Unilateral MR-guided laser interstitial thermal therapy thalamotomy for refractory multiple sclerosis tremor: 12-month outcomes in four patients.

BACKGROUND: Tremor affects many people with multiple sclerosis (MS), is frequently disabling and responds poorly to medication. Deep brain stimulation, gamma-knife radiosurgery and MR-guided focused ultrasound are options, but each has constraints. MR-guided laser interstitial thermal therapy (MRgLITT) is a hardware-free stereotactic thalamotomy providing real-time thermometry, its role in MS tremor is unknown. OBJECTIVE: To describe 12-month clinical, safety and imaging outcomes after unilateral MRgLITT thalamotomy for refractory MS tremor. METHODS: Single-centre, consecutive case series of

Simultaneous hyperkinetic movement disorders phenotyping: a cross-cohort pediatric transfer study using routine videos, markerless pose estimation and a tabular foundation model.

BACKGROUND: Accurate recognition of movement disorders (MDs) phenomenology remains a demanding task in pediatric practice, compounded by mixed and evolving motor presentations, inter-examiner expertise variability, and the scarcity of structured digital tools validated across age groups. A child with combined MDs may receive different phenomenological diagnoses from different examiners, with direct consequences for etiological guidance, treatment planning, neuromodulation candidacy, and longitudinal monitoring. Video-based examination is already central to expert practice, but its systematic a

Sedation and anaesthesia in pulsed-field ablation for atrial fibrillation: mechanisms, procedural requirements, and real-world practice.

Pulsed-field ablation (PFA) has rapidly emerged as an important energy source for atrial fibrillation ablation, offering tissue selectivity and procedural efficiency while introducing specific anaesthetic and sedation requirements. This narrative review examines the rationale for anaesthesia and sedation during PFA, the mechanisms underlying pain, involuntary movement, and neuromuscular stimulation, and the implications of catheter design, waveform characteristics, and patient-related factors for peri-procedural management. Available evidence indicates that both deep sedation and general anaes

Training Non-Neurologist Healthcare Professionals in the Recognition and Management of Movement Disorders in Malawi.

BACKGROUND: Many patients with neurological disorders in Malawi are seen by non-neurologist healthcare professionals due to the low number of neurologists available in the country. Movement disorders are a growing concern at neurology clinics and patients often present late, having spent years before getting a neurology referral. In line with the WHO's global action plan on improving neurological care, we organized a workshop for non-neurologist healthcare professionals on the clinical overview of common movement disorders. METHODS: The 2-day in-person workshop was held in Blantyre city and i

Imaging Characteristics in the Ataxia-Neuropathy Spectrum Related to POLG Mutation.

Mutations in the mitochondrial DNA polymerase gamma (POLG) gene cause a broad spectrum of neurodegenerative disorders, including the ataxia-neuropathy spectrum (ANS). Substantial phenotypic heterogeneity and overlapping clinical features can delay diagnosis. We report a 38-year-old woman with no significant past medical history who presented with a three-year history of slowly progressive sensory ataxia that worsened in the dark. Examination revealed mild dysarthria, large-fiber sensory loss, and a positive Romberg sign. Extensive metabolic, toxic, autoimmune, and infectious work-up was unreve

Hierarchical reorganization of cortical and cerebellar functional gradients in Parkinson's disease.

INTRODUCTION: Parkinson's disease (PD) is a common movement disorder with heterogeneous clinical manifestations, and its tremor-predominant presentations may overlap with essential tremor (ET), complicating clinical differentiation in some patients. Functional gradient analysis provides a continuous framework for characterizing macroscale brain hierarchical organization and may offer candidate imaging features for disease characterization. METHODS: We first investigated surface-based resting-state functional measures and cerebral perfusion in 50 patients with idiopathic PD and 33 healthy cont

Cytoplasmic TDP-43 Accumulation in Autosomal Recessive Spastic Ataxia of Charlevoix-Saguenay.

BACKGROUND: Autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS) is a rare, early-onset neurodegenerative disorder characterized by progressive cerebellar ataxia, spasticity and peripheral neuropathy. It is caused by biallelic mutations in the SACS gene, which encodes sacsin, a large protein involved in cytoskeletal organization and mitochondrial homeostasis, and protein quality control. Despite advances in understanding sacsin biology, the molecular mechanisms underlying neurodegeneration in ARSACS remain incompletely understood. Here, we report the first neuropathological evide

Familial COQ8B-Associated Glomerulopathy: Divergent Long-Term Outcomes and Adult Detection Through Cascade Testing.

COQ8B (formerly ADCK4)-associated glomerulopathy is an autosomal recessive primary coenzyme Q10 deficiency that typically presents with proteinuria, focal segmental glomerulosclerosis and progressive kidney dysfunction during childhood or adolescence. We report extended longitudinal follow-up of a family carrying the homozygous COQ8B c.1339dupG, p.(Glu447Glyfs*10) variant, demonstrating marked intrafamilial phenotypic heterogeneity despite an identical genotype. Two homozygous siblings developed kidney failure in childhood or adolescence and underwent living-related kidney transplantation, wit

Single-nucleotide polymorphism-directed design of allele-specific antisense oligonucleotides for spinocerebellar ataxia type 3.

Spinocerebellar ataxia type 3 (SCA3), also called Machado-Joseph disease, the most common form of autosomal dominant cerebellar ataxia, is caused by a CAG repeat expansion in ATXN3. The mutant protein is prone to aggregation and exerts toxic gain-of-function effects, whereas the wild-type protein has important neuroprotective roles as a deubiquitinase. Current treatment strategies for SCA3 lack specificity for the mutant allele. Identifying genetic variants such as single-nucleotide polymorphisms (SNPs) in linkage with the pathogenic CAG expansion offers a promising avenue to develop allele-sp

Correlation Between Cognitive Function and Serum Irisin Levels in Parkinson's Disease Patients.

INTRODUCTION: Cognitive impairment is a common non-motor symptom of Parkinson's disease (PD) that markedly reduces patients' quality of life. Given the clinical heterogeneity of PD-related cognitive impairment (PDCI), further clarification of clinical and circulating biological factors associated with cognitive status may help improve our understanding of cognitive dysfunction in PD. METHODS: In this cross-sectional study, 243 patients with PD and 90 age- and sex-matched healthy controls (HCs) were enrolled. Cognitive function was evaluated using the Montreal Cognitive Assessment (MoCA), and

CRISPR Screening Identifies SMARCAL1 and MRN as Modulators of WRN Dependency in MSI-H Colorectal Cancer.

UNLABELLED: Microsatellite instability-high (MSI-H) colorectal cancer cells depend on the Werner syndrome helicase (WRN) to resolve cruciform DNA structures that arise from expanded TA-dinucleotide repeats. Loss of WRN induces replication stress and double-strand breaks, a vulnerability that can be recapitulated with the selective WRN inhibitor (WRNi) HRO761 in MSI-H cancer cells. To uncover the mechanisms governing sensitivity to WRN inhibition, we conducted genome-wide CRISPR/Cas9 screens in colorectal cancer cell lines treated with or without HRO761. These screens identified SMARCAL1 as a k

Beyond the Classic Triad: A Case of GQ1b Antibody-Positive Bickerstaff Brainstem Encephalitis Overlap Syndrome.

Anti-GQ1b antibody syndrome encompasses related postinfectious neurologic disorders, including Miller Fisher syndrome, Guillain-Barré syndrome, and Bickerstaff brainstem encephalitis. Recognizing overlap among these phenotypes can be difficult in the intensive care unit because profound unresponsiveness and loss of brainstem reflexes may be attributed to sedation or structural brain injury. A middle-aged man developed perioral numbness, blurred vision, ataxia, extremity paresthesias, and facial weakness after an upper respiratory infection. His condition progressed rapidly to respiratory failu

Diaphragm Ultrasound-Guided PEEP Titration in COPD with Dynamic Hyperinflation.

Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation, dynamic hyperinflation, and intrinsic positive end-expiratory pressure (iPEEP), which increase inspiratory threshold load and complicate ventilatory support. Non-invasive ventilation (NIV) is the preferred initial ventilatory support for COPD exacerbations complicated by acute or acute-on-chronic respiratory acidosis, while invasive mechanical ventilation (IMV) may be required when NIV fails. External PEEP may reduce the inspiratory threshold load in patients with expiratory flow limitation, although excessive

Childhood-onset parkinsonism, dystonia and severe muscle wasting in a Sudanese girl caused by a novel homozygous DnaJ heat shock protein family (heat shock protein 40) member C6 frameshift variant, with an incidental pathogenic breast cancer 1 gene secondary finding: a case report.

BACKGROUND: DNAJC6-related juvenile parkinsonism is a rare autosomal recessive disorder of auxilin, a co-chaperone essential for clathrin-mediated synaptic vesicle recycling. Fewer than 50 kindreds have been reported worldwide, and the condition is easily missed when first-line investigations come back normal. We describe a Sudanese adolescent in whom a previously unreported homozygous DNAJC6 frameshift variant was identified only after a prolonged diagnostic journey that included a period during which her illness was mistakenly attributed to a functional neurological disorder. Clinical whole-

The optics of introspection: a two-axis model of self-reflective distortion.

What kind of instrument do we use when we examine ourselves-and how much of what we see is shaped by that instrument rather than by what we actually are? I propose here that self-reflective cognition operates through a system subject to two separable classes of aberration. Directional calibration bias (Axis I) describes a stable functional tendency to systematically inflate (positive bias) or deflate (negative bias) the self-image across domains. The mediating field (Axis II) describes the temporal, interpersonal, cultural, and perspectival conditions through which self-information passes befo

A study of gender-specific differences in online platform in medical education.

BACKGROUND: This study explores gender-specific learning patterns among medical students using a national online educational platform in France. Although the proportion of women admitted to medical schools has steadily increased, persistent gender disparities remain in medical career progression. These inequalities may already emerge during university training, influencing learning behaviors. Understanding when and how these differences appear is crucial to designing targeted interventions that promote equity in medical education. METHODS: We analyzed anonymized data from all sixth-year medic
