Movement disorders : official journal of the Movement Disorder Society2025eng
BACKGROUND: With disease-modifying drugs for degenerative ataxias on the horizon, ecologically valid measures of gait performance that can detect patient-relevant changes in trial-like time frames are highly warranted.
OBJECTIVES: In this 2-year longitudinal study, we aimed to unravel ataxic gait measures sensitive to longitudinal changes in patients' real lives using wearable sensors.
METHODS: We assessed longitudinal gait changes of 26 participants with degenerative cerebellar disease (Scale for the Assessment and Rating of Ataxia [SARA]: 9.4 ± 4.1) at baseline, 1-year, and 2-year follow-u

Spinocerebellar ataxia type 2 (SCA2) is an autosomal dominant neurodegenerative disorder marked by cerebellar dysfunction, ataxic gait, and progressive motor impairments. SCA2 is caused by the pathologic expansion of CAG repeats in the ataxin-2 (ATXN2) gene, leading to a toxic gain-of-function mutation of the ataxin-2 protein. Currently, SCA2 therapeutic efforts are expanding beyond symptomatic relief to include disease-modifying approaches such as antisense oligonucleotides (ASOs), high-throughput screening (HTS) for small molecule inhibitors, and gene therapy aimed at reducing ATXN2 expressi

Pembrolizumab is an immune checkpoint inhibitor targeting programmed cell death protein 1 , used for stage IIB/IIC melanoma. While effective, pembrolizumab can lead to immune-mediated toxicities in various systems; however, nervous system effects are less well-documented. We report the case of a 34-year-old man who developed delayed immune-mediated encephalomyelitis with antiglial fibrillary acidic protein (GFAP) antibodies after completing nine cycles of adjuvant pembrolizumab for stage IIC melanoma. Four months posttreatment, he presented with neurological symptoms, including ataxic gait, se

BACKGROUND/OBJECTIVES: Joubert syndrome (JS, MIM 213300) is a rare genetic condition characterized by respiratory control disturbances, abnormal eye movements, ataxia, cognitive impairment, and the notable agenesis of the cerebellar vermis. The molar tooth sign visible in magnetic resonance imaging of the brain serves as a diagnostic tool for JS. Variants in the TCTN3 gene can lead to the development of several diseases, including JS type 18, Orofaciodigital syndrome IV, and Meckel-Gruber syndrome.
METHODS: We performed whole-exome sequencing (WES) in a 49-year-old woman with JS characterized

BACKGROUND: This study evaluated the effects of difelikefalin, a kappa opioid receptor (KOR) agonist, on the inflammatory response and renal dysfunction in a rat model of acute kidney injury induced by renal ischemia-reperfusion injury, comparing it with other KOR agonists.
METHODS: One week after right nephrectomy, rats received one of three drugs (difelikefalin, nalfurafine hydrochloride, or U-50488H). Thirty minutes later, ischemia was induced in the left kidney by clamping its artery and vein for 45 min. Rats were then transferred to metabolic cages for 24-h urine collection, and blood wa

Prolactinomas are the most common functional pituitary adenomas in the pediatric population, though they remain rare overall. Male adolescents often present with larger and more aggressive tumors than female adolescents, with delayed symptoms such as visual disturbances, headaches, and hypogonadism due to the absence of early hormonal signs. We report the case of a 15-year-old previously healthy male patient who presented with a one-month history of severe frontal headache, followed by blurred vision and vomiting. On examination, he exhibited bilateral mydriatic pupils unresponsive to light, l

BACKGROUND: Hemophagocytic lymphohistiocytosis (HLH) is a syndrome of hyperinflammation. It is categorized into a primary or secondary form. Familial HLH is considered a subset of primary HLH that results from a biallelic mutation in one of the four genes that regulate granule-dependent cytotoxicity: PRF1, UNC13D, STX11, and STXBP2. The reported ocular findings in association with HLH are diverse and variable, with few reports in particular in patients with primary HLH. As far as our knowledge goes, this is the first documented instance of a genetically confirmed case of PRF1-associated famili

As gaitpathology assessment systems improve both in accuracy and efficiency, the prospect of using these systems in real healthcare applications is becoming more realistic. Although gait analysis systems have proven capable of detecting gait abnormalities in supervised tasks in laboratories and clinics, there is comparatively little investigation into making such systems explainable to healthcare professionals who would use gait analysis in practice in home-based settings. There is a "black box" problem with existing machine learning models, where healthcare professionals are expected to "trus

BACKGROUND: Chronic lymphocytic inflammation with pontine perivascular enhancement responsive to steroids is a rare, subacute inflammatory disorder of the central nervous system with an unknown etiology. It is characterized by distinct clinical (diplopia, ataxia, dysarthria, and altered facial sensation), radiological (punctiform lesions detected on magnetic resonance imaging), and histopathological (predominantly perivascular lymphocytic infiltration, mainly affecting the pons and cerebellum) features. The condition typically demonstrates a favorable response to corticosteroid therapy.

We report the case of a 62-year-old male presenting with progressive gait disturbances, recurrent syncope, and episodes of myoclonic epilepsy over three months, with underlying sporadic Pick's disease diagnosed clinically. Imaging revealed a well-defined, extra-axial tumor in the right parietal region, consistent with a meningioma. Surgical intervention using neuronavigation technology facilitated a complete tumor resection. The patient's postoperative course was stable, with a resolution of syncope and improvement in gait. This case underscores the importance of interdisciplinary management i

Gait and balance disorders are a leading cause of morbidity, mortality, and disability in central and peripheral neurologic disorders. Neurologic gait disorders are classically evaluated with a clinical examination and visual pattern recognition. Gait patterns such as a parkinsonian or ataxic gait may have distinct etiologies and are assessed using the neurologic physical examination and validated clinical scales. Technological advances have made gait analysis more accessible, allowing precise objective measurement of gait and balance deficits. Gait analysis may be more sensitive at detecting

Neurodegenerative diseases commonly manifest as progressive deterioration of neurological function in varied forms. Damage to the cerebellum disrupts the coordination of limb and eye movements, impairs balance, and decreases muscle tone. A 11-year-old male child born to consanguineous parents, developmentally normal until five years of age, presented with a history of frequent falls while walking and speech difficulty from the age of five years. On examination, bilateral ocular motor apraxia was seen with an ataxic gait and slurred speech. All the other cerebellar signs were present. Magnetic

Glycosylphosphatidylinositol (GPI) anchors are a class of post-translational modifications observed on over 150 proteins. Pathogenic variants in the GPI biosynthesis enzyme, PIGA, in humans are associated with several brain anomalies such as hypomyelination, cerebellar hypoplasia, ataxic gait, and can lead to premature mortality. We previously genetically deleted Piga from the embryonic mouse brain which led to early postnatal death and significant structural brain malformations similar to those observed in humans with PIGA variants. The current treatment options for PIGA patients only manage

Mutations in the SACS gene are associated with autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS), a clinically and genetically heterogeneous neurodegenerative disorder. ARSACS typically manifests as slowly progressive ataxia with spasticity and sensorimotor neuropathy. Nevertheless, an array of additional features may also be observed, including hearing impairment, epileptic seizures, and even the absence of spasticity. Reports of SACS mutations in Polish patients are rare. Here we report a compound heterozygous pathogenic variants in the SACS gene, a novel duplication of exon

BACKGROUND: Recessive mutations in POLR3A exhibit considerable phenotypic diversity, spanning from severe childhood-onset hypomyelinating leukodystrophic syndrome to less severe gait disorders, which may present later in life and may be accompanied by additional non-neurological symptoms. In this study, we report a new case of rare POLR3A variants in a 6-year-old female patient sharing common genetic and neuropsychological profiles of POLR3-related disorders, although without revealing the classic MRI phenotype and severe clinical signs of POLR3-related leukodystrophy, such as diffuse hypomyel

European spine journal : official publication of the European Spine Society, the European Spinal Deformity Society, and the European Section of the Cervical Spine Research Society2025eng
BACKGROUND & OBJECTIVES: Basilar invagination (BI) represents a complex anomaly of the craniovertebral junction, characterized by the displacement of the odontoid process towards the foramen magnum. Current surgical interventions include anterior decompression and combined anterior-posterior decompression with posterior fusion. Traditional methods for odontoid resection encompass transoral, transnasal, and endonasal approaches. However, these techniques are fraught with significant risks. Furthermore, the restricted exposure provided by the endonasal corridor's anatomical limitations hampers s

BACKGROUND: Spinocerebellar ataxia (SCA) and Friedreich's ataxia (FA) frequently give rise to progressive scoliosis, which impairs respiratory and postural function. Although spinal fusion is the most reliable method for correcting deformities, perioperative neurological vulnerability and monitoring failures complicate decision-making. This study combines a systematic review with a detailed pediatric case to clarify current evidence on surgical management, complications, and outcomes.
METHODS: We present a 12-year-old girl who underwent posterior fusion (T4-L4) for presumed adolescent idiopat

BACKGROUND: Image-guided splanchnic neurolysis has been performed for decades to relieve abdominal pain due to cancer. Severe complications associated with the procedure are rare but could happen. Different from the previously reported cases of paraplegia due to anterior artery (cord) syndrome, here we presented a case of transient dorsal cord syndrome involving bilateral lower extremities after splanchnic neurolysis.
CASE REPORT: The patient had numbness and impaired proprioception of both legs, ataxic gait, and bowel and bladder dysfunctions shortly after bilateral splanchnic neurolysis. Ma
