Ex vivo Liver Autotransplantation for Alveolar Echinococcosis with Brain and Lung Metastases: A Case Report.

Ex vivo liver resection and autotransplantation (ELRA) in extensive abdominal alveolar echinococcosis (AE) is a relatively rare treatment. We present the first case, ever reported in Kazakhstan, of successfully performed ELRA of a patient with extensive abdominal AE with invasion into the inferior vena cava (IVC). A 64-year-old woman was diagnosed with AE of the brain, alveococcectomy from the left frontal lobe was performed. At the same time, liver AE metastased to the lungs and brain was diagnosed. The patient was admitted to our center for the following surgical treatments: (1) laparotomy,

Host Proteins in Echinococcus multilocularis Metacestodes.

Metacestodes of Echinococcus multilocularis are the causative agents of alveolar echinococcosis, a neglected, life-threatening, zoonotic disease. To study these metacestodes in vitro, a model system using a culture medium conditioned by rat hepatoma cells is available. A key question is how the parasite interacts with the host and, in particular, which host-derived compounds are taken up. In this study, we focus on the uptake of host-derived proteins. Studies with artificially labeled proteins suggest that this uptake may occur independently of protein size or charge. Closer investigation usin

Progress on the HIF-1α/VEGF/VEGFR2 signal pathway in hepatic alveolar echinococcosis.

Alveolar echinococcosis (AE), a lethal parasitic zoonosis mimicking malignant tumors, progresses via hepatic infiltration and metastatic spread, causing multiorgan failure. Despite its clinical resemblance to cancer, molecular drivers of its aggressiveness remain poorly defined. Recent studies highlight perilesional angiogenesis as pivotal for lesion invasiveness, mediated by VEGF-driven pathological vascularization. VEGF not only fuels parasitic proliferation by creating nutrient-rich microenvironments but also engages crosstalk with host-parasite interactions, including immune evasion by Ech

Impact of Multimodal Fast-Track Protocols on Recovery in End-Stage Hepatic Alveolar Echinococcosis Patients Undergoing ex vivo Liver Resection and Autotransplantation: a Preliminary Study.

BACKGROUND: While ex vivo liver resection combined with autotransplantation has been reported from various perspectives, the use of fast-track protocols for this procedure in treating end-stage hepatic alveolar echinococcosis has not been documented. METHODS: We retrospectively analyzed the outcomes of 21 patients with end-stage hepatic alveolar echinococcosis who underwent ex vivo liver resection and autotransplantation, followed by a fast-track recovery protocol at our institution between 2014 and 2017. RESULTS: All patients successfully underwent liver autotransplantation with no intraope

Alveolar Echinococcosis: Is Non-Radical Liver Resection a Game Changer in the Current Treatment Approach to Patients with Advanced Alveolar Echinococcosis Disease? Single Center Experience of Poland.

BACKGROUND: Alveolar echinococcosis is parasitic disease caused by Echinococcus multilocularis and spread endemically in the northern hemisphere. Although alveolar echinococcosis is considered a rare disease, with approximately 18,000 new cases diagnosed annually, it continues to present significant diagnostic and therapeutic challenges. Alveolar echinococcosis affects the liver through slow, asymptomatic infiltration over many years. At the time of diagnosis, approximately 70% of cases are not eligible for radical lesion resection. The aim of the study was to evaluate whether non-radical surg

Hepatic alveolar echinococcosis with chest wall metastasis: a case report.

Alveolar echinococcosis (AE), a zoonotic parasitic disease caused by Echinococcus multilocularis infection, predominantly colonizes the liver and may metastasize to the lungs or brain in advanced stages. Involvement of extrapulmonary sites such as the chest wall or subcutaneous tissues is exceedingly rare, even in endemic regions. The nonspecific clinical manifestations and imaging features of chest wall AE pose diagnostic challenges, necessitating histopathological confirmation. We present a case of a 48-year-old female admitted with a chief complaint of a right supra-mammary mass persisting

Comprehensive Survival Analysis of Alveolar Echinococcosis Patients, University Hospital Zurich, Zurich, Switzerland, 1973-2022.

Alveolar echinococcosis (AE) is a zoonotic disease of increasing concern worldwide. Before benzimidazole drug therapy, 10-year death rates were 90% without surgical resection. In unresectable patients, long-term benzimidazole therapy is highly effective in stabilizing the disease course. We performed a retrospective study of 334 AE patients treated at the University Hospital Zurich, Zurich, Switzerland, during 1973-2022. Annual diagnoses increased over time, and more cases were detected by chance at earlier stages. Ninety patients died, mostly from causes unrelated to AE. Relative survival of

[Infectious cysts and pseudocysts: When parasites want to mimic tumors!].

Parasites can form true tissular cysts, with cavity(ies) delimited by a wall, covered by a specialized parasitic epithelium. In humans, these tissue cysts are caused by cestodes (segmented flatworms), which cannot complete their full cycle (e.g., in hydatidosis, alveolar echinococcosis or cysticercosis). Parasites can also form tissue pseudocysts, the wall of which consists of a resorptive granulomatous reaction to the presence of the parasite. In humans, these pseudocysts are caused by both nematodes (roundworms) and cestodes. These pathogens are rarely encountered by pathologists, and their

Regulation of Host Regulatory T Cell Differentiation by emu-let-7-5p in Echinococcus multilocularis Infection Through Targeting NFκB2.

Regulatory T cells (Treg) play a crucial role in creating an immunosuppressive microenvironment surrounding the metacestode during chronic alveolar echinococcosis (AE). However, the mechanisms by which E. multilocularis induces Treg differentiation, particularly the role of parasite-derived microRNAs (miRNAs), remain largely unexplored. Here, we demonstrate that E. multilocularis can significantly induce the differentiation of Treg in mice. Emu-let-7-5p is upregulated in peripheral blood lymphocyte cells (PBLC) and splenic lymphocytes of E. multilocularis-infected mice. Exosomes enriched with

Echinococcus multilocularis Calreticulin Inhibits Lectin Pathway of Complement Activation by Directly Binding to Mannose-Binding Lectin.

Alveolar Echinococcosis (AE) is a serious zoonotic disease caused by infection of Echinococcus multilocularis larvae. To survive within the host, E. multilocularis has developed a complex immune evasion mechanism including the inhibition of complement activation. This study focused on a calreticulin secreted by E. multilocularis (EmCRT) and its role in binding ability to human MBL and inhibiting MBL-mannose-mediated lectin pathway of complement activation. Results demonstrated the binding of recombinant EmCRT protein to both external and natural MBL in serum and the subsequent inhibition of MB

Double Trouble on the Lower Leg-Unique Human Coinfection with Echinococcus granulosus and Echinococcus multilocularis Without Liver Involvement.

The tapeworms Echinococcus granulosus and Echinococcus multilocularis cause two different clinical manifestations in humans: cystic echinococcosis (CE) and alveolar echinococcosis (AE), respectively. Both forms of echinococcosis manifest primarily in the liver, while other organs or tissues are less frequently affected. Simultaneous occurrence of CE and AE is extremely rare, and all previously reported patients exhibited affected livers, while simultaneous infection without liver involvement has not yet been described. Herein, we present an exclusively extrahepatic E. granulosus and E. multilo

Minimally Invasive Surgical Strategies for Alveolar Echinococcosis of the Liver.

BACKGROUND: Alveolar echinococcosis (AE) is a significant health concern worldwide, primarily affecting the liver. The standard treatment of surgical removal faces significant challenges due to the complex nature of AE. Despite advancements in pharmacological treatments, surgery complemented by at least 2 years of adjuvant medical treatment post-surgery remains the primary curative approach for AE. SUMMARY: The introduction of minimally invasive techniques, such as laparoscopic and robot-assisted surgery, has expanded surgical options, offering reduced recovery times and minimized surgical tr

Two-Stage Automatic Liver Classification System Based on Deep Learning Approach Using CT Images.

Alveolar echinococcosis (AE) is a parasitic disease caused by Echinococcus multilocularis, where early detection is crucial for effective treatment. This study introduces a novel method for the early diagnosis of liver diseases by differentiating between tumor, AE, and healthy cases using non-contrast CT images, which are widely accessible and eliminate the risks associated with contrast agents. The proposed approach integrates an automatic liver region detection method based on RCNN followed by a CNN-based classification framework. A dataset comprising over 27,000 thorax-abdominal images from

The value of nomogram analysis in predicting pulmonary metastasis in hepatic alveolar echinococcosis.

Hepatic alveolar echinococcosis (HAE) is a rare zoonotic parasitic disease that closely resembles malignant tumors in both behavior and appearance. It can cause infiltration of affected organs and chronic liver damage. In advanced stages, it may metastasize or invade surrounding organs, resembling liver cancer, and is clinically referred to as "parasitic cancer." However, the prognosis of HAE with pulmonary metastasis is poor, and no reliable method currently exists to predict lung metastasis. This study aims to investigate the efficacy of a nomogram model, based on CT and MRI imaging features

Application of 11C-CHO in the imaging of cerebral alveolar echinococcosis: A prospective study.

11C-acetylcholine (11C-CHO) is a positron emission tomography (PET) radiotracer that has been utilized in the diagnosis and treatment of prostate, esophageal, and lung cancers. This preliminary study aimed to assess the feasibility of using 11C-CHO PET/computed tomography (CT) for imaging cerebral alveolar echinococcosis (CAE). In a prospective single-center study, patients with CAE underwent 18F-fludeoxyglucose (18F-FDG) and 11C-CHO PET/CT imaging. The number and location of lesions detected by the 2 tracers were compared, along with determining the maximum standard uptake value (SUVmax) of t

Hepatic alveolar echinococcosis infection induces a decrease in NK cell function through high expression of NKG2A in patients.

UNLABELLED: Echinococcus multilocularis larval tapeworm infection in humans is considered a serious public health issue. The immune interaction between parasites and their hosts is extremely important. NK cells are known innate immune cells that play important roles during infection and tumour progression. However, the possible role of NK cells in hepatic alveolar echinococcosis is not completely clear. In this study, we investigated the functional decrease in NK cells in hepatic alveolar echinococcosis (AE) patients. METHODS: Using human liver tissue samples from 10 patients with hepatic AE,
