This report details a case of a patient in his 60s who exhibited ataxic gait, frequent falls, limb incoordination, tremors and rigidity, leading to a diagnosis of progressive supranuclear palsy with cerebellar predominance (PSP-C). Clinical assessment indicated slurred speech, increased muscle tone, deep tendon reflexes that were mildly reduced asymmetrically on the left side and reduced strength, while sensations remained intact. PSP-C is an uncommon variant of PSP often misidentified as multiple system atrophy-cerebellar type (MSA-C) because of shared characteristics. The patient participate

Mounting evidence suggests that antiganglioside antibodies play a crucial role in the pathogenesis of Guillain-Barré syndrome (GBS) and its variants. Among them, anti-GT1a ganglioside immunoglobulin G (IgG) antibody (anti-GT1a IgG) is most strongly associated with cranial nerve involvement. We report a rare case of a 26-year-old woman who developed ocular flutter and mild ataxia following an upper respiratory tract infection. Neurological examination revealed rapid, conjugate horizontal saccadic oscillations without intersaccadic intervals, along with a mildly ataxic gait. Brain magnetic reson

BACKGROUND: Peripheral neuropathy is a common complication of Lyme disease. Cranial mononeuropathy, particularly that affecting the facial nerve, can be a presenting symptom, and at times, it can be associated with polyradiculopathies or plexopathies. However, isolated femoral neuropathy has not yet been reported in Lyme disease; therefore, we felt the need to present this case.
CASE PRESENTATION: Laboratory investigations were performed on a 67-year-old man living in a region at high risk for Lyme disease after he developed erythema migrans on his chest, accompanied by the swelling of his le

An 83-year-old male presented with a months-long ataxic gait and recent frequent falls. Initial evaluation revealed ventriculomegaly and acute ischemic infarction to the right cerebellar and occipital regions in addition to severe cervical stenosis. Syphilis serology with reflex returned positive, and neurosyphilis was confirmed on cerebrospinal fluid with a reactive Venereal Disease Research Laboratory test, lymphocytic pleocytosis, and elevated protein. The patient was put on a two-week course of intravenous penicillin G while completing inpatient physical rehabilitation and demonstrated sig

Extraintestinal pathogenic Escherichia coli (ExPEC) are opportunistic pathogens in pigs and can cause systemic infections with neurological signs. Cases in suckling piglets have rarely been reported. In November 2024, suckling piglets aged 1-3 weeks on a commercial pig farm developed neurological signs, including altered consciousness, vertical nystagmus, ataxic gait, and recumbency. Within two affected batches comprising 1150 and 1200 piglets, 17 and 26 animals, respectively, developed clinical signs (morbidity 1.48% and 2.17%). Initial treatment with amoxicillin and dexamethasone was ineffec

It is estimated that Angelman syndrome (AS) accounts for up to 6% of all children presenting with severe mental retardation and epilepsy. The main clinical features of AS may not be apparent early in life. Clinical findings present in all patients include developmental delay, which becomes apparent by 6-12 months of age, severely impaired expressive language, ataxic gait, tremulousness of limbs, and a typical behavioral profile, including a happy demeanor, hypermotoric behavior, and low attention span. Seizures, abnormal electroencephalography, microcephaly, and scoliosis are observed in >80%

BACKGROUND: Tumor necrosis factor-alpha (TNF-α) inhibitors are widely used for immune-mediated inflammatory diseases, including Crohn's disease. Although generally effective, TNF-α blockade has been linked to uncommon but clinically significant paradoxical immune-mediated neurologic adverse events, including inflammatory demyelinating disorders of the central nervous system (CNS).
CASE PRESENTATION: A 32-year-old woman with Crohn's disease (perianal fistula) treated with infliximab for one year presented with progressive dizziness, headache, and bilateral lower-limb weakness. Neurologic exami

Gait disturbance is a common motor symptom in Angelman syndrome (AS), but its characteristics have been poorly studied quantitatively. This study aimed to analyze gait characteristics in school-age children with AS using three-dimensional gait analysis (3DGA). Patients with clinically and genetically confirmed AS and healthy children aged 6-15 years were included. For gait assessments, 3DGA was performed using an eight-camera motion analysis system and eight force plates. Gait metrics, including gait speed, step length, step width, gait variability, gait deviation index, and kinematic and kine

Dysfunctions in autophagy, a cellular mechanism for breaking down components within lysosomes, often lead to neurodegeneration. The specific mechanisms underlying neuronal vulnerability due to autophagy dysfunction remain elusive. Here we show that autophagy contributes to cerebellar Purkinje cell (PC) survival by safeguarding their glycolytic activity. Outside the conventional housekeeping role, autophagy is also involved in the ATG5-mediated regulation of glucose transporter 2 (GLUT2) levels during cerebellar maturation. Autophagy-deficient PCs exhibit GLUT2 accumulation on the plasma membra

BACKGROUND: Pose estimation algorithms applied to two-dimensional videos evaluate gait disturbances; however, a few studies have used this method to evaluate ataxic gait.
OBJECTIVE: The aim was to assess whether a pose estimation algorithm can predict the severity of cerebellar ataxia by applying it to gait videos.
METHODS: We video-recorded 66 patients with degenerative cerebellar diseases performing the timed up-and-go test. Key points from the gait videos extracted by a pose estimation algorithm were input into a deep learning model to predict the Scale for the Assessment and Rating of At

Cytarabine is widely used in the treatment of hematological malignancies. Its common toxicities include myelosuppression and gastrointestinal disturbances. Additionally, it can cause central nervous system (CNS) symptoms, which include hoarseness, ataxic tremor, ataxic gait, nystagmus, dysmetria, and dysdiadochokinesia. In this article, we present the first case report of dysphonia, absent of CNS symptoms, induced by high-dose cytarabine (HiDAC) in a patient with acute myeloid leukemia. The patient's voice began to change 3 days following the first cycle of HiDAC chemotherapy, and dysphonia re

CTNNB1, which encodes β-catenin, plays an essential role in the Wnt signaling pathway and regulates cellular homeostasis. Mutations in this gene can lead to neurodevelopmental disorder with spastic diplegia and visual defects (NEDSDV). This study aimed to identify CTNNB1 mutations in two patients presenting with global developmental delay and compare their distinct phenotypes. Next-generation sequencing (NGS) was performed to detect mutations in CTNNB1. Longitudinal clinical observations were conducted to analyze the clinical features of the patients. The first patient was a 7-year-old boy who

Adult polyglucosan body disease is a rare genetic condition caused by biallelic pathogenic variants in GBE-1 gene. Affected patients typically have urinary dysfunction, progressive gait disturbance and cognitive impairment. We report a 63-year-old woman with urinary incontinence, walking difficulty and episodes of forgetfulness. She had symmetrical limb weakness with upper motor neurone signs, distal sensory loss and a broad-based ataxic gait. MR scans of the brain and spine showed white matter changes with cerebellar and spinal cord atrophy. Sural nerve biopsy identified intra-axonal polygluc

Background and aim: Cerebral palsy (CP) is a nonprogressive neurological disorder characterized by permanent developmental disorders of movement and posture. One of the most common goals of rehabilitation is the treatment of gait disorders. Ataxic gait disorder tends to worsen in the adolescent period. Research indicates a positive therapeutic effect of the combined application of conventional rehabilitation, robotic neurorehabilitation (RNR) and virtual reality, but there is no consensus on the length of treatment and frequency of application. The aim of this case report was to contribute to

The Lentiform Fork sign is an imaging finding on brain MRI described by bilateral, symmetric hyperintensities in the basal ganglia. It is described in the setting of metabolic acidosis of many etiologies and is a sign of vasogenic edema. Both imaging findings and symptoms are often reversible with correction of the acidosis and removal of any identifiable triggers. We report a case of confusion and ataxic gait in a patient with end stage renal disease and ongoing metformin use. The mechanism of metformin-induced encephalopathy remains unclear, but could be due to vulnerability of mitochondria

Spinocerebellar ataxia 13 (SCA13) is an autosomal dominant neurological disorder caused by mutations in KCNC3. Our previous studies revealed that KCNC3 (Potassium Voltage-Gated Channel Subfamily C Member 3) mutation R423H results in an early-onset form of SCA13. Previous biological models of SCA13 include zebrafish and Drosophila but no mammalian systems. More recently, mouse models with Kcnc3 mutations presented behavioral abnormalities but without obvious pathological changes in the cerebellum, a hallmark of patients with SCA13. Here, we present a novel transgenic mouse model by bacterial ar

INTRODUCTION/AIMS: Charcot-Marie-Tooth disease (CMT), the most common inherited peripheral neuropathy, is clinically and genetically heterogeneous with over 100 genes identified to date. Recently, next-generation sequencing (NGS) has enabled molecular diagnosis in previously unidentified CMT cases. However, less progress has been achieved in sub-Saharan African (SSA) populations. We report rare CMT variants found in four unrelated Malian families.
METHODS: Patients went through a thorough neurological examination and Nerve Conduction Studies (NCS) were performed. DNA was extracted for genetic
