Demographic and socioeconomic status differences in perceptions of difficulty with mobility in late life.

OBJECTIVES: This study assesses whether socioeconomic and demographic differences in reported mobility limitations are attributable to differential perceptions of mobility difficulty that result in the differential use of response categories. METHODS: Data come from the Social Environment and Biomarkers of Aging Study and its parent study, the Taiwan Longitudinal Study of Aging. Ordered probit models with person-specific cut-points are used to test whether, after controlling for underlying mobility using objective performance measures, cut-points for reporting mobility limitations vary across

Developmental dysplasia of the hip in twins: the importance of mechanical factors in the etiology of DDH.

BACKGROUND: Developmental dysplasia of the hip (DDH) is a disease of multifactorial etiology. Genetic and intrauterine mechanical factors influence the development of the hip joint and the incidence of DDH. Breech position is a risk factor for DDH. The aim of this study is to analyze whether in twins, the intrauterine mechanical factors, such as a breech position, limitation of fetal mobility, limitation of hip abduction, and increase the incidence of DDH may, therefore, be considered as risk factors. METHODS: A pair of twins with either breech or cephalic presentation, and no family history

Onset and persistence of disabling foot pain in community-dwelling older adults over a 3-year period: a prospective cohort study.

BACKGROUND: Foot pain and related disability in older adults are common yet understudied problems. This study aimed to determine the onset and persistence of disabling foot pain in community-dwelling older adults over a 3-year period. METHODS: A 3-year follow-up postal survey was conducted in a population sample of older adults aged 50 years and older, recruited previously as part of the North Staffordshire Osteoarthritis Project. Disabling foot pain was defined as the report of problems on at least 1 of the 10 function items of the Manchester Foot Pain and Disability Index occurring on most/

Mapping onto Eq-5 D for patients in poor health.

BACKGROUND: An increasing amount of studies report mapping algorithms which predict EQ-5 D utility values using disease specific non-preference-based measures. Yet many mapping algorithms have been found to systematically overpredict EQ-5 D utility values for patients in poor health. Currently there are no guidelines on how to deal with this problem. This paper is concerned with the question of why overestimation of EQ-5 D utility values occurs for patients in poor health, and explores possible solutions. METHOD: Three existing datasets are used to estimate mapping algorithms and assess exist

Differential proteomic analysis highlights metabolic strategies associated with balhimycin production in Amycolatopsis balhimycina chemostat cultivations.

BACKGROUND: Proteomics was recently used to reveal enzymes whose expression is associated with the production of the glycopeptide antibiotic balhimycin in Amycolatopsis balhimycina batch cultivations. Combining chemostat fermentation technology, where cells proliferate with constant parameters in a highly reproducible steady-state, and differential proteomics, the relationships between physiological status and metabolic pathways during antibiotic producing and non-producing conditions could be highlighted. RESULTS: Two minimal defined media, one with low Pi (0.6 mM; LP) and proficient glucose

Estimating transition probabilities in mobility and total costs for medicare beneficiaries.

OBJECTIVE: To examine how persons move back and forth along levels of mobility disability. DESIGN: Self-reported mobility limitations were used to create categories of annual transition states. The total cost to Medicare associated with each year was calculated for each participant. In addition, we examined cost relative to transition state, adjusting for demographic and other health status variables. SETTING: National survey. PARTICIPANTS: Participants in the longitudinal Medicare Current Beneficiary Survey from 1992 to 2005. INTERVENTIONS: Not applicable. MAIN OUTCOME MEASURES: Annual s

Heart rate response and factors affecting exercise performance during home- or class-based rehabilitation for knee replacement recipients: lessons for clinical practice.

OBJECTIVES: Patients undergoing total knee replacement (TKR) are typically de-conditioned and manifest medical co-morbidities associated with a lack of fitness. Consequently, an argument exists for rehabilitation programmes to target cardiovascular fitness. Doubt exists, however, as to the capacity of TKR recipients to exercise intensely and effectively. This preliminary study aimed to: (1) determine whether patients participating in a home- or class-based exercise programme can exercise in their heart rate (HR) training zone, and (2) identify confounding factors influencing performance. Prevalence of obesity-related chronic health conditions in overweight adolescents with disabilities.

The prevalence of 15 common obesity-related chronic health conditions was examined in a convenience sample of adolescents, ages 12-18 years old, with mobility and non-mobility limitations (n = 208 and 435, respectively). In both groups, overweight adolescents (BMI ≥ 85th %ile) had a significantly higher number of obesity-related chronic health conditions than their counterparts with healthy weight status (2.74 versus 1.74 for the mobility limitation group, and 1.79 versus 1.45 for the non-mobility limitation group). Prevalence of high blood cholesterol was significantly higher among overweight

Patient-reported outcome after fast-track hip arthroplasty: a prospective cohort study.

BACKGROUND: A fast-track intervention with a short preoperative optimization period and short postoperative hospitalization has a potential for reduced convalescence and thereby a reduced need for postoperative rehabilitation. The purpose of this study was to describe patient-related outcomes, the need for additional rehabilitation after a fast-track total hip arthroplasty (THA), and the association between generic and disease specific outcomes. METHODS: The study consisted of 196 consecutive patients of which none received additional rehabilitation beyond an instructional exercise plan at di

Mood disorders and physical functioning difficulties as predictors of complex activity limitations in young U.S. adults.

BACKGROUND: There is established research that shows associations between basic physical functional difficulties and complex activity limitations. In addition, there is some research that shows associations between mood disorders and complex activity limitations. However, there is limited research looking at the joint association between mood disorders and physical functioning and complex activity limitations. Furthermore, because mood disorders and physical functioning limitations increase with age, there is a lack of information available on younger adults. OBJECTIVES: We assess the impact

Limitations of self-reported estimates of functional capacity using the Walking Impairment Questionnaire.

OBJECTIVE: A potential issue with the Walking Impairment Questionnaire (WIQ) is that it is relatively complex. We estimated the number of errors made by patients when self-completing the WIQ, and assessed the benefit of correcting missing, duplicate or paradoxical (i.e., reported lower difficulty for a higher-intensity task) answers. DESIGN: Prospective non-interventional study. MATERIALS: All consecutive new patients with claudication over a 3-month period. METHODS: The WIQ was self-completed before patients performed a constant-load treadmill walking test (maximised to 750 m). MAIN OUTCO

Using video animation to assess mobility in older adults.

BACKGROUND: Although the importance of the context of task performance in the assessment of mobility in older adults is generally understood, there is little empirical evidence that demonstrates how sensitive older adults are to subtle changes in task demands. Thus, we developed a novel approach to examine this issue. METHODS: We collected item response data to 81 animated video clips, where various mobility-related tasks were modified in a systematic fashion to manipulate task difficulty. RESULTS: The participants (N = 234), 166 women and 68 men, had an average age of 81.9 years and a varie

Prolonged healing of venous leg ulcers: the role of venous reflux, ulcer characteristics and mobility.

OBJECTIVE: To assess whether a difference in venous reflux pattern, ulcer size or duration, regular medications or the daily use of walking aids can predict the healing of a chronic venous leg ulcer (VLU). METHOD: In this prospective, randomised, single-centre study, 110 consecutive patients with chronic leg ulcers were assessed. Ninety-nine patients met the inclusion criteria and a controlled, conservative 3-month treatment period was initiated, in which 90 patients were managed with standardised local treatment combined with compression therapy. In this group, 62 VLUs healed within 12 weeks

A young man with an ulcerated lesion on the right ankle.

We present a case of epitheloid sarcoma that was referred with a 2x3 cm ulcerated lesion on the right ankle and edema of the lower leg. Foot drop of the right side was present that had caused walking difficulty. After a few months, he developed several sporthricoidal nodular lesions on the medial aspect of right thigh, inguinal lymphadenopathy, weight loss, anorexia and respiratory symptoms. Chest x-ray and HRCT showed pulmonary metastasis. Histopathological evaluation and immunohistochemical profile of both skin lesion and involved inguinal lymph node were consistent with epitheloid sarcoma.

Predominant sensory ataxic neuronopathy showing marked improvement after resection of a thymoma followed by intravenous immunoglobulin therapy.

A 64-year-old man was admitted to our hospital because of difficulty in walking and numbness in his lower extremities. Upon investigation, the patient was diagnosed as having predominant sensory ataxic neuronopathy associated with thymoma. Surgical resection of the thymoma followed by intravenous immunoglobulin therapy resulted in marked improvement of the patient's clinical symptoms; therefore immunological mechanisms related to the presence of the thymoma were suspected to underlie the neuropathy in this patient. We did not find any previous reports of an association of sensory ataxic neuron

Feasibility of a gait retraining strategy for reducing knee joint loading: increased trunk lean guided by real-time biofeedback.

The purpose of this feasibility study was to examine changes in frontal plane knee and hip walking biomechanics following a gait retraining strategy focused on increasing lateral trunk lean and to quantify reports of difficulty and joint discomfort when performing such a gait modification. After undergoing a baseline analysis of normal walking, 9 young, healthy participants were trained to modify their gait to exhibit small (4°), medium (8°), and large (12°) amounts of lateral trunk lean. Training was guided by the use of real-time biofeedback of the actual trunk lean angle. Peak frontal plane
