Delayed-onset central neurotoxic syndrome with predominant cerebellar dysfunction following combined chlorpyrifos and cypermethrin poisoning: a case report.

Organophosphate (OP) poisoning typically manifests in three phases: the acute cholinergic crisis, the intermediate syndrome, and OP-induced delayed neuropathy (OPIDN). Delayed central nervous system sequelae with predominant cerebellar dysfunction are exceptionally rare and poorly characterized. We report the case of a woman in her early thirties with a history of chronic alcohol use who ingested a commercial insecticide containing chlorpyrifos (50%) and cypermethrin (2%). Following the resolution of an acute cholinergic crisis and a brief period of cardiac arrest (return of spontaneous circul

Thoracic Spontaneous Spinal Epidural Hematoma in a Young Adult: A Case Report.

Spontaneous spinal epidural hematoma (SSEH) is an uncommon condition that may present with acute back pain and neurological deficits. Prompt diagnosis and timely intervention are important to optimise neurological recovery. A 21-year-old female presented with a 5-day history of back pain associated with right lower limb weakness, gait instability, and bilateral foot numbness. Clinical examination demonstrated reduced muscle power (4/5) in the right lower limb and an ataxic gait. Magnetic resonance imaging (MRI) revealed a posterior epidural hematoma extending from T6 to T8 with significant spi

Acute Bilateral Visual Loss as an Atypical Manifestation of Wernicke Encephalopathy Secondary to Hyperemesis Gravidarum: A Case Report and Narrative Literature Review.

Wernicke encephalopathy (WE) is a severe neurological condition caused by thiamine deficiency that remains underreported in pregnancy. We report a clinical diagnosis of WE in a 41-year-old woman, gravida 2, para 1, at 16 weeks of gestation. She presented with acute, painless visual impairment, severe dietary restriction, and marked weight loss secondary to severe hyperemesis gravidarum (HG). Examination revealed reduced visual acuity, horizontal nystagmus, psychomotor retardation, and an ataxic gait. The initial diagnosis was delayed due to her atypical presentation and the absence of MRI or b

Astrocyte-specific deletion of LRRC8A causes neurological dysfunction but not chronic white matter edema.

Volume-regulated anion channels (VRACs) are central to cell volume homeostasis. They mediate swelling-induced efflux of chloride and organic osmolytes to drive regulatory volume decrease. In the brain, VRACs have been proposed to play a key role in astrocytic volume regulation. Genetic defects in astrocytic VRAC modulating proteins (MLC1, GlialCAM, Aquaporin-4, GPRC5B) cause the leukodystrophy Megalencephalic leukoencephalopathy with subcortical cysts (MLC), characterized by chronic white matter edema and myelin vacuolization. Disrupted VRAC activity in MLC-patient-derived lymphoblasts and pri

Whisking Behaviour Reveals Stronger Evidence of Habituation in Homozygous Reeler Mice Compared to Controls.

Reeler mice have a mutation in the reelin gene. As a result, Reeler mice lack cortical layers, yet their brains are still largely functional. However, Reeler mice display strong motor phenotypes, including ataxic gait and tics, and we posit that their whisking behaviour might also be disrupted. We used high-speed video to film and track whisker movements in 9 adult Reeler mice and 9 age-matched controls in three whisker movement assessment tasks, including our established novel object exploration and open field tasks, as well as a new open field habituation task. Overall, whisker movements in

Nitrous Oxide-Induced Myelopathy: A Case From French-Speaking Switzerland.

Recreational nitrous oxide (N₂O) use has increased worldwide, driven by its accessibility, low cost, and perception as a harmless "party drug." Chronic or heavy use can cause neurological complications through functional vitamin B₁₂ deficiency. Despite its growing prevalence, such cases remain rarely documented in French-speaking Switzerland. Our patient is a 27-year-old man of Cape Verdean origin who presented with rapidly progressive paresthesia and gait ataxia following heavy N₂O inhalation. Neurological examination revealed profound sensory ataxia and loss of vibration sense in the lower l

How I treat Angelman syndrome: an expert opinion.

INTRODUCTION: Angelman syndrome (AS) is a neurodevelopmental disorder arising from loss of maternal UBE3A function, characterized by intellectual disability, absent speech, ataxic gait, and epilepsy in 80-90% of patients. AREAS COVERED: Seizure management remains symptomatic, guided by retrospective cohort data, caregiver surveys, and expert opinion in the absence of randomized controlled trials. This review systematically addresses the complete neurology-centered clinical management pathway for AS - from diagnostic recognition, neurodevelopmental and seizure characterization, delineation of

Clinical Reasoning: A 28-Year-Old Man with Subacute-Onset Ataxic Gait.

The diagnostic approach to subacute progressive polyneuropathy can be challenging, particularly when accompanied by systemic features. We present a 28-year-old man with subacute-onset ataxic gait. Four months before his initial evaluation at our center, he was assessed by orthopedics for a poorly differentiated tumor of the right tibia. He also reported intermittent fever and unintentional weight loss. Neurologic examination revealed marked weakness in all four limbs, with deep tendon reflexes absent in the lower extremities. Sensory testing demonstrated a stocking-glove distribution of hypoes

Homozygous PTRHD1 Mutation in Intellectual Disability and Atypical Parkinsonism.

Five different homozygous PTRHD1 mutations, two of them in more than one family, have been reported as responsible for intellectual disability (ID) and parkinsonism. In all 10 families, onset of ID was early childhood, and in parkinsonism, later childhood to the fourth decade. We report on a family with four siblings presenting with mild to moderate ID and mildly ataxic gait without spasticity or hemiparesis that is not consistent with parkinsonism. The signs of parkinsonism such as bradykinesia, tremor, slow to response, dementia, and gait problems appeared in the fourth decade. There was no

Skin Hyperpigmentation as Initial Manifestation of Vitamin B12 Deficiency: A Case of Delayed Diagnosis With Late-Onset Subacute Combined Degeneration of the Cord.

Vitamin B12 deficiency may present with a myriad of symptoms. Rarely, skin hyperpigmentation may be the sole initial manifestation of this micronutrient deficiency, which can easily be overlooked, leading to delayed diagnosis. This case report describes a 32-year-old Ghanaian male who presented with a 4-year history of progressive darkening of the dorsal aspects of the interphalangeal joints of both hands, the palms, and soles of the feet, for which he did not seek immediate medical attention. He later developed a worsening unsteady gait. Neurological assessment revealed mild weakness in both

Monitoring the Progression of Pre-Ataxic Gait in SCA2 with Inertial Sensors Over Four Years.

The search for digital biomarkers of gait ataxia is a key research priority in spinocerebellar ataxias (SCAs), especially in the early stages when traditional clinical scales are less effective. Despite existing evidence supporting the effectiveness of suitable digital biomarkers, their use in assessing early disease progression remains limited. This study was aimed to evaluate the progression of digitally measured gait ataxia features in preclinical SCA2. Twenty-seven preclinical carriers of the SCA2 mutation were monitored four times over four years. Participants completed a 10-meter walking

Toward a deeper and more comprehensive understanding of neurological symptoms related to Allgrove syndrome: a case report.

INTRODUCTION AND IMPORTANCE: Allgrove syndrome (AS) is a rare autosomal recessive disorder caused by triple A syndrome gene mutations, characterized by alacrima, achalasia, and adrenocorticotropic hormone (ACTH)-resistant adrenal insufficiency, with potential neurological complications. Early signs include absent tears, feeding difficulties, vomiting, and growth issues, while adrenal crises and neurological symptoms may appear later. Diagnosis is often delayed when only one feature is present. CASE PRESENTATION: We report a 13-year-old boy with AS on oral hydrocortisone who presented with rec

Expanding the clinical phenotype of HIST1H1E syndrome: cerebellar atrophy and bilateral optic neuropathy, a case report.

BACKGROUND: HIST1H1E syndrome is caused by frameshift variants in the HIST1H1E gene; while strabismus and refractive errors have been previously reported, this is the first case describing optic and cerebellar atrophy in an affected individual to our knowledge. METHODS: A 51-year-old woman with intellectual disability, recurrent falls, and progressive visual difficulties required comprehensive ophthalmic, neurologic, and genetic evaluation. RESULTS: Systemic examination revealed characteristic facial dysmorphism, brachydactyly, ataxic gait, tremor, and positive Romberg's sign. Ophthalmic exa

Autopsy-Confirmed Non-Paraneoplastic Lambert-Eaton Myasthenic Syndrome with Cerebellar Degeneration: A Case Report.

Background and Clinical Significance: Lambert-Eaton myasthenic syndrome (LEMS) is mediated by antibodies against P/Q-type voltage-gated calcium channels (VGCCs) and is classified as paraneoplastic (T-LEMS) or non-paraneoplastic (NT-LEMS). Cerebellar degeneration is recognized in T-LEMS, but pathological confirmation in NT-LEMS has not been reported. Case Presentation: A 79-year-old man developed progressive ataxic gait and dysarthria at age 76 and was diagnosed with LEMS based on repetitive nerve stimulation findings and anti-P/Q-type VGCC antibodies. No malignancy was identified during more t

Prominent Self-Injurious Behavior Masking the Diagnosis of Rett Syndrome: A Case Report from a Resource-Limited Setting.

Rett syndrome (RTT) is a rare neurodevelopmental disorder characterized by developmental regression, loss of purposeful hand use, and stereotypic movements after apparently normal early development. We report a 26-month-old girl who presented with severe self-injurious behavior (SIB), including repetitive biting of the left forearm and head banging, associated with restlessness, frequent yelling, and poor sleep. Further evaluation revealed regression of motor, language, and hand skills with stereotypic hand wringing. Examination showed impaired social interaction, ataxic gait, and hypertrophic

Linezolid-induced peripheral neuropathy in a patient with multidrug-resistant pulmonary tuberculosis: a case report and comprehensive literature review.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) remains a major global health challenge due to the limited availability of effective therapeutic options. Among second-line agents, linezolid has demonstrated excellent efficacy in both MDR-TB and extensively drug-resistant tuberculosis (XDR-TB), owing to its high oral bioavailability, good tissue penetration, and low resistance profile. However, its long-term use is frequently limited by adverse effects-most notably peripheral neuropathy, along with optic neuritis and myelosuppression-which often lead to early treatment discontinuation and

Anaesthetic Management of a Spontaneous Haemorrhagic Stroke Following Severe Preeclampsia: A Case Report.

Haemorrhagic stroke (HS) during pregnancy, especially in the context of preeclampsia, is a rare but critical emergency that poses significant health risks to both the mother and the foetus. Even so, rapid recognition and appropriate management are of utmost importance to minimise unfavourable outcomes. We present a case of the management of HS in a pregnant woman with severe preeclampsia, resulting in favourable outcomes for both mother and neonate. A 39-year-old female (gravida 8, para 4) at 32 weeks and six days of gestation was admitted with sudden-onset severe headache, vomiting, hypertens

Wernicke Encephalopathy Secondary to Cannabinoid Hyperemesis Syndrome in a 15-Year-Old Male Patient: A Case Report.

The use of cannabis and high-potency tetrahydrocannabinol (THC) products has risen substantially among adolescents, and cannabinoid hyperemesis syndrome (CHS) is now an increasingly recognized complication of chronic use. CHS is characterized by recurrent episodes of intractable nausea, vomiting, and abdominal pain that resolve with cannabis cessation. Wernicke encephalopathy (WE), a neurologic syndrome caused by thiamine deficiency, is classically associated with chronic alcohol use but is increasingly reported in nonalcoholic and pediatric patients, in whom atypical presentations complicate
