Delayed-onset functional ischemia 16 years after digital replantation: A Raynaud's-like vasospasm case report.

Reports of long-term complications following finger replantation are scarce. Vascular crises occurring more than a decade after surgery are particularly rare. We present the case of a 27-year-old male who developed with acute cyanosis and reduced skin temperature of the left index finger, occurring 16 years after digital replantation. The patient had undergone replantation of the left index and middle fingers in childhood. He reported chronic smoking and frequent cold-water exposure prior to symptom onset. Imaging demonstrated narrowing of the proper palmar digital artery of the index finger,

Risk stratification for adult patients with pulmonary arterial hypertension associated with congenital heart disease. A scientific statement of the ESC Working Group on Pulmonary Circulation & Right Ventricular Function, the ESC Working Group on Adult Congenital Heart Disease, and the Association of Cardiovascular Nursing & Allied Professions of the ESC.

For patients with pulmonary arterial hypertension (PAH), current guidelines recommend a 3- and 4-strata risk stratification model at baseline and follow-up, respectively. Risk stratification models in PAH are mainly derived from idiopathic PAH cohorts and are not automatically applicable to all patients with PAH associated with congenital heart disease (CHD), especially in those with Eisenmenger syndrome, given the differences in pathophysiology and clinical phenotype. Additional features such as shunt location, complexity of CHD, degree of cyanosis, iron deficiency, syndromic co-morbidity and

Reversing the irreversible? A case of successful surgical repair in a late-presenting aortopulmonary window with severe pulmonary hypertension.

BACKGROUND: Aortopulmonary window (APW) is a rare congenital heart defect, accounting for only 0.2-0.6% of all congenital cardiac anomalies, and is usually identified and surgically corrected within the first year of life to prevent irreversible pulmonary hypertension (PH). Once pulmonary vascular resistance (PVR) becomes fixed, surgical repair is generally considered contraindicated. However, emerging evidence suggests that pulmonary vascular reactivity may persist in select adolescents, opening a potential therapeutic window beyond infancy. CASE PRESENTATION: We report a 15-year-old male wi

Palliative Balloon Pulmonary Valvotomy Enhances Immediate Postoperative Outcomes in Severely Cyanotic Patients With Tetralogy of Fallot.

BackgroundOlder patients with severe cyanosis from unrepaired tetralogy of Fallot (TOF) face high perioperative morbidity despite technically straightforward surgical repair. Chronic hypoxemia leads to myocardial metabolic adaptation, polycythemia, and increased susceptibility to reperfusion injury. Preoperative optimization may improve outcomes in this high-risk group.ObjectiveTo evaluate whether palliative balloon pulmonary valvotomy (BPV) before intracardiac repair (ICR) improves immediate postoperative outcomes in severely cyanotic patients with uncomplicated TOF.MethodsA prospective obser

Case Report: Transcatheter occlusion of a rare pulmonary artery to left atrium fistula using an atrial septal defect occluder device.

A right pulmonary artery to left atrium fistula represents a rare cause of cyanosis in childhood. It may be diagnosed late due to the presence of minimal symptoms. We report the case of a 3-year-old child who presented with dyspnea and cyanosis. Transthoracic echocardiography revealed an anomalous fistulous connection between the right pulmonary artery and a large aneurysm communicating with the roof of the left atrium. The patient underwent successful transcatheter occlusion using an atrial septal defect occluder device. The patient had an uneventful recovery on follow-up with resolution of c

Transient Neonatal Cyanosis Exacerbated by Inhaled Nitric Oxide.

BACKGROUND: Methemoglobinemia in newborns presents with cyanosis and hypoxemia, which can be mistaken for congenital heart disease or pulmonary hypertension. CASE REPORT: A term infant presented with cyanosis and low SpO 2 (70s) immediately after birth, despite continuous positive airway pressure (CPAP) and 100% inspired oxygen. The patient was intubated and started on inhaled nitric oxide (iNO) and prostaglandin E1 infusion. Chest X-ray showed bilateral pneumothoraces; the echocardiogram was normal. Arterial blood gases demonstrated normal pH and elevated PaO 2 . iNO and prostaglandin E1 (PG

Main branches pulmonary artery stenting in congenital heart diseases: a case series.

BACKGROUND: Branch pulmonary artery (PA) stenosis is a common complication in congenital heart disease (CHD) that can result in unequal pulmonary perfusion, cyanosis, and increased ventricular workload. Although surgical repair remains an option, pulmonary artery stenting has emerged as a less invasive and effective alternative for restoring vessel patency. METHODS: This single-center retrospective study included 18 pediatric patients (median age 5.4 years, range 3 months-17 years) who underwent PA stenting for branch stenosis associated with complex CHD. Patient demographics, procedural char

Avoidance of Hyperoxemia Versus Routine Hyperoxia During Cardiopulmonary Bypass in Children With Cyanotic Congenital Heart Disease-A Systematic Review and Meta-Analysis.

BACKGROUND: Children with cyanotic congenital heart disease undergoing cardiac surgery with cardiopulmonary bypass (CPB) are exposed to varying oxygenation strategies, and the optimal oxygenation strategy and the impact of limiting hyperoxic exposure remain uncertain. This study aimed to compare oxygenation strategies designed to avoid hyperoxemia versus routine hyperoxic management in this population. METHODS: A systematic search was conducted in four databases. The primary outcome was postoperative intubation time. Secondary outcomes included intensive care unit (ICU) length of stay (LOS),

Cardiorenal Biomarkers and Cerebrovascular Risk in Patients with Congenital Heart Disease.

Background/Objectives: Adults with congenital heart disease (CHD) have a substantially higher risk of ischemic stroke than the general population. Circulating biomarkers such as N-terminal pro B-type natriuretic peptide (NT-pro-BNP), high-sensitivity C-reactive protein (hs-CRP), and microalbuminuria have been associated with adverse cardiovascular outcomes in CHD, but their role in predicting cerebrovascular events remains uncertain. Methods: Prospective cohort study including 372 adults with CHD [median age 34 years (IQR 23-42); 57.8% male] followed at a tertiary center between 2017 and 2022.

Adolescent presentation of L-type ccTGA with pulmonary valve hypoplasia and RV dysfunction: from cardiogenic shock to surgical repair.

BACKGROUND: Congenitally corrected transposition of the great arteries (ccTGA) is an uncommon congenital heart defect involving both atrioventricular (AV) and ventriculoarterial (VA) discordance. This condition is further complicated by additional anomalies such as a hypoplastic pulmonary valve, ventricular septal defect (VSD), and dextroversion. These abnormalities may contribute to an increased risk of heart failure and cardiogenic shock, making early detection and appropriate intervention essential for better patient outcomes. CASE PRESENTATION: A 16-year-old female presented with progress

We report the diagnosis, treatment, and follow-up of a rare giant superior vena cava (SVC) aneurysm in a 39-year-old woman, managed through a combination of surgical and interventional techniques. The patient underwent off-pump SVC aneurysmorrhaphy without cardiopulmonary bypass, followed by balloon angioplasty to address early postoperative stenosis manifested by facial cyanosis. The interventions resulted in complete symptom resolution and durable vessel patency, as confirmed by computed tomography at 3 months and maintained at 1 year. This case highlights that off-pump aneurysmorrhaphy offe
