Hunting and Stray Dogs as Reservoirs of Echinococcus Species.

In Serbia, human echinococcosis cases are predominantly diagnosed as the cystic form, caused by Echinococcus granulosus sensu lato (s. l.) complex tapeworms. Alveolar echinococcosis, a more severe disease caused by Echinococcus multilocularis, emerged recently. Domestic dogs have been implicated in the transmission of E. granulosus sensu stricto G1/3 to humans. In an ongoing screening of definitive hosts for the presence of Echinococcus spp. to discover reservoirs of E. multilocularis, fecal samples of hunting (n = 29) and stray dogs (n = 66) were examined. Taeniid eggs were collected using fl

Magnetic Resonance Imaging Features of Hepatic Hydatid Disease: A Pictorial Review with Emphasis on Atypical Presentations and Differential Diagnosis.

Hepatic echinococcosis, caused by Echinococcus species, remains a significant global health concern, with cystic echinococcosis (CE) being widespread and alveolar echinococcosis (AE) representing a rarer but more aggressive form. CE generally demonstrates characteristic imaging features, allowing straightforward diagnosis, whereas atypical presentations can closely mimic other hepatic lesions, leading to diagnostic uncertainty. AE poses an even greater diagnostic challenge due to its infiltrative, tumor-like growth pattern. Magnetic resonance imaging (MRI), with its superior soft-tissue contra

The spread of Echinococcus multilocularis in the Balkans is facilitated by the golden jackal.

INTRODUCTION: Human alveolar echinococcosis (AE), caused by the zoonotic tapeworm Echinococcus multilocularis, is an emerging disease in the Balkans. The presence of this tapeworm has been detected in several wildlife species, including the golden jackal, a confirmed definitive host. The golden jackal is distributed throughout the Balkans, with a large resident population in Serbia. The aim of this study was to identify Echinococcus spp. circulating in golden jackals, identify regions with high occurrence of Echinococcus multilocularis and gain insight into the geographical spread of this tape

Drug Repurposing Against Cestode Fatty Acid Binding Proteins Through Integrated Virtual Screening and In Vitro Validation.

Echinococcosis is a neglected tropical disease (NTD) caused by Echinococcus granulosus and Echinococcus multilocularis, the etiological agents of cystic and alveolar echinococcosis, respectively. These infections pose a significant public health burden, particularly in endemic regions. Cestodes lack key enzymes involved in lipid metabolism and must acquire lipids from their hosts. Fatty-acid binding proteins (FABPs), which mediate lipid trafficking and intracellular transport, have therefore emerged as essential and potentially druggable targets. In this study, we implemented an integrated vir

Clinical Challenges in Echinococcosis Treatment and Frontiers in New Drug Development: Current Status and Future Outlook.

Echinococcosis, a zoonotic disease caused by larval Echinococcus tapeworms, manifests primarily as Cystic Echinococcosis (CE) and Alveolar Echinococcosis (AE). It poses a serious threat to the global livestock industry and public health. Based on a systematic review of the disease's epidemiological features, pathogen biology, life cycle, and clinical management, this review systematically assesses the progress, core strengths, and key translational barriers of four mainstream anti-echinococcosis drug development avenues over the past five years. It was emphasized that the in vivo and in vitro

Locally Aggressive Hepatic Echinococcosis With Colonic Involvement Requiring a Right Hemicolectomy: A Case Report and Focused Literature Review.

Echinococcosis most commonly presents as a slowly enlarging hepatic cyst, whereas extensive inflammatory extension with colonic involvement is uncommon. We report the case of an 80-year-old man who presented with abdominal pain and fever and reported having been aware of a hepatic echinococcal cyst for approximately 30 years. CT showed a giant calcified hepatic cyst containing an air-fluid level and intralesional gas, suggestive of secondary infection; intraoperative cyst-fluid culture subsequently grew Escherichia coli. Emergency laparotomy revealed dense inflammatory adherence to the transve

The establishment of a universal blocking ELISA for echinococcosis using immunoglobulin against a new conserved linear B-cell epitope.

Echinococcosis is a neglected tropical disease that poses a severe threat to global public health and socioeconomic development. Given its alarming global spread, there is an urgent need to develop accurate diagnostic methods to support targeted and personalized treatment. Echinococcus multilocularis Em18 has been demonstrated to exhibit the most prominent diagnostic relevance in the progression of alveolar echinococcosis (AE). In this study, six highly specific monoclonal antibodies (mAbs) against the Em18 protein-designated 8G6F, 11E2E, 11E7C, 11E7E, 11E8C, and 11E10D-were generated using hy

Host genetic regulation of immune tolerance and disease persistence in human echinococcosis.

Human echinococcosis, caused by Echinococcus spp., remains a significant public health concern in endemic regions and manifests primarily as cystic echinococcosis (CE) and alveolar echinococcosis (AE), both characterized by chronic infection and heterogeneous clinical outcomes. This review synthesizes current evidence on host genetic determinants that influence immune responses, susceptibility, and disease persistence in CE and AE. Available studies indicate that polymorphisms in genes encoding key immunoregulatory cytokines, particularly IL10 and TGFB1, are associated with an anti-inflammator

Screening for alveolar echinococcosis in household members of affected patients: A cross-sectional study with a pooled comparison to endemic population screenings.

BACKGROUND: Alveolar echinococcosis (AE) is an emerging and potentially fatal zoonosis caused by accidental ingestion of infective eggs of the cestode Echinococcus multilocularis. Household members of AE patients share the same human-animal-environment interface and may therefore have similar risks for exposure to infective eggs. METHODS: We conducted a cross-sectional study at the University Hospital of Ulm, Germany. Unrelated household members of AE patients who shared the same household for ≥5 years prior to the patient's diagnosis were invited for screening with serological testing and ab

Minimally invasive versus open liver resection for hepatic alveolar echinococcosis within a disease-control oriented surgical strategy: a single-center retrospective study.

BACKGROUND: The surgical strategy for hepatic alveolar echinococcosis (AE) differs from that for typical malignant tumors, as treatment is not solely determined by the feasibility of complete (R0) resection. While complete resection remains the preferred goal, cytoreductive surgery combined with albendazole therapy is considered in selected cases. The role of minimally invasive surgery (MIS) within this disease-control oriented strategy remains unclear. METHODS: We retrospectively analyzed 126 patients who underwent liver resection for hepatic AE between January 2010 and March 2026: 87 underw

Poly(hydroxybutyrate) nanoparticle-encapsulated albendazole enhances in vivo efficacy against the Echinococcus multilocularis germinal layer.

Alveolar echinococcosis (AE), caused by the larval stage of Echinococcus multilocularis, is a severe zoonosis requiring prolonged anthelmintic therapy. Albendazole (ABZ) remains the treatment of choice; however, its poor aqueous solubility and low systemic bioavailability limit therapeutic efficacy and necessitate prolonged high-dose regimens. Improving the delivery of existing antiparasitic drugs therefore represents a valuable therapeutic strategy. This study evaluated the in vivo chemoprophylactic and therapeutic efficacy of an innovative nanomedicine formulation comprising ABZ encapsulated

Preparation of monoclonal antibodies against Echinococcus multilocularis CIP2A and development of blocking and sandwich ELISAs.

Alveolar echinococcosis (AE), caused by the metacestode stage of Echinococcus multilocularis, is a highly lethal zoonosis. Its insidious progression frequently precludes diagnosis until advanced stages, and the paucity of effective early detection tools remains a critical impediment to disease control. In this study, we aimed to evaluate the diagnostic utility of EmCIP2A as a target antigen and develop monoclonal antibody-based tools for the serological and non-invasive detection of E. multilocularis infection. Six monoclonal antibodies (mAbs) were generated against EmCIP2A, a germinative cell
