Effect of parental presence on intravenous sedation in children with cyanotic heart disease undergoing CT angiography: a prospective randomized trial.

PURPOSE: Congenital heart disease (CHD) affects 9.3 per 1000 live births in Asia, with 25% being cyanotic. Advanced imaging such as CT angiography is essential, but requires effective sedation in this high-risk group. While parental presence is known to reduce anxiety during mask induction in operating rooms, its effect on intravenous sedation in non-operating room anesthesia (NORA) settings, particularly for diagnostic imaging, has not been studied. This trial evaluates the effect of parental presence during intravenous procedural sedation on ketamine requirements for children with cyanotic C

Methemoglobinemia in Infancy.

Methemoglobinemia represents a rare blood disorder which occurs through the oxidation of hemoglobin (Hb) from its ferrous (Fe 2+ ) to ferric (Fe 3+ ) state, thus disabling its oxygen transport function. The condition leads to functional hypoxia which manifests as cyanosis that does not respond to oxygen therapy. The condition affects neonates most severely because their enzymatic systems are not fully developed and their fetal Hb has a tendency to become oxidized. We report a case of a 4-week-old male term infant who developed severe cyanosis after receiving excessive EMLA cream (lidocaine/pri

Renal regional oxygenation during pediatric cardiac surgery predicts acute kidney injury: a prospective cohort study with model comparisons.

BACKGROUND: Acute kidney injury (AKI) is a common complication after pediatric cardiac surgery using cardiopulmonary bypass (CPB). This study evaluated renal regional oxygen saturation (R-rSO₂), measured via near-infrared spectroscopy (NIRS), as an AKI predictor. METHODS: 120 pediatric patients undergoing CPB-assisted congenital heart surgery were prospectively enrolled. Continuous intraoperative R-rSO₂ monitoring was performed. Four multivariable logistic regression models (adjusted for age, pre-SCr, pre-hemoglobin, RACHS-2, cyanosis, and CPB time) assessed distinct R-rSO₂ metrics during CPB

Experimental Models of Acute Lung Injury to Study Inflammation and Pathophysiology: A Narrative Review.

Acute lung injury (ALI) is characterized by acute respiratory insufficiency, including tachypnea, cyanosis refractory to oxygen, decreased lung compliance, and diffuse alveolar infiltrates, which is a condition associated with high morbidity and mortality that usually results in the development of multiple organ dysfunction. Acute lung injury in humans is histopathologically characterized by neutrophilic alveolitis, injury of the alveolar epithelium and endothelium, hyaline membrane formation, and microvascular thrombi. Different animal models of experimental lung injury have been used to inve

Psychomotor Development in Pediatric Patients with Congenital Heart Defects Prior to Surgical Intervention: Findings from a Prospective Cross-Sectional Study.

Background and Objectives: Psychomotor developmental delay is a frequent comorbidity in children with congenital heart defects (CHD), especially after surgical correction of the CHD and exposure to risk factors such as anesthesia, cardiopulmonary bypass and postoperative complications. Yet psychomotor delay is present in these patients before surgical correction but is under-recognized. Evidence focusing solely on unrepaired CHD remains limited. Materials and Methods: This prospective cross-sectional study evaluated 153 and included 77 children under 6 years of age with unrepaired CHD, stratif

First Isolation of Klebsiella pneumoniae from Septicaemic Piglets in Poland.

Klebsiella pneumoniae is commonly known to cause a vast range of community-acquired or nosocomial infections. The isolation of K. pneumoniae has also been noted in diseased food-producing animals, including swine. The main goals of this study were to document clinical manifestation of a septicaemia outbreak in suckling piglets due to K. pneumoniae ST25 and provide molecular characterisation of the isolates. For the purpose of this investigation, 13 dead suckling piglets with cyanosis were selected. All the isolates obtained from affected lungs were susceptible to apramycin, ceftiofur, gentamyc

Thoracoscopic near-infrared localization and division of H-type tracheoesophageal fistulas in newborns: A case series.

PURPOSE: Congenital H-type tracheoesophageal fistula (H-TEF) is a rare congenital esophageal malformation. Treatment involves minimally invasive surgery; however, accurate fistula localization remains challenging. Real-time imaging via near-infrared (NIR) thoracoscopy facilitates the distinction between different tissue structures. Therefore, we aimed to evaluate the clinical efficacy of the NIR-thoracoscopic simple clip method for treating congenital H-TEFs in neonates. METHODS: Neonates with H-TEFs underwent thoracoscopic NIR localization using indocyanine green (ICG) injection, followed by

Recessive congenital methemoglobinemia: a systematic review of reported cases.

BACKGROUND: Recessive congenital methemoglobinemia (RCM) is a rare autosomal recessive disorder characterized by a deficiency of NADH-cytochrome b5 reductase. Under normal conditions, cytochrome b5 reductase and NADH reductase maintain methemoglobin in the physiological range by keeping heme iron in the Fe2+ state. RCM presents in two clinical forms. Type I presents with isolated cyanosis, while type II presents also with severe neurocognitive manifestations. Genetic analysis of the CYB5R3 gene is often used for diagnosis. Treatment with methylene blue, ascorbic acid, or riboflavin can reduce

Successful Treatment of Phlegmasia Cerulea Dolens by Surgical Thrombectomy: A Case Report.

Phlegmasia cerulea dolens (PCD) is a rare, life-threatening manifestation of deep vein thrombosis that can clinically resemble acute arterial occlusion. Accurate differentiation is essential for appropriate management. Delayed-phase contrast-enhanced computed tomography is particularly valuable in distinguishing between these two conditions. Here, we report the case of an 86-year-old man who presented with suspected acute limb ischemia, subsequently diagnosed with PCD by contrast-enhanced computed tomography. The patient was successfully treated with emergency surgical venous thrombectomy, fol

Persistent methaemoglobinaemia due to unsupervised use of topical dapsone-adapalene.

A woman in her 40s with cognitive impairment presented with acute-onset central cyanosis unresponsive to supplemental oxygen therapy. She was diagnosed with methaemoglobinaemia based on a marked saturation gap and elevated methaemoglobin levels on co-oximetry. Despite multiple doses of methylene blue, methaemoglobin levels repeatedly rebounded. Extensive investigations failed to identify either an endogenous or ingested oxidant source until nursing staff observed the patient applying a topical dapsone-adapalene cream brought from home. Discontinuation of the cream resulted in complete and sust

Computed tomography angiography imaging of double superior vena cava in a case of tetralogy of fallot.

Tetralogy of Fallot is characterized by ventricular septal defect, right ventricular hypertrophy, overriding of aorta and pulmonary tract obstruction. This leads to the development of cyanosis in the baby. However, cyanosis is not apparent in the early days of neonate due to the presence of patent ductus arteriosus (PDA). As the PDA closes, the obstruction of the pulmonary tract leads to a reduction in blood flow to the lungs, causing the development of cyanosis. Tetralogy of Fallot can be present in isolation or with various cardiovascular or extra-cardiovascular abnormalities. This case demo

Point of care INR monitoring in cyanotic congenital heart disease-related compensatory erythrocytosis.

INTRODUCTION: People with cyanotic congenital heart disease (CHD) frequently require anticoagulation with warfarin. Point of care (POC) devices offer significant benefits such as patient-managed warfarin and rapid testing, however, there is limited experience with the use of POC in adult patients with CHD and high haematocrit. METHODS: This exploratory prospective cohort study recruited people between September 2021 and December 2023. Inclusion criteria were cyanotic CHD and oxygen saturations <93% and a Hct ≥0.51. Exclusion criteria were pregnancy, antiplatelet agents, recent thromboembolic

Interpretable machine learning and deep neural networks for ICU admission prediction in paediatric respiratory patients.

Respiratory disorders represent a significant health challenge globally, especially in paediatric populations. Traditional diagnostic methods including clinical evaluation, imaging, pulmonary function tests, and invasive procedures, etc., often involve high costs, discomfort, and risks of misdiagnosis. In this context, machine learning (ML) techniques, particularly Deep Neural Networks (DNNs), offer a promising alternative by effectively modelling complex clinical and demographic data to improve prediction accuracy. This study proposes different ML based approaches, and DNN based approaches co

Incidental diagnosis of a lingular pulmonary arteriovenous malformation in a 22-year-old female: a rare case report from Syria.

BACKGROUND: Pulmonary arteriovenous malformations (PAVMs) are abnormal communications between pulmonary arteries and veins that bypass the normal pulmonary capillary bed. They are most commonly associated with hereditary hemorrhagic telangiectasia (HHT), while idiopathic cases are rare. PAVMs usually occur in the lower lobes and are more frequent in females, particularly during pregnancy. Although often clinically silent, they can present with dyspnea, hypoxemia, and cyanosis. This case is reported due to its rarity and as, to the best of our knowledge, the first documented surgically resected

Effects of sacubitril/valsartan, an angiotensin receptor-neprilysin inhibitor, on renal impairment in patients with cyanotic congenital heart disease.

Sacubitril/valsartan (angiotensin receptor-neprilysin inhibitor [ARNI]) is widely used to manage heart failure, a common complication in patients with cyanotic congenital heart disease (CCHD). Its effects on the kidneys, especially in patients with long-standing cyanosis, remain poorly understood. We report three cases of CCHD who were started on ARNI for ventricular dysfunction. During treatment, all three developed features of cyanotic nephropathy. The discontinuation of ARNI led to a gradual improvement in renal function in all cases. This highlights a possible correlation between the use o
