Clinical and molecular overlap between nucleotide excision repair (NER) disorders and DYRK1A haploinsufficiency syndrome.

Nucleotide excision repair (NER) disorders are genetic conditions caused by defects in the pathway responsible for repairing DNA lesions due to UV radiation. These defects lead to a variety of heterogeneous disorders, including Cockayne syndrome (CS) and trichothiodystrophy (TTD). In this study, we report 11 patients initially suspected of having CS or TTD who were ultimately diagnosed with DYRK1A haploinsufficiency syndrome using high-throughput sequencing. Comparing clinical presentations, we observed that DYRK1A symptoms overlapped with CS, with shared features such as intellectual disabili

Two Siblings with LRPPRC Mutation: Mitochondrial Complex IV Deficiency: Case Report.

INTRODUCTION: Mitochondrial diseases caused by mutations in the LRPPRC gene are rare and lead to multisystemic dysfunction. We report two siblings from consanguineous Iraqi parents, both harboring a rare homozygous deletion in LRPPRC (c.2726_2728del; p.Lys909del), previously reported in one other patient. These cases contribute to the expanding phenotypic and geographic spectrum of LRPPRC-related mitochondrial disease. CASE PRESENTATION: The younger sibling, a 9-year-old girl, presented with severe growth retardation, global developmental delay, hypotonia, spastic ataxic gait, and lactic acid

Multifocal hemorrhagic metastases with cerebellar predominance following prolonged remission of pancreatic adenocarcinoma: illustrative case.

BACKGROUND: Brain metastases from pancreatic adenocarcinoma are extremely rare, particularly when involving the cerebellum or presenting with atypical dural or hemorrhagic lesions. Only 2 other reported cases demonstrate multifocal brain metastases with cerebellar involvement following remission of pancreatic cancer. OBSERVATIONS: A 55-year-old man with stage IV pancreatic ductal adenocarcinoma and liver metastases achieved systemic remission following chemotherapy and chemoradiation therapy. Brain MRI 1 year postdiagnosis was negative for metastases. Twenty-seven months postdiagnosis, he dev

Eighty-Year-Old Male With Normal Pressure Hydrocephalus Presenting With Dementia and Parkinsonism Treated Using Electroencephalography (EEG)-Guided H4 Coil Protocol: A Case Report.

Idiopathic Normal Pressure Hydrocephalus (iNPH) is a rare syndrome of apractic or ataxic gait, mild to moderate dementia, and urinary incontinence. The cerebrospinal fluid tap test is widely used in the diagnosis of NPH. Using deep Transcranial Magnetic Simulation (DTMS) with H coils, we developed a novel treatment method that incorporates electroencephalography (EEG)-based customization of the treatment protocol. An 80-year-old male presented with clinical signs of slowness of gait and activity, rigidity, and urinary incontinence, which progressed to bradykinesia, magnetic, short, and shuffli

Anatomically Precise Microsurgical Resection of a Posterior Fossa Cerebellar Metastasis in an Elderly Patient with Preservation of Venous Outflow, Dentate Nucleus, and Cerebrospinal Fluid Pathways.

Background and Clinical Significance: Adults suffering from cerebellar metastases are often at high risk for rapid deterioration of their neurological status because the posterior fossa has limited compliance and the location of these metastases are close to the brain stem and important cerebrospinal fluid (CSF) pathways. In this paper, we present a longitudinal, patient-centered report on the history of an elderly individual who suffered from cognitive comorbidities and experienced a sudden loss of function in her cerebellum. Our goal in reporting this case is to provide a comparison between

Misleading EEG in CACNA1A mutation: A case of late-onset episodic ataxia type 2.

BACKGROUND: Episodic ataxia type 2 (EA2) is a rare, autosomal dominant paroxysmal neurological disorder caused by mutations in the CACNA1A gene. Its hallmark features include transient episodes of ataxia and dysarthria, often responsive to acetazolamide. However, misdiagnosis as epilepsy may occur due to overlapping electroencephalographic findings. CASE PRESENTATION: We report a 64-year-old woman with no prior history nor family history of neurological illness, who developed daily episodes of dysarthria and ataxic gait. Brain MRI did not show definite signs of cerebellar atrophy. Interictal

Metronidazole-induced encephalopathy and polyneuropathy.

Though effective, metronidazole poses multiple side effects, especially with prolonged therapy or high dosage. Among these are metronidazole-induced peripheral neuropathy and encephalopathy, which are severely debilitating. We describe a middle-aged man with a liver abscess who was treated with metronidazole 2.4 g/day for nearly 2 months, and who went on to develop dysarthria, ataxic gait and sensory loss, including numbness and burning paraesthesia in his limbs. Neurological assessment comprised nerve conduction studies (NCS) and magnetic resonance imaging (MRI). NCS indicated axonal neuropat

Hypertrophic cardiomyopathy with ataxic gait: a cardiac clue to a neurologic diagnosis.

Hypertrophic cardiomyopathy (HCM) is an autosomal dominant cardiac myocyte disease characterised by left ventricular hypertrophy that develops in the absence of causative haemodynamic factors. In addition to mutations in sarcomeric genes, which are the most significant cause of HCM in adults and children, paediatric HCM also exhibits a high prevalence of non-sarcomeric causes. These include inherited metabolic disorders, malformation syndromes, neuromuscular diseases and mitochondrial diseases collectively accounting for about 35% of cases. In this case report, we describe a case of non-sarcom

A Rare Presentation of Guillain-Barré Syndrome: A Case Report and Literature Review.

Guillain-Barré syndrome (GBS) is a term that includes a group of autoimmune-mediated peripheral neuropathies affecting varying nerve distributions, typically characterized by an acute onset of areflexic paralysis with albumino-cytologic dissociation. Given the wide range of subtypes and clinical presentations, GBS can be difficult to diagnose; however, the life-threatening nature of the disorder makes prompt diagnosis and treatment a top priority. A 36-year-old athletic young male with a history of recent upper respiratory infection presented to the emergency department complaining of difficul

DPPX antibody-mediated disease mimicking Wernicke's encephalopathy.

Anti-dipeptidyl-peptidase-like protein 6 antibody-mediated disease is a rare autoimmune encephalitis typically presenting with diarrhoea and/or weight loss, central nervous system hyperexcitability and cognitive dysfunction. We present a case of a young woman with 10 days of diplopia and unsteadiness in the context of dysthymia and significant weight loss over 2 months. Initial examination demonstrated mixed dysconjugate nystagmus and ataxic gait. Presumed diagnosis was Wernicke's encephalopathy and intravenous thiamine was given, resulting in partial improvement. Subsequent examination reveal

A Case of Wernicke's Encephalopathy With Objective Evaluation of Pupil Findings Using a Pupillometer.

Wernicke encephalopathy is a nutritionally disabling encephalopathy resulting from thiamine deficiency. The typical triad of symptoms in Wernicke's encephalopathy includes impaired consciousness, ocular motility disturbances, and ataxic gait. Although constricted pupils and diminished light reflexes are recognized features of Wernicke's encephalopathy, few reports have specifically focused on these findings. Here, we highlight the pupil diameter and light reflex abnormalities observed in this case. A 73-year-old man was admitted to our hospital because of dizziness and then referred to our dep

Wernicke's Encephalopathy Masquerading as an Acute Cerebellar Stroke.

Wernicke's encephalopathy (WE) is an acute neurological emergency resulting from severe thiamine deficiency, frequently mimicking acute cerebellar stroke due to its abrupt onset and neurological manifestations. Early recognition is crucial to prevent severe complications. Here, we report the case of a 52-year-old male with alcohol use disorder who presented with acute dizziness, vomiting, and gait instability. Neurological examination revealed left facial droop, dysarthria, and an ataxic gait with a positive Romberg sign. Given the presentation, an acute stroke was initially suspected (Nationa

Wernicke's Encephalopathy Following 40 Days of Water-Only Fasting: A Case Report.

Wernicke's encephalopathy (WE) is an acute neurological disorder that develops due to thiamine (vitamin B1) deficiency. While it is common in chronic alcoholism, WE can also manifest due to other conditions, including extreme fasting. This report presents a unique case of WE occurring after a 40-day water-only fast, emphasizing the importance of recognizing nonalcoholic causes of WE. A 36-year-old male presented with diplopia, gait instability, and confusion following a 40-day water-only fast. Neurological examination revealed bilateral sixth cranial nerve palsy, ataxic gait, and confusion. Ma

Research-Based Whole Genome Sequencing Identifies Biallelic Loss of Function Variants in DOCK3 Gene Causing DOCK3-Related Disorder: The End of a Diagnostic Journey for This Family.

The DOCK3 gene (NM_004947.5) is located on chromosome 3p21.2 spanning 53 exons and encodes the dedicator of cytokinesis 3 protein. DOCK3 belongs to the family of guanine nucleotide exchange factors (GEFs) that activate GTPases. DOCK3 is expressed almost exclusively in the central nervous system and has been shown to promote axonal outgrowth. Biallelic disruptions of DOCK3 are implicated in a neurodevelopmental disorder presenting with intellectual disability, hypotonia and ataxia (OMIM: 618292). We report a 9-year-old female with global developmental delay, moderate intellectual disability, wi

Creutzfeldt-Jakob disease mimicking limbic encephalitis as a cause of rapid neurological deterioration.

BACKGROUND: A broad range of inflammatory and neurodegenerative conditions manifest with progressive cognitive and behavioural changes. A diagnostic challenge is the differentiation of limbic encephalitis (LE) from Creutzfeldt-Jakob disease (CJD). LE and CJD are distinct neurological conditions with distinct variations in their clinical course, with overlapping clinical presentations. LE can be subdivided into autoimmune paraneoplastic and non-paraneoplastic subtypes, under the umbrella of autoimmune LE. CJD is the most prevalent form of human prion disease and the subtype sporadic CJD (sCJD)
