Cognitive function is associated with the progression of non-tremor motor symptoms in Parkinson's disease.

BACKGROUND: Parkinson's disease (PD) is characterized by motor impairment which consists of tremor and non-tremor symptoms. Cognitive function may overlap with specific aspects of voluntary movement and action initiation. This study aims to investigate associations between global cognition and the severity and longitudinal progression of tremor and non-tremor motor symptoms in PD. METHODS: As part of the Oxford Quantification in Parkinsonism (OxQUIP) study, 84 participants with PD were tested over seven visits at three-month intervals. At each visit, participants completed standardized global

Robotic-Assisted Radical Resection for Hilar Cholangiocarcinoma.

Radical resection for hilar cholangiocarcinoma is particularly challenging due to complex anatomy and high operative risks. While conventional laparoscopic surgery has been used with reported R0 resection rates of 86.3%, it is limited in precision and complex vascular or biliary reconstruction. Robotic-assisted surgery overcomes these constraints through three-dimensional visualization, enhanced instrument maneuverability, and tremor filtration. This approach is associated with reduced intraoperative blood loss compared to laparoscopic and open techniques and demonstrates superior capability i

Paired regional complementarity in diffusion MRI reveals disease-specific microstructural profiles in PD, MSA, and PSP: a feasibility study.

Parkinson’s disease (PD) is a prevalent neurodegenerative disorder characterized by tremor, rigidity, bradykinesia, and postural instability. Atypical Parkinsonian syndromes such as progressive supranuclear palsy (PSP) and multiple system atrophy (MSA) share overlapping clinical features, complicating accurate diagnosis. While prior diffusion MRI studies have used large, opaque machine-learning models, this study explored the best two diffusion metrics to differentiate Parkinsonian syndromes. This strategy introduces a compact and interpretable approach tailored for clinically relevant cohort

Impact of PSA- versus STN-DBS on effective connectivity in Parkinson's disease - a 3.0T resting-state fMRI study.

Subthalamic nucleus deep brain stimulation (STN DBS) is an established treatment for advanced Parkinson's disease (PD), whereas the posterior subthalamic area (PSA) has been proposed as an alternative target for tremor-dominant cases. However, their underlying therapeutic mechanisms have not been directly compared. Leveraging the single-trajectory dual-target DBS technique, this work utilizes high-field 3.0 T resting-state functional magnetic resonance imaging data and spectral dynamic causal modeling to investigate the differential modulatory effects of PSA and STN stimulation on effective co

Diffusion Tensor Imaging and Fiber Tractography Analysis in Patients with Pelizaeus-Merzbacher Disease.

INTRODUCTION: Pelizaeus-Merzbacher Disease (PMD) is a hypomyelinating disorder with X-linked recessive inheritance caused by mutations in the proteolipid protein 1 (PLP1) gene on chromosome Xq22. In the early stages of PMD, head tremor and pendular nystagmus are observed, while in the later stages, psychomotor developmental delay, choreoathetosis, ataxia, and spasticity are added to the clinical presentation. This study aimed to investigate the relationship between diffusion tensor imaging-fiber tractography (DTI-FT) findings, the clinical and pathogenetic features in PMD patients. METHODS: N

T2-FLAIR hyperintensities in the inferior cerebellar peduncles and their association with clinical symptoms, molecular and MRI markers in male FMR1 premutation carriers.

BACKGROUND AND OBJECTIVES: FMR1 premutation carriers (55-200 CGG repeats) are at risk of developing fragile X-associated tremor/ataxia syndrome (FXTAS), a neurodegenerative disorder associated with motor and cognitive impairment. Bilateral hyperintensities of the middle cerebellar peduncles (MCP sign) are the major radiological hallmarks of FXTAS. The inferior cerebellar peduncles (ICP) contain fibers related to proprioception and vestibular functions (such as the rostral and posterior spinocerebellar tracts and the juxta restiform body), which are clinically associated with cerebellar gait at

Long period quakes linked to Mefite D'Ansanto (Italy) mantellic CO2 emissions.

Mefite D’Ansanto is a remarkable mantle origin CO2 emissions system, and the understanding of the relative source mechanisms is one of the fundamental aspects for the comprehension of the evolution of the southern Apennines active dynamics. Moreover, due to its lively activity in a nearly quiet place, Mefite also represents a natural laboratory for the investigation of hydrothermal system dynamics. The seismic wavefield sourced by these CO2 emissions is complex, characterized by a background stationary hydrothermal tremor, with intermittent superposition wavetrains of variable duration. Here w

Incarcerated geriatric patients' experiences of aging and healthcare.

Older adults comprise the fastest growing demographic in correctional facilities in the United States. Studies have shown older adults in prison report difficulty accessing healthcare and navigating the prison environment. Little is known about how older adults navigate aging and access healthcare in the jail setting. We sought to explore experiences of older adults incarcerated in a county jail. We conducted semi-structured qualitative interviews with 20 older adults (> 50 years old) who were incarcerated at a local county jail and were referred to the emergency department for acute care need

Perioperative quantification of MDS-UPDRS-III tremor measurements in patients with Parkinson's disease using accelerometry.

Tremor in patients with Parkinson's disease (PD) is usually evaluated using the Movement Disorder Society Unified PD Rating Scale (MDS-UPDRS). Limitations of the MDS-UPDRS include subjectivity and rater-dependency, which may be overcome by accelerometry. This study focused on relating the MDS-UPDRS hand tremor scores to an objective scoring method with 3D-accelerometry. An accelerometric algorithm to measure and classify tremor of the hands according to MDS-UPDRS criteria is proposed. Sixty-four PD patients and 64 healthy controls (matched on sex and age) were included. Two raters assessed MDS

Effects of long-term treatment with transcranial pulsed electromagnetic fields in Parkinson's disease on kinematic and electrophysiological tremor.

BACKGROUND: Previous studies of the use of transcranial pulsed electromagnetic fields (T-PEMF) as a treatment for Parkinson's disease (PD) for a short period have shown positive results. The aim was to investigate the effect of long-term treatment with T-PEMF on tremor quantified by forearm muscle activation and hand acceleration in PD. METHODS: Patients (n = 14) received three 8-week periods of daily T-PEMF treatment (±50 V, 3 ms, 55 Hz, 30 min/day). Furthermore, a control group with PD (n = 8, no treatment) and a healthy reference group (n = 5) participated. The participants performed a pos

Wearable sensors for monitoring drug pharmacokinetics in patients with Parkinson's disease.

Wearable sweat sensors have attracted significant attention in recent years due to their potential for noninvasive health monitoring and point-of-care treatment. In this study, we developed and clinically evaluated wearable electrochemical sweat sensors in 32 patients with Parkinson’s disease (PD) at Taichung Veterans General Hospital, Taiwan. PD patients were treated with standard oral L-DOPA formulations, in terms of Madopar or Sinemet. PD symptoms were assessed using standard MDS-UPDRS scores, 10 clinical movement metrics, wearable inertial measurement units (IMUs) data, and sweat L-DOPA co

Focused ultrasound for blood-brain barrier opening in brain tumor patients: Technical nuances.

INTRODUCTION: Focused ultrasound (FUS) is making rapid advances in the field of neurological sciences. While proven effective for tremor, pain and dystonia, FUS hold promises in experimental applications like blood-brain barrier opening (BBBO). BBBO is an emerging non-invasive option for intracranial brain tumor treatments by enhancing drug delivery and enabling liquid biopsies. Clinical trials underscore the need for consistent use of FUS for BBBO, which requires sophisticated facilities and standardized protocols to ensure efficacy and reproducibility. This manuscript outlines a single-cente

Familial SCA14: A case report with review.

Spinocerebellar ataxia type 14 (SCA14) is a rare autosomal dominant neurodegenerative disorder caused by mutations in the PRKCG gene, which encodes protein kinase Cγ (PKCγ). The clinical manifestations are heterogeneous, ranging from slowly progressive pure cerebellar ataxia to complex phenotypes with sensory or extrapyramidal involvement. To the best of our knowledge, the present report is the first to describe a Han Chinese family carrying the PRKCG c.424T>G (p.C142G) mutation, which has previously only been described in Danish and Japanese cohorts. The proband, a 72-year-old man, developed
