Fenestration Re-creation for Early Fontan Deterioration: Long-Term Hemodynamic and Clinical Outcomes.

Spontaneous closure of a Fontan fenestration may precipitate early hemodynamic deterioration in vulnerable patients. In such situations, fenestration re-creation may be considered as a rescue strategy when medical therapy is insufficient. However, the long-term hemodynamic consequences of this intervention remain poorly defined. We retrospectively reviewed 62 patients who underwent the Fontan procedure between January 2011 and December 2020 and subsequently experienced spontaneous fenestration closure. Patients were divided into two groups according to management strategy: those who underwent

Sex differences in response to COVID-19 infection: a retrospective study based on emergency medical team interventions.

The COVID-19 pandemic adversely affected several aspects of daily life and seriously threatened public health. The main aim of this study was to investigate the differences in human response to SARS-CoV-2 virus infection depending on sex, based on emergency medical team (EMT) interventions. The study was conducted in north-eastern Poland and were based on 12,110 EMT interventions, in which SARS-CoV-2 virus infection was indicated as an ICD-10 codification-based diagnosis. The study used descriptive statistics, Mann-Whitney U, and Chi-square tests. Patients infected with SARS-CoV-2 accounted fo

Emergency medicine updates: Pediatric brief resolved unexplained event.

INTRODUCTION: Brief resolved unexplained events (BRUEs) can occur in a large number of pediatric patients, with most having a benign etiology. However, in many cases, these can result in unnecessary evaluations and admissions among low-risk patients. Therefore, it is critical for clinicians to be aware of the current evidence regarding BRUE in the emergency department (ED) setting. OBJECTIVE: This paper evaluates key evidence-based updates concerning pediatric BRUE for the emergency clinician. DISCUSSION: A BRUE is defined as a clinical entity occurring in an infant less than 1 year of age t

Incidence and predictive factors for development of ADHD post congenital heart disease surgery-a single tertiary center experience from India.

INTRODUCTION: A recent shift in mortality trends in children with congenital heart disease (CHD) has led to more patients reaching adulthood. Attention-deficit hyperactivity disorder (ADHD) is a prevalent and often underdiagnosed condition, with increased risk observed in children with CHD due to disease- and surgery-related factors. This study aimed to determine the incidence of ADHD one year after cardiac surgery in children and to identify associated clinical and operative risk factors. METHODS: Children aged 3-5 years and 6-18 years who were to undergo cardiac surgery for CHD between Dece

Does preservation of antegrade pulmonary blood flow reduce pulmonary arteriovenous malformations after bidirectional Glenn? A retrospective observational study.

BACKGROUND: Pulmonary arteriovenous malformations (PAVMs) are a recognised cause of progressive cyanosis following bidirectional superior cavopulmonary anastomosis (BDG). The role of antegrade pulmonary blood flow (APBF) in preventing PAVM formation remains controversial. While preserved APBF may improve oxygen saturation and pulmonary artery growth, its protective effect against PAVMs is uncertain. Increasing evidence suggests that exclusion of hepatic venous blood from the pulmonary circulation, rather than absence of pulsatile flow, may be the primary mechanism underlying PAVM development.

Hematological Adaptation Without Differences in Systemic Inflammatory Indices in Cyanotic and Acyanotic Congenital Heart Disease.

Background: Chronic hypoxemia in cyanotic congenital heart disease triggers well-recognized hematological adaptation; however, whether hypoxemia also drives systemic inflammatory activation remains uncertain. This study aimed to evaluate hematological parameters and inflammatory indices in cyanotic and acyanotic congenital heart disease (CHD) to better characterize the relationship between hypoxemia and systemic inflammatory status. Methods: In this single-center retrospective study, 260 children with congenital heart disease were classified as cyanotic (n = 158) or acyanotic (n = 102). Preope

Bluetongue virus serotype 8 (BTV-8) in Serbia, 2025.

Bluetongue (BT) is a vector-borne disease of ruminants caused by bluetongue virus (BTV). After more than a decade without reported cases, BT re-emerged in Serbia in 2025. In August 2025, clinical signs consistent with BT were observed in a sheep flock in western Serbia, including fever, submandibular and head oedema, oral haemorrhages, and cyanosis of the tongue. Real-time RT-PCR targeting segment 10 confirmed BTV infection, and serotyping identified BTV-8. Virus isolation in embryonated chicken eggs yielded multiple BTV-8 isolates, which underwent whole-genome sequencing using Oxford Nanopore

Severe Methemoglobinemia Due to Accidental Ingestion of Nitrite-Containing Nail Polish Remover: A Case Report.

BACKGROUND Methemoglobinemia is an uncommon hemoglobinopathy caused by oxidation of heme iron from its ferrous (Fe²⁺) state to the ferric (Fe³⁺) state, resulting in functional anemia that does not improve with supplemental oxygen. "Poppers" is a slang term for recreational drugs marketed as nail polish removers or room deodorizers, often sold under brand names such as "Jungle Juice," which may cause methemoglobinemia due to their nitrite content. CASE REPORT A 67-year-old White woman accidentally ingested "Jungle Juice" nail polish remover containing isobutyl nitrite, resulting in severe methe

Reversible Lesions of the Genu of the Corpus Callosum and White Matter Detected by MRI in a Neonate With Apnea: A Case Report.

In this report, we describe a 19-day-old female neonate who presented with recurrent apnea, poor feeding, and cyanosis. Brain diffusion-weighted MRI revealed hyperintense lesions in the genu of the corpus callosum and bilateral frontal white matter with corresponding low apparent diffusion coefficient values. Antibiotic therapy and supportive management were initiated, and the apnea resolved by hospital day 6. A follow-up brain MRI performed on day 49 demonstrated the complete resolution of the previously observed lesions. No neurological sequelae were observed up to 11 months of age. This cas

Navigating the complexities of dual CNV findings: a case of DEE7 caused by a de novo KCNQ2 deletion and a Co-occurring chromosome 13 duplication - a case report and literature review.

BACKGROUND: Developmental and epileptic encephalopathy 7 (DEE7) is a severe neurodevelopmental disorder stemming from mutations in the KCNQ2 gene, which encodes a critical voltage-gated potassium channel subunit. While KCNQ2 mutations are known to cause a spectrum of diseases, from benign familial neonatal seizures (BFNS1) to drug-resistant DEE7, the clinical outcomes specifically associated with large deletions-a rare mechanism causing haploinsufficiency-are not well defined. This study reports a de novo KCNQ2 large deletion and aims to delineate the associated clinical trajectory and treatme

Hb F-SickKids (HBG2: C.308A > G): A Novel γ-Globin Variant Associated with Transient Neonatal Cyanosis.

We report a novel fetal hemoglobin variant, HBG2:c.308A > G [Gγ(102(G4)Asn > Ser], in a male neonate presenting with cyanosis and low oxygen saturation that resolved three months after birth, coinciding with the temporal decrease in the variant hemoglobin. Further molecular analysis confirmed maternal inheritance of this variant. The HBG2:c.308A > G variant parallels known low-oxygen-affinity mutations at the same position in the HBB and HBG2 genes, supporting its role in transient neonatal cyanosis. This variant has been designated Hb F-SickKids. Identifying such variants is important for tim
