C3 Glomerulopathy in Autoinflammatory-Pancytopenia Syndrome: A Case Report.

Autoinflammatory-pancytopenia syndrome (AIPCS) is a rare autosomal recessive disease caused by a mutation in the deoxyribonuclease II (DNASE2) gene. It has mainly haematological manifestations and recurrent fevers. The kidney could be involved in the form of proteinuria, membranoproliferative glomerulonephritis (MPGN) or immune complex deposition rather than acute kidney injury (AKI) or C3 glomerulopathy (C3G). A six-year-old male child presented with high-grade fever, AKI, generalized edema and hypertension. He had a history of recurrent hospital admission with fever, arthritis, skin rash and

Case Report: Myelodysplastic changes and 7q deletion in a case of vitamin B12 deficiency: a diagnostic pitfall.

Vitamin B12 plays a crucial role in DNA synthesis, and its deficiency can occasionally present with atypical hematological manifestations mimicking myelodysplastic syndrome (MDS). There is limited literature describing reversible chromosomal abnormalities associated with vitamin B12 deficiency. We report an 11-year-old girl who presented with generalized body aches for 1 month and fever, easy fatigability, and respiratory distress for 2 days. Examination revealed pallor and hepatomegaly, with no splenomegaly. Investigations showed severe anemia (hemoglobin 3.1 g/dL), thrombocytopenia, a low co

Clinical and etiological profile of patients with bitemporal optic disc pallor visiting at a tertiary eye hospital.

BACKGROUND: Bitemporal optic disc pallor may result from chiasmal compression as well as bilateral toxic, nutritional, hereditary, or inflammatory optic neuropathies. Data describing its clinical and etiological profile in Nepal are limited. This study was conducted with the aim to describe the clinical and etiological profile of patients with bitemporal optic disc pallor and to evaluate the relationship between clinical, visual-field, neuroimaging, and OCT findings. METHODS: A hospital-based prospective, cross-sectional study was conducted among patients with clinically documented bitemporal

Transient Whitish Change in the Gastric Mucosa Induced by Water-Jet Flushing.

Localized transient whitening of the gastric mucosa after water-jet flushing through the auxiliary water channel is rarely described. We report a 49-year-old man who underwent endoscopic submucosal dissection for a 5-mm gastric-type neoplasm on the greater curvature of the lower gastric body. During washing of adjacent non-lesional mucosa, strong auxiliary water-channel flushing for approximately 2-5 s was followed by a focal whitish linear change confined to the flushed area, which became evident approximately 10 s after flushing was stopped and persisted for approximately 30 s before resolvi

Increased mean corpuscular hemoglobin concentration: A case report as an early clue of chronic severe hyponatremia.

RATIONALE: Mean corpuscular hemoglobin concentration (MCHC) is a stable parameter, and its true elevation typically indicates hematological disorders. Although occasional reports suggest an association with electrolyte imbalances, the underlying mechanisms remain unclear. PATIENT CONCERNS: An 81-year-old female was admitted with a 3-day history of altered mental status. Complete blood count revealed a markedly elevated MCHC (369 g/L), and peripheral blood smear showed reduced central pallor and mildly hyperchromic erythrocytes, suggestive of cellular dehydration. DIAGNOSES: Chronic severe hy

PLCG1 gene mutation in a child with autoimmune hemolytic anemia: a case report.

Autoimmune hemolytic anemia (AIHA) is a highly heterogeneous disease characterized by immune-mediated destruction of red blood cells through cellular and humoral mechanisms, complement activation, cytokine release, and bone marrow compensation. Clinically, AIHA exhibits a broad spectrum of severity, ranging from mild or compensated anemia to life-threatening anemia. Traditionally, therapeutic approaches have relied on corticosteroids, immunosuppressive agents, and splenectomy as the mainstay treatment. Increasing evidence suggests that genetic factors, including variants in the PLCG1 gene-whic

Drainage of a Tense Propofol Extravasation Through a Pre-existing Intravenous Tract in an Adolescent: A Case Report.

Propofol extravasation is often self-limited but can cause tissue necrosis and, rarely, compartment syndrome. A healthy 13-year-old girl (35 kg) undergoing hysteroscopy developed extravasation at the antecubital fossa during the manual administration of 10 mL of propofol 1% through a previously placed 24-gauge peripheral intravenous catheter in the right cephalic vein. A 5-mL saline flush immediately beforehand caused neither pain nor visible swelling. Near the completion of the propofol administration, pain developed with marked swelling, pallor, and capillary refill of approximately four sec

Seven-Year Delayed Diagnosis of a Giant Suprasellar Arachnoid Cyst Presenting With Progressive Gait Disturbance: Successful Endoscopic Ventriculocystocisternostomy-A Case Report.

Suprasellar arachnoid cysts are uncommon pediatric lesions that may cause obstructive hydrocephalus and optic pathway compromise, often with insidious clinical progression. We report an 11-year-old boy who presented with a 7-year history of progressive gait disturbance and imbalance. Neurological examination was largely normal, and visual acuity was preserved; however, fundoscopic examination revealed bilateral optic disc pallor. Brain magnetic resonance imaging demonstrated a giant cerebrospinal fluid-intensity cyst centered in the third ventricle with inferior extension into the suprasellar

Phlegmasia alba dolens caused by external iliac vein compression from inflatable penile prosthesis reservoir malposition.

Phlegmasia alba dolens (PAD) is a rare manifestation of extensive iliofemoral deep venous thrombosis characterized by near-complete venous outflow obstruction, resulting in severe pain, diffuse limb swelling, and pallor, whereas arterial inflow remains preserved. PAD is a vascular emergency as progression to phlegmasia cerulea dolens and venous gangrene may occur without prompt restoration of venous outflow. A 62-year-old man with a past medical history of prostate cancer status post prostatectomy presented with dyspnea, chest pain, and acute right lower extremity swelling 1 week after inflata

Clinical Relevance of the Eating and Drinking Ability Classification System in Young Children with Cerebral Palsy: Associations with Child and Family Outcomes Relevant to Rehabilitation.

BACKGROUND: The Eating and Drinking Ability Classification System (EDACS) is a validated functional classification system for assessing feeding ability in individuals with cerebral palsy (CP). Its clinical relevance in relation to broader child and family outcomes has received limited attention, particularly in young children. This study evaluated associations between EDACS and motor function, communication ability, nutritional status, behavioral profile and caregiver quality of life in young children with CP. METHODS: This cross-sectional study included 100 children aged 18-60 months with CP

Clinicoetiological Profile and Outcomes of Shock in Mechanically Ventilated Children in a Pediatric Intensive Care Unit.

INTRODUCTION: Shock in mechanically ventilated children is associated with severe circulatory failure, tissue hypoperfusion, multiorgan dysfunction, prolonged intensive care requirement, and high mortality. Early bedside recognition of hemodynamic instability is essential, particularly because hypotension may appear late in pediatric shock. OBJECTIVES: To evaluate the clinicoetiological profile and outcomes of shock in mechanically ventilated children admitted to the pediatric intensive care unit (PICU), determine the common type of shock, assess morbidity and mortality, identify factors asso

Interrater Reliability of a Modified Bronchoscopy Scoring Tool in Children With Cystic Fibrosis.

BACKGROUND: Flexible bronchoscopy (FB) is widely used in the management of children with Cystic Fibrosis (CF) to visualize airway abnormalities, assess inflammation and detect infection. While previous scoring systems have been proposed to quantify visual airway findings in general pediatric populations, no standardized tool exists for assessing airway inflammation specific to CF. METHODS: We modified the previously proposed pediatric bronchoscopy scoring tool by adding four features relevant to CF pathology: mucus plugging, secretion viscosity, bleeding, and vascular drawing (abnormal or enh

Emergency medicine updates: Pediatric brief resolved unexplained event.

INTRODUCTION: Brief resolved unexplained events (BRUEs) can occur in a large number of pediatric patients, with most having a benign etiology. However, in many cases, these can result in unnecessary evaluations and admissions among low-risk patients. Therefore, it is critical for clinicians to be aware of the current evidence regarding BRUE in the emergency department (ED) setting. OBJECTIVE: This paper evaluates key evidence-based updates concerning pediatric BRUE for the emergency clinician. DISCUSSION: A BRUE is defined as a clinical entity occurring in an infant less than 1 year of age t

Too much of a good thing.

A 38-year-old man with poorly controlled type 2 diabetes mellitus and arterial hypertension on semaglutide presented with acute, painless, vision loss in the left eye. Ophthalmic examination revealed optic disc edema (ODE) in the left eye and optic disc pallor (ODP) in the right eye, accompanied by severe nonproliferative diabetic retinopathy and cystoid macular edema in both eyes. The combination of ODE in one eye and ODP in the fellow eye raised concern for the Foster Kennedy syndrome secondary to an intracranial mass; however, neuroimaging and systemic evaluation were unremarkable. Addison-like Clinical Phenotype Without Adrenal Insufficiency: A Rare Manifestation of Pernicious Anemia.

Severe vitamin B12 deficiency can clinically mimic Addison disease, with hyperpigmentation despite preserved adrenal function. We report a 34-year-old woman presenting with fatigue, an unintended weight loss of about 20 kg, and mucocutaneous hyperpigmentation raising concern for primary adrenal insufficiency. Examination showed pallor, mild scleral icterus, and elevated blood pressure. Laboratory testing demonstrated severe cobalamin deficiency with macrocytic anemia and biochemical evidence of intramedullary hemolysis, whereas morning cortisol and ACTH levels excluded adrenal failure. Intrins
