Spinocerebellar ataxia type 27B (SCA27B) is a recently described autosomal dominant ataxia caused by a pathogenic GAA repeat expansion in the FGF14 gene. The frequency and the distribution of FGF14 repeat alleles remain largely unexplored in underrepresented populations, particularly in Latin America. To investigate the frequency of SCA27B and characterize the allelic distribution of the GAA-FGF14 microsatellite locus in a Peruvian cohort of individuals with ataxia of unknown molecular etiology. We analysed a cohort of affected individuals evaluated at a referral center for neurogenetic disord

BACKGROUND: Tremor affects many people with multiple sclerosis (MS), is frequently disabling and responds poorly to medication. Deep brain stimulation, gamma-knife radiosurgery and MR-guided focused ultrasound are options, but each has constraints. MR-guided laser interstitial thermal therapy (MRgLITT) is a hardware-free stereotactic thalamotomy providing real-time thermometry, its role in MS tremor is unknown.
OBJECTIVE: To describe 12-month clinical, safety and imaging outcomes after unilateral MRgLITT thalamotomy for refractory MS tremor.
METHODS: Single-centre, consecutive case series of

Mutations in the mitochondrial DNA polymerase gamma (POLG) gene cause a broad spectrum of neurodegenerative disorders, including the ataxia-neuropathy spectrum (ANS). Substantial phenotypic heterogeneity and overlapping clinical features can delay diagnosis. We report a 38-year-old woman with no significant past medical history who presented with a three-year history of slowly progressive sensory ataxia that worsened in the dark. Examination revealed mild dysarthria, large-fiber sensory loss, and a positive Romberg sign. Extensive metabolic, toxic, autoimmune, and infectious work-up was unreve

BACKGROUND: Autosomal recessive spastic ataxia of Charlevoix-Saguenay (ARSACS) is a rare, early-onset neurodegenerative disorder characterized by progressive cerebellar ataxia, spasticity and peripheral neuropathy. It is caused by biallelic mutations in the SACS gene, which encodes sacsin, a large protein involved in cytoskeletal organization and mitochondrial homeostasis, and protein quality control. Despite advances in understanding sacsin biology, the molecular mechanisms underlying neurodegeneration in ARSACS remain incompletely understood. Here, we report the first neuropathological evide

COQ8B (formerly ADCK4)-associated glomerulopathy is an autosomal recessive primary coenzyme Q10 deficiency that typically presents with proteinuria, focal segmental glomerulosclerosis and progressive kidney dysfunction during childhood or adolescence. We report extended longitudinal follow-up of a family carrying the homozygous COQ8B c.1339dupG, p.(Glu447Glyfs*10) variant, demonstrating marked intrafamilial phenotypic heterogeneity despite an identical genotype. Two homozygous siblings developed kidney failure in childhood or adolescence and underwent living-related kidney transplantation, wit

Spinocerebellar ataxia type 3 (SCA3), also called Machado-Joseph disease, the most common form of autosomal dominant cerebellar ataxia, is caused by a CAG repeat expansion in ATXN3. The mutant protein is prone to aggregation and exerts toxic gain-of-function effects, whereas the wild-type protein has important neuroprotective roles as a deubiquitinase. Current treatment strategies for SCA3 lack specificity for the mutant allele. Identifying genetic variants such as single-nucleotide polymorphisms (SNPs) in linkage with the pathogenic CAG expansion offers a promising avenue to develop allele-sp

UNLABELLED: Microsatellite instability-high (MSI-H) colorectal cancer cells depend on the Werner syndrome helicase (WRN) to resolve cruciform DNA structures that arise from expanded TA-dinucleotide repeats. Loss of WRN induces replication stress and double-strand breaks, a vulnerability that can be recapitulated with the selective WRN inhibitor (WRNi) HRO761 in MSI-H cancer cells. To uncover the mechanisms governing sensitivity to WRN inhibition, we conducted genome-wide CRISPR/Cas9 screens in colorectal cancer cell lines treated with or without HRO761. These screens identified SMARCAL1 as a k

Anti-GQ1b antibody syndrome encompasses related postinfectious neurologic disorders, including Miller Fisher syndrome, Guillain-Barré syndrome, and Bickerstaff brainstem encephalitis. Recognizing overlap among these phenotypes can be difficult in the intensive care unit because profound unresponsiveness and loss of brainstem reflexes may be attributed to sedation or structural brain injury. A middle-aged man developed perioral numbness, blurred vision, ataxia, extremity paresthesias, and facial weakness after an upper respiratory infection. His condition progressed rapidly to respiratory failu

Cellular DNA is constantly exposed to endogenous and exogenous genotoxic factors causing mutations. Therefore, safeguarding the integrity of the human genome is a conditio sine qua non for normal cell function. To maintain genomic integrity, complex multiprotein networks, known as DNA damage response (DDR) signaling pathways, sense DNA lesions and repair mistakes or induce cell death when extensive unrepairable damage has occurred. Two of the major DDR pathways are the ataxia-telangiectasia mutated (ATM) and ataxia-telangiectasia and Rad3-related (ATR) cascades. The ATM pathway primarily respo

We report progressive myoclonic epilepsy in two brothers. The younger brother was diagnosed with progressive myoclonic epilepsy type 1 (EPM1) by genetic testing, and the elder brother underwent pathological examination. The parents were cousins. Both brothers presented with epileptic seizures in their early teens, followed by progressive cognitive impairment and cerebellar ataxia. In later life, epileptic seizures became intractable. The elder brother died at age 71, and the younger brother died at age 63. Genetic examination of the younger brother revealed a mutation in the CSTB gene. Patholo

The paper presents a method for genotyping Charolais and Angus beef bulls for the causative mutation within KIF1C gene responsible for Progressive Ataxia (PA). The method involves amplifying a 406 bp fragment of the KIF1C gene using PCR and detecting the polymorphic site with the Mvn I restriction enzyme. Due to its simplicity, speed, and low cost, this method is accessible to an average laboratory equipped with a PCR thermocycler. Using this method, 62 bulls (46 Charolais and 16 Angus) currently used in artificial insemination in Poland were tested, among which 7 PA carriers were identified,

Friedreich's ataxia (FA) is a rare autosomal recessive neurodegenerative disorder caused by reduced expression of frataxin, a mitochondrial protein important for iron-sulfur cluster assembly and mitochondrial homeostasis. Although FA has traditionally been attributed to neuronal dysfunction, increasing evidence suggests that glial cells play a critical role in disease progression, although their contribution remains poorly defined. Using the FXNI151F mouse model, we investigated cell-type-specific metabolic and redox alterations in neurons and glial populations from the cerebrum, cerebellum, a
