Therapeutic Hybrid Intelligence with Neural and Knowledge-based Expert Reasoning for SRS (THINKERS): an AI Model for Functional Outcome Prediction After Gamma Knife Thalamotomy for Essential Tremor.

Outcome prediction after Gamma Knife thalamotomy for essential tremor remains largely guided by baseline tremor severity, target anatomy, radiosurgical dose parameters, and institutional experience rather than individualized estimates of functional improvement, latency to benefit, and adverse radiation effect risk. We developed THINKERS-ET, a mixture-of-experts (MoE) artificial intelligence framework for personalized functional outcome prediction and dose-policy evaluation after Gamma Knife thalamotomy. We performed a retrospective single-center treatment-level study of patients with medically

A Diphenhydramine Overdose Mimicking Encephalitis, Complicated by Acute Kidney Injury Associated with Anticholinergic Urinary Retention: A Case Report.

A 26-year-old man presented with fever, an altered consciousness, tremor, mydriasis, tachycardia, and a dry oral mucosa and was empirically treated for suspected encephalitis. A rapid neurological improvement and anticholinergic findings prompted the reconsideration of intoxication. A family assisted room search revealed empty over-the-counter medication bottles, and he later reported ingesting 2.4 g of diphenhydramine at once. Acute kidney injury developed in association with diphenhydramine-induced urinary retention, and his urine output and renal function improved after catheterization. In

Longitudinal Changes in Brain Connectivity and Structure after MRgFUS VIM Thalamotomy for Essential Tremor.

Essential Tremor (ET) is the most common movement disorder. Magnetic Resonance-guided Focused Ultrasound (MRgFUS), an incisionless surgery involving thermal ablation of targeted brain regions, like the Ventral Intermediate Nucleus of the Thalamus (VIM) and surrounding white matter tracts, is an effective treatment option. Fractional Anisotropy (FA), a metric of diffusion tensor imaging, can be used to determine white matter structural integrity and their changes following MRgFUS. This study investigates white matter and morphological brain remodeling after MRgFUS VIM thalamotomy and its associ

AI-enhanced intraoperative decision-making in robotic surgery: from augmented reality navigation to foundation models.

Robotic surgical platforms have improved instrument dexterity, tremor filtration, and ergonomic control, yet intraoperative decisions still depend on a surgeon interpreting a moving, partially observed field under time pressure. Artificial intelligence (AI) is increasingly used to close parts of this loop by recognizing anatomy, tracking instruments, anticipating events, and-experimentally-planning or executing selected actions. This review maps the computational technologies that support intraoperative decision-making in robotic surgery and appraises the clinical evidence for them.Because the

Development and Validation of the 9-item Parkinson's Disease Screening Questionnaire.

OBJECTIVE: We aimed to develop and validate a brief, self-administered questionnaire integrating motor and prodromal non-motor symptoms for screening Parkinson's disease (PD). METHODS: We conducted a three-phase study. Phase 1 used a prospectively registered systematic review of 107 studies to generate a 52-item draft. In Phase 2, the draft was refined in 205 participants (122 with early PD and 83 healthy controls) using item-level areas under the receiver operating characteristic curves (AUCs) and age- and sex-adjusted multivariable logistic regression. Phase 3 validated the 9-item Korean PD

A clinically interpretable model for Parkinson's disease: insights from complementary MRI modalities.

Accurate identification of Parkinson's disease may remain challenging in certain clinical scenarios, highlighting the need for a clinical decision-support model based on complementary MRI modalities. The case-control study included 462 participants, split into training [247 Parkinson's disease, 122 normal controls (NC)] and testing (60 Parkinson's disease, 33 NC) cohorts to construct and validate a clinical decision-support model. We employed least absolute shrinkage and selection operator and logistic regression analysis to develop the model utilizing quantitative susceptibility mapping, arte

DHDDS variants in epilepsy and movement disorders: Two novel mutations and an updated literature review.

INTRODUCTION: Recent evidence suggests that dehydrodolichol diphosphate synthase (DHDDS, MIM*608,172) gene variation can cause neurological diseases. However, the complete clinical and genetic spectrum of DHDDS-related disorders has not been fully elucidated. This study aims to systematically describe the clinical and genetic features of five patients with molecularly confirmed DHDDS variants and to provide an updated review of previously reported cases. METHODS: We retrospectively enrolled patients with DHDDS gene variants in the Epilepsy Center of Beijing Tiantan Hospital, Capital Medical U

Newly Diagnosed Graves' Disease Followed by the Discovery of a Cavernous Internal Carotid Artery Aneurysm Causing Panhypopituitarism: A Case Report.

Sellar and parasellar vascular lesions are rare causes of pituitary dysfunction and may mimic pituitary adenomas or other sellar masses. Cavernous internal carotid artery (ICA) aneurysms can cause hypopituitarism through compression of the pituitary gland or stalk, but diagnosis may be delayed when symptoms are initially attributed to a more common endocrine disorder. We report a 37-year-old man who presented with six weeks of unintentional weight loss, fatigue, heat intolerance, excessive sweating, anxiety, tremor, and tachycardia. Initial evaluation showed suppressed thyroid-stimulating horm

Simultaneous hyperkinetic movement disorders phenotyping: a cross-cohort pediatric transfer study using routine videos, markerless pose estimation and a tabular foundation model.

BACKGROUND: Accurate recognition of movement disorders (MDs) phenomenology remains a demanding task in pediatric practice, compounded by mixed and evolving motor presentations, inter-examiner expertise variability, and the scarcity of structured digital tools validated across age groups. A child with combined MDs may receive different phenomenological diagnoses from different examiners, with direct consequences for etiological guidance, treatment planning, neuromodulation candidacy, and longitudinal monitoring. Video-based examination is already central to expert practice, but its systematic a

Training Non-Neurologist Healthcare Professionals in the Recognition and Management of Movement Disorders in Malawi.

BACKGROUND: Many patients with neurological disorders in Malawi are seen by non-neurologist healthcare professionals due to the low number of neurologists available in the country. Movement disorders are a growing concern at neurology clinics and patients often present late, having spent years before getting a neurology referral. In line with the WHO's global action plan on improving neurological care, we organized a workshop for non-neurologist healthcare professionals on the clinical overview of common movement disorders. METHODS: The 2-day in-person workshop was held in Blantyre city and i

Hierarchical reorganization of cortical and cerebellar functional gradients in Parkinson's disease.

INTRODUCTION: Parkinson's disease (PD) is a common movement disorder with heterogeneous clinical manifestations, and its tremor-predominant presentations may overlap with essential tremor (ET), complicating clinical differentiation in some patients. Functional gradient analysis provides a continuous framework for characterizing macroscale brain hierarchical organization and may offer candidate imaging features for disease characterization. METHODS: We first investigated surface-based resting-state functional measures and cerebral perfusion in 50 patients with idiopathic PD and 33 healthy cont

Correlation Between Cognitive Function and Serum Irisin Levels in Parkinson's Disease Patients.

INTRODUCTION: Cognitive impairment is a common non-motor symptom of Parkinson's disease (PD) that markedly reduces patients' quality of life. Given the clinical heterogeneity of PD-related cognitive impairment (PDCI), further clarification of clinical and circulating biological factors associated with cognitive status may help improve our understanding of cognitive dysfunction in PD. METHODS: In this cross-sectional study, 243 patients with PD and 90 age- and sex-matched healthy controls (HCs) were enrolled. Cognitive function was evaluated using the Montreal Cognitive Assessment (MoCA), and
