Activation of the Wnt signaling pathway and its role in epithelial-mesenchymal transition and hepatic fibrosis in alveolar echinococcosis.

OBJECTIVE: This study examined the effects of Echinococcus multilocularis infection on the activation of the Wnt signaling pathway in hepatocytes, its association with epithelial-mesenchymal transition (EMT), and its role in E. multilocularis-induced hepatic fibrosis. METHODS: Hepatic lesion tissues were obtained from 20 patients with clinically diagnosed alveolar echinococcosis (AE). These tissues were categorized into near-lesion and distant-from-lesion groups. Additionally, a murine model of AE infection was developed through the injection of E. multilocularis protoscoleces. The mice were

Settlement characteristics and transmission of echinococcosis: a cross-sectional study in nomadic communities on the Qinghai-Tibet Plateau, China.

BACKGROUND: Echinococcosis remains highly endemic in some nomadic communities on the Qinghai-Tibet Plateau, China, where alveolar echinococcosis (AE) and cystic echinococcosis (CE) exhibit notably high prevalence rates of 3.64% and 2.37%, respectively. Recent settlement expansion in the region has raised concerns, as smaller, remote settlements often lacked waste disposal and sewage systems, potentially facilitating echinococcosis transmission. The aim of this study is to investigate how settlement characteristics influence echinococcosis transmission. FINDINGS: The study was conducted from 2

First human infection and management of Versteria rafei infection restricted to the liver with phylogenetic analysis in the context of known cases.

A recently described cestode genus, Versteria, has been implicated in disseminated human disease in North America. We report the first known case of infection with a recently reported species, Versteria rafei, localised to the liver with attempted surgical resection. Through review of other patients known to be infected with this parasite, we established that this is a rare zoonotic infection involving contact with minks and mustelids. Although Versteria belongs to the same family as Echinococcus, there are several important differences. Known Versteria infections in humans typically are sympt

EmsB Microsatellite Analysis of Echinococcus multilocularis Specimens Isolated from Belgian Patients with Alveolar Echinococcosis and from Animal Hosts.

Alveolar echinococcosis (AE), caused by Echinococcus multilocularis (E. multilocularis), is a severe parasitic zoonosis that is potentially fatal for humans. The parasite is primarily transmitted by wildlife, with red foxes acting as definitive hosts and rodents as intermediate hosts, while humans can become accidental but dead-end hosts. The aim of this study is to use EmsB typing on E. multilocularis isolates from human AE cases and local animals such as foxes and rodents. In this study, retrospective EmsB typing was performed on 39 samples, including 11 tissue samples from 10 patients, 18 f

Preliminary Proteomic and Metabolomic Analyses Reveal Potential Serum Biomarkers for Identifying Alveolar Echinococcosis in Mice.

Alveolar echinococcosis (AE) is a chronic and potentially fatal zoonotic parasitic disease that seriously affects the host's health. It is caused by the proliferation of Echinococcus multilocularis larvae within the liver. Due to its long incubation period following host infection, early diagnosis of the disease is currently not feasible. Treatment options are extremely limited, with the only choice being curative surgical resection combined with benzimidazole medication. Thus, the development of early, rapid, and minimally invasive diagnostic methods is crucial for enhancing patient prognosis

Unveiling the incidences and trends of alveolar echinococcosis in Europe: a systematic review from the KNOW-PATH project.

Human alveolar echinococcosis is a notifiable parasitic infectious disease in most European countries; however, in practice, it is under-reported by national health systems. To fill this knowledge gap, data on the number, incidence, and trend of cases in Europe were extracted through a systematic review approach from both the scientific and grey literature, covering 1997-2023. This systematic review identified 4207 human alveolar echinococcosis cases from 28 of the 40 European countries investigated. Historically endemic Austria, France, Germany, and Switzerland accounted for 2864 (68·08%) of

First detection of Echinococcus multilocularis in golden jackals (Canis aureus) in Bosnia and Herzegovina.

Echinococcus multilocularis, the causative agent of alveolar echinococcosis (AE), is a parasitic zoonosis of increasing public health significance in Europe. Although previously confirmed in red foxes in Bosnia and Herzegovina, this study provides the first report of E. multilocularis in golden jackals (Canis aureus) in the country. Between December 2024 and March 2025, a total of 44 golden jackals were examined across 15 localities in Bosnia and Herzegovina, with E. multilocularis detected in 6.8 % of the samples from three sites in Western and Central Bosnia and Herzegovina. Adult Echinococc

Simultaneous combined surgery for hepatic-renal double organ alveolar or cystic echinococcosis: A retrospective study.

BACKGROUND: Alveolar and cystic echinococcoses are lethal zoonotic diseases caused by Echinococcus multilocularis and Echinococcus granulosus infections, leading to alveolar echinococcosis (AE) or cystic echinococcosis (CE), respectively. No study has hitherto reported effective treatment approaches for AE or CE with concurrent hepatorenal involvement. AIM: To investigate the feasibility and efficacy of simultaneous combined surgery (SCS) as a comprehensive treatment approach for patients with hepatorenal echinococcosis. METHODS: Clinical datasets of hepatorenal AE (n = 10) and CE (n = 11) p

IDO1 promotes Echinococcus multilocularis infection by regulating the formation of neutrophil extracellular traps.

The widespread prevalence of alveolar echinococcosis (AE) caused by Echinococcus multilocularis infection poses a significant threat to human health. E. multilocularis is found primarily in the Northern Hemisphere. Given the limitations of current treatment methods, primarily surgical resection, there is a pressing need for more effective therapeutic options. We established a mouse model of E. multilocularis infection by injecting E. multilocularis protoscoleces into C57BL/6 mice. The formation of neutrophil extracellular traps (NETs) following E. multilocularis infection was identified and va

Molecular survey of taeniid cestodes with special emphasis on Echinococcus species in free-roaming dogs and wild carnivores in Armenia.

Echinococcosis, caused by cestodes of the genus Echinococcus, poses significant public health and veterinary concerns globally. In Armenia, cystic echinococcosis (CE) is well-documented in livestock and humans, while alveolar echinococcosis (AE) has long been considered non-endemic. However, a recent retrospective study identified human AE cases, suggesting an underestimation of the parasite's presence. To address knowledge gaps, a pilot survey was conducted to identify Echinococcus species and other taeniids in free-roaming dogs and wild carnivores in Armenia. Fecal samples (n = 112) were opp

Parasite diversity in grey wolves (Canis lupus) from Tuscany, central Italy: a copromicroscopical investigation.

Parasite monitoring is considered an important issue for wildlife conservation as well as for veterinary and public health. In Italy, data on endoparasites of the grey wolf (Canis lupus) are still limited. A copro-microscopical study was performed to investigate endoparasites of grey wolf communities in three Apennine natural reserves and a hilly area in northern Tuscany, central Italy. A total of 66 fresh faecal samples were collected and examined using the Mini-FLOTAC technique with a high-density flotation solution. Apparent prevalence and 95 % confidence intervals (CI) were estimated for e

Chasing intermediate hosts of Echinococcus multilocularis at the southern edge of its European distribution using red fox stomach content analysis.

Echinococcus multilocularis (Em) is a Taeniidae cestode circulating among canids (definitive hosts) and among voles (intermediate hosts). Humans can develop alveolar echinococcosis (AE) following egg ingestion. In Italy, Em is present in red foxes (Vulpes vulpes) from the Trentino-Alto Adige/Südtirol Region, with reports in canids from Liguria (wolf, Canis lupus, and domestic dog, Canis lupus familiaris) and Tuscany (wolf and red fox) Regions. The first autochthonous case of human AE was recently confirmed in the Province of Bolzano. Despite its relevance, the intermediate hosts maintaining th

First molecular confirmed patient of alveolar echinococcosis in India: Case report from a tertiary care hospital in North India.

Alveolar echinococcosis, caused by Echinococcus multilocularis, is rare in India. We present the first molecularly confirmed AE case from Kashmir, India who presented with a hepatic mass mimicking metastatic malignancy. Imaging revealed a large liver lesion with intrahepatic biliary obstruction. Cytological examination showed necrosis and laminated membranes without hooklets or brood capsules. Polymerase chain reaction (PCR) identified E. multilocularis. The patient underwent albendazole therapy and right hepatectomy, but her condition deteriorated due to post-hepatectomy liver failure, leadin

CD155 as a therapeutic target in alveolar echinococcosis: insights from an Echinococcus multilocularis infection mouse model.

INTRODUCTION: Alveolar echinococcosis (AE) is a life-threatening zoonotic parasitic disease caused by the metacestode stage of Echinococcus multilocularis, characterized by granulomatous lesions and liver fibrosis. Immune exhaustion is the key mechanism by which E. multilocularis evades host immune responses and maintains long-term parasitism. Although CD155 is recognized as an immune checkpoint molecule, its specific role and underlying mechanism in AE remain unclear. METHODS: A mouse model of E. multilocularis infection was used to investigate the role of CD155 in AE progression. Flow cytom

The initial stage of alveolar echinococcosis is a diagnostic challenge: a case report.

BACKGROUND: Alveolar echinococcosis is a rare, potentially fatal parasitosis with the main manifestation site in the liver. Diagnosis already in the initial stage of the disease is important to prevent further exacerbation and possible secondary complications by early targeted therapy. Identifying alveolar echinococcosis lesions on imaging can be difficult, and making the diagnosis can be an interdisciplinary challenge, even in a specialized center. CASE PRESENTATION: In a clinically symptom-free 65-year-old white female patient with type 2 diabetes mellitus, an abdominal ultrasonography perf

Echinococcosis on the Tibetan Plateau, where to go?

The post-COVID-19 era has exacerbated challenges in controlling echinococcosis on the Tibetan Plateau, the epicentre of alveolar and cystic echinococcosis, where reduced funding for neglected tropical diseases (NTDs) coincides with growing tourism and trade. This convergence heightens transmission risk, and we provide a novel synthesis of context-specific, integrated control strategies.

Seasonal population dynamics and dietary switching of Vulpes spp. amplify Echinococcus spp. transmission in the Eastern Tibetan plateau: implications for wildlife-mediated zoonotic risks.

BACKGROUND: Echinococcosis, a severe zoonosis caused by Echinococcus spp., poses a significant public health challenge in the eastern Tibetan Plateau. This study aimed to investigate the interplay among seasonal shifts in definitive host ecology (Vulpes ferrilata and V. vulpes), dietary shifts, and infections with E. multilocularis and E. shiquicus within a high-altitude ecosystem. RESULTS: Statistical analyses revealed that V. ferrilata dominated the local fox community (90.48%, n = 441), with the highest number of samples collected in fall (46.49%), likely linked to dispersal behaviors afte

Acute peritoneal propagation of alveolar echinococcosis in a 12-year-old child.

Herein we present a case of a 12-year-old child with acute symptoms (abdominal pain, fever). Preliminary imaging suggested pyogenic liver abscess. Despite the broad-spectrum antibiotic therapy, which was started after hospital admission, no improvement was perceived. Rising eosinophilia and multiplex focal lesions detected by ultrasound and MRI forced serological investigation by which Echinococcus granulosus s.l. seropositivity was detected. Antihelminthic therapy was initiated and upon multidisciplinary consultation surgical intervention was performed with the removal of a cystic lesion whic

A high-throughput LC-MS/MS method for simultaneous analysis of albendazole, albendazole sulfoxide and albendazole sulfone in human plasma.

Albendazole is a benzimidazole derivative with a broad spectrum of antihelmintic activity. Its primary metabolite, albendazole sulfoxide, is responsible for the treatment's efficacy. On the other hand, it contributes to side effects and toxicity. Therefore, their quantification in plasma may increase therapeutic success and reduce associated risks by maintaining values within the therapeutic range. The present study introduces an LC-MS/MS method for the simultaneous quantification of albendazole, albendazole sulfoxide, and albendazole sulfone in human plasma. A simple and sensitive LC-MS/MS me
