Balancing two defects: surgical decision-making in low-birth-weight neonates with esophageal atresia and tetralogy of Fallot-a case series.

BACKGROUND: Esophageal atresia with tracheoesophageal fistula (EA/TEF) and tetralogy of Fallot (TOF) are both life-threatening congenital malformations in the neonatal period. Their coexistence poses substantial challenges for treatment planning, particularly in low-birth-weight infants, for whom no consensus management strategy has yet been established. CASE PRESENTATION: We report the diagnostic and therapeutic courses of two preterm, low-birth-weight infants with concomitant EA/TEF and TOF. Case 1 was a male neonate with a birth weight of 1.7 kg. Intraoperative exploration revealed an appro

Transcatheter Occlusion of Hepatic Vein-Atrial Communication After Modified Fontan in a Child Post Kawashima Surgery.

BACKGROUND: The hepatic vein-azygos vein connection is a rare Fontan modification used in patients with single ventricle physiology and interrupted inferior vena cava, following an initial Kawashima procedure. We report an unusual case of severe, late-onset hypoxemia due to recanalization of a hepatic vein-atrial communication after this operation. CASE SUMMARY: A girl with heterotaxy, mesocardia, unbalanced atrioventricular septal defect, and interrupted inferior vena cava underwent Kawashima surgery at 1 year of age, followed by modified Fontan completion at 4 years, in which hepatic veins

Truncal valve repair combined with right atrial appendage valved right ventricular-to-pulmonary connection in truncus arteriosus type II with single coronary artery.

We present a challenging case of truncus arteriosus type II repair in a 2-month-old, 3.3 kg infant admitted with heart failure and cyanosis. Right ventricular-to-pulmonary arterial continuity was restored using a right atrial free-wall autograft, while a neopulmonary valve was constructed from the right atrial appendage, providing an autologous valved connection. The patient also exhibited moderate-to-severe truncal valve stenosis and mild regurgitation, which were addressed with concomitant valve repair. The patient also had a single coronary artery.

Surviving Adulthood with Rare Combined Congenital Heart Defects: Complete AV Canal Defect, Ebstein's Anomaly, and Right Ventricular Hypoplasia.

Background/Objectives. Ebstein's anomaly (EA), which accounts for fewer than 1% of congenital heart diseases, and atrioventricular canal defect (AVCD), present in approximately 4-5% of cases, exceptionally coexist, with this combination observed in fewer than 0.5% of patients with AVCD. We aim to report the oldest documented case of a 45-year-old female with the exceptionally rare combination of complete AVCD, EA, and right ventricular hypoplasia and to provide a concise review of these anomalies. Case presentation. Diagnosed in early childhood with a complete AVCD, pulmonary stenosis, and rig

Hereditary methemoglobinemia diagnosed in adulthood: Type 1 cytochrome B5 reductase deficiency.

INTRODUCTION: Type 1 CYB5R3 deficiency is a rare cause of hereditary methemoglobinemia. Although it is typically diagnosed during childhood, it may also be identified in older adults. CASE: A 62-year-old male patient presenting with cyanosis and shortness of breath was diagnosed with Type 1 CYB5R3 deficiency. The diagnosis was supported by the chocolate-brown appearance of arterial blood, a methemoglobin (metHb) level of 17%, hypoxemia inconsistent with pulse oximetry findings, and the identification of a homozygous pathogenic mutation in CYB5R3 on sequence analysis. The patient is currently

Reversal of Glenn-associated diffuse pulmonary arteriovenous malformations after total cavopulmonary connection: a case report.

BACKGROUND: Diffuse pulmonary arteriovenous malformations (PAVMs) are a rare but serious complication after the Glenn procedure. Clinically, they typically present with progressive hypoxemia, which can lead to worsening cyanosis, reduced exercise tolerance, and may even progress to heart failure. CASE SUMMARY: We report an 8-year-old boy with complex congenital heart disease, including double outlet right ventricle, ventricular septal defect, atrial septal defect, and pulmonary stenosis. He underwent a bidirectional Glenn procedure at 6 months of age and subsequently developed progressive cya

The blue dilemma: a rare presentation of dual hemoglobinopathy in a pregnant patient.

Methaemoglobinaemia is a rare disorder due to an increase in the amount of a variant of haemoglobin known as methaemoglobin (MetHb), which decreases the oxygen-carrying capacity of blood. It is usually caused by enzyme deficiency. Hb Q India is another rare alpha-chain variant haemoglobinopathy. A young primigravida was referred due to low oxygen saturation. She was otherwise asymptomatic. Patient was received with peripheral cyanosis and in a tachypneic state due to advanced labour. On the pulse oximeter, oxygen saturation was found to be 40%; hence, the patient was immediately intubated, and

Hair tourniquet syndrome in a neonate: a case report highlighting bedside diagnosis and preventative strategy in a resource-limited setting, Woldia, Ethiopia, 2025.

BACKGROUND: Hair tourniquet syndrome (HTS) is a rare but potentially serious condition where a strand of hair or fiber constricts an appendage, leading to ischemia and tissue damage. It most commonly affects infants, with toes, fingers, and genitalia being the most frequently involved sites. Due to its rarity and nonspecific presentation, diagnosis may be delayed, leading to complications. This case report contributes to the limited literature on HTS by highlighting the importance of early recognition in atypical presentations and emphasizing innovative, non-invasive diagnostic and management

Infant with Complex Congenital Cardiac Anomalies: A Case Report and Review of Literature.

The coexistence of multiple complex congenital cardiac anomalies in a single infant - particularly unbalanced Atrioventricular Septal Defect; Double Outlet Right Ventricle; Infracardiac Total Anomalous Pulmonary Venous Connection, situs inversus, and pulmonary hypertension - is exceptionally rare and presents significant surgical and physiological challenges. A full-term male neonate was diagnosed at 25 days of age with this rare combination of defects. Despite low-flow oxygen, the baseline oxygen saturation remained at 60%-65%, and growth was severely impaired. Initial management at a tertiar

New insight into anomalies of coronary arteries in children: diagnosis, treatment with perioperative and postoperative course and prognosis.

BACKGROUND: Coronary artery anomalies (CAAs) are rare and heterogeneous congenital abnormalities. However, some of them are associated with serious consequences, including myocardial ischemia, heart failure, and even death. This study aimed to characterize symptoms, course, treatment, and prognosis of different types of CAAs. METHODS: This single-centre retrospective cohort study included 38 consecutive paediatric patients hospitalised in the Department of Paediatric Cardiology due to a diagnosis of CAAs between 01.2012-03.2025. Patients were divided into two groups: anomalous origin of the c

Acute methemoglobinemia- Diagnosing and managing in family practice.

Methemoglobinemia is a rare but serious disorder in which hemoglobin is oxidized to methemoglobin, impairing oxygen transport. This condition can present in primary care with cyanosis and oxygen desaturation and is potentially misdiagnosed as respiratory or cardiac failure. We present the case of a 38-year-old male driver who was brought to the emergency department unconscious, with cyanosis, and unresponsive to oxygen therapy. His oxygen saturation was 86% on pulse oximetry, although arterial blood gas analysis showed a normal partial pressure of oxygen. The source of poisoning was unknown at

Beyond epilepsy: convulsive syncope revealing restrictive cardiomyopathy in a child.

Seizure-like episodes may occasionally arise from cardiovascular causes rather than primary neurological disease, leading to diagnostic delay. We report an adolescent girl who presented with recurrent generalised tonic episodes with cyanosis and transient loss of consciousness. Initial evaluation for epilepsy, including video electroencephalography during an event, was unremarkable. Cardiac assessment revealed marked biatrial dilatation with restrictive ventricular physiology on echocardiography, consistent with restrictive cardiomyopathy (RCM). Ambulatory monitoring documented atrial tachycar

First palliation for the fragile: Short-term outcomes of ductal stenting versus surgical shunt in small infants with restricted pulmonary blood flow.

BACKGROUND: In congenital heart disease (CHD) with restricted pulmonary blood flow, infants require urgent palliation, either with a modified Blalock-Taussig-Thomas (m-BTT) shunt or ductal stenting. However, evidence in infants weighing under 5 kg remains limited. This study aims to compare short-term outcomes after the two approaches in infants weighing <5 kg. METHODS: This retrospective cohort study included infants weighing under 5 kg diagnosed with CHD and restricted pulmonary blood flow who underwent either ductal stenting or m-BTT shunt between 2018 and 2023 at the National Cardiovascul

Right atrial appendage for neopulmonary valve reconstruction in a child with tetralogy of Fallot with absent pulmonary valve.

Absent pulmonary valve is a rare congenital heart anomaly frequently associated with tetralogy of Fallot. We present the case of a 12-year-old child who came with symptoms of cyanosis and a heart murmur. Echocardiographic evaluation revealed severe infundibular pulmonary stenosis and a large subaortic ventricular septal defect. Intraoperative findings confirmed the presence of a rudimentary pulmonary valve, with thin, chordae-like structures connected to the pulmonary leaflet remnants. The child underwent successful intracardiac repair, which involved the reconstruction of a neopulmonary valve
