Peripheral Nerve Stimulation for Essential Tremor: a Systematic Review and Meta-Analysis of Effectiveness and Safety.

BACKGROUND: Essential tremor (ET) is the most prevalent movement disorder worldwide. There are limited current pharmacological therapies, and their side effects often preclude adequate doses. Surgical therapies, such as Deep Brain Stimulation (DBS) and Focused Ultrasound (FUS), are reserved for patients with tremor that debilitates activities of daily living (ADLs). Peripheral nerve stimulation (PNS), including transcutaneous afferent patterned stimulation (TAPS) and transcutaneous peripheral nerve stimulation (TPNS), has emerged as a promising non-invasive neuromodulatory approach for many pa

CXCL9-driven immune rewiring and tumor-intrinsic ATR signaling inhibition restore radiotherapy response in lung cancer.

BACKGROUND: Non-small cell lung cancer (NSCLC) accounts for ~ 85% of lung cancers and is associated with poor outcomes in advanced stages. Radiotherapy (RT) is central to NSCLC treatment, yet locoregional relapse remains frequent. Increasing evidence indicates that, beyond tumor-intrinsic adaptations, the tumor microenvironment (TME) and immune contexture critically influence response to irradiation. However, most radioresistance models rely on human cell lines unsuitable for immunocompetent systems, limiting insight into immune-tumor interactions. METHODS: We generated radioresistant (RR) is

Moderate-risk variants in hereditary breast and ovarian cancer: seven-year experience from a specialized center.

Moderate-risk gene variants represent an increasingly recognized component of hereditary breast and ovarian cancer (HBOC) predisposition, yet their clinical interpretation and management remain less standardized than those of high-penetrance genes. This study provides a seven-year real-world evaluation of genetic variants in ATM, BARD1, CHEK2, RAD51C, and RAD51D in individuals undergoing germline testing at a specialized HBOC center, with a particular focus on variant distribution and associated clinical management. We retrospectively analyzed germline testing data from 3416 individuals evalua

A person-centered continuity pathway for pediatric contrast-enhanced imaging: sustaining one multisensory context across home, hospital, and scanner transitions.

BACKGROUND: Pediatric contrast-enhanced computed tomography (CT) and magnetic resonance imaging (MRI) require sustained cooperation, procedural immobility, and venous access, creating operational and experiential challenges for children, families, and imaging services. Existing nonpharmacological interventions generally address isolated procedural segments and do not preserve a familiar context across home, hospital, and scanner transitions. INNOVATION: Euterpe-Radiology (Euterpe-RAD) is a consent-gated, five-module institutional pathway that sustains one autobiographically anchored multisens

Local infiltration anesthesia (anterior field block) vs. general anesthesia for thyroidectomy: effects on operative time, complications, and cost-a GRADE-assessed systematic review and meta-analysis with subgroup analysis.

INTRODUCTION: Thyroidectomy involves the removal of part or all of the thyroid gland, commonly performed for goiters, hyperthyroidism, or carcinomas, with 450,000-500,000 procedures performed annually in the United States. General anesthesia is traditionally used, providing optimal surgical conditions but associated with risks such as cardiovascular complications and longer hospital stays. Local anesthesia is gaining interest for high-risk patients, offering potentially similar outcomes to general anesthesia, while reducing postoperative complications and costs. The choice of anesthetic techni

A Narrative Review on Intra-Articular Injection Therapies for Knee Osteoarthritis: Current Evidence and Future Perspectives.

This narrative review summarises current evidence on intra-articular injection therapies for knee osteoarthritis. Knee osteoarthritis (OA) is a major global cause of disability, particularly among older adults, prompting growing interest in non-operative strategies that alleviate symptoms, preserve joint function, and delay surgical intervention. Established options including corticosteroids and hyaluronic acid, remain widely utilized for short and intermediate-term symptom relief. Regenerative approaches such as platelet-rich plasma (PRP) and mesenchymal stem cells (MSCs) have gained prominen

Edited HSPCs rescue frataxin deficiency and inflammation-driven multisystemic pathology in Friedreich's ataxia mice.

Friedreich's ataxia (FRDA) is a multisystemic neurodegenerative disorder caused by GAA repeat expansion in intron 1 of frataxin (FXN), resulting in FXN deficiency and mitochondrial dysfunction. Current therapies do not correct the underlying genetic defect. Here, we demonstrate that ex vivo CRISPR/Cas9 excision of the GAA expansion in hematopoietic stem and progenitor cells (HSPCs) provides durable, systemic therapy for FRDA. A single infusion of gene-edited Sca1+ HSPCs, mPPL-001, into myeloablative, humanized YG8JR mice resulted in long-term engraftment of donor-derived cells across hematopoi

The role of transcranial direct current stimulaton on ataxia: a scoping review.

Cerebellar ataxias are a diverse group of hereditary and acquired disorders that impair balance, gait, posture and ocular movements. With limited treatment options, particularly in hereditary forms, neuromodulation has emerged as a promising therapeutic approach particularly the transcranial direct current stimulation (tDCS). This scoping review aims to discuss the role of tDCS as a potential therapy in reducing ataxia symptoms in both hereditary and acquired forms. A total of 787 studies were found after searching Pubmed, EMBASE, and Cochrane. After removing 273 duplicates, 514 studies remain

Directional Confinement Governed Ion Transport for Efficient Actuation.

Ionic transport kinetics in electrolytes are a key determinant of ionic actuator performance. Conventional random porous electrolytes are constrained by porosity and tortuosity, leading to an intrinsic limitation in ion mobility within the Bruggeman framework. Here, structural anisotropy is introduced to establish a unified framework that analyzes ion transport from random diffusion to directionally confined regimes. To validate this concept, anisotropic electrolytes with oriented channels are fabricated via directional freeze-drying. These structures exhibit pronounced directional dependence,

Spinal neurocysticercosis: synthesizing evidence on clinical presentations, anatomical patterns, diagnostic approaches, and therapeutic strategies.

Spinal neurocysticercosis (NCC) is an uncommon manifestation of NCC involving the spinal cord, meninges, nerve roots, and subarachnoid spaces. Although less common than intracranial disease, spinal involvement can cause substantial neurological morbidity because even small lesions within the confined spinal canal may produce progressive myelopathy. Disease may result from hematogenous dissemination or cerebrospinal fluid (CSF) spread of Taenia solium larvae and may present as intramedullary, intradural extramedullary, subarachnoid, or, rarely, extradural lesions. Clinical manifestations are di

Early mobilization in neurocritical care as a threshold-regulated intervention: a narrative review of mechanistic integration and implications for neurological recovery and physiological stability.

The integration of early mobilization into neurocritical care remains constrained by uncertainty regarding how heterogeneous physiological systems respond to graded activity during neurological instability. Although early mobilization is increasingly used in intensive care, the relationships between intervention timing, intensity, neurological recovery, functional outcomes, and physiological tolerance remain incompletely defined in acute neurocritical populations. This narrative review integrates clinical and mechanistic evidence to examine how immobility and reactivation may influence interco

Thoracolumbar pain associated with cervical radiculopathy successfully managed with combined cervical periradicular injection and ultrasound-guided percutaneous electrical nerve stimulation therapy: a case report.

BACKGROUND: Cervical radiculopathy may occasionally present with atypical pain patterns extending beyond the classical cervical and upper-limb distribution, potentially leading to diagnostic uncertainty. The thoracodorsal nerve originates predominantly from the C6-C8 roots and innervates the latissimus dorsi muscle, providing a possible anatomical basis for referred pain involving the posterior thoracolumbar region. This case describes an unusual presentation of cervical radiculopathy manifesting predominantly as severe thoracolumbar pain and its management using a combined interventional appr

The isolated HEPN domain of SACSIN exhibits RNA-binding activity.

Autosomal Recessive Spastic Ataxia of Charlevoix-Saguenay (ARSACS) is a neurodegenerative disorder caused by mutations in the SACS gene, though the molecular function of its protein product, SACSIN, remains elusive. Therapeutic strategies for ARSACS are limited, mostly due to the exceptionally large size of SACSIN (~ 520 kDa), which precludes conventional gene therapy and standard molecular delivery methods. Over 200 pathogenic variants, including missense, nonsense and frameshift mutations, have been identified. Regardless of variant type, patients uniformly exhibit cerebellar ataxia, spastic

Hemi-see-saw nystagmus with superimposed pendular oculopalatal tremor: case report and systematic review.

BACKGROUND: Hemi-see-saw nystagmus (HSSN) is an uncommon subtype, characterized by elevation and intorsion of one eye with contralateral depression and extorsion, typically in a jerk pattern. It is associated with unilateral lesions of the interstitial nucleus of Cajal and the medial longitudinal fasciculus, both of which are involved in the ocular tilt reaction (OTR). Although its association with oculopalatal tremor (OPT) has been described, it is rare. METHODS: We performed a systematic review of seven databases on HSSN, extracting data on demographics, etiology, lesion location, oculomoto

CNPY3 variants associated with a recessive developmental and epileptic encephalopathy affect inflammasome and TLR function.

BACKGROUND: Canopy FGF signalling regulator 3 (CNPY3) is an endoplasmic reticulum (ER)-resident chaperone essential for toll-like receptor (TLR) folding, trafficking and inflammasome activation. In rare paediatric cases, biallelic CNPY3 variants have been associated with developmental and epileptic encephalopathy (DEE), but available data remain sparse. METHODS: 50 individuals from 38 unrelated families with biallelic CNPY3 variants and DEE were evaluated for exome and genome findings, standardised clinical phenotype descriptions, EEG results, and neuroimaging data.
