Assessing injury-related movement difficulties: A method for analyzing the association between functional limitations and social participation.

BACKGROUND: The conceptualization of disability has shifted from a medical to a social model with a consequent focus away from impairments and toward activities and participation. The International Classification of Functioning, Disability and Health (ICF) provides a common point of reference and a common language in a developing disability discourse. OBJECTIVES: We sought to apply a model for the measurement of disability based on the activity and participation constructs of the ICF to persons with movement difficulty as a result of injury-related causes. METHODS: Data from sample adults ag

Fear of falling and falls in people with Parkinson's disease treated with deep brain stimulation in the subthalamic nuclei.

BACKGROUND: No previous study prospectively investigated the effects of subthalamic deep brain stimulation on fear of falling (FOF) and falls. AIM: The aim was to prospectively explore whether FOF and fall rate were affected after STN stimulation in people with Parkinson's disease (PD). METHODS: Twenty participants (mean age: 65, SD 6.4) were included. Falls and near falls were recorded (fall diary) during 3 months before and 1 year after surgery. FOF was evaluated using the Falls-Efficacy Scale, Swedish version, FES(S), and the modified Survey of Activities and Fear of Falling in the Elderl

A new method in the treatment of postburn and post-traumatic scar contractures: Double-opposing Z- and V- (K-M-N) plasty.

BACKGROUND: To date, many techniques for the surgical treatment of postburn scar contractures have been described. Some of the most popular techniques are Z-plasty, V-Y-plasty and their analogues. A major limitation of these techniques is that the excess tissue requires excision of the dog ear. The current study presents a new modification of the double-opposing Z- and V-plasty, called 'K-M-N plasty'. METHODS: Twenty postburn scar contractures were successfully treated with K-M-N plasty. The postoperative results depict the versatility of this technique in the surgical treatment of postburn s

The effect of collateral ligament release on ankle dorsiflexion: an anatomical study.

BACKGROUND: Ankle stiffness is a common complication after ankle fracture, reconstructive surgery or total ankle replacement, and the usual limitation is in dorsiflexion. There are few articles in the literature concerning this frequent problem, and furthermore they are not recent and tend to be controversial. The purpose of this anatomical study was to evaluate and quantify the effect of ankle collateral ligament release on dorsiflexion, specifically the amount of increase in ankle dorsiflexion following section of the two ligaments most often implicated in ankle stiffness: the deep posterior

Primary lumbosacral intradural hydatid cyst in a child.

Primary spinal hydatid cyst is very rare lesion affecting less than 1% of the total cases of the hydatid disease. In this study, we report a case of spinal hydatid in a 5-year old boy presented with a history of backache, leg pain, difficulty in walking, and bowel and bladder incontinence for 4 months. An intradural nonenhancing cystic lesion was detected using magnetic resonance imaging from L4 to sacral region, and histopathological findings were suggestive of hydatid cyst.

A huge infiltrating squamous cell carcinoma of the neck 46 years after radiotherapy of a haemangioma.

BACKGROUND: There is a higher risk for malignant tumours in patients who underwent any kind of radiotherapy. MAIN OBSERVATIONS: An 87-year-old patient with an extended squamous cell carcinoma of the neck was referred to our hospital. The large ulcerated tumour showed a deep infiltration of the para-vertebral muscles and the spinal processes C6-C7. The tumour was known since about six years and appeared about 46 years after low-dose radiotherapy of a cervical haemangioma of the neck in 1961. The large dimension of the tumour leaded to a radical surgical removal and a sufficient fast reconstruc

Clinical evaluation of the efficacy of Khadiradi yoga avachoornana in Kachchu with special reference to genitoinguinal intertrigo.

Kachchu is a simple localized dermatological infection of the genitoinguinal region, which occurs because of unhygienic observance, and manifests in the form of inflammation followed by secondary bacterial or fungal infections that can be best correlated to genitoinguinal intertrigo. Even though it rarely causes systemic manifestations, its mere presence itself is disturbing because of intractable itching and pain. It may delay the proper healing of the episiotomy wound and may cause difficulty in walking because of severe pain. Candida powder is the highest selling medicine for the intertrigo

Chronic obstructive pulmonary disease in the absence of chronic bronchitis in China.

BACKGROUND AND OBJECTIVE: COPD has a variable natural history and not all individuals follow the same course. The aim of this study was to assess the prevalence of COPD in the absence of chronic bronchitis (CB) based on a population survey in China, and to identify the determinants of CB in patients with COPD. METHODS: A multi-stage cluster sampling strategy was used to survey a population from seven different provinces/cities of China. All residents over 40 years of age were interviewed using a standardized questionnaire and spirometry was measured. A post-bronchodilator FEV(1)/FVC < 70% was

Injection-related venous disease and walking mobility.

Injection users are at risk for vascular injuries resulting in chronic venous disease (CVD). The authors examined walking mobility in relation to CVD for 713 individuals in methadone treatment. The authors used a cross-sectional, comparative design that was stratified on age, sex, ethnicity, and drug use. CVD was present in 92.3% of participants. The structural equation model supported the causal link between leg injection and CVD (.40, P<.001). The worse the mobility, the greater was the CVD classification (-.21, P<.001). CVD had an indirect effect on mobility through pain. CVD and pain need

Ischemic fasciitis in an 85-year-old debilitated female.

Ischemic fasciitis is a rare reactive, non-neoplastic proliferation of atypical fibroblasts typically seen in older debilitated patients at pressure points. We describe the case of an 85-year-old female nursing home resident with a past medical history of advanced dementia and immobility who developed an enlarging nodule on the posterior aspect of her right shoulder. A needle biopsy was performed under ultrasound guidance, and the histology was consistent with ischemic fasciitis. Surgical resection of the lesion was performed and anatomical pathology was consistent with ischemic fasciitis. Isc

Secondary analysis of a scoping review of health promotion interventions for persons with disabilities: Do health promotion interventions for people with mobility impairments address secondary condition reduction and increased community participation?

BACKGROUND: Secondary conditions can have very serious outcomes for people with physical disabilities. Such consequences can range from immobility due to pressure sores to withdrawal and isolation due to depression, decreasing participation in the community. OBJECTIVE/HYPOTHESIS: To further investigate these assumptions, we conducted a review of the literature on health promotion interventions that include physical activity for adults with disabilities to determine whether they have a positive effect on the reduction of secondary conditions and increased community participation. METHODS: We

A treatment option for post-injection sciatic neuropathy: transsacral block with methylprednisolone.

BACKGROUND: Accidental intraneural injection induced nerve injury is an iatrogenic tragedy and intramuscular injection (IM) is the most common injury mechanism affecting the sciatic nerve. The most frequent presentation of sciatic nerve injury includes radicular pain and paresthesia with almost immediate onset of variable motor and sensory deficit. OBJECTIVES: Intraneural injection is a common injury mechanism of the sciatic nerve and generates neuropathic pain with inflammatory neuritis. Steroids inhibit the production of inflammatory mediators and reduce ectopic discharges on damaged neural
