Synchronous cardiac and hepatic echinococcosis in a pediatric patient: a case report.

INTRODUCTION: Hydatidosis caused by Echinococcus granulosus rarely involves the heart (0.5-2%), posing risks of rupture, tamponade, or anaphylaxis. Synchronous cardiac and hepatic disease is exceptionally uncommon in children. CASE PRESENTATION: An 8-year-old Arab male presented with abdominal pain, vomiting, fever, and dry cough; there was no prior instability or arrhythmia. Imaging revealed multiple calcified hepatic cysts and a subepicardial cyst compressing the left ventricle (with no coronary involvement). After administration of albendazole (15 mg/kg/day for 2 weeks preoperatively), a s

Primary Paraspinal Intramuscular Hydatid Cysts Mimicking a Soft Tissue Tumor: A Case Report.

Primary intramuscular hydatid disease of the paraspinal muscles is exceptionally rare, particularly in the absence of hepatic or pulmonary involvement. Its nonspecific clinical and radiological features often mimic soft tissue tumors, creating diagnostic challenges for neurosurgeons practicing in endemic regions. A 22-year-old woman presented with a slowly enlarging, mildly painful right lumbar paraspinal mass without animal exposure or systemic symptoms. Magnetic resonance imaging revealed a multiloculated cystic lesion extending from L2 to L4 that lacked definitive hydatid features and could

Surgical approaches for complex hepatic and perihepatic tumors: a single-center experience with ante situm and ex situ techniques.

BACKGROUND: Ante situm and ex situ liver surgery may offer potential treatment options for patients with locally advanced hepatic and perihepatic tumors deemed unresectable by conventional techniques. However, the complexity of the procedures and historically high morbidity and mortality rates have limited their widespread adoption. This study aims to evaluate perioperative morbidity, mortality, and long-term survival outcomes following ante situm and ex situ liver surgery. MATERIALS AND METHODS: This single-center cohort study examines a consecutive series of patients who underwent ante situ

Primary Isolated Intramyometrial Cystic Echinococcosis Presenting with Life-Threatening Anemia.

Cystic echinococcosis usually involves the liver and lungs, whereas uterine involvement is uncommon. A surgically confirmed case of primary intramyometrial echinococcosis in northern Kenya is described. A 29-year-old woman presented with severe anemia secondary to heavy menstrual bleeding. Ultrasound revealed a 12 × 12 × 14 cm intramyometrial anechoic lesion initially interpreted as a degenerating leiomyoma. An open myomectomy allowed for an intact excision without rupture. Histopathology revealed laminated acellular membranes and protoscolices consistent with cystic echinococcosis. Hepatic ul

Clinical evaluation of the Cystic Echinococcosis Complication Score for preoperative risk stratification and surgical planning in hepatic cystic echinococcosis: A retrospective derivation and comparative cohort study.

BACKGROUND: Surgical management of hepatic cystic echinococcosis is frequently associated with substantial postoperative morbidity. The absence of a validated, disease-specific risk stratification tool hinders optimal patient referral, surgical planning, and perioperative resource allocation. This study evaluates the clinical utility of the Cystic Echinococcosis Complication Score for preoperative risk assessment and its association with postoperative outcomes. METHODS: The Cystic Echinococcosis Complication Score was derived from a retrospective cohort of 746 patients who underwent echinococ

Imaging comparisons of cystic echinococcosis in HIV-positive and HIV-negative patients.

BACKGROUND: Radiological imaging is crucial for diagnosing hepatic cystic echinococcosis (HCE) and assessing complications. Case studies suggest HCE with human immunodeficiency virus (HIV) co-infection is associated with larger, more multilocular cysts, increased extrahepatic dissemination, rupture, secondary infection and biliary invasion. OBJECTIVES: To compare the radiological presentation of HCE in HIV-positive (HIV+) and HIV-negative (HIV-) patients and determine whether HIV co-infection influences imaging features. METHOD: Pre-operative imaging (chest X-rays, ultrasound, CT and MRI of

Primary diaphragmatic hydatid cyst: A rare location of echinococcosis.

INTRODUCTION: Hydatid disease most commonly affects the liver and lungs. Primary diaphragmatic involvement is exceptionally rare and often misdiagnosed as hepatic or pulmonary hydatidosis on imaging due to anatomic proximity. CASE PRESENTATION: We report a 47-year-old Tunisian woman presenting with intermittent right hypochondrium pain. Laboratory tests and hydatid serology were normal. Computed tomography (CT) imaging revealed a cystic lesion at the dome of the right hemidiaphragm with close contact to the liver, raising suspicion of a hepatic-origin hydatid cyst. Surgical exploration throug

Polarization of Hepatic Macrophages in Alveolar Echinococcosis and Its Role in Remodeling the Immune Microenvironment.

Alveolar echinococcosis (AE), caused by Echinococcus multilocularis larvae, is a severe zoonotic disease mimicking tumors, primarily affecting the liver with high mortality if untreated. Host immunity plays a pivotal role, shifting from Th1/Th17-mediated clearance to Th2/Treg-driven tolerance, enabling parasite survival. Liver macrophages, including Kupffer cells, polarize towards M2 phenotype under parasite antigens (e.g., phytic acid, exosomes), promoting immunosuppression, fibrosis, and T cell exhaustion via IL-10/TGF-β. This reshapes the tumor-like immune microenvironment with M2 macrophag

Teaching NeuroImage: Cervical Myelopathy Due to Alveolar Spinal Echinococcosis Cysts.

A 64-year-old male farmer with a history of prostate adenocarcinoma, treated with curative radiotherapy 2 years prior, presented with neck pain and weakness. On examination, he had an incomplete spastic tetraparesis (3/5 strength in upper extremities, 5/5 strength in lower extremities) and sensory level below C5. Cervicothoracic MRI showed garland-like compressive epidural masses, suggestive initially of metastases (Figure 1). Stable prostate-specific antigen and negative PET-CT argued against metastases, leading to CT-guided spinal biopsy, which revealed Echinococcus multilocularis cysts. Ser
