The influence of motor and non-motor characteristics of Parkinson's disease on motor imagery vividness.

Motor imagery (MI) can facilitate movement in healthy individuals and patient populations. People with Parkinson's (PwP) experience debilitating motor symptoms, yet appear to have relatively intact MI capabilities, which may position MI as an effective therapeutic adjunct to existing interventions for PwP. However, individual differences in healthy individuals, such as cognitive factors and depression, as well as symptom severity in PwP, have been shown to influence MI, which may moderate its therapeutic impact. In the present study, data from 56 PwP were analysed using linear mixed-effect mod

Environmental enrichment ameliorates the impairments of a rodent model of prodromal Parkinson's disease.

Parkinson's disease (PD) pathogenesis is multifactorial and involves both genetic and environmental factors, converging to induce mitochondrial and synaptic dysfunction, oxidative stress, and neuroinflammation. Although the disease is characterized by cardinal motor symptoms (bradykinesia, rigidity, and/or resting tremor), in a majority of PD patients non-motor symptoms, including disorders of mood and affect, sensory and autonomic dysfunction, pain, disturbances of the sleep-wake cycle and cognitive impairment, anticipate the onset of motor symptoms and aggravate the burden of symptomatology

Decoding the RNA Shield: HNRNPC and YTHDF2 Act as Guardians of Neurons in Parkinson's Disease.

Parkinson’s disease (PD) is a progressive neurodegenerative disorder characterized by the selective loss of dopaminergic neurons in the substantia nigra, resulting in hallmark motor symptoms such as tremor, bradykinesia, rigidity, and postural instability. Beyond these classical features, emerging evidence highlights the critical role of RNA modifications, particularly N6-methyladenosine (m6A), in regulating neuronal health and disease. The m6A modification is dynamically controlled by writers (METTL3, METTL14, WTAP), erasers (FTO, ALKBH5), and readers (e.g., YTHDF2, HNRNPC), which together in

OpenStride: an inexpensive, open-source force plate actometry system for quantification of rodent motor activity and behaviour.

Force plate actometry (FPA) enables quantitative assessment of rodent motor and behavioural activity. Through tracking centre of mass at high temporal and spatial resolution, FPA can directly quantify motor performance and various behaviours in the context of operator-independent, naturalistic movement. Previously designed systems have been expensive and non-customisable, and they are no longer commercially available. We designed OpenStride, an open-source FPA system consisting of both hardware and software. Our system was designed to be able to be constructed with standard tools worldwide at

Parkinson's disease in real life healthcare organization database: a medication-based algorithm.

BACKGROUND: Accurate identification of Parkinson’s disease (PD) in large electronic health record (EHR) population-based databases is challenging due to diagnostic heterogeneity in routine care, with a substantial proportion of individuals diagnosed with PD had not been diagnosed by a specialist. Our aim was to develop and validate a simplified rule-based medication algorithm to identify PD in a nationwide healthcare registry and apply it to estimate long-term incidence, prevalence, and pre-diagnostic diagnoses. METHODS: Using Clalit Health Services EHR data covering over five million individu

A mouse model of autosomal dominant spastic ataxia and myopathy caused by a mutation in Tuba4a.

Hereditary ataxias are a heterogeneous group of neurodegenerative disorders characterized by impaired balance and coordination, often due to cerebellar dysfunction. Despite advances in identifying genetic causes, animal models remain essential for dissecting underlying mechanisms and testing therapeutic strategies. Here we describe a mouse model of spastic ataxia and myopathy caused by a missense mutation in Tuba4a (n.A626C, p.Gln176Pro). In an ENU mutagenesis screen, a male C57BL/6J mouse exhibiting muscle wasting and an intention tremor starting at approximately 4 weeks-of-age was identified

Navigating transcranial mr guided focused ultrasound complexities with machine learning: Overcoming obstacles and expanding therapeutic horizons.

Transcranial magnetic resonance-guided focused ultrasound surgery (MRgFUS) is increasingly recognized as a promising therapeutic option for patients with conditions including pain, tremor, and epilepsy. However, challenges such as slow magnetic resonance imaging acquisition and physical restrictions imposed by stereotactic frames limit the realization of this technology’s full potential. Machine learning technologies have recently seen rapid growth in their clinical applicability, reflected by a corresponding increase in regulatory approvals for clinical artificial intelligence. Advancements i

EMG-Driven Musculoskeletal Modelling Framework for Virtual Simulation of Upper Limb Activation-Modulated Impairment Scenarios.

Background and Objectives: Surface electromyography (EMG) is widely used to assess muscle activation. However, direct interpretation of its functional biomechanical consequences remains challenging. This study aimed to develop and evaluate an EMG-driven musculoskeletal simulation framework for investigating how controlled modifications of muscle activation patterns influence joint-level biomechanics in the upper limb. The objective was not to reproduce specific clinical pathologies but to enable systematic virtual scenario analysis of activation-dependent movement alterations. Materials and Me

Neurotransmitter Dysregulation in Parkinson's Disease: Pathophysiological Insights and Therapeutic Perspectives.

Parkinson's disease (PD) is a prevalent neurodegenerative movement disorder characterized by bradykinesia, rigidity, and resting tremor, progressing insidiously over time. Central to its pathophysiology is the degeneration of dopaminergic neurons in the substantia nigra, leading to a significant decrease in striatal dopamine (DA) levels. This dopaminergic deficit disrupts basal ganglia circuitry, impairing motor function and contributing to the core symptoms of the disease. While the etiology of PD remains incompletely understood, a combination of genetic predispositions and environmental expo

Diagnostic pathways and reported latency of Parkinson's disease among Chinese immigrants in New York city: A retrospective chart review.

BACKGROUND: Diagnostic delay in Parkinson's disease (PD) is influenced by symptoms and clinical phenotypes, healthcare access, and sociocultural context. Minority and immigrant populations face distinct barriers to diagnosis, but data on Chinese immigrants are limited. OBJECTIVE: To describe reported diagnostic latency and selected clinical, social, and healthcare-related factors among Chinese immigrants with Parkinson's disease in New York City. METHODS: We conducted a retrospective chart review of Chinese immigrant patients with idiopathic PD seen at Parkinson's Center of Excellence at Col

Dual-target magnetic resonance-guided focused ultrasound of the subthalamic and ventral intermediate nuclei for Parkinson's disease: illustrative case.

BACKGROUND: Magnetic resonance-guided focused ultrasound (MRgFUS) has emerged as a minimally invasive alternative for medication-refractory Parkinson's disease tremor. While single-target ablation of either the subthalamic nucleus (STN) or the ventral intermediate nucleus (VIM) is standard, intraoperative adaptation to address incomplete symptom relief remains largely unexplored. OBSERVATIONS: The authors present the case of a 70-year-old man with an 8-year history of Parkinson's disease who underwent planned unilateral left STN MRgFUS. Following lesioning, marked improvement in bradykinesia

A Tremor Suppression Method for the Master-Follower Surgical Robot Manipulator Based on Kalman Filter Algorithm.

BACKGROUND: Master follower surgical robotic systems are widely used in invasive procedures but remain vulnerable to end effector tremor transmitted from the operator's physiological tremor, reducing precision. METHODS: Physiological tremor was characterised with a three-dimensional optical motion capture system and integrated into master follower control strategy enhanced with Kalman filter based tremor suppression algorithm. The method was validated on a custom platform using 24 predefined motion trajectories executed via a force feedback haptic master and robotic slave manipulator. RESULT

Frequency-specific and spatiotemporal dynamics of β-γ phase-amplitude coupling in Parkinson's disease.

Cross-frequency coupling (CFC) has been proposed to facilitate neural information transfer across spatial and temporal scales. Phase-amplitude coupling (PAC), a type of CFC in which the amplitude of a faster brain oscillation is coupled to the phase of a slower brain oscillation, is implicated in various higher-order cognitive functions and was shown to be pathologically altered in neurological and psychiatric disease. In Parkinson's disease (PD), the coupling between gamma amplitude (50-150 Hz) and beta phase (13-35 Hz) is exaggerated. Enhanced β-γ PAC was found in the subthalamic nucleus and

Cortical gyrification deficits in early-stage Parkinson's disease: the importance of bradykinesia.

Timely, accurate diagnosis of Parkinson's disease is still challenging for clinicians. It is therefore crucial to identify novel biomarkers to better characterize the early stages of the disease. Here, we assessed cross-sectional brain structure differences between healthy control (HC) and Parkinson's disease participants. We also explored potential correlations between brain structure and distinctive Parkinson's disease clinical features. We analysed T1-weighted brain images from 381 Parkinson's disease patients, primarily in the early stages, and 139 HC participants obtained from the Parkins

Psychometric reliability of patient-reported visual analogue scales in subthalamic nucleus deep brain stimulation programming for Parkinson's disease.

Subthalamic nucleus deep brain stimulation is an established therapy for Parkinson's disease, yet its programming relies heavily on subjective patient feedback. Visual analogue scales have been proposed to structure patient-reported outcome measures during programming, but their psychometric reliability has not been systematically evaluated. In this study, fifteen patients with bilateral subthalamic nucleus deep brain stimulation completed four structured experiments to assess the reliability of visual analogue scales: test-retest consistency, the effect of stimulation duration (15, 60, 120 s)

Rapidly Progressive Dementia Secondary to Vitamin B12 Deficiency: A Case Report.

INTRODUCTION: While vitamin B12 deficiency is classically associated with hematological abnormalities and subacute combined degeneration, its presentation as rapidly progressive cognitive decline with episodic movement disorders remains an underrecognized clinical phenomenon. This case highlights the diagnostic challenges posed by such non-classical neurological manifestations and underscores the need for increased clinical suspicion in cases of unexplained cognitive-motor decline. CASE PRESENTATION: A patient presented with rapidly progressive memory impairment and episodic limb tremors in t
