The clinical findings in 50 children with intermediate or mild spinal muscular atrophy are reviewed. An early age of onset and the presence of tongue fasciculation carry a poor prognosis for disability. Scoliosis appears early in nearly all children with the more severe forms of spinal muscular atrophy and early energetic treatment is indicated. Contractures are also found in both the more severe and the milder forms and are the main cause of progressive disability, since continuous deterioration of muscle power was not evident in the present series. The oesophagic affectation seems very frequent in Darier's disease. The authors have found oesophagic lesions in 4 cases examined. The radiologic aspect of this localization shows an oesophagic border in "post stamp" and a definite irregularity of the fasciculation. This fibroscopic examination shows the presence of keratotic papules similar to the skin lesions of this disease.

In addition to the 19 cases of neuromytonia syndrome previously published, one further observation by the author is presented. The symptoms for neuromyotonia are an involuntary tensing of the musculature with impairment of movement, fasciculation and myokymia as well as specific electromyographic criteria. These symptoms are causes by a functional disturbance in the peripheral nervous system. They can be considerably improved by administering phenytoin and carbamazepine.

The characteristics of neuromuscular block produced by alpha-bungarotoxin, a post-junctionally active polypeptide toxin purified from snake venoms, have been studied in vivo in 12 anaesthetized cats, using the sciatic nerve-tibialis anterior muscle preparation. The frequency of muscle pain following suxamethonium was studied in 50 pregnant (8-13 weeks' gestation) and 100 non-pregnant women, undergoing laparoscopic tubal ligation. The incidence of muscle pain in the non-pregnant group was 42%, but only 20% in the pregnant women. The intensity of fasciculation was less in the pregnant patients. It appears that pregnancy protects the patient from suxamethonium pains, even during the first trimester.

A technique of 'crash induction' using thiopentone and suxamethonium with cricoid pressure was studied in 100 unselected patients at risk from vomiting or regurgitation. No episode of regurgitation occurred. There was difficulty with intubation due to poor relaxation in 7 patients and, in 61 cases, it was believed that there was some evidence of a potential hazard from raised intra-abdominal pressure other than due to fasciculation. Systolic blood pressure rose more than 20% in 19 patients, and fell more than 20% in 9 patients.

In order to clarify the state of bioelectrical muscular activity in relation to the severity of the disease and the localization of the pathological process in Urowsche disease, the author examined 107 patients and 27 normals. The achieved data point to quantitative and qualitative differences of the bioelectrical muscular activity in patients and normals. In Urowsche disease in all stages of rest, in deep inhalation and synergia, electrical activity was not present. In a voluntary muscular contraction in 45.5% of the muscles there was type I in the EMG, 33.4% -- type II and 21.1% -- type IV.

16 patients suffering from syringomyelia were checked for additional peripheral nerve lesions. On voluntary effort, often in conformity with the decrease of muscle strength, a moderate rarefication of the discharge patterns was apparent. Fasciculation potentials were common. Motor conduction velocities of the median and ulnar nerve were in the normal range in cases without additional peripheral nervous lesion at the elbow or the carpal. Depending on the severity of the peripheral nervous lesion, we found the duration of the muscle action potentials increased.

Small discrete spikes appearing on the electrocardiogram (ECG) of a man with a pacemaker turned out to be fasciculation potentials picked up from leg muscles. To find out how common these spikes were all routine ECGs made in one month were reviewed. Spike potentials were found in six patients, and in each case they originated from somatic musculature. Spikes could nearly always be detected on ECGs that were recorded subsequently in patients known to have lower motor neurone lesions that produced fasciculation. Interference spikes were noted on the electrocardiogram of a patient with an implanted demand pacemaker. Runaway malfunction was suspected and the generator replaced. Subsequent investigation showec that the interference originated from fasciculation in the left leg caused by underlying neuromuscular disease. Skeletal muscle potentials can produce an electrocardiographic appearance closely resembling 'runaway' pacemaker. Such abnormalities should prompt a search for occult neuromuscular disease.
