Ultrasonographic insights into the complex anatomy and biomechanical dynamics of the Achilles tendon and its fascicles: a pictorial essay.

This pictorial essay clarifies the complex anatomical and ultrasonographic features of the Achilles tendon. Utilizing high-frequency ultrasound probes (12-18 MHz), the study visualizes the tripartite segmentation of the tendon as it relates to anatomical fascicles. Notably, there is variability in the ultrasound visibility of fascicles and this partition influences biomechanical properties and susceptibility to injury. This essay not only enhances the understanding of Achilles tendon anatomy but also underscores the importance of anatomical knowledge for comprehending associated pathologies. T

Suspected chlorfenapyr poisoning in a horse.

Chlorfenapyr is a pesticide that interferes with mitochondrial function, leading to cell death and mortality. Although poisoning is primarily documented in humans and dogs, it can result in severe clinical signs, including sweating, respiratory distress, and neurological dysfunction. This report describes the clinical and laboratory findings of a horse with suspected accidental intoxication with chlorfenapyr. A 4-year-old Quarter Horse mare was referred to the veterinary hospital with signs of excessive sweating, hyperthermia, tachypnea, muscle fasciculation, and fearful facial expression. The

Nervous system-wide analysis of all C. elegans cadherins reveals neuron-specific functions across multiple anatomical scales.

Differential expression of cell adhesion proteins is a hallmark of cell-type diversity across the animal kingdom. Gene family-wide characterization of their organismal expression and function is, however, lacking. Using genome-engineered reporter alleles, we established an atlas of expression of the entire set of 12 cadherin gene family members in the nematode Caenorhabditis elegans, revealing differential expression across neuronal classes, a dichotomy between broadly and narrowly expressed cadherins, and several context-dependent temporal transitions in expression across development. Enginee

Fasciculation distribution in a healthy population assessed with diffusion tensor imaging.

Fasciculations, a hallmark of motor neuron diseases, also occur in healthy individuals, highlighting the need to understand fasciculation intensity and distribution. Motor unit MRI (MUMRI) can assess fasciculations in large volumes but is not widely applied. We hypothesize that a more common MRI technique, diffusion tensor imaging (DTI), can also detect fasciculation when correcting for low signal-to-noise ratios and signal variability. We first systematically compared MUMRI and DTI in upper leg muscles of healthy subjects (n = 5). Secondly, we retrospectively determined fasciculation intensit

[A case of amyotrophic lateral sclerosis complicated by syringomyelia associated with Chiari type I malformation].

The patient was a 78-year-old woman. She underwent foramen magnum decompression for syringomyelia associated with Chiari type I malformation, which had developed with difficulty in raising the left upper limb and muscle weakness in both upper limbs. One year after surgery, weight loss of 20 ‍kg, progressive muscle atrophy and weakness in the extremities, paralytic dysarthria, and fasciculation in the bilateral anterior thighs were observed, and needle electromyography showed acute denervation and chronic denervation in the medial vastus muscle. The rapid postoperative progression of symptoms a

Characteristics of Neuromuscular Ultrasound in Patients with Amyotrophic Lateral Sclerosis.

INTRODUCTION: Neuromuscular ultrasound has been increasingly used in the detection and diagnosis of amyotrophic lateral sclerosis (ALS), commonly characterized by peripheral nerve atrophy, degeneration, and muscle fasciculations. The aim of this study was to assess the ultrasound characteristics of ALS patients. METHODS: A total of 67 consecutive patients with sporadic ALS and 19 with ALS mimics (63.16% peripheral neuropathy) were recruited. Ultrasound and electrophysiological examinations were performed; the peripheral nerve cross-sectional area (CSA) and fasciculation grades were compared b

Sex-Difference in Olfactory Interhemispheric Malformation Caused by Pax6 Haploinsufficiency.

Sex differences are increasingly recognized as critical factors influencing the onset and manifestation of various neurodevelopmental disorders, including autism spectrum disorder (ASD). Among ASD-associated genes, Pax6, which encodes a transcription factor, has been implicated in sex-dependent phenotypes when mutated. The anterior commissure (AC), a major commissural pathway involved in higher cognitive and social functions, exhibits structural abnormalities both in individuals with ASD and in those carrying Pax6 mutations. Anatomically, the AC consists of two principal fiber bundles: the ant

Progressive Bulbar Palsy (PBP) or Bulbar Onset MND: "A Case Report".

A patient with enhancing bulbar palsy, a type of efferent neuron disease that causes hypertrophy and twitching of the tongue's musculature, dysphagia, dysarthria, and an excessive buildup of secretions, is described. Enhancing bulbar palsy is a degenerative disorder of the efferent nuclei in the medulla. The patient may consult a dentist at first. Clinicians must possess knowledge regarding the telltale signs and symptoms of this terminal illness to promptly refer patients for neurologic evaluation and initiate appropriate symptomatic treatments.

Genetic compensation highlights the importance of neural cell adhesion molecule Ncam1 paralogs in balancing signaling pathways during zebrafish lateral line development.

The neural cell adhesion molecule NCAM1 is essential for neuronal development and enables organized cell migration, axon growth, and fasciculation. As a result of genome duplication in zebrafish, the paralogs Ncam1a and Ncam1b arose. Our previously published findings using morpholino knockdown experiments demonstrated the essential role of Ncam1b in the development of the zebrafish lateral line system, a mechanosensory organ critical for detecting water movements. ncam1b morphants exhibited severe defects, including impaired primordium migration, disrupted proneuromast deposition, and reduced

Myospasm and Fasciculation After Botulinum Toxin Injection.

Botulinum toxin type A (BTA) is commonly used to treat dystonia; however, reports of dystonia caused by BTA are rare. In this case study, the authors aimed to illustrate the instances of dystonia after cosmetic injections of BTA. Three cases were retrospectively analyzed, with follow-up evaluations conducted to assess the development and resolution of symptoms. Injection techniques, including microdroplet and intradermal methods, were examined, and potential hypotheses regarding the etiology of dystonia were explored. Recommendations for mitigating adverse effects were formulated based on the

"Smart" Nerves Sprout and Assemble in an Extracellular Matrix-Based 3D Nerve-in-a-Chip Microfluidic Model.

Controlling directional neurite outgrowth and Schwann cell (SC) migration remains critical for nerve regeneration. While 2D anisotropic topographies guide nerve growth via contact guidance, these models fail to recapitulate the 3D extracellular matrix microenvironment of native nerves. Herein, a microfluidic "nerve-in-a-chip" platform is customized and filled with decellularized nerve matrix hydrogel (DNM-G) to investigate how microchannel dimensions regulate 3D neurite extension and SC dynamics. Mid-sized microchannels optimally enhanced longitudinal neurite growth and fasciculation, whereas

Paclitaxel neurotoxicity is triggered by epidermal EG5-dependent microtubule fasciculation and X-ROS formation.

Taxanes are frontline chemotherapeutics that stabilize microtubules, induce mitotic arrest, and drive tumor remission. However, their off-target effects in healthy tissues, most notably cutaneous axon degeneration underlying chemotherapy-induced peripheral neuropathy (CIPN), remain poorly understood. Here, we show that paclitaxel induces microtubule fasciculation in epidermal keratinocytes through the mitotic kinesin Eg5, thereby initiating CIPN. Mechanistically, paclitaxel enhances Eg5-dependent fasciculation of detyrosinated (stabilized) microtubules, which constrict and breach the nuclear l

Differential Response to Corticosteroids in Immune-Mediated Myokymia vs Grouped Fasciculation Discharges.

Myokymia and grouped fasciculation discharges (GFD) are electromyographic manifestations of peripheral nerve hyperexcitability syndromes. This case series explores the differential response to corticosteroids in patients with myokymia compared to those with GFDs. Six patients were included in this report: three with predominant myokymia and three with predominant GFDs, reviewed between March 2023 and February 2024. Electromyographic examinations were performed on various muscle groups, with the gastrocnemius EMG findings serving as an index. The neurologist conducting the EMG was blinded to th

Fasciculation in limbs serves as the predictor of ALS progression: an ultrasound study.

OBJECTIVE: To explore the predictive effects of fasciculation by ultrasound in amyotrophic lateral sclerosis (ALS) progression. METHODS: Sporadic ALS patients were consecutively recruited and followed up 3 to 6 months after the initial visit. Muscle ultrasound examination was conducted at the baseline to detect the severity score of fasciculations on bilateral elbow flexor and extensor, ankle dorsiflexor and plantar flexor of each patient, the sum of which was defined as the total fasciculation score. Baseline and follow-up ALS functional research scale-revised (ALSFRS-R) score and muscle str

Functional role for Cas cytoplasmic adaptor proteins during cortical axon pathfinding.

Proper neural circuit organization requires individual neurons to project to their targets with high specificity. While several guidance molecules have been shown to mediate axonal fasciculation and pathfinding, less is understood about how neurons intracellularly interpret and integrate these cues. Here we provide genetic evidence that the Crk-Associated Substrate (Cas) family of intracellular adaptor proteins is required for proper fasciculation and guidance of two cortical white matter tracts: the Anterior Commissure (AC) and thalamocortical axons (TCAs). Using a Cas Triple Conditional Knoc

Unravelling the role of Neurotrimin (NTM), a member of the IgLON family, in mild intellectual disability and anxiety-like behaviors.

Neurotrimin (NTM) (OMIM #607938), a member of the IgLON family, collaborates with other neural cell adhesion proteins to regulate neurite outgrowth, axonal fasciculation, and synapse formation. Disruption of NTM function in animal models has been associated with deficits in emotional learning and increased anxiety-like behaviours. In this study, we examined an individual presenting with mild intellectual disability, anxiety-like behaviours, and low frustration tolerance, along with generalised hypotonia. Trio-based whole exome sequencing (WES) identified a de novo nucleotides insertion, c.788_

Clinically visible but often unperceived: Low awareness of fasciculations in amyotrophic lateral sclerosis.

BACKGROUND: Fasciculations are a key clinical sign of amyotrophic lateral sclerosis (ALS) but also occur in other conditions such as benign fasciculation syndrome. Patients often present with perceived twitching fearing ALS; however, the extent to which ALS patients themselves perceive fasciculations has not been systematically examined. We therefore aimed to clarify how often ALS patients are aware of fasciculations that are clinically visible. METHODS: We prospectively studied 34 ALS patients. First, a structured questionnaire assessed initial symptoms, chief complaints, and awareness of tw
