Cystic echinococcosis in patients with leukemia: Clinical challenges and review of reported cases.

OBJECTIVES: Cystic echinococcosis (CE) rarely coexists with hematologic malignancies. We report two Peruvian men with leukemia and CE. CASE REPORT: One patient had chronic lymphocytic leukemia and a hepatic CE2 cyst managed with albendazole and surgery before chemoimmunotherapy, and another patient had B-cell acute lymphoblastic leukemia and prior hepatic and pulmonary CE treated with concurrent albendazole and induction chemotherapy. Both patients lived in endemic highland areas and had childhood exposure to dogs. DISCUSSION: A literature review identified only a few cases, mostly acute mye

Apical cardiac hydatid cyst compressing the left ventricle: An uncommon manifestation of echinococcosis.

BACKGROUND: Cardiac hydatidosis is a rare but serious manifestation of Echinococcus granulosus infection, accounting for less than 2 % of all hydatid disease cases. Ventricular involvement is exceptional, and cyst degeneration can mimic infection. Prompt recognition and surgical intervention are essential to prevent complications such as rupture, tamponade, or coronary compression. CASE PRESENTATION: A 36-year-old woman with prior hepatic hydatid cystectomy presented with progressive dyspnea. Echocardiography and CT revealed a large cystic lesion compressing the left ventricular apex. The cys

Salvianolic acid C attenuates liver fibrosis and inhibites hepatic stellate cell activation by targeting FAP/TβRI/SMAD axis.

BACKGROUND: Liver fibrosis remains a major clinical challenge with limited effective therapeutic options. Salvianolic acid C (SAC), a major water-soluble compound derived from Salvia miltiorrhiza, has shown anti-fibrotic activity in various disease models; however, its role and underlying mechanisms in liver fibrosis remain unclear. METHODS: A CCl4-induced mouse model was used to assess the in vivo anti-fibrotic activity of SAC. LX-2 hepatic stellate cells (HSCs) were used to investigate the underlying mechanisms in vitro. Network pharmacology, machine learning, molecular docking and molecula

Surgical treatment of extrahepatic intraabdominal hydatid cysts: A single-centre retrospective analysis.

Cystic echinococcosis is a zoonotic disease caused by Echinococcus granulosus. Although Echinococcal cysts are most frequently located in the liver (59-75%), in rare circumstances they may develop in extra-hepatic locations. Because extrahepatic cysts are rare and lack the characteristic appearance of hepatic cysts, diagnosis can be challenging. In the present retrospective study, the medical records of 41 patients surgically treated for intra-abdominal extrahepatic hydatid cyst disease were analysed to collect the following: Demographic data, cyst localization, serological test results, surgi

Glycogen content and expression of CD10 and α-SMA in the microenvironment of viable human hepatic hydatid cysts.

BACKGROUND: While hepatic cystic echinococcosis (CE) has been extensively studied in animal models, its impact on the human liver microenvironment remains unclear. Elucidating its pathogenic mechanisms in humans may reveal novel prognostic biomarkers and potential therapeutic targets. We aimed to investigate the impact of CE on the glycogen content of hepatocytes, CD10 expression in the bile canaliculi, and the profibrotic α-smooth muscle actin (α-SMA) in the adjacent portal areas. METHODS: The study involved 20 cases with hepatic hydatid cysts and 20 controls. Histopathological and viability

Hepatic Echinococcosis as an Incidental Finding in a Patient With a Perforated Gastric Ulcer: A Case Report.

Echinococcosis is a zoonotic helminth infection with global distribution and a large public health impact. In Brazil, cases are traditionally associated with the Southern region, but mortality records show a substantial proportion of cases occurring in the Northeast, supporting underrecognition outside historically endemic areas. Herein, we report a case of a 67-year-old man who presented in early April 2025 with an acute abdomen due to a perforated gastric ulcer. He reported childhood exposure to sheep and dogs in rural Paraíba. Computed tomography (CT) demonstrated pneumoperitoneum and an in

Hepatic mucinous cystic neoplasm with hematoma: A case report.

We report a case of mucinous cystic neoplasm that presented with intratumoral bleeding. The tumor showed unusual imaging morphology mimicking echinococcosis and other cystic liver lesions. A woman in her 60s with frequent contact with multiple dogs presented with an incidentally detected liver mass. Computed tomography and magnetic resonance imaging revealed a 53-mm lobulated hepatic mass with a central hematoma and peripheral cysts. Differential diagnoses included hemorrhagic cyst, echinococcosis, hepatocellular adenoma, mucinous cystic neoplasm, and metastatic tumors. Serologic testing for e

When Silence Speaks: A Perforated Hepatic Cyst, Intact Omental Cyst, and Cholelithiasis.

A 39-year-old man from an echinococcosis-endemic region presented with acute left upper quadrant pain. Imaging revealed cholelithiasis, a hepatic and a large abdominal cystic masses, and significant free peritoneal fluid. Urgent laparotomy unexpectedly disclosed three concurrent pathologies: chronic calculous cholecystitis, a perforated hepatic hydatid cyst, and an intact omental hydatid cyst. The exact source of his symptoms remains uncertain. This case highlighted the diagnostic challenge of coexisting surgical diseases in endemic regions.

Role of interventional radiology in the management of hepatic hydatid disease.

Hydatid disease, caused by the larval stage of Echinococcus granulosus, is a significant public health problem in endemic regions and worldwide. The liver is the most frequently affected organ, followed by the lungs, in adults. Hepatic hydatid cysts (HHC) may remain asymptomatic for years and are often incidentally detected on imaging. Radiological evaluation plays a pivotal role in the diagnosis and classification of hydatid cysts into World Health Organization types (cystic lesion and cystic echinococcus 1 through cystic echinococcus 5). Further, imaging also helps plan appropriate managemen

A Novel Approach to Recurrent Hepatic Hydatid Cyst Using EUS-Guided Aspiration and Ethanol Injection: A Case Report and Focused Literature Review.

BACKGROUND: Hepatic hydatid disease, a manifestation of cystic echinococcosis caused by Echinococcus granulosus, remains endemic in many regions and poses a persistent therapeutic challenge. Although surgery is often employed for definitive management, recurrence, procedural risks, and anatomical constraints have led to growing interest in minimally invasive alternatives such as percutaneous aspiration and endoscopic interventions. CASE PRESENTATION: We report a 52-year-old male with an incidentally detected hepatic hydatid cyst who initially underwent surgical cystectomy and cholecystectomy.

Differential activation of hepatic stellate cells by Echinococcus multilocularis and Echinococcus granulosus protoscoleces.

Echinococcosis, caused by Echinococcus multilocularis (Em) and Echinococcus granulosus (Eg), induces distinct patterns of liver fibrosis. Understanding how protoscoleces (PSCs) from these species interact with hepatic stellate cells (HSCs) is crucial to elucidating the mechanisms of fibrogenesis. In this study, HSC-LX2 cells were co-cultured with Em and Eg PSCs. ELISA was used to quantify collagen-I, α-smooth muscle actin (α-SMA), osteopontin (OPN), and TNF-α levels, while transmission electron microscopy (TEM) examined ultrastructural changes. Both E. multilocularis- and E. granulosus-derived

Traumatic rupture of a hepatic hydatid cyst in a polytrauma patient: a case report highlighting diagnostic and management challenges.

INTRODUCTION AND IMPORTANCE: Traumatic rupture of hepatic hydatid cysts is a rare but life-threatening complication of echinococcal disease. Rupture following blunt abdominal trauma can lead to anaphylaxis, massive peritoneal contamination, and rapid hemodynamic deterioration. Early recognition is essential, especially in endemic regions. CASE PRESENTATION: A 19-year-old, previously healthy male sustained polytrauma in a high-speed motor vehicle accident. Initial assessment showed a Glasgow Coma Scale of 3 and a fixed left pupil. Computed tomography demonstrated a left subdural hemorrhage and

A Rare Case of Extensive Relapsing Disseminated Hydatid Disease with Multi-Organ Involvement: A Case Report.

BACKGROUND: Hydatid disease, caused primarily by Echinococcus granulosus, remains a significant public health challenge in endemic regions. While hepatic (80-85%) and pulmonary (15-20%) involvements are common, multi-organ dissemination involving rare sites such as the pericardium, diaphragm, and mediastinum occurs in less than 0.1-2% of cases. CASE PRESENTATION: We present a rare case of a 26-year-old male, a farmer for 10 years, with occupational exposure to dogs and horses, with a personal history of multiple surgically treated abdominal cysts in 2016, admitted after abdominal computed tom

Diagnosis and treatment of hepatic hilar lymph node metastasis in hepatic alveolar echinococcosis patients: a real-world single-center experience.

BACKGROUND: In patients with hepatic alveolar echinococcosis (HAE), germinal cells of Echinococcus multilocularis (EM) might invade regional lymph nodes (LNs) causing LN metastasis. If infected LNs are not removed during surgery, the risk of postoperative recurrence significantly increases. We aimed to develop a preoperative prediction model for HAE hepatic hilar LN metastasis using clinical and imaging data, summarize our center's surgical experience, and analyze the long-term prognosis. MATERIALS AND METHODS: We retrospectively reviewed patients with HAE who underwent radical hepatectomy co
