CAG repeat length shapes bidirectional functional connectome alterations in spinocerebellar ataxia type 3: a multimodal MRI study.

The ATXN3 gene is the primary driver of brain damage in spinocerebellar ataxia type 3 (SCA3). We aimed to elucidate the patterns of connectome alterations shaped by cytosine-adenine-guanine (CAG) repeat length in SCA3. Eighty-nine genetically confirmed patients with SCA3 and 89 age-, sex-, education-matched healthy controls underwent multimodal MRI and comprehensive clinical assessments. Connectome-based predictive modeling (CPM) was used to identify functional connectivity patterns associated with CAG repeat length. Structural brain damage was assessed using voxel-based morphometry. Mediation

CFI-400945 enhances radiosensitivity in colorectal cancer models by promoting mitotic dysregulation and DNA damage-associated apoptosis.

BACKGROUND: Radioresistance remains a major barrier to improving radiotherapy (RT) efficacy in colorectal cancer (CRC). Since mitotic fidelity and centrosome homeostasis influence cell fate under genotoxic stress, we investigated whether CFI-400945 enhances the radiation response in CRC models and whether this effect is associated with polo-like kinase 4 (PLK4)-related mitotic dysregulation. METHODS: HCT116 and HT29 CRC cells were treated with CFI-400945 and/or irradiation. Radiation response was evaluated using MTT viability and clonogenic survival assays, alongside analyses of cell-cycle di

Polyomavirus Drives Cerebellar Lesions in AIDS Monkeys With Ataxia.

Cognitive and motor dysfunction occurs in 30%-50% of people with HIV/AIDS. Within this clinical spectrum, HIV-associated ataxia is an uncommon but debilitating disorder with poorly defined neuropathological mechanisms. This study investigated two rhesus macaques with chronic simian immunodeficiency virus (SIV) infection that developed progressive ataxia, characterized by postural imbalance and nystagmus. Comparative neuropathological profiling of cerebellar tissue from ataxic SIV-infected macaques, non-ataxic SIV-infected macaques, and uninfected controls revealed that ataxia was associated wi

Generation of Spinocerebellar Ataxia 27B iPSC Lines and Isogenic Controls.

Spinocerebellar Ataxia 27B (SCA27B) is a late-onset neurodegenerative disorder caused by a heterozygous pathogenic GAA•TTC repeat expansion in intron 1 of FGF14. Despite being a common genetic cause of adult-onset cerebellar ataxia, the underlying molecular mechanisms remain unclear. Here, we performed simultaneous reprogramming and CRISPR-Cas9 genome editing to generate four induced pluripotent stem cell (iPSC) lines and their respective isogenic gene-corrected controls. All lines exhibited normal karyotypes, pluripotency, and trilineage differentiation capacity. These paired iPSC lines provi

Longitudinal Changes and Responsiveness of the Cerebellar Cognitive Affective Syndrome Scale in Patients with Acute Cerebellar Stroke.

Cognitive and affective disturbances due to cerebellar lesions are termed cerebellar cognitive affective syndrome (CCAS). Although CCAS is frequent after acute cerebellar stroke, CCAS-Scale responsiveness during acute hospitalization remains unclear. Objective: To examine longitudinal changes and responsiveness of the CCAS-Scale and compare its changes with global cognitive function, ataxia, and activities of daily living (ADL). In this single-center prospective observational study, 21 patients with first-ever cerebellar stroke were assessed 7 days after onset and at discharge (median, 21 days

Acute cerebellitis as a post-infectious complication of primary varicella in an immunocompetent young adult: A case report.

BACKGROUND: Acute cerebellitis is a well-documented neurological sequela of primary varicella-zoster virus (VZV) infection. Although frequently reported in children, this complication remains uncommon in adults and may present diagnostic difficulties, especially when neuroimaging studies are normal. CASE PRESENTATION: A previously healthy, immunocompetent 20-year-old man was admitted with a four-day history of progressive gait instability and imbalance. Nine days prior to admission, he had developed a generalized vesicular rash consistent with primary varicella, initially involving the trunk

Predicted structure of the complete CPLANE complex reveals novel interactors and mechanisms of Jbts17 during ciliogenesis.

Cilia are highly conserved organelles that use hundreds of unique proteins to drive extracellular motility and serve as signaling hubs for eukaryotic cells. The Ciliogenesis and Planar Polarity Effector (CPLANE) protein complex controls basal body docking during early ciliogenesis and recruits the intraflagellar transport (IFT) proteins to the base of growing cilia. Leveraging recent advantages in deep learning neural networks and high-performance computing, we predict the structure of JBTS17, the largest CPLANE subunit, and the tertiary structure of the complete CPLANE complex, revealing dire

Ataxia Therapeutics: A Comprehensive Overview of the Clinical Development Pipeline.

Ataxias are a heterogeneous group of rare neurological disorders characterized by progressive impairment of motor coordination and balance, caused by a broad range of genetic, immune-mediated, and degenerative mechanisms. Despite significant advances in molecular diagnostics and disease characterization, effective disease-targeted treatments remain limited, with omaveloxolone currently representing the only approved therapy for Friedreich's ataxia. In this review, we provide a comprehensive and up-to-date overview of the clinical drug development pipeline for ataxias, based on a systematic ana

Generation of Spontaneously Beating Neuromyocardial Organoids with Neural Crest-Derived Proprioceptive-Biased Neurons and Real-Time Respirometry.

The neural crest gives rise to diverse cell types, including the proprioceptive sensory neurons that relay limb and organ position to the central nervous system. Studying how these neurons interact with their peripheral targets in a human context has been constrained by the lack of integrated in vitro models that combine neural crest derivatives with functional effector tissue. Here, we present a protocol for generating spontaneously beating neuromyocardial organoids from human pluripotent stem cells (hPSCs), incorporating neural crest-derived proprioceptive-biased neurons. Differentiation pro

DHDDS variants in epilepsy and movement disorders: Two novel mutations and an updated literature review.

INTRODUCTION: Recent evidence suggests that dehydrodolichol diphosphate synthase (DHDDS, MIM*608,172) gene variation can cause neurological diseases. However, the complete clinical and genetic spectrum of DHDDS-related disorders has not been fully elucidated. This study aims to systematically describe the clinical and genetic features of five patients with molecularly confirmed DHDDS variants and to provide an updated review of previously reported cases. METHODS: We retrospectively enrolled patients with DHDDS gene variants in the Epilepsy Center of Beijing Tiantan Hospital, Capital Medical U

Clinical heterogeneity in chronic inflammatory demyelinating polyneuropathy: data-driven subtypes challenge current classification.

BACKGROUND: Chronic inflammatory demyelinating polyneuropathy (CIDP) presents with substantial phenotypic heterogeneity, yet current classifications do not adequately capture clinically meaningful subgroups. Thus, we aimed to discover novel, clinically meaningful CIDP subgroups using unbiased clustering. METHODS: In this observational, multicenter retrospective cohort study we screened 425 patients diagnosed with typical CIDP or CIDP variants from 10 German centers. Standardized data including demographic, clinical, electrophysiological, laboratory, comorbidity, and treatment information were

Management of fragile X-associated tremor ataxia syndrome: a review for clinicians.

Fragile X-associated tremor ataxia syndrome (FXTAS) is a multi-faceted and progressive neurodegenerative disorder that manifests clinically with action tremor, gait ataxia, parkinsonism, peripheral neuropathy, and cognitive and psychiatric features. The disease is highly heterogeneous and there are no FDA approved medications for FXTAS as a specific indication. As such, treatment is symptomatic and must be tailored to the specific needs of a given patient. This review provides expert opinion-based treatment recommendations for the core and ancillary clinical features of FXTAS. Movement disorde

Transient ATM inhibition enhances knock-in efficiency in hematopoietic stem cells by attenuating the DNA damage response.

Precise genome editing in hematopoietic stem cells (HSCs) holds great potential for treating inherited blood disorders; however, low knock-in (KI) efficiency and loss of functional HSCs after genome editing remain major obstacles. Here, we demonstrate that transient inhibition of Ataxia-Telangiectasia Mutated (ATM) kinase markedly enhanced KI efficiency in mouse HSCs genome-edited with Cas9/ribonucleoprotein (RNP) and AAV donor DNA. Phosphoproteomic and capillary western blot analyses revealed that ATM inhibition suppressed aberrant Cas9-AAV-induced ATM activation and the subsequent DNA damage

[Clinical heterogeneity and prediction complexity associated with nucleotide sequence abnormalities in the SCN2A gene].

Numerous variants of the SCN2A gene have been identified, including missense mutations, deletions, insertions, frameshifting, nonsense mutations, and splice site mutations. The emergence of severe phenotypes is generally associated with de novo and missense variants. The clinical manifestations linked to SCN2A abnormalities exhibit considerable diversity, including varying severities of epileptic episodes with onset in neonatal or subsequent periods, autism spectrum disorders, schizophrenia (including catatonic manifestations), and episodic ataxia. With rare exceptions, all identified phenotyp
