia is a vascular emergency that occurs when blood flow to a limb suddenly decreases, putting the limb at risk if not treated promptly. Acute limb ischemia accounts for 11% of all vascular emergencies, among which arm ischemia is relatively rare. It is caused by occlusion of the brachial artery, most commonly by vascular thromboembolism. We report a female patient with left upper limb ischemia induced by occlusion of the brachial artery. PATIENT CONCERNS: We report a 45-year-old female who presents with an acute onset of severe pain, numbness, and pallor in her left

Ultrasound-guided Percutaneous Ulnar Nerve Release at the Elbow: A Stepwise Approach.

Cubital tunnel syndrome is the second most common compressive neuropathy in the upper limb. A wide range of surgical techniques have been described, from simple in situ decompression to anterior transposition, both through open and endoscopic approaches. Recent advances in ultrasonography and the development of dedicated instrumentation have enabled the emergence of ultrasound-guided cubital tunnel release as a feasible, effective, minimally invasive alternative. However, concerns regarding potential complications and inconsistent outcomes have limited widespread adoption. The aim of this stud

Management of Small Fiber Neuropathy: A Clinical Perspective.

Small fiber neuropathy (SFN) is a common neurological diagnosis with a multitude of symptoms, including dysautonomia symptoms (e.g., orthostatic dizziness) as well as sensory disturbances, such as a tingling sensation or burning pain. The disease can be due to metabolic, autoimmune, or hereditary factors, and treatment plans stem from SFN etiology. Diabetes, autoimmune disease, infection, vitamin deficiencies, post-vaccination syndromes can all cause SFN. Needle electromyography and nerve conduction studies can be performed to exclude other neuromuscular conditions by ruling out large fiber ne

Peripheral Nervous System Complications after COVID-19 Vaccination.

While vaccination against severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) remains crucial, neurological complications have been detected following the coronavirus disease 2019 (COVID-19) vaccination. The neurological complications of COVID-19 vaccination can be seen in both the central nervous system (CNS) and the peripheral nervous system (PNS). In this study, we reviewed PNS complications after COVID-19 vaccination, their underlying mechanisms, diagnosis, and management. Inflammatory polyneuropathy, small fiber neuropathy, Parsonage-Turner syndrome (PTS), cranial mononeuropathie

IgE elevation temporally associated with dermal Glufosinate ammonium exposure in an elderly patient: A case report from Vietnam.

Glufosinate ammonium (GLA) is a nonselective herbicide of the phosphinic acid group, which is widely used in agriculture. Dermatologic toxicity has been rarely reported, especially in the elderly. We present the case of a 94-year-old female patient who developed irritant contact dermatitis and paresthesia after using her feet to stir 20 vials of Glufosinate ammonium herbicide in water. The patient presented with pruritus, numbness of the extremities, and a very high immunoglobulin E (IgE) elevation (855.5 IU/mL), which gradually decreased over time. This is the first report in Vietnam describi

Oral prolonged-release dienogest 2 mg and ethinylestradiol 0.02 mg in a 24/4-day regimen for polycystic ovary syndrome-associated hirsutism: a double-blind, randomised, placebo-controlled trial.

BACKGROUND: Current guidelines for polycystic ovary syndrome (PCOS) recommend combined oral hormonal contraceptives as first-line pharmacological treatment to reduce androgen excess and to treat clinical hyperandrogenic skin manifestations, such as hirsutism. However, these are used off-label. We aimed to demonstrate the efficacy of a prolonged-release oral formulation in a 24/4-day regimen for up to nine 28-day consecutive cycles compared with placebo for the clinical management of hirsutism in women with PCOS. METHODS: This was a multinational, multicentre, double-blind, placebo-controlled

A Rare and Atypical Manifestation of Intraosseous Hemangioma in the Zygomatic Bone.

Intraosseous hemangiomas (IH) are rare intrabony lesions that represent less than 1% of intraosseous tumors. IH are mostly seen in the axial skeleton and skull. Most commonly, the frontal bone, zygomatic, sphenoid, maxilla, ethmoid, and lacrimal bone can manifest IH. Currently, IH is classified as a developmental condition of endothelial origin. According to WHO, the five histological types of IH are cavernous, capillary, epithelioid, histiocytoid, and sclerosing. IH of the zygoma is an extremely rare condition with female predominance. A systematic review recently estimated that there were 78

Functional and Aesthetic Outcomes of the Subtarsal Approach in Zygomaticomaxillary Complex Fractures With Orbital Floor Involvement: A Prospective Cohort Study.

Introduction Zygomaticomaxillary complex (ZMC) fractures, often resulting from high-impact trauma, frequently involve the orbital floor, leading to functional and aesthetic complications such as diplopia, enophthalmos, and facial asymmetry. The subtarsal approach, which involves an incision through the skin, provides access to the orbital floor while potentially reducing complications. This study aimed to evaluate the efficacy and safety of the subtarsal approach in managing ZMC fractures with orbital floor discontinuity, focusing on functional outcomes (nerve paresthesia, ocular movement, and

Homoeopathic Approach in the Treatment of Postherpetic Neuralgia: A Case Report.

INTRODUCTION: The varicella zoster virus, which also causes chicken pox in unvaccinated individuals, is the cause of herpes zoster. Vaccinated individuals with partial protection acquired against the virus during a previous chicken pox episode may be susceptible to herpes zoster. When affected by chicken pox, the virus lodges in the posterior nerve root ganglion. From then on, anytime the patient encounters a decrease in cell-mediated immunity during their lifetime, the patient is at a risk of developing an acute eruption that appears like tense vesicles and occurs in groups with surrounding e

Detection of sensory deficits in fine nerve fibres in leprosy diagnosis.

OBJECTIVES: The purpose of this study was to determine reference points for thermal perception in cutaneous lesions of leprosy, a disease caused by Mycobacterium leprae characterised by hypoesthesia in skin lesions due to nerve and Schwann cell infection. Early diagnosis is essential to control transmission and effectively treat the disease. METHODS: Quantitative thermal testing (QTT) has been proposed as a valuable tool for early detection of the disease, initiation of treatment, and monitoring of nerve damage. A thermal analyser was used to determine warm and cold perception thresholds (WPT
