Electroacupuncture mitigates oxidative stress and neuroinflammation in Parkinson's disease via mitigating autophagy-lysosome dysfunction.

BACKGROUND: Parkinson's disease (PD) is a neurodegenerative disorder characterized by autophagic dysfunction. Acupuncture is a well-established intervention recognized for its immunomodulatory and neuroprotective effects. PURPOSE: This study aimed to explore the mechanisms by which electroacupuncture (EA) interventions can ameliorate mitochondrial damage in PD. METHODS AND RESULTS: EA was applied to the chorea and tremor control area on the scalp of MPTP-induced mice. The cognitive abilities and motor functions of mice were evaluated through behavioral experiments. EA treatment alleviated be

Associations Among Pain, Depression, Cognition, and Motor Function in Parkinson's Disease.

OBJECTIVE: The authors examined the associations among depression, pain, motor function, and cognitive performance in Parkinson's disease (PD) to determine whether these relationships differ between the tremor-dominant (TD) and postural instability and gait difficulty (PIGD) motor subtypes of PD. METHODS: Cognitive performance was assessed with a broad neuropsychological test battery. Pain was measured with the King's Parkinson's Disease Pain Scale, Numeric Rating Scale, Color Analog Scale, and Douleur Neuropathique 4; motor function with the Movement Disorder Society Unified Parkinson's Dise

Explainable multimodal feature fusion networks for Parkinson's disease prediction.

Parkinson's disease (PD) is a progressive neurodegenerative disorder characterized by motor and non-motor impairments, where early diagnosis remains challenging due to reliance on subjective clinical assessments. Recent artificial intelligence (AI)-based approaches have demonstrated promise in identifying subtle PD biomarkers from individual modalities such as speech, gait, and handwriting; however, unimodal systems often fail to capture the heterogeneity of the disease and provide limited interpretability. To address these limitations, this study proposes a multimodal deep learning framework

A Gaze-Driven Robotic System for Post-Stroke Active Ankle Rehabilitation Training.

Lower-limb rehabilitation benefits from an input channel that conveys intent cleanly while actuation remains predictable and safety-bounded. We present a clinic-friendly, binocular gaze-driven paradigm that maps quadrant fixations to discrete commands for a two-degree-of-freedom ankle robot (dorsiflexion/plantarflexion and internal/external axial rotation), while inversion/eversion can be left compliant or mechanically constrained as needed. Pupil centers from a near-infrared tracker are mapped to a unit-normalized screen plane using low-order regression with ArUco-guided homography and rapid

An Autopsy Case With Fragile X-Associated Tremor/Ataxia Syndrome Presenting Intranuclear Inclusion Bodies Mainly in the Limbic System.

Fragile X-associated tremor/ataxia syndrome (FXTAS) is a neurodegenerative disorder characterized by a late onset and slow progression caused by a premutation (55-200 CGG repeat) in the fragile X mental retardation (FMR1) gene. Here, we report the case of a Japanese patient with FXTAS which is the first case autopsied in Japan. The patient was a 74-year-old man with a family history of fragile X syndrome at the time of death. The clinical features included postural tremors, mild cognitive decline, and ataxia. Magnetic resonance imaging (MRI) showed a high-intensity lesion in the bilateral midd

Effect of rhythmic auditory stimulation (RAS)® with and without melody on Parkinson's disease (PD) patients with deep brain stimulation (DBS): A study protocol.

Parkinson's Disease (PD) is characterized by motor impairments, including gait abnormalities that contribute to instability and an increased risk of falls as the disease progresses. While deep brain stimulation (DBS) of the subthalamic nucleus (STN) or the globus pallidus internus (GPi) can be highly effective for managing tremor, rigidity and limb bradykinesia, its impact on gait remains limited, highlighting the need for dedicated gait therapies. Rhythmic auditory stimulation (RAS)® has been used to improve PD gait parameters, such as cadence, velocity and stride length. This exploratory pil

A multimodal biomechanics dataset with synchronized kinematics and internal tissue motions during reaching.

Tissue motions within body segments, such as the relative movements of muscles, fascia, and bone, remain largely unexplored despite their relevance to movement dysfunction, force transmission, and motor skill. Here, we present a time-synchronized multimodal dataset that bridges this gap by capturing both internal tissue dynamics and conventional biomechanical measurements during arm reaching. Thirty-six participants across three expertise levels (world-class athletes, regional athletes, and untrained individuals) performed slow, rhythmic reaching movements while we recorded data using B-mode u

Separable, symptom specific alterations in brain microstructure associated with early-stage Parkinson's disease.

INTRODUCTION: Parkinson's Disease (PD) is diagnosed based on motor symptoms (bradykinesia, resting tremor, rigidity); yet non-motor symptoms such as sleep abnormalities, autonomic dysfunction, and cognitive changes often precede motor signs, fulfilling the criteria for prodromal PD. How motor and non-motor symptoms emerge from dopamine depletion and whether they involve separable neural substrates remains unclear. METHODS: We applied correlational tractography based on multi-shell, diffusion-weighted magnetic resonance imaging in early-stage PD to assess microstructural changes throughout the

Striatal Dopamine Transporter and Rest Tremor in Parkinson Disease: A Clinical Validation.

BACKGROUND AND OBJECTIVES: The mechanisms underlying tremor generation in Parkinson disease (PD) remain unclear. Previously, we demonstrated a connection between rest tremor amplitude and higher dopamine transporter (DAT) binding in the ipsilateral striatum among the Parkinson Progression Markers Initiative cohort. Here, we investigated the association of parkinsonian motor symptoms with striatal DAT binding in a sizable and clinically representative sample of patients with parkinsonian signs to validate the previously observed ipsilateral relationship in PD. METHODS: This observational cross

Infrathalamic ablation in advanced Parkinson's disease: Three-year outcomes support a safe and accessible therapeutic strategy.

BACKGROUND: Advanced Parkinson's disease (PD) presents significant therapeutic challenges, especially in resource-limited settings. While deep brain stimulation (DBS) remains the gold standard, its accessibility is restricted. Infrathalamic stereotactic ablation has emerged as a promising alternative. OBJECTIVE: To evaluate the long-term clinical outcomes of combined stereotactic ablation targeting the caudal zona incerta and Fields of Forel in patients with advanced PD and a tremor-rigid phenotype. METHODS: A longitudinal study with 36-month follow-up was conducted in ten patients with unil

Ciprofloxacin-Induced Unilateral Tremor and Weakness Mimicking Acute Ischemic Stroke: A Case Report.

Fluoroquinolones are widely prescribed antibiotics and are generally well-tolerated; however, they are associated with rare but clinically significant neurological adverse effects that may mimic acute cerebrovascular events. We describe the case of a 50-year-old man who developed intermittent unilateral coarse tremors and weakness of the left upper limb approximately two weeks after initiating ciprofloxacin therapy following endoscopic retrograde cholangiopancreatography (ERCP) for suspected choledocholithiasis. The acute presentation prompted a Code Stroke activation. Initial computed tomogra

A Review of Therapeutic Strategies to Enhance ;Neuroplasticity in Parkinson's Disease: ;Implications for Slowing Disease Progression ;and Promoting Recovery.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder characterised by the degeneration of dopaminergic neurons in the substantia nigra pars compacta, resulting in motor symptoms such as tremor, rigidity and bradykinesia, along with cognitive impairments. While conventional research has largely focused on pathological degeneration, recent advances highlight the role of neuroplasticity-the brain's ability to reorganise and adapt neural circuits-as a potential mechanism for functional recovery and disease modification. SUMMARY: This review examines therapeutic strateg

Pentanucleotide repeat instability and transmission in benign adult familial myoclonic epilepsy types 1, 4, and 8.

OBJECTIVE: Benign adult familial myoclonic epilepsy (BAFME) is a rare autosomal dominant disorder characterized by tremor and myoclonic seizures. To date, seven genetic subtypes have been identified, all caused by noncoding pentanucleotide repeat expansions-typically TTTTA expansions with additional TTTCA insertions-across distinct loci. Although aberrant repeat expansions are a shared hallmark, it remains unclear whether different BAFME subtypes exhibit similar patterns of repeat length variability and inheritance. We aimed to characterize pentanucleotide repeat structure, repeat length varia

M5 positive allosteric modulation alleviates parkinsonian motor deficits.

Parkinson's disease is a neurodegenerative movement disorder which is characterized by cardinal motor symptoms of tremor at rest, rigidity, bradykineasia, and postural instability. Underlying these cardinal motor symptoms is thought to be death and dysfunction of nigrostriatal dopamine neurons, and the gold-standard treatment of Parkinson's disease is dopamine replacement therapy with the dopamine precursor L-DOPA. While efficacious, L-DOPA does not treat all motor symptoms and can have serious treatment-related side effects called L-DOPA induced dyskinesias, indicating an immense need for new

Temporal Dynamics of Parkinson's Disease Tremor: Clinical and Neuroimaging Insights.

BACKGROUND: Parkinson's disease (PD) is a progressive neurodegenerative disorder clinically defined by three cardinal motor symptoms: bradykinesia, rigidity, and tremor. Although the natural history of bradykinesia and rigidity is well described, the evolution of tremor as the disease progresses remains controversial. OBJECTIVES: The goal was to clinically characterize long-term trajectories of rest, postural, and action tremor in PD and examine their neural network correlates using longitudinal resting-state functional magnetic resonance imaging (rs-fMRI). METHODS: In this retrospective lon

Treatment Continuation and Long-Term Outcomes of Perampanel in Primary Orthostatic Tremor; A Cohort Study.

BACKGROUND: Primary orthostatic tremor (POT) is a rare hyperkinetic movement disorder with limited and largely ineffective treatment options. Perampanel has been suggested as a therapy, but has so far only been investigated in small studies with short duration. OBJECTIVE: To (1) describe longitudinal perampanel use and its association with key clinical outcomes and baseline characteristics in a well-defined cohort of POT, and (2) evaluate factors possibly associated with treatment continuation. METHODS: A prospective observational cohort study in POT patients who started perampanel treatment
