Cognitive reappraisal training as an intervention in patients with functional movement disorders: a placebo-controlled EEG pilot study.

Functional movement disorders (FMD) are common and disabling neurological conditions characterized by impaired voluntary motor control in the absence of structural disease. Accumulating evidence suggests that emotion regulation deficits contribute to the pathophysiology of FMD. This randomized, placebo-controlled pilot study investigated the clinical and neural effects of a digital cognitive reappraisal training program in patients with FMD, targeting emotion regulation as a potential therapeutic mechanism. Twenty adult patients diagnosed with clinically definite FMD (mean age = 50.7 years, 11

Orthostatic tremor and its subtypes: a single centre cohort of 74 patients.

BACKGROUND: Orthostatic tremor (OT) is a rare, heterogenous disorder, recently sub-classified into primary OT (isolated 13-18 Hz tremor), OT-plus (OT with additional neurological features) and pseudo-OT (OT with frequencies < 13 Hz). However, to our knowledge no study to date has compared clinical characteristics between all three subgroups. OBJECTIVES: We aim to further define and compare the clinical characteristics of the three different OT subgroups, utilising one of the largest described single centre cohorts to date. METHODS: A retrospective analysis was undertaken of clinical records

Endoscopic ultrasound-guided radiofrequency ablation of pancreatic insulinoma.

A man in his mid-30s presented to the emergency department following a motor vehicle accident with critically low blood glucose of 1.1 mmol/L (3.9-5.6 mmol/L) noted by emergency medical service and altered mental status. The patient reported progressive symptoms including tremor, sweating and difficulty concentrating, requiring continuous eating throughout the day, weight gain and hypoglycaemia unawareness.A 72-hour supervised fast confirmed endogenous hyperinsulinaemia, after 5 hours, inappropriately elevated insulin of 26 pmol/L (diagnostic cut-off ≥18 pmol/L) and C-peptide >0.33 nmol/L (exp

Microglia, Astrocytes, and Oligodendrocytes in Parkinson's Disease: Neuroinflammatory Crosstalk and Emerging Therapeutic Strategies.

Parkinson's disease (PD) is a progressive neurodegenerative disorder marked by the degeneration of dopaminergic neurons in the substantia nigra, resulting in cardinal motor symptoms such as tremor, rigidity, and bradykinesia. Neuroinflammation is increasingly recognized as a central driver of PD onset and progression in which oligodendrocytes, astrocytes, and microglia engage in complex bidirectional crosstalk that shapes the inflammatory milieu of the central nervous system. Pathological activation of glial cells triggers the release of pro-inflammatory cytokines, chemokines, and reactive oxy

Hereditary Ataxias: From Pathogenesis and Clinical Features to Neuroimaging, Fluid, and Digital Biomarkers-A Scoping Review.

Hereditary ataxias are a heterogeneous group of disorders with overlapping clinical presentations but diverse genetic and molecular etiologies. Biomarkers are increasingly essential to improve diagnosis, refine prognosis, and accelerate the development of targeted therapies. Following PRISMA-ScR guidelines, we conducted a scoping review of PubMed and complementary sources (2010-2025) to map and describe the current landscape of genetic, imaging, fluid, electrophysiological, and digital biomarkers across the most prevalent hereditary ataxias, including SCA1, SCA2, SCA3, SCA6, SCA7, SCA17, SCA27

Impedance Problems and Their Causes-A Single-Center Analysis of 601 Patients with De Novo Deep Brain Stimulation.

Background/Objectives: Patients with deep brain stimulation (DBS) require regular follow-up. When a sudden loss of therapeutic effect occurs, impedance abnormalities are often the underlying cause. If reprogramming cannot restore clinical benefit, revision surgery may be necessary to replace defective hardware. Since all three major manufacturers are used at our center, we analyzed our patient cohort to determine the incidence and causes of impedance abnormalities. Methods: All 601 patients who underwent de novo DBS implantation in Hamm between 2009 and 2025 were evaluated for impedance abnorm

Psychometric Validation of the French-Canadian Version of the Tampa Scale of Kinesiophobia for Parkinson's Disease.

BACKGROUND: Kinesiophobia is defined as an excessive and irrational fear of movement and physical activity. Individuals living with Parkinson's disease (PD) can be at risk of developing this phobia, due to the debilitating nature of the disease's motor symptoms such as impaired balance, bradykinesia, rigidity and tremor. This is particularly problematic, as exercise is crucial for people with PD, especially considering its potential to slow down disease progression. The Tampa Scale of Kinesiophobia for Parkinson's disease (TSK-PD) is a valid and reliable instrument for measuring kinesiophobia

Roflupram Reduces 6-OHDA-Induced Cellular Damage in SH-SY5Y Cells by Inhibiting PDE4.

BACKGROUND: The main symptoms of Parkinson's disease (PD) include olfactory impairment and tremor. Current treatment methods for PD generally have limitations such as short duration and severe side effects. The novel phosphodiesterase 4 (PDE4) inhibitor Roflupram (Roflu) mitigates inflammatory responses and enhances cognitive functions in individuals with neurological conditions. However, it remains unknown whether Roflu provides neuroprotection in a PD model induced by 6-hydroxydopamine (6-OHDA). METHODS: Cell viability was assessed using a 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazoliu

Insights into the seismogenesis and tectonic implications of an isolated intraplate earthquake (M4.0) on February 17, 2025, in Delhi.

On February 17, 2025, a magnitude 4.0 earthquake occurred near Jheel Park in South Delhi, approximately 6 km from the epicenter of the November 25, 2007, M4.6 event, one of the most significant regional earthquakes in recent decades. The tremor was widely felt across Delhi and adjoining regions, accompanied by a distinct rumbling sound. For the first time, a comprehensive seismo-geodetic investigation was conducted to characterize the causative source mechanism and fault geometry of this event. Data from the dense network of the National Seismological Network (NSN) enabled precise focal mechan

Adverse events related to buspirone: a real-world pharmacovigilance study using the FAERS database.

Buspirone, a widely used anxiolytic, has incompletely characterized adverse effects. This study aimed to comprehensively characterize buspirone's safety profile by a large-scale analysis of real-world data from the Food and Drug Administration Adverse Event Reporting System (FAERS) database, covering Q1 2006 to Q1 2025. We collected 1744 buspirone-related adverse event reports and analyzed them utilizing various signal detection techniques, including proportional reporting ratio (PRR), reporting odds ratio (ROR), multi-item gamma Poisson shrinker (MGPS), and Bayesian confidence propagation neu

The apolipoprotein gene: a modulating role on brain volume and cognitive function in carriers of the fragile X premutation.

Fragile X-associated tremor/ataxia syndrome (FXTAS), caused by the FMR1 premutation allele, is associated with brain degeneration, yet the mechanisms behind this neurodegeneration still need to be elucidated. Apoε polymorphism has been widely implicated in brain aging in cognitively healthy individuals and brain deterioration in Alzheimer's disease. This study aimed to examine the interaction of Apoε genotypes, FXTAS clinical symptoms, FMR1 molecular measures, and age, towards brain pathophysiology and cognitive functions. This longitudinal study includes MRI data collected from 205 male premu

A Novel Gain-of-Function ITPR1 Variant Associated With a Movement Disorder Characterized by Tremor and Dystonia.

The 1,4,5-trisphosphate receptor type 1 (ITPR1) gene encodes an endoplasmic reticulum calcium release channel, in which loss-of-function mutations have been associated with spinocerebellar ataxias and related neurological phenotypes. Only one gain-of-function mutation in the highly conserved suppressor domain of ITPR1 has been previously reported. We report a novel de novo ITPR1 variant (p.(Tyr131His)) detected by whole genome sequencing in a child with an unexplained movement disorder, characterized by tremor and dystonia, concurrent with a second diagnosis of Myhre syndrome. Single-station analysis of Campi Flegrei (Italy) seismic signals using multiscale entropy and unsupervised learning.

Seismic activity in volcanic regions such as Campi Flegrei (Italy) provides essential insights into subsurface dynamics and potential hazards. However, high background noise and continuous data volume challenge event detection and classification. Here, we apply a Self-Organizing Map (SOM) approach, combined with Linear Predictive Coding (LPC), STA/LTA ratios, and Multiscale Entropy (MSE), to analyze single-station seismic data. The method successfully identifies uncatalogued events and anomalies associated with fumarolic tremor, and reveals temporal relationships between clustering variation,

Ex vivo feasibility study of vasovasostomy utilizing a novel microsurgery platform versus manual microsurgery.

BACKGROUND: The use of the microsurgical vasovasostomy technique allows for greater precision and the use of extra-fine sutures, resulting in improved outcomes for vasectomy reversal procedures. The challenging nature of these microsurgical procedures has led to the investigation of robotic-assisted microsurgical vasovasostomy for decreased hand tremor and increased dexterity and hand-eye coordination. To date, no research has investigated vasovasostomy using the Symani Surgical System, a robotic device approved by the US Food and Drug Administration (FDA) for microsurgical procedures. This st

Persistent Tremor in Bipolar Disorder: A Case Report of Idiopathic Parkinson's Disease Superimposed on Lithium and Antipsychotic Effects.

We report a 58-year-old woman with bipolar I disorder on long-term lithium and aripiprazole who developed a progressive asymmetric resting tremor and rigidity. Despite stopping both agents, the tremor persisted for more than a year. Dopamine transporter imaging showed reduced uptake in the left putamen, confirming idiopathic Parkinson's disease (PD) with superimposed drug-induced parkinsonism (DIP). Management included discontinuing lithium, switching aripiprazole to quetiapine to limit motor worsening, and starting carbidopa-levodopa. Motor symptoms improved, but hypomanic symptoms emerged an
