Staged bilateral MR-guided focused ultrasound thalamotomy in a patient with cerebral cavernous malformations.

MRI-guided focused ultrasound (MRgFUS) thalamotomy is an effective ablative procedure for medication-refractory essential tremor (ET). However, limited data exist regarding its safety in patients with coexisting intracranial disease such as cerebral cavernous malformations. We present a man in his late 60s with medication-refractory ET who underwent staged bilateral MRgFUS thalamotomy. Preoperative MRI demonstrated heterogeneously T1-hyperintense and T2-hyperintense lesions in the left caudate head and basal ganglia consistent with cavernous malformations, with an associated developmental veno

Functional Neurological Disorder Management in a Pediatric Hospital: A Retrospective Chart Review.

BACKGROUND: Functional neurological disorder (FND) is a complex, multinetwork brain disorder that is characterized by the presence of a broad range of neurological symptoms in the absence of neurological disease or damage. For pediatric patients requiring an in-patient stay, misdiagnosis, lack of provider consistency and diagnostic education, frequent testing and work-up, and invasive procedures are often reported. This review will characterize the population, identify current state for receiving diagnosis, detect variability in service delivery, and provide information to evaluate use of reso

The Role of Biologically Effective Dose in Clinical Improvement and Hyper-Response After Gamma Knife Radiosurgery for Essential Tremor.

BACKGROUND AND OBJECTIVES: Stereotactic radiosurgery (SRS) is a minimally invasive alternative for drug-resistant essential tremor (ET). The objective of this study was to evaluate whether biologically effective dose (BEDGy2.47) is relevant for tremor arrest after SRS for ET. METHODS: Two hundred sixty-one cases were treated in Marseille University Hospital (CHU Timone) between January 2009 and June 2021. The median age was 71.9 years (39.9-91.5). The treated side was the left ventrointermediate nucleus in 237 cases (90.8%). 130 Gy was prescribed in all cases. The median BEDGy2.47 was 4119.3

Exploratory radiomics analysis of T1-weighted and FLAIR MRI in relation to Parkinson's disease motor subtypes.

BACKGROUND: Parkinson's disease motor subtypes, tremor-dominant (TD) and postural instability/gait difficulty (PIGD), differ in disease trajectory and prognosis, yet their classification depends on MDS-UPDRS-based clinical ratings that are susceptible to inter-rater variability and medication state. PURPOSE: This exploratory study investigated whether T1-weighted and FLAIR MRI radiomics show differential associations with TD and PIGD motor subtypes in Parkinson's disease, and whether the two sequences contribute complementary subtype-relevant information when combined in a multimodal framewor

Acute ring-enhancing change in a previously solidly enhancing meningioma.

BACKGROUND: Meningiomas are typically slow-growing, benign extra-axial tumours that exhibit stable radiological characteristics over long periods. Although peritumoural oedema and mass effect may develop gradually, acute structural changes with rapid neurological deterioration are unusual, particularly in World Health Organisation (WHO) grade I lesions. Ring enhancement is extremely rare and is often associated with necrosis, haemorrhage, or cyst formation. METHODS: We describe an adult patient with multiple right frontal meningiomas who had been followed radiologically for almost a decade. S

The Influence of Symptoms and Psychosocial Factors on Quality of Life and Life Satisfaction in Parkinson's Disease.

BACKGROUND: Parkinson's disease (PD) symptoms affects quality of life (QoL), but effects on life satisfaction (LS) are less studied. OBJECTIVES: This study explored influences of motor and non-motor symptoms on QoL and LS in people with PD in Ireland. METHODS: Eight hundred twenty nine people with PD completed a survey of validated tools, exploring motor and non-motor symptoms, social connectedness, self-efficacy, and stigma, and QoL and LS. Linear and multiple regression analyses assessed influences on QoL and LS. RESULTS: Stigma and low self-efficacy were associated with poor QoL and LS;

Modelling and Correcting the Effects of Tremor on Touchscreen Interaction.

Essential tremor and Parkinson's tremor are common movement disorders, affecting approximately 7% of adults over 65, increasing with age. Although numerous studies have used keyboard timing, stand-alone sensors, or smartphone sensors to identify and measure tremor severity, relatively little research has isolated and examined how tremor impacts overall mobile interaction and usability models. This study investigated controlled fixed levels of induced tremor in a standardised ISO pointing task to quantify the effect on usability models of different levels of tremor. Tremor was induced using a w

Differential associations between tongue pressure and handgrip strength with MDS-UPDRS part III motor symptoms in Parkinson's disease.

Tongue pressure is a swallowing-related measure that correlates with handgrip strength, an indicator used in the assessment of sarcopenia. However, reduced tongue pressure in Parkinson's disease (PD) may reflect not only generalized muscle weakness but also PD-related motor control impairment, raising the possibility that tongue pressure and handgrip strength may show different patterns of association with PD motor symptoms. This study examined the patterns of association between tongue pressure and handgrip strength and motor symptoms assessed using Part III of the Movement Disorder Society-s

Cryoprobe to current practice: neurosurgical legacies of Dr. Irving S. Cooper in movement disorders.

Before the advent of levodopa therapy for Parkinson's disease, surgical lesioning of the motor thalamus or globus pallidus represented the primary therapeutic intervention for Parkinson's disease and essential tremor at a time when effective medical alternatives were severely limited. In this context, Irving S. Cooper directed one of the most prolific and controversial lesioning practices of the mid-20th century, performing thousands of procedures and documenting outcomes. Through chemopallidectomy, chemothalamectomy, and later cryosurgical techniques, Cooper investigated the relationship betw

The association between upper limb motor performances and gait dynamics in essential tremor: an emerging relationship.

Essential tremor (ET) is a complex disorder extending beyond upper limb action tremor. Both slowing in upper limb movements and impaired gait performances have been widely described. The present study aimed to assess clinical-instrumental relationship between wearable-sensor recorded upper limb motility, tremor and walking performances in ET and explore their role in disease characterization. A gyroscope taped over the index finger was used to quantify both tremor in three tasks (forward outstretched; lateral-wing; instrumental finger-nose, iFN) and upper limb performances in the iFN. Maximum

Learning Curve in Robotic-Assisted Microsurgery: A Systematic Review of Skill Acquisition.

Background Robotic-assisted microsurgery offers enhanced precision, tremor filtration, and improved ergonomics, but adoption is limited by undefined learning curves. This systematic review evaluates the learning curve of microvascular anastomosis performed on dedicated robotic platforms. Methods A systematic review was conducted across MEDLINE, EMBASE, Web of Science, and Cochrane databases (inception to June 2025). Evaluated outcomes included time metrics (anastomosis time, time per stitch), microsurgical skills and anastomosis quality for the robot-assisted (RA) and hand-sewn (HS) techniques

Dual-Tasking Exacerbates Force and Neural Control Unsteadiness in Older Adults With Sarcopenia.

BACKGROUND: Sarcopenia is associated with impaired physical function. Dual-task conditions, which increase cognitive demand during motor performance, may reveal deficits in neuromuscular control that are not evident during isolated motor tasks. We investigated whether older adults with sarcopenia exhibit poorer force and neural control (i.e., greater force and motor unit inter-spike variability and altered common synaptic input) during single- and dual-task conditions compared with non-sarcopenic controls and masters athletes. METHODS: Sarcopenia was defined using Sarcopenia Definitions and O

Safety of Staged Bilateral MRgFUS Thalamotomy: Associations Between Lesion Characteristics, Stereotactic Coordinates, and Adverse Events.

BACKGROUND AND OBJECTIVES: Staged bilateral magnetic resonance-guided focused ultrasound (MRgFUS) thalamotomy is increasingly performed for medication-refractory essential tremor, yet the relationship between lesion characteristics and profiles of adverse events (AE) across sessions remains poorly characterized. We sought to examine how lesion volume, stereotactic coordinates, and session order relate to AE profiles in staged bilateral MRgFUS thalamotomy. METHODS: Retrospective analysis of 50 patients who underwent staged bilateral MRgFUS thalamotomy at a single center. AE were assessed at po
