Characteristics of accelerometric activity of the head in healthy subjects at rest and in postures.

OBJECTIVE: Head tremor occurs in various neurological disorders, yet quantitative data on head movements in healthy individuals are lacking. We aimed to provide the first accelerometric database of physiological head movements. METHODS: Accelerometric activity of the head and EMG of neck muscles of healthy volunteers was recorded. Quantitative characteristics (intensity, peak frequency, center frequency, frequency dispersion) were determined at rest and in various postural positions. Data were statistically analysed. RESULTS: Power spectra of the accelerometric and EMG activity did not show

An acoustic-only multivariable diagnostic model for Parkinson's disease using bowel sound acoustic signatures.

BackgroundEarly detection of Parkinson's disease (PD) remains challenging due to the lack of reliable prodromal biomarkers. Non-invasive markers of gastrointestinal dysfunction are lacking.ObjectivesTo characterize bowel sound acoustic phenotypes in PD and evaluate their potential as non-invasive diagnostic markers by developing an acoustic-based predictive model.MethodsStandardized 5-min bowel sound recordings were collected from 120 PD patients and 100 controls. Thirteen acoustic features were extracted. LASSO (least absolute shrinkage and selection operator) selected predictors for logistic

EMG-Driven Phase-Locked Spinal Cord Stimulation Physiologically Modulates Essential Tremor.

Essential tremor (ET) is the most prevalent movement disorder, yet existing pharmacological and surgical treatments often lack efficacy or involve significant risks. Phase-locked neuromodulation offers a targeted approach to disrupt pathological oscillations, but its implementation through non-invasive spinal cord stimulation (tSCS) has remained largely unexplored. In this study, we present a novel closed-loop tSCS framework for ET based on real-time tremor phase estimation from surface electromyography (EMG). The system implements a Fourier Linear Combiner (FLC) algorithm to track the demodul

Perspectives of robotic surgeons on the strengths, weaknesses, opportunities, and threats of robotic surgery in india: a cross-sectional survey.

Robotic surgery is increasingly being adopted in India, but its wider implementation is associated with economic, training, and resource-related challenges. This study assessed the perspectives of surgeons performing robotic surgery in India regarding the perceived strengths, weaknesses, opportunities, and threats (SWOT) associated with robotic surgery. A cross-sectional questionnaire-based survey was conducted from June 2023 to January 2025 among surgeons performing robotic surgery in India. The questionnaire comprised 25 five-point Likert-scale statements addressing different aspects of robo

Clinical and Electrophysiological Heterogeneity of Orthostatic Tremor: An Individual Patient Data Analysis.

BACKGROUND: Orthostatic tremor (OT) is traditionally classified using tremor frequency and the presence of associated neurological signs. However, increasing reports of overlapping frequencies, fluctuating electrophysiological profiles, and diverse clinical associations indicate that current classification systems may not fully capture the clinical heterogeneity of OT. METHODS: A systematic review of PubMed/MEDLINE from inception to February 2026 identified case reports, case series, and original studies reporting electromyography-confirmed OT with individual patient data. Demographic, clinic

Tremor or myoclonus? A practical clinical approach to distinguishing two common hyperkinetic movement disorders.

Tremor and myoclonus are among the most common hyperkinetic movement disorders encountered in neurological practice, with tremor being considerably more common than myoclonus in everyday clinical practice, yet distinguishing between them remains a frequent diagnostic challenge. Although their definitions are conceptually distinct, several disorders - including cortical tremor, dystonic tremor, and generalized polymyoclonus - can mimic one another, leading to diagnostic uncertainty and inappropriate management. This practical review provides clinicians with a structured approach to recognizing

Alterations in relative perfusion and synaptic patterns in Parkinson's disease using dynamic SV2A PET.

PURPOSE: Dynamic [ 11 C]UCB-J PET can simultaneously provide information about synaptic density and relative perfusion in Parkinson's disease (PD). We investigated whether these complementary biomarkers could explain a higher percentage of motor symptom severity variation in PD and inform other PD-specific physiological alterations and symptomatology. METHODS: Binding potential ( BP ND ) and relative perfusion ( R 1 ) images were derived 30 PD and 30 controls imaged with [ 11 C]UCB-J PET. A spatial covariance analysis was used on R 1 to derive PDRP-a known metabolic network alteration in PD.

Bilateral multi-condition surface electromyographic characterization of tremor in Parkinson's disease and essential tremor.

OBJECTIVES: Clinical differentiation between Parkinson's disease (PD) and essential tremor (ET) remains challenging, particularly in early or overlapping cases. This study aimed to characterize bilateral tremor features using multi-condition surface electromyography (sEMG) and to evaluate task-dependent modulation patterns relevant to differential diagnosis. METHODS: Thirty patients with PD and forty-one with ET underwent bilateral sEMG recordings from agonist-antagonist muscle pairs. Tremor frequency, burst duration, amplitude, and activation patterns were systematically analyzed under four

Grid to glide: A simple visual cue that alleviates upper-limb freezing.

Freezing of the upper limb (FoUL) is a poorly explored manifestation of Parkinson's disease. This report describes a case of a 54-year-old woman with FoUL predominantly affecting handwriting. Writing on grid paper with small squares (0.5 × 0.5 cm) markedly reduced FoUL frequency. A subsequent daily home-based writing and calligraphic training program led to sustained improvements in handwriting legibility, drawing ability, and transfer to writing on blank paper without the grid, without any change in medication. The immediate response to grid paper suggests that simple visual cueing-possibly b

Wearable Electromyography and Inertial Sensor Phenotypes Across Clinical Subtypes of Parkinson's Disease.

INTRODUCTION: Parkinson's disease (PD) is clinically heterogeneous, yet whether the mild motor-predominant, intermediate, and diffuse malignant (MMP/IM/DM) subtypes show objective neuromuscular and kinematic phenotypic differences remains unclear. METHODS: We studied 120 patients with PD (55 men and 65 women) in the medication ON state. Subtypes were assigned using motor, cognitive, rapid eye movement sleep behavior disorder, and autonomic domains. Synchronized surface electromyography and inertial recordings were obtained from the more affected upper limb during three tremor and three moveme

Deep brain stimulation for arm tremor: one-year blinded follow-up of a randomized trial.

OBJECTIVE: To present the secondary endpoints at one-year blinded follow-up from our randomized controlled study, that compared the efficacy of DBS in the ventral-intermediate thalamic nucleus (VIM) with the posterior subthalamic area (PSA) three months each in crossover design, and showed significantly greater reduction of the Sum dominant arm tremor score (Fahn-Tolosa-Marin) during the PSA-DBS period. The outcomes at one year are now reported to verify target choice and sustained benefit with clinically optimized free programming. METHODS: Forty-five patients with isolated or combined arm a

The first UK report of robotic-assisted Lymphaticovenous anastomoses (LVA) for the management of lymphoedema.

Lymphoedema remains a significant complication following axillary lymph node clearance, affecting at least 20% of breast cancer patients. One of the surgical approaches that has been shown to benefit such patients in a therapeutic setting is lymphaticovenous anastomosis surgery. Robotic-assisted microsurgery has emerged as a promising advancement in this domain, enhancing surgical precision through motion scaling and tremor elimination. We report the first UK case of a robot-assisted lymphaticovenous anastomoses (LVA) using the Symani Surgical System for the management of lymphoedema post axil

A case of acute lithium encephalopathy associated with elevated myelin basic protein levels in cerebrospinal fluid.

BACKGROUND: Acute lithium intoxication can cause severe central nervous system (CNS) manifestations, sometimes leading to long-term sequelae known as the syndrome of irreversible lithium-effectuated neurotoxicity (SILENT). However, its underlying pathophysiology remains unclear. We report a case of acute lithium encephalopathy presenting with elevated myelin basic protein (MBP) levels in the cerebrospinal fluid (CSF), offering insights into its pathogenesis. CASE PRESENTATION: A 50-year-old man with schizophrenia developed coma following acute lithium intoxication. Two days prior to symptom o

Staged bilateral MR-guided focused ultrasound thalamotomy in a patient with cerebral cavernous malformations.

MRI-guided focused ultrasound (MRgFUS) thalamotomy is an effective ablative procedure for medication-refractory essential tremor (ET). However, limited data exist regarding its safety in patients with coexisting intracranial disease such as cerebral cavernous malformations. We present a man in his late 60s with medication-refractory ET who underwent staged bilateral MRgFUS thalamotomy. Preoperative MRI demonstrated heterogeneously T1-hyperintense and T2-hyperintense lesions in the left caudate head and basal ganglia consistent with cavernous malformations, with an associated developmental veno

Functional Neurological Disorder Management in a Pediatric Hospital: A Retrospective Chart Review.

BACKGROUND: Functional neurological disorder (FND) is a complex, multinetwork brain disorder that is characterized by the presence of a broad range of neurological symptoms in the absence of neurological disease or damage. For pediatric patients requiring an in-patient stay, misdiagnosis, lack of provider consistency and diagnostic education, frequent testing and work-up, and invasive procedures are often reported. This review will characterize the population, identify current state for receiving diagnosis, detect variability in service delivery, and provide information to evaluate use of reso
