Bed-related deaths are rare in forensic practice and are usually associated with falls, restraint-related entrapment, or unsafe infant sleeping environments. Fatal mechanical asphyxia involving lift-up storage beds is an exceptional occurrence, and the few published cases involve adult victims. We report the first pediatric fatality caused by neck entrapment in the closing mechanism of an ottoman-style storage bed. An 8-year-old boy was found prone and unconscious, with his head trapped between the lowered mattress-support frame and the storage compartment below. Facial cyanosis and congestion

BACKGROUND: Left ventricular assist devices (LVAD) are an established treatment for advanced heart failure. Due to the continuous flow, pulse palpation, noninvasive blood pressure measurement and pulse oximetry are often unreliable in acute care.
OBJECTIVE: Case-based analysis of the acute care of patients with left ventricular assist devices in emergency medical services and emergency departments, exemplified by a patient supported with HeartMate 3® and derivation of practical recommendations for action.
MATERIAL AND METHODS: Anonymized prehospital and clinical documentation of a 67-year-ol

BACKGROUND: Congenital heart disease (CHD) is a major contributor to infant morbidity and mortality worldwide. Although prenatal screening, neonatal assessment, pulse oximetry screening, and routine pediatric evaluation contribute to CHD detection, some cases may remain unrecognized until after discharge or later in infancy. Parental recognition of warning signs may support timely healthcare-seeking, but it should be considered one component of a broader diagnostic and referral pathway. This study assessed parental knowledge, awareness, and intended healthcare-seeking responses toward CHD amon

The Kawashima procedure is a variant of the bidirectional Glenn used in patients with interrupted inferior vena cava (IVC) and azygos continuation, though optimal timing and the role of fenestration remain debated. We report a 5-month-old boy with functional single ventricle physiology, transposition of the great arteries, pulmonary valve atresia with ventricular septal defect, interrupted IVC with azygos continuation, and bilateral superior vena cavae, who presented with cyanosis and failure to thrive. Imaging confirmed complex venous anomalies with hepatic venous drainage directly into the r

Heterotaxy syndrome comprises a spectrum of laterality defects frequently associated with complex congenital heart disease and extracardiac anomalies. We report a 2-year-old male who presented with persistent cyanosis, recurrent respiratory infections, and failure to thrive. Cardiac computed tomography demonstrated ambiguous situs with dextrocardia, reversed atrial arrangement, L-looped ventricles, large atrial and ventricular septal defects, hypoplastic right ventricle, absent right ventricular outflow tract, and double-outlet left ventricle. Additional findings included polysplenia, right-si

A neonate with common arterial trunk presented with severe cyanosis rather than pulmonary overcirculation. Echocardiography, cardiac CT, catheterisation, and surgery showed a pulmonary trunk arising from a truncal valvar sinus, with systolic obstruction by a dysplastic truncal cusp. Staged palliation restored antegrade pulmonary flow, allowed pulmonary arterial growth, and enabled successful complete repair.

Subglottic hemangioma (SGH) is a rare but potentially fatal cause of infantile stridor that is frequently misdiagnosed as laryngomalacia, croup, or idiopathic subglottic stenosis. Circumferential involvement is an uncommon and particularly dangerous variant whose endoscopic appearance overlaps with that of other concentric subglottic lesions, including congenital cricoid cartilage malformation and idiopathic subglottic stenosis. We present a two-week-old female with a history of preterm birth (36 5/7 weeks) and progressive biphasic stridor, cyanosis, and increasing respiratory distress. Operat

BACKGROUND: Persistent right venous valve (PRVV), a remnant of the fetal right valve of the sinus venosus, may facilitate right-to-left atrial shunting through the patent foramen ovale (PFO). Although PRVV has been reported to cause severe neonatal cyanosis requiring surgical intervention, its clinical significance in otherwise healthy term neonates remains poorly defined.
METHODS: We retrospectively analyzed consecutive term neonates (≥ 37 weeks' gestation) with stable general conditions who exhibited periodic breathing-like oxygen saturation (SpO2) fluctuations and were found to have PRVV-a

Congenital pulmonary airway malformation (CPAM) is a rare developmental lung anomaly that may present in the neonatal period with respiratory distress, and its association with dextrocardia is exceptional, potentially leading to diagnostic challenges. We report the case of a 26-day-old male neonate, born at term, admitted for cyanosis and respiratory distress. Clinical examination revealed tachypnea (68 breaths per minute), oxygen desaturation (SpO₂ 65% in room air), and signs of respiratory distress, with cardiac auscultation suggesting right-sided heart sounds. Imaging studies demonstrated a

BACKGROUND & AIMS: Patients with cyanotic CHD and those with metabolic dysfunction-associated steatotic liver disease are at risk of liver fibrosis. We compared hepatic extracellular volumes and native T1 values to better understand the burden of liver disease in these populations.
METHODS: The sample comprised 136 patients in 5 groups: control (n = 23), metabolic dysfunction-associated steatotic liver disease [mild (F0-F1) and significant (F2-F4) fibrosis; n = 45], repaired tetralogy of Fallot (n = 30), and Fontan circulation (n = 38). Differences were assessed using linear regression models

BACKGROUND: Congenital methemoglobinemia is a rare inherited disorder caused by impaired reduction of methemoglobin to functional hemoglobin, resulting in functional hypoxia despite normal arterial oxygen tension. Due to its often-mild clinical phenotype, diagnosis is frequently delayed or made incidentally.
METHODS: This prospective case series of surgical patients was conducted at a tertiary care center in South India over a one-year period. Patients incidentally diagnosed with methemoglobinemia during perioperative or acute medical evaluation were included. Diagnosis was established using

BACKGROUND: Milrinone is commonly used after congenital heart surgery, yet its role following late repair of tetralogy of Fallot remains poorly defined. In many low- and middle-income country settings, children present for repair at older ages after prolonged exposure to right ventricular pressure overload and chronic cyanosis, resulting in post-operative physiology that differs fundamentally from early infant repair. Extrapolation from heterogeneous paediatric cardiac surgery populations may therefore be inappropriate. This review examines the pathophysiologic substrate of late-presenting tet

BACKGROUND: Early prediction of the clinical course may aid clinical decisions for patients with suspected sepsis. We examined how variables observed by ambulance personnel predicted the clinical course in patients with suspected sepsis.
METHODS: We prospectively collected prehospital data from patients with suspected sepsis in the ambulance service and supplemented the data with in-hospital findings. Prehospital data were used to calculate outputs of quick Sequential Organ Failure Assessment (qSOFA) and National Early Warning Score-2 (NEWS2). Outcomes were (1) sepsis severity after hospital

Methaemoglobin (MetHb) is an oxidised form of haemoglobin (Hb) unable to bind oxygen. Raised levels of MetHb reduce the blood's oxygen-carrying capacity, causing potentially severe hypoxaemia and possible death. The condition arises from three main pathologies: mutations in globin genes causing Haemoglobin-M, inherited deficiency of the enzyme cytochrome b5 reductase (CytB5R) responsible for reducing MetHb back into functional Hb, and exposure to some oxidising agents. Well-documented causative agents include dapsone and cocaine-derived anaesthetics, with emerging evidence highlighting an incr

BACKGROUND: Pregnancy in Eisenmenger syndrome (ES) confers high risk of fetal, obstetric, and maternal complications including death.
CASE SUMMARY: A previously healthy nulliparous 22-year-old woman underwent a caesarean section at 37 weeks because of intrauterine growth restriction and preeclampsia. Postpartum, she developed hypoxia and was found to have differential cyanosis and suprasystemic pulmonary pressures because of an unrepaired patent ductus arteriosus. Pulmonary vasodilator therapy was commenced. She was discharged from hospital at day 12 postpartum and remained stable when review

BACKGROUND: Anomalous drainage of the right superior vena cava to the left atrium is a rare form of congenital heart disease. This typically results in mild cyanosis when present as an isolated defect.
CASE SUMMARY: An asymptomatic term female demonstrated an SpO2 level of 88% despite supplemental oxygen. Transthoracic echocardiography with agitated saline contrast injection revealed drainage of upper-extremity venous blood to the left atrium. Advanced imaging confirmed the diagnosis and revealed partial anomalous pulmonary venous return. Hemodynamic instability noted following elective repai

OBJECTIVE: To describe the clinical signs, treatment, and outcome in a dog after ingestion of heroin packets and repeated exposure.
CASE SUMMARY: A 3-year-old intact female American Pit Bull Terrier was referred for management of suspected drug intoxication after possible exposures to marijuana, cocaine, oxycodone, and heroin. The dog was initially treated with naloxone (0.02 mg/kg, IM) after presenting to the primary care veterinarian with cyanosis and unresponsiveness. The dog showed improvement, was then transferred to the authors' hospital, and was discharged the next day. Two days after

OBJECTIVE: Patients with failing single-ventricle physiology at the bidirectional Glenn stage remain among the most challenging candidates for mechanical circulatory support. Historically, outcomes after ventricular assist device (VAD) implantation were poor with early reports describing survival as low as 30%; unique aspects of Glenn circulation may limit effective ventricular decompression and exacerbate cyanosis. More contemporary multicenter experience suggests improving survival. We evaluated the outcomes of a stepwise VAD strategy in patients with failing Glenn circulation, consisting of

The aim was to investigate and clarify the cause of death of a 26-year-old male found dead in his apartment. It was known that the individual used drugs of abuse. In the apartment, the police found amongst others a "brown powder" and a glass bottle labeled with "2-methyl-2-butanol" (2m2b). Autopsy and chemical analyses were performed. The autopsy revealed unspecific morphological signs of an intoxication (cerebral swelling and cyanosis, pulmonary edema, full urine bladder). Headspace gas chromatography with flame ionization detection (HS-GC-FID) was used to determine 2m2b and ethanol. Concentr

Spontaneous pulmonary artery (PA) dissection is an extremely rare and life-threatening complication of chronic pulmonary hypertension, most commonly associated with congenital heart disease. We report a 54-year-old woman who presented with sharp chest pain, progressive dyspnea, and differential cyanosis between the upper and lower extremities. Laboratory evaluation revealed secondary erythrocytosis and elevated NT-proBNP. Transthoracic echocardiography demonstrated a markedly dilated pulmonary trunk, severe pulmonary hypertension, and a large patent ductus arteriosus with bidirectional shuntin
